Indications for bone perforation include mainly: various blood system diseases (such as anemia, thrombocytopenia, leukemia, multiple myeloma, myelodysplastic syndrome, myelofibrosis, malignant histiocytosis and metastatic tumor), some parasitic diseases (such as malaria and kala-azar), enlargement of liver, spleen and lymph nodes and the like.
The position and parts of the osteopathy usually selected include sternum and anterior superior iliac spine, and the position of the osteopathy is required to be horizontal; lateral position of spinous process and posterior superior iliac spine; the posterior superior iliac spine can also be selected to be in a prone position; the small shin baby can be in a supine position, the elderly can be in a sitting position, the shank is bent, the sole and the hip are in a horizontal line, and the popliteal fossa forms an obtuse angle.
The bone puncture operation process comprises the following steps: 1. the operator teaches the purpose and necessity of the puncture to the patient and family members, and then executes the puncture after signing consent. 2. The patient's bed number, name, hospital number are checked again, and the contraindication of bone puncture is eliminated. 3. An operator washes hands, stands on the left side of the patient, assists the patient to put a corresponding body position according to the illness state of the patient, fully exposes the puncture part, and gives a mark for positioning. 4. The puncture package is opened and the operator wears sterile gloves. (under strict aseptic conditions, an assistant hands the disposable hole towel and the injector to an operator and places the disposable hole towel and the injector in the puncture bag), checks whether the articles in the puncture bag are complete or not, and checks whether the injector leaks or not. 5. The assistant holds the holding forceps to clamp 0.5% iodophor and 75% alcohol cotton balls into 2 disinfection cups respectively (note that the holding forceps should be held horizontally or downwards, the whole process avoids pollution). An operator holds the forceps with the left hand, holds an iodophor cotton ball, horizontally crosses into the bent hemostatic forceps of the right hand, disinfects local skin for 3 times in a clockwise direction by taking a puncture point as a center (1/3 is pressed on each 1 circle), disinfects the local skin for 3 times by using an alcohol cotton ball after drying the local skin with the diameter of about 15cm., the range is smaller than that of one time, and the last 1 time exceeds the outermost layer of the iodophor. During disinfection, the bent disc is placed on the side of a patient, and the disinfected cotton balls and the bent hemostatic forceps are placed in the disinfection bowl and taken away by an assistant. 6. As mentioned above, when the puncture needle is used for bone puncture, the needle head is easy to bend, and when tumor cells in bone marrow are large, fat is high, and bone dregs exist, the problem of tube blockage is easily caused, which results in collection failure. Therefore, the invention provides a children bone marrow puncture collection needle and a component thereof; the invention will now be further described with reference to the drawings and specific examples.
Referring to fig. 1, a child bone marrow puncture collection needle designed according to the present invention is illustrated, comprising: the puncture needle comprises a syringe 1, a flexible tube 2 and a puncture needle 3, wherein one end of the flexible tube 2 is provided with an enlarged part 4 which is a tubular part with the diameter larger than that of the flexible tube 2 so as to accommodate a filter; the nipple of the syringe 1 is connected to an enlargement portion 4, and the syringe has a size of 10 ml. The existing clinical use generally adopts a 20ml syringe, but the syringe has large force bearing area and reduced suction force, and when the suction force is too small during the bone marrow suction, the bone marrow suction is easy to fail or the bone marrow is easy to agglutinate indirectly. The 10ml syringe has small volume and strong suction force, the generated pressure is safe and applicable in the central venipuncture process, and the bone puncture operation is greatly facilitated.
The flexible pipe 2 is made of PE. The food-grade plastic has the characteristics of strong flexibility and easier operation of the bone marrow obtaining process, and the food grade is safer, so that the detection result of the blood product cannot be influenced by the material factors.
The one end of pjncture needle 3 is pinpoint portion, the other end of flexible pipe 2 is connected with the one end of keeping away from pinpoint portion of pjncture needle 3, pjncture needle 3 is 9 syringe needles, and the crooked phenomenon of syringe needle can not generally take place for it to satisfy older children's bone and wear. And the inner diameter of the 9-gauge needle is larger, which is helpful to prevent the blockage of the tube and also increases the output port of the marrow blood.
The amplifying part 4 is provided with a filter 5, and the filter 5 is a filter screen with the aperture of 25mm-30 mm. By arranging the filter, tumor cells, fat and bone residues in bone marrow can be filtered out, and the passing of the bone marrow is not influenced. The enlargement part is arranged at one end of the flexible tube 2, so that a place is provided for installing a filter for filtering tumor cells, fat and bone residues in bone marrow, and meanwhile, the enlargement part is also used as a connecting part with a syringe nipple, so that the connection between the syringe and the flexible tube is met, and the bone marrow puncture collection needle has the function of filtering bone marrow. In addition, the collecting needle of the present invention can collect a part of bone residues, thereby reducing the bone residues remaining in the bone marrow.
A holding part 6 is fixed on the side wall of the puncture needle 3, a side hole 7 is formed in the position, close to the needle point, on the side wall of the puncture needle 3, and the side hole 7 is located on one side of the inclined plane of the needle point part of the puncture needle.
Further, with continued reference to fig. 1, in other embodiments, the enlargement portion 4 and the nipple of the injector 1 are detachably connected, that is, the nipple is inserted into the enlargement portion 4, so as to facilitate the injector to be removed after the bone marrow collection is completed for further processing of the bone marrow. Due to the integral design of the enlargement part 4 and the flexible tube 2, the enlargement part 4 is also made of flexible material, which is beneficial to forming a sealing connection between the nipple of the syringe 1 and the flexible tube 4 after the nipple is inserted into the enlargement part, and bone marrow leakage during collection is avoided.
Further, in some other embodiments, the amplifying part 4 is detachably connected with the flexible tube 2, specifically, the amplifying part and the flexible tube are detachably connected in a screwing manner, and once impurities filtered by the filter block the tube in the bone marrow obtaining process, the amplifying part can be directly replaced to continue the bone marrow obtaining, so that the interruption of the operation process is avoided.
Further, with continued reference to fig. 1, in some embodiments, the flexible tube 2 is further provided with a scale 8 extending from one end of the flexible tube near the puncture needle to the other end, and the conventional bone marrow collection volume is typically 0.2ml, and therefore, the scale ranges from 0ml to 0.2 ml. In addition, the scale range can be 0ml-1.0ml, and the device can be suitable for being used when more bone marrow needs to be collected under specific conditions; the main function of the scale is to accurately control the bone marrow collection amount. Because the conventional bone marrow is collected in an amount of 0.2-1ml at present for definite diagnosis, and 0.2-0.5ml is generally sufficient for children, the existing scalp needle is long in length and far exceeds 1ml after the bone marrow reaches the estimated measurement of the injector, so that the waste of the bone marrow is caused.
In addition, the length of the flexible pipe 2 is controlled to be between 4.5 cm and 6cm, which is practical, and of course, the length of the flexible pipe can be selected according to actual needs.
Further, with continued reference to fig. 1, in some embodiments, the grip portion 6 is formed by two arcuate rigid members that are symmetrically secured to the needle 3 at an end thereof adjacent the flexible tube. The grip part mainly has the functions of applying acting force to the puncture needle for puncture and pulling out the puncture needle after the collection of bone marrow is completed. Another advantage of using the grip portion 6 to assist the puncture is that the puncture needle itself is not touched, since the portion of the puncture needle needs to enter the skin and bone marrow of the collector, and if there is no grip portion, the puncture needle needs to be held for puncture and extraction, which not only is inconvenient for puncture, but also increases the probability of infection.
Further, with reference to fig. 1, in some embodiments, the number of the side holes 7 is 1, one of the functions of the side holes is to prevent the needle tip from being blocked, so that bone marrow can be collected, for a conventional puncture needle, bone marrow can be sucked only through the hole at the needle tip of the puncture needle, once the blockage occurs, a new puncture needle needs to be used to puncture again after the puncture needle is pulled out, which increases pain of a patient, and the occurrence of the above situation can be prevented to a certain extent by providing the side holes, thereby reducing the probability of blockage. It will be appreciated that other numbers of side holes 7, such as 2, may be provided as required, but not too many, as would otherwise tend to break the needle portion.
Referring to fig. 1 and 2, the invention discloses a children bone marrow puncture assembly, which comprises a children bone marrow puncture collection needle in the above embodiment and a puncture point fixing device, wherein the puncture point fixing device comprises a first restraint part 9, a second restraint part 10 and an adhesion part 11, the first restraint part 9 and the second restraint part 10 are both arc-shaped structures, the two arc-shaped structures form a detachably connected restraint part through a buckle 12, the restraint part is a ring body with an opening 13, the restraint part is an elastic structure, the adhesion part 11 is fixed at both ends of the opening, and the adhesion surface of the adhesion part 11 faces the inner side of the ring body. The puncture point fixing device of the embodiment is very suitable for fixing the puncture points of the spinous process and the posterior superior iliac spine in the lateral decubitus position and the anterior superior iliac spine in the horizontal decubitus position.
Further, referring to fig. 3, in use, the restricting parts are sleeved on the trunk of a patient, the adhering parts 10 are respectively adhered to two sides of the puncture point 14, then the first restricting part and the second restricting part are connected through the buckle 12, and the respective adhering parts are pulled through the restricting parts, so that the skin is pulled to two sides to be tensed, and then the puncture position is selected in the tensed area of the skin, thereby fixing the puncture point. In addition, the left thumb and the index finger are tightened and fix the skin in the puncturing process, the skin is fixed and tightened in four directions by matching with the puncturing point fixing device, particularly, the skin is not easy to shift or slip when the skin punctures periosteum, and the efficiency and the success rate of the operation are greatly facilitated. In addition, through the split design of the restriction component, the tightness degree of the restriction component can be adjusted according to the requirements of the body size of the patient.
The above description is only a preferred embodiment of the present invention and is not intended to limit the present invention, and various modifications and changes may be made by those skilled in the art. Any modification, equivalent replacement, or improvement made within the spirit and principle of the present invention should be included in the protection scope of the present invention.
