Description
Technical Field
The invention belongs to the technical field of medical instruments, and particularly relates to a novel knee joint peripheral ligament insertion fixing nail.
Background
Knee joint collateral ligament injury is commonly seen in patients with obesity, osteoporosis, inflammatory arthropathy, severe knee valgus deformity, and previous surgical history. Although rare complications, can still lead to post-operative knee instability, accelerated implant wear, eventual failed revision or secondary surgery, among other undesirable consequences. For the body injury of the collateral ligament, suture, autologous tendon transplantation and synthetic materials can be adopted for repair. Injuries to ligament insertion, particularly avulsion, require treatment with suture anchors, spacer screws, staples, or other fixation devices. However, none of the above methods of fixation provides sufficient gripping force to fix the torn bone pieces and/or the transplanted tendon ligament, and re-tearing of the ligament insertion points is likely to occur.
Therefore, how to ensure the effective fixation of the collateral ligament creates good conditions for the healing of the collateral ligament and avoids the patellar ligament from tearing off, which becomes a difficult problem in front of clinicians.
In order to solve the problem of poor gripping force in the traditional fixing mode, scientific researchers invented a novel ligament fixer for knee joint surgery. The design of the fixing plate is adopted, so that the compression area can be effectively increased, and the ligament structure is uniformly stressed. The cross section of the fixing plate for knee joint collateral ligament injury is rectangular, four corners are provided with conical stand columns, and the stand columns are provided with circular grooves to strengthen occlusion. The fixation plate has a central hole for screw attachment and 6 needle holes around it for threading to reinforce the suture ligament. Fixing the torn bone blocks in situ by using a fixing plate in the operation, and lightly hammering to ensure that the fixing plate is occluded with the bone; then preparing a channel by using a 3.2mm drill through a central hole of the fixing plate, and evaluating the length of the channel by using a depth finder; finally, an expansion screw is screwed in and comprises an expansion screw with an inner cavity and an inner core embedded in the inner cavity, and the expansion screw expands and is tightly pressed against the femoral condyle.
Although the above-mentioned ligament fixator for knee joint surgery improves the grasping force, the following disadvantages have been inherent: firstly, a fixing plate and a fixing nail are required to be assembled and fixed; secondly, the fixing plate is not provided with a holding instrument, and a drill bit and a depth measuring ruler are required to be firstly used for implanting the fixing nail; thirdly, the fixing nail has two kinds: one is that the common spongy bone screw with the diameter of 6.5mm needs double cortical fixation and can not be fixed by single cortical fixation, and the other is that the expansion type screw needs complicated assembling operation.
In conclusion, how to ensure the effective fixation of the collateral ligament creates good conditions for the healing of the collateral ligament and avoids the patellar ligament from tearing off, which is a difficult problem in front of clinicians.

Detailed Description
The following disclosure provides many different embodiments, or examples, for implementing different features of the invention. To simplify the disclosure of the present invention, the components and arrangements of specific examples are described below. Furthermore, the present invention may repeat reference numerals and/or letters in the various examples. This repetition is for the purpose of simplicity and clarity and does not in itself dictate a relationship between the various embodiments and/or configurations discussed. It should be noted that the components illustrated in the figures are not necessarily drawn to scale. Descriptions of well-known components and processing techniques and procedures are omitted so as to not unnecessarily limit the invention.
Referring to fig. 1 to 4, the invention provides a novel knee joint peripheral ligament insertion fixing nail. The staple includes a staple body 10 and a washer 20. The fixing nail body 10 comprises a nail head 11 and a nail tail 12. The nail head 11 is a self-tapping nail, so that auxiliary instruments such as a drill bit and the like are not needed in the specific implantation process, and the operation is simpler.
The washer 20 is sleeved outside the nail tail 12. Referring to fig. 3, the inner edge of the gasket 20 is provided with a second serration structure 23. Referring to fig. 4, the outer edge of the nail tail 2 of the fixing nail body 10 is provided with a first sawtooth structure 13. The first saw tooth structure 13 and the second saw tooth structure 23 are matched. When the fixing nail body 10 and the washer 20 are assembled, the first sawtooth structure 13 and the second sawtooth structure 23 are matched and connected, so that the fixing nail body and the washer can be stably fixed.
The fixing nail body 10 is of an integrated structure, so that the fixing performance of the structure is better, and the production process is simpler. According to the research on the characteristics of ligament insertion points around the knee joint and through a large amount of experimental data analysis, some numerical parameters of the optimal fixing nail body 10 are obtained. The diameter of the fixing nail body 10 is 8.0 mm. The length of the fixing nail body 10 is between 2cm and 8cm, for example: 2cm, 6cm or 8 cm. In order to reduce the production cost while ensuring the practicability, it is more preferable that the length of the staple body 10 is varied in a gradient of 0.5cm, for example: the length of the fixing nail body 10 is 2cm, 2.5cm or 7.5 cm.
The screw thread on the fixing nail body 10 provided by the invention is deep, and the holding force for fixing cancellous bone is good, so that single-cortical fixation can be carried out. In addition, the prosthetic knee joint prosthesis can be used together with a revision knee joint prosthesis with an extension rod.
Preferably, at least one conical projection 21 is provided on the washer 20. When carrying out the ligament fixed, the toper arch 21 can promote the power of grabbing of staple itself, and the displacement is difficult for appearing, and stability can obtain further promotion.
Notably, the staple body 10 is a universal screw. Which may be disposed at an angle to the gasket 20. Preferably, the angle between the staple body 10 and the washer 20 is in the range of 30 °, i.e. not more than 30 °.
Depending on the injury, in addition to staple fixation, it is sometimes necessary to perform suture to further fix the injury site and promote healing. In order to meet different requirements, preferably, a threading hole 22 is further disposed on the gasket 20, and the threading hole 22 is used for threading a suture. The diameter of the threading hole 22 is not too large due to the limited area of the washer 20; however, if the diameter is too small, it may cause difficulty in running the suture. Therefore, it is more preferable that the diameter of the threading hole 22 is 1.2 mm. In order to further improve the stability of the sewing, it is preferable that at least two threading holes 22 are provided; and a plurality of threading holes 22 are arranged at equal intervals.
Since the staple needs to be implanted for use in vivo, safety, stability and lifespan are very important. After conducting a great deal of research, the inventors have made the staple using a titanium alloy material, including the staple body 10 and the washer 20. Considering that bone ingrowth is a long-term process, the titanium alloy material is also coated with a hydroxyapatite coating. Preferably, the hydroxyapatite coating layer has a thickness of 50 μm.
The following illustrates a specific application process of the staple provided by the present invention: when the femoral cut point of the medial collateral ligament is torn and broken in the operation, firstly, a gasket 20 (the back surface of the gasket is provided with a conical bulge 21, so that the clamping force is better) is used for fixing a fracture block in situ; then, the staple body 10 is screwed in the most appropriate direction (within 30 degrees); finally, a suture thread is inserted into the threading hole 22 for sewing so as to enhance the fixing effect.
The fixing nail provided by the invention has the advantages of good fixing effect, simplicity in operation and low cost, and is beneficial to large-scale popularization and use.
Although the present invention has been described in detail with respect to the exemplary embodiments and advantages thereof, it should be understood that various changes, substitutions, and alterations can be made hereto without departing from the spirit and scope of the invention as defined by the appended claims.
