3. The remote health monitoring and management system of claim 1, wherein: the data transmission intermediate equipment comprises a wireless gateway, a smart phone, a computer and a set top box.
4. The remote health monitoring and management system of claim 1, wherein: the remote monitoring center is a monitoring PC or a monitoring mobile phone.
5. The remote health monitoring and management system of claim 1, wherein: the system server platform is a network server unit.
6. A remote health monitoring management method, comprising:
collecting and uploading health monitoring data;
analyzing and processing health monitoring data;
and analyzing and processing result feedback of health monitoring data, health early warning and health management.

The remote health monitoring and management method of claim 6, wherein: the health monitoring data acquisition comprises the acquisition of blood glucose concentration, blood pressure, body temperature, sleep data, blood oxygen concentration, weight, pulse and electrocardiogram data, the health monitoring data is uploaded to be connected to data transmission intermediate equipment through Bluetooth wireless transmission or USB, and the health monitoring data is uploaded to the Internet through the data transmission intermediate equipment.

The remote health monitoring and management method of claim 8, wherein: the disease condition evaluation rule is used for evaluating the disease condition by combining the type of the disease condition parameter, the weight of the disease condition parameter and the disease condition grade evaluation threshold of the disease condition parameter, the grade corresponding to the disease condition grade evaluation threshold comprises an initial stage, a development stage, a middle and later stage, and the type of the disease condition parameter comprises one or more of the combination of blood sugar concentration, blood pressure, body temperature, sleep data, blood oxygen concentration, weight, pulse and electrocardiogram data.
10. The remote health monitoring and management method of claim 6, wherein: the health early warning is to perform health early warning when one or more collected health monitoring data exceed an illness state grade evaluation threshold value.

The remote health monitoring and management method of claim 6, wherein: the health management comprises health record establishment, health positioning and tracking, self-care ability assessment, diagnosis and treatment appointment reminding, health evaluation assessment, health knowledge education, health information consultation, mental health guidance, rehabilitation physical therapy suggestion, nutrition catering suggestion, health care suggestion and exercise suggestion.
