Description
Technical Field
The invention relates to an umbrella-shaped recyclable venous thrombus filter, and belongs to the technical field of venous thrombus filters.
Background
At present, the incidence of Deep Vein Thrombosis (DVT) formation is increased day by day, and once the vein thrombosis falls off, the vein thrombosis can enter and block the pulmonary artery along with the blood flow, so that the clinical manifestation of pulmonary artery thrombosis Plug (PE) is caused. The incidence of Deep Vein Thrombosis (DVT) is about 1/1000 per year in the united states. Approximately 1/3 untreated symptomatic patients were combined into pulmonary artery thromboembolism (PE).
The most effective physical therapy for the prevention of pulmonary thromboembolism (PE) is the placement of a thrombus filter in the vein. The thrombus filter is divided into two types: permanent filters and recyclable filters. The permanent filter can provide a lifelong filtering function, has the maximum lumen vein intimal fixation force, and has research verification that the incidence of PE can be effectively reduced after the filter is implanted, but the popularization is limited because the long-term implantation in vivo can cause complications such as inferior vena cava perforation, filter displacement and secondary thrombus, inferior vena cava occlusion, DVT recurrence and the like; the recyclable filter has an advantage that it can be placed in the body as a permanent filter for a long period of time and can also be taken out as a semipermanent filter, but has a disadvantage that the filter is placed in the body as a semipermanent filter for a short period of time, generally only 2 weeks, and after exceeding the taking-out time window, the filter is adhered to the blood vessel wall to be integrated due to the problem of endothelialization caused by the barbs, and thus is not easily taken out. Clinical data at home and abroad show that less than 30% of the recoverable filters which can be successfully taken out remain in the body, so that the advantages of the selectable filters are not obvious. The reason for the low extraction rate is that the filter has a short extraction window and the extraction requires more complicated operating techniques and additional equipment, which increases the treatment costs.

the umbrella-shaped filter screen is a hollow conical-structure memory metal wire filter screen formed by cutting or weaving memory metal through laser;
the tail end of each memory metal wire around the opening of the umbrella-shaped filter screen is provided with a supporting pin, the supporting pins are parallel to the central axis of the umbrella-shaped filter screen, the supporting pins and the outwards-protruded umbrella-shaped filter screen jointly form a folding structure with a V-shaped section, at least one layer of outward-facing positioning barbs are arranged on the supporting pins, and each layer of positioning barbs is formed by uniformly distributing a plurality of hook bodies in each direction;
and a recovery pipe is fixed at the top of the umbrella-shaped filter screen, and a recovery hook groove is formed in the recovery pipe.
As a preferred example, the umbrella-shaped filter screen is made of a nickel-titanium alloy material.
As a preferred example, the support pins are made of 316L stainless steel material or nitinol material.
As a preferred example, the end of the supporting pin is provided with a layer of positioning barbs, and the hook bodies of the positioning barbs are formed by bending the end of the supporting pin outwards.
As a preferable example, a layer of positioning barb is arranged at the joint of the supporting pin and the umbrella-shaped filter screen, and the hook body of the positioning barb is welded on the outer side of the supporting pin.
The invention has the beneficial effects that:
(1) the filter adopts an open-loop design with an opening at one end, a plurality of supporting pins are uniformly distributed along the circumferential direction, the supporting pins play a supporting role in the circumferential direction to prevent the blood vessel from deforming, and the open-loop design (the open structure is adopted at the far end) ensures that the filter can not be completely wrapped by the endothelial growth after the filter is placed for a long time, thereby prolonging the window period for taking out the filter;
(2) the near end of the filter adopts an umbrella-shaped filter screen structure, and the umbrella-shaped filter screen is conical, so that the secondary blockage of the blood vessel caused by the fact that endothelium is attached to the near end of the filter can be prevented;
(3) the support pins are provided with at least one layer of outward positioning barbs, so that the implanted filter can be prevented from shifting, the hook bodies of the positioning barbs are formed by bending the tail ends of the support pins outwards, the integrity is better, the manufacturing cost is saved, the other layer of positioning barbs are welded at the connecting parts of the support pins and the umbrella-shaped filter screen, and the filter is positioned more firmly by the double layers of positioning barbs;
(4) the filter is well jointed with the blood vessel, is stable in support, is easy to take out after long-term implantation, can be used as a permanent filter for long-term implantation, and can also be used as a recyclable filter for short-term use.

the umbrella-shaped filter screen 1 is a hollow conical-structure memory metal wire filter screen formed by laser cutting or weaving memory metal;
the tail end of each memory metal wire around the opening of the umbrella-shaped filter screen 1 is provided with a supporting pin 3, the supporting pins 3 are parallel to the central axis of the umbrella-shaped filter screen 1, the supporting pins 3 and the outwards-protruded umbrella-shaped filter screen 1 jointly form a folding structure with a V-shaped section, at least one layer of outward-facing positioning barbs 4 are arranged on the supporting pins 3, and each layer of positioning barbs 4 is formed by uniformly distributing a plurality of hooks in each direction;
a recovery pipe 2 is fixed on the top of the umbrella-shaped filter screen 1, and a recovery hook groove 21 is arranged on the recovery pipe 2.
The umbrella-shaped filter screen 1 is made of nickel-titanium alloy material.
The support pins 3 are made of 316L stainless steel material or nitinol material.
Example 1
As shown in fig. 1, only one filter layer with positioning barbs 4 is provided. The tail end of the supporting pin 3 is provided with a layer of positioning barb 4, and the hook body of the positioning barb 4 is formed by bending the tail end of the supporting pin 3 outwards. The periphery of the opening of the umbrella-shaped filter screen 1 is provided with 6 memory metal wires and 6 supporting pins 3. Usually 6-8 memory wires and corresponding support stitches 3 are provided at the opening. The umbrella-shaped filter screen 1 and the supporting pins 3 are formed into an integral structure by laser cutting of memory metal or are formed into an integral structure by adopting a welding mode.
Example 2
As shown in fig. 2, a filter is provided with a double layer of positioning barbs 4. Besides the layer of positioning barbs 4 arranged at the tail ends of the supporting pins 3, another layer of positioning barbs 4 is arranged at the connecting positions of the supporting pins 3 and the umbrella-shaped filter screen 1, hooks of the another layer of positioning barbs 4 are welded on the outer sides of the supporting pins 3, and the welded hooks are made of 316L stainless steel materials or nickel-titanium alloy materials.
The working principle is as follows:
folding the filter through a V-shaped fold configuration to form an elongated tubular structure prior to implantation of the filter; the filter is released to a preset position in the blood vessel through the thrombus filter releasing device, after the filter is released, the memory metal wire restores to a natural state, the blood vessel is propped up, and thrombus is filtered through the umbrella-shaped filter screen 1. During the recovery, the recovery hook groove 21 on the recovery pipe 2 is hooked by a special catcher, and the filter is taken out.
