Abstract
植入式的血管内神经刺激器支架系统由微米级血管交感神经刺激器固定在锌合金支架梁上,通过介入方法与支架一起植入到靶血管,刺激器收集外界的射频能量通过控制电路输出恒定电流,随着锌合金支架的降解,血管交感神经刺激器逐渐从内膜层扩散到外弹力层,体外的超声刺激诱发刺激器电路,直接产生电信号刺激血管周围的交感神经,促进靶血管的收缩和舒张功能,促使重建血管生理稳态的恢复。

技术领域
本发明涉及医疗器械技术领域,尤其是涉及调节血管功能的血管内神经刺激器支架系统。
背景技术
血管疾病包括冠心病、颅内缺血性疾病以及下肢和内脏等血管粥样硬化性以及梗阻性疾病,是影响人类健康的重大疾病,其中因心血管疾病死亡的人数占比达到了41.1%,已成为我国居民的首要死因。根据著名医学杂志Lancet在2019年6月24日发表的中国疾病负担结果,中风和缺血性心脏病已成为疾病负担的主要原因。
介入治疗成为血管阻塞性疾病主要治疗手段,以心血管治疗为例,目前主要的治疗手段是药物洗脱支架,但药物抑制再狭窄同时会导致血管内壁愈合过程延迟,增加了晚期血栓风险。支架永久存在于靶病变处,可能会导致延迟愈合、内皮功能障碍、支架内血栓形成、新生动脉粥样硬化以及支架与生长的血管的匹配等问题。由此,可降解支架基础和临床研究成为热点。Abbott公司“Absorb BVS”是世界上研究最为广泛的高分子可降解冠脉支架,2016年被FDA批准上市,18个月后全球停止应用,主要原因是由于聚乳酸支架材料本身机械性能的欠缺,晚期支架内血栓的事件风险增加。镁基支架是研究最为广泛的可降解金属支架,Biotronik 公司生产的Magmaris 镁合金支架2016年已获得CE证书,由于该支架体内降解速率过快,大约2周到12周,再狭窄率和血栓发生率较高。2001年还曾首次报道了关于铁基可降解支架研究,结果表明铁基可降解支架降解速率过慢长达5年以上。
Zn的标准电极电位(-0.763 V) 处于Mg(-2.372V)与Fe (-0.447V)之间,降解速率适宜,是生物体必需的微量金属元素,对维持机体生理功能和正常代谢十分重要。至少300种酶以锌为辅助因子参与糖类、脂质、蛋白质和核酸代谢。成年人每天需要摄入约15mg锌(和一支架重量相当)来满足新陈代谢的需求。锌还在预防心脏病方面扮演积极的角色,如维持内皮细胞(EC)的完整性,通过增加内源性成纤维细胞生长因子的水平刺激EC的扩散改善心脏功能,以及防止进一步损害,如缺血和梗死。纯锌支架的兔腹主动脉研究显示,支架植入后生物相容性良好,锌基支架有望成为新一代完全可生物降解金属支架。
植物神经系统是连接中枢神经系统和内脏效应的传入和传出神经元的集合,可无意识地调节身体机能,如心率,消化,呼吸速率等。通过对血管周围的植物神经的调控,可以改善血管的功能。肾交感传入和传出神经在高血压的发生和发展中起重要的调节作用。肾交感神经通过钠盐平衡、肾素、外周血管调节血压,可长期调控血压已得到证实。
鉴于目前支架植入后引发的并发症,本发明在前期工作的基础上制作一种微米级血管交感神经刺激器并固定在锌合金支架梁,通过介入方法与支架一起植入到靶血管,神经刺激器直接产生电信号刺激血管周围的交感神经,促进靶血管的收缩和舒张功能,促使重建血管生理稳态的恢复。
发明内容
本发明一种血管内神经刺激器支架系统,制作微米级血管交感神经刺激器并通过激光焊接的方式固定在锌合金支架梁,通过介入方法与支架一起植入到靶血管,刺激器收集外界的射频能量通过控制电路输出恒定电流,可以促进锌合金支架表面形成的局部电解电离,减少表面金属化合物沉淀,促进锌合金支架的完全降解,随着锌合金的降解,血管交感神经刺激器逐渐从内膜层扩散到外弹力层,体外的超声刺激诱发刺激器电路,直接产生电信号刺激血管周围的交感神经,促进靶血管的收缩和舒张功能,促使重建血管生理稳态的恢复。
微型血管调控刺激器由神经刺激器、体外程控系统及射频能量收集器组成。由外界射频(RF)能量驱动工作的设计具有尺寸小、质量轻的优点(刺激器重量为4mg,在典型的神经刺激条件下的平均功耗为20μW)。神经刺激器和射频收集器采用激光焊接的方式固定在医用钛合金外壳内。外壳留有2个电极孔,正负电极柱伸出电极孔,其中负极与支架梁接触,正极裸露在血液中。电极与外壳的空隙通过陶瓷封密封,以防止血液等体液进入刺激器内造成短路。

具体实施方案
本发明是植入式血管内神经刺激器设备,该设备包括锌合金材料的支架结构以及嵌入在支架上内含微电路的神经刺激器及体外电脑控制系统组成。
其中支架本体形状上通常为管状或半开放式,具有支撑作用,该支撑结构可以根据靶血管病变位置设置其长度和管径大小,上面设计有使其本身撑开的花纹图案和嵌入的含有微电路的神经刺激器设备。整体的系统通过支架介入手术的输送工具植入到靶血管的内部。
该系统的支架结构的柔韧性和可压缩性使得其可以到达靶血管位置,该系统被压缩和传送到靶血管,一旦到达合适的位置,利用气球或传送系统控制的驱动轴,支架设备被膨胀撑开。该操作过程只有很小的切口和对组织最小的伤害。当血管内设备被膨胀时,该部分血管会比其他位置稍微粗大,从而保证了支架系统在固定的位置不会移位和脱落。该系统的支架既可以单纯的作为神经刺激器的载体起支撑作用,又可起到打开目标堵塞血管或使目标血管保持通畅的作用。所有之前的植入式支架的设计花型均可被借用,S形交叉样或弯曲重复样的杆。
其中微型血管调控刺激器由神经刺激器件、体外程控系统及射频能量收集器组成。由外界射频(RF)能量驱动工作的设计具有尺寸小、质量轻的优点(刺激器重量为4mg,在典型的神经刺激条件下的平均功耗为20μW)。神经刺激器和射频收集器采用激光焊接的方式固定在医用钛合金外壳内。外壳留有2个电极孔,正负电极柱伸出电极孔,其中负极与支架梁接触,正极裸露在血液中。电极与外壳的空隙通过陶瓷封密封,以防止血液等体液进入刺激器内造成短路。本发明的血管内神经刺激器支架系统及微型刺激器示意图如图1。
神经刺激器和体外控制器通过蓝牙进行通讯,对靶点神经提供脉冲幅度(1 -30mA)、脉冲占空比(50-1000us)、脉冲频率(10-1000Hz)和关断时间可调的电脉冲刺激。该刺激器通过射频能量收集器捕获外界环境的射频能量将其转换为直流电压从而给体内神经刺激器供电。外界收集到的微弱的射频能量首先需要通过倍压整流电路以逐级放大累加的方式将信号放大并转换成电能,再通过升压电路将电能提升,达到驱动体内神经刺激器运行的额定电压值,之后经过稳压电路,输出稳定的直流电压,然后通过储能电路,将一部分电能储存,做备用电压源,最后将电能输送到体内神经刺激器驱动其工作。
神经刺激器的外壳或周圈可埋入显影剂或金色的显影圈,从而在带着神经刺激器的支架系统进入患者血管内时可以清楚地看到刺激器的位置。
关键地,该系统的支架结构可在治疗2-3年后被降解,可采用Zn-Cu合金材料,众所周知,铜和锌均为人体最为基本的必要微量元素,降解后的支架对人体不会造成伤害且可以补充人类所需的微量元素。支架的表面可以进行药物涂层,例如紫杉醇或者地塞米松磷酸钠,起到消炎或抑制细胞增生等作用。根据前期有研究表明将锌合金支架植入植入猪的冠状动脉,研究表明9个月开始降解,12个月降解70%,随着支架的降解,微型神经刺激器随着支架的降解产物从内膜层转移到外弹力层,即使支架完全降解后,嵌入在心血管内壁的神经刺激器会由于长期存在捏紧血管壁而被组织包埋,仍然固定在血管内,还可通过体外控制调节血管的功能,本发明的支架降解及微型刺激器移动过程如图2所示。
应当强调的是本发明的上述实施例,特别是任何优选实施例,仅仅是所为实施的示例,仅仅是为清楚地理解本发明的原理而列举。在本发明的上述实施例进行各种改变和变更,均属于本发明的保护范围之内。
Claims (4)
1.一种具有调节血管功能的锌合金支架系统,包括:可植入血管内的可降解锌合金支架、可嵌入并固定到支架上的微型血管调控刺激器及体外控制器;
所述的锌合金支架材料采用Zn-Cu合金材料;
所述的植入式支架可在体内完全降解;
所述的微型血管调控刺激器由神经刺激器件、体外程控系统及射频能量收集器组成;
所述的神经刺激器外壳留有2个电极孔,正负电极柱伸出电极孔,其中负极与支架梁接触,正极裸露在血液中;
所述的电极与外壳的空隙通过陶瓷封密封;
所述的刺激器收集外界的射频能量通过控制电路输出恒定电流;
所述的神经刺激器的外壳或周圈可埋入显影剂或金色的显影圈。
2.如权利要求书1所述的系统,其特征在于:所述的神经刺激器和射频收集器采用激光焊接的方式固定在医用钛合金外壳内。
3.如权利要求书1所述的系统,其特征在于:所述的神经刺激器与所述的体外控制器通过蓝牙进行通讯。
4.如权利要求书1所述的系统,其特征在于:所述的支架系统是无线的。
