背景技术

Promega的ADCC报道测定是51Cr释放测定的一种商业替代。这种方法依赖于使用以冷冻形式提供的经工程化NK细胞系,其只需解冻即可使用。这些细胞系经工程化以通过基于ADCC的NFAT信号传导途径表达萤光素酶。与经典的51Cr释放测定相比,这种方法没有放射性材料,并且从总体测定可变性中去除了效应细胞相关的可变性。因此,如果不需要对NK细胞响应进行精细表征(释放哪些介导物并且以何种量释放),则这种方法是可用的,只要不需要考虑供体/患者异质性即可。
不管是考虑经典的51Cr测定还是ADCC报道测定,这些策略的主要限制是它们提供单一读出参数,这可能不足够。当目标包括响应的精细表征(例如评估多种mAb的可比性或针对治疗个性化的患者分层)时,这样的单一读出参数无法提供响应的完整表征。
当必须考虑患者的异质性时或者当需要激活状态的更精细表征时,基于流式细胞术的技术是可用的,因为这些方法提供了高灵活性和精细表征能力。可调整测定的内容以适合大多数特定情况的需要,并且所提供的信息的量可以很大,因为多色流式细胞术提供多个参数的同时测量。
目前ADCC研究所使用并商业化的大多数方法均依赖于效应细胞或靶细胞的细胞系的使用。在这样的情况下,它们可表现出标准化和相对稳健的优势,但没有考虑患者异质性。当开发体外方法以尽可能多地与体内遇到的情况相匹配时,它们达到了其极限。如果一些先前的方法(例如51Cr释放测定)也可用PBMC进行,并因此考虑了患者异质性,则它们迅速变得非常费力并且易于变化。
当开发流式细胞术解决方案以满足上述要求时,这些方法是高度“手工艺性的(artisanal)”。它们与可需要多中心分析和纵向随访的临床研究不良地相容。
不管考虑哪种方法,所有当前商业化的技术均依赖于其储存应受良好控制(2至8℃、-20℃或-90℃)的试剂,并且需要多个移液步骤。这些储存和处理要求对即用型(ready-to-use)解决方案施加了重大限制。例如,Promega的ADCC报道测定以解冻使用(thaw-and-use)的形式提供,但如上所述,其不允许朝向患者分层的途径。如今,可获得的解决方案没有可被个性化至可解决患者特异性问题的程度。
本发明解决了这些和其他需求。
发明概述
在一些实施方案中,本发明包括评价针对治疗的反应的方法。所述方法包括以下步骤:将来自患者的血细胞暴露于治疗性抗体,将所暴露的血细胞与多个经标记的报道分子组合以产生经标记的细胞,测量来自经标记的细胞的信号,以及将所测量的信号组合成测定输出。所述测定输出指示患者针对治疗性抗体的反应。
所述方法可包括靶细胞(例如,B细胞)耗竭、白细胞亚群的活化或白细胞的细胞因子产生中的一种或更多种作为测定输出。白细胞亚群可包括NK细胞。多个经标记的报道分子可包括细胞表面标志物和细胞因子。这些报道分子中可以是针对CD137和针对CD69的抗体以及针对TNF-α和针对IL8的抗体。另一些经标记的报道分子可包括针对CD66b和CD11c的抗体或针对CD63、CD3、CD19、CD56、CD54、CD107a、CD107b、CD11b、INF-γ、IL6和CD45的抗体。
在一些实施方案中,治疗性抗体靶向可溶性蛋白,例如细胞因子(例如,TNFα)。在这些实施方案中,检测到对由可溶性蛋白介导的机制的阻断。
在另一些实施方案中,本发明包括评价针对治疗性抗体的响应的方法,其具有将血细胞的第一等分试样暴露于第一治疗性抗体并且将血细胞的第二等分试样暴露于第二治疗性抗体的步骤。每个等分试样可与相应的多个经标记的报道分子组合以产生经标记的细胞的第一等分试样和经标记的细胞的第二等分试样。可在流式细胞仪中从每个等分试样的经标记的细胞中测量信号。评价所测量的信号的步骤确定了针对第一治疗性抗体和针对第二治疗性抗体的细胞响应。
在一些实施方案中,多个经标记的报道分子包括针对CD107a、CD69和CD54的经标记的抗体,并且其中血细胞包括NK细胞、中性粒细胞、嗜碱性粒细胞和单核细胞中的一种或更多种。
在另一些实施方案中,本发明包括表征生物药剂的方法。所述方法包括以下步骤:测定血细胞针对生物药剂的功能性响应,以及比较该血细胞的功能性响应与参照组合物的功能性响应。可通过与参照组合物的功能性响应相比较的功能性响应来表征生物药剂。
测定步骤可包括将血细胞与多个经标记的报道分子组合,并且在流式细胞仪中测量来自血细胞的信号。多个经标记的报道分子可包括针对CD107a、CD69和CD54的经标记的抗体。血细胞可包括NK细胞、单核细胞或多种细胞类型的混合物。在一些实施方案中,多个经标记的报道分子可包括与血细胞的表面标志物结合的经标记的抗体。
在另一些实施方案中,(或另外地),多个经标记的报道分子可包括针对细胞因子的经标记的抗体。

发明详述
本发明提供了基于使用干燥和室温稳定的试剂的方法,所述试剂能够进行全血样品分析。特别地,本发明提供了用于通过流式细胞术研究给定治疗性mAb(或任何其他生物制剂)对给定全血样品的作用或功能的即用型方法。在一个实施方案中,以干燥、室温稳定且即用型的形式提供染色试剂(荧光染料缀合的抗体)和治疗剂(治疗性mAb或任何其他生物制剂)二者。在这种情况下,仅需将全血添加到干燥试剂管/容器中,这使得本发明的测定非常简单且高度标准化,因此在临床研究环境中是高度期望的。无论是用mAb表征或者患者分层进行处理,本发明在不同情况下都是令人感兴趣的。
抗CD20抗体
为了说明本发明的用途和利益,在此考虑了抗CD20治疗性mAb的情况。本领域技术人员将容易理解,本文中公开的方法和组合物可用于研究任何治疗性抗体或药剂的作用或功能。这样的治疗性抗体和药剂的一些非限制性实例包括抗CD38、抗CD19、抗PDL1和抗PD1抗体。这是一个相关的研究案例,因为当今围绕此类mAb有许多活动。抗CD20 mAb目前用于治疗多种病理状况(从B细胞淋巴瘤到自身免疫病),并且已被商业化多于15年。
当前在抗CD20治疗领域中遇到不同的挑战。第一个挑战是,利妥昔单抗(在1997年被批准的第一个抗CD20治疗性mAb)的生物仿制药一旦被开发、测试并且批准,立刻就可商业化。数家公司可尝试开发这种分子的生物仿制药,因为利妥昔单抗的年收入非常大(超过$7B)。因此,能够在数种分子之间进行精细比较的工具的可用性至关重要。第二个挑战与患者分层和治疗个性化有关。市场上有数种靶向CD20抗原的治疗性mAb,而更多的分子(生物仿制药、生物改良药或其他起始者)则处于开发的后期阶段。因此,可选择多于一种抗CD20分子来治疗给定的患者。除了应表现类似的生物仿制药之外,医生必须最佳地确定待用于给定患者的抗CD20治疗性mAb。实际上,根据所选择的mAb,不仅治疗的成功可改变,而且副作用的程度和潜在的严重性也可改变。一种特殊的副作用称为“输注相关反应”(infusionrelated reaction,IRR)。对于一些患者,其在用利妥昔单抗进行治疗时发生,而当使用奥比妥珠单抗时,其频繁得多地(>10%)发生。这些IRR可以是非常激烈的、可能致命的副反应,并且可包括过敏性休克或细胞因子释放综合征(cytokine release syndrome,CRS)。CRS是由受免疫治疗影响的细胞因子从免疫细胞大量、迅速地释放到血液中引起的。细胞因子释放综合征的体征和症状包括发热、恶心、头痛、疹、心跳加快、低血压和呼吸困难。能够预测给定个体发生这样的反应使得可改变治疗以尽可能多地降低风险是非常重要的。
预测成功、失败或副作用的能力对于患者分层和治疗个性化是至关重要的同时,其在生物制药开发的背景下同样非常重要。
已经开发了本发明以潜在地解决前述问题。从原理上讲,其已被开发使得可在体外模拟体内可发生什么,同时仅依赖于非常简单且直接的实验程序。这些目标可用于患者分层和治疗性mAb可比性评估二者。
我们制备了数个细胞术组以解决不同的目标。
表1胞外组
上面的胞外组(表1)使得能够在用(例如)抗CD20治疗性抗体进行刺激时同时监测B细胞耗竭以及NK细胞活化。在本形式中,染色试剂和治疗剂二者均已准备好作为干燥试剂在样品瓶中使用,并且血液只需在于37℃下孵育之前添加以开始测定。
针对细胞组分CD45、CD19、CD56、CD3、CD14和杀伤免疫球蛋白样受体(killerimmunoglobulin-like receptor,KIR)的混合物的经标记的报道分子(通常是经荧光标记的抗体)是设门试剂,而针对CD107a、CD69和CD54的经标记的报道分子有助于表征NK细胞和单核细胞(CD69和CD54)二者的活化状态。上图的第一行示出了用于激发与每种经标记的报道分子相关的荧光染料的激光。调节该实施例的激光和标记以匹配申请人的CytoFLEX细胞仪的可用变化形式的激光和标记。第二行指示与每种经标记的报道分子相关的标记;第三行指示特异性:经标记的报道分子所针对的细胞组分。
图3示出了在有和没有通过奥比妥珠单抗治疗的情况下来自不同供体的细胞之间的变化。使用上述胞外组分析细胞,并在CytoFLEX细胞仪设门中进行测量以分离NK细胞。条示出了在来自不同供体的NK细胞上的CD16表达。注意,NK细胞在与奥比妥珠单抗结合之后内化CD16的能力非常异质。
图4示出了使用上述同一胞外组的来自同一组供体的细胞之间的变化。当与静止的NK细胞相比时,完全活化的NK细胞表现出更高的KIR阳性细胞百分比。这部分地说明了即使在同一组的情况下的不同测定输出的效用。
以下示出了胞外标志物的测试结果。观察到受试的不同供体之间的响应的大的异质性。所研究参数与B细胞耗竭的程度之间没有明显的相关性。三阳性NK细胞(CD69+CD107a+CD54+)的百分比与活化最相关。完全活化的NK细胞比静止的NK细胞的KIR阳性要显著得多。

本发明的胞内组示于上表2中;该表的每一行的描述与胞外组的每一行的描述相同。如在胞外组的情况下一样,已经开发了胞内组以使得能够在用抗CD20治疗性抗体进行刺激时同时监测B细胞耗竭以及NK细胞活化。目前尚不清楚哪种细胞因子和/或哪种细胞类型最能引起IRR。令人感兴趣的是,在此采用(follow)的治疗性mAb/融合蛋白细胞因子确实已经存在。如果发现一种细胞因子在给定患者中更特别地引起潜在IRR,则可将对抗该细胞因子作用的合适药物添加到所考虑患者的治疗性混合物,以使IRR发生的可能性最小化。同样地,本发明可用于确定一种细胞因子是否更特别地在给定患者中引起细胞因子释放综合征(CRS),并且可向患者施用对抗该细胞因子作用的合适药物。
如在胞外组的情况下一样,观察到受试的不同供体之间的响应的大的异质性。当关注单核细胞(n=13)时,已经鉴定出三个主要特征:主要表达IL8的供体(n=10)、主要表达IL8和IL6的供体(n=2)和主要表达IL8和TNFα的供体(n=1)。在测试条件下在单核细胞上未观察到IL10表达。在NK细胞上观察到IFN和TNFα的产生,但所需的布雷菲德菌素(brefeldine)条件不同于单核细胞。
考虑到过敏性休克也被列为潜在的IRR,在下表3中确定嗜碱性粒细胞活化的组(包括CD203c、CD63、CD3、CRTH2和CD45)也可用于表征治疗性mAb刺激之后嗜碱性粒细胞活化状态,特别是当考虑具有鼠氨基酸序列和/或糖基化部分的mAb时。

为了帮助完成模型,每个患者可能需要另外的信息,例如CD16变体,并且将来的实验将有助于我们表征每个上述组的重要性。

抗TNFα抗体
上面证明的方法也可应用于可溶性分子,例如TNFα。由可溶性(sTNF)和膜结合(tmTNF)介导的作用机制示于图7中。本发明的方法可用于例如将患者分层并随时间推移使患者个性化。可用于抗TNFα mAb的组示于表6和7中。

Claims (19)
1.评价针对治疗的反应的方法,所述方法包括:
将来自患者的血细胞暴露于治疗性抗体;
将所暴露的血细胞与多个经标记的报道分子组合以产生经标记的细胞;
测量来自所述经标记的细胞的信号;以及
将所测量的信号组合成测定输出,所述测定输出指示所述患者针对所述治疗性抗体的反应。
2.权利要求1所述的方法,其中所述测定输出包括靶细胞耗竭、白细胞亚群的活化或白细胞的细胞因子产生中的一种或更多种。
3.权利要求2所述的方法,其中所述靶细胞是B细胞。
4.权利要求3所述的方法,其中所述治疗性抗体与选自CD20、CD38、CD19、PDL1和PD1的至少一种抗原结合。
5.权利要求2所述的方法,其中所述白细胞亚群包括NK细胞。
6.权利要求1所述的方法,其中所述治疗性抗体结合细胞因子。
7.权利要求6所述的方法,其中所述细胞因子是TNFα。

将所暴露的血细胞的每个等分试样与多个经标记的报道分子组合以产生经标记的细胞的第一等分试样和经标记的细胞的第二等分试样;
在流式细胞仪中测量来自每个等分试样的所述经标记的细胞的信号;以及
评价所测量的信号以确定针对所述第一治疗性抗体和针对所述第二治疗性抗体的细胞响应。
14.权利要求13所述的方法,其中所述多个经标记的报道分子包括针对CD107a、CD69和CD54的经标记的抗体,并且其中所述血细胞包括NK细胞和单核细胞中的一种或更多种。
15.表征生物药剂的方法,所述方法包括:
测定血细胞针对所述生物药剂的功能性响应;
比较该血细胞的功能性响应与参照组合物的功能性响应,其中所述生物药剂通过与所述参照组合物的功能性响应相比较的功能性响应来表征。
16.权利要求15所述的方法,其中测定步骤包括将所述血细胞与多个经标记的报道分子组合,并且在流式细胞仪中测量来自所述血细胞的信号。
