在缺血中,器官或组织的含血量减少。缺血可以是全身性贫血的局部表现或是局部血液循环障碍的结果。缺血类型包括:1):可以由例如肿瘤、绷带过紧和积液对动脉血管的压迫导致血管腔变狭窄或闭塞所致的压迫性缺血。临床上,由于卧侧血管受到压迫,长期躺卧形成的痔疮或溃疡是缺血造成的组织坏死的适例,这可以引起肌肉损伤。2)因动脉内血栓形成或栓塞的阻塞性缺血可以造成血管阻塞,导致供给至例如肢体或心脏的血液受阻。3)可以由大量血液急剧流入腹腔脏器而造成其它器官和组织缺血造成侧肢性缺血。
患有下肢周边动脉疾病的患者年龄大多超过60岁,且这些患者中约一半患有糖尿病。目前,针对缺血性下肢进行血管新生治疗已有药物上市。例如,AutoloGel system是通过抽取患者自体的高浓度富含血小板的血浆(PRP),并添加促进伤口愈合的生长因子和细胞因子以形成胶状物质制备的伤口敷料。然而,这类治疗方法仅仅用于治疗慢性伤口,但它们不能治疗潜在的缺血。其它治疗方法如血管绕道手术(bypass grafting)、血管扩张术和放置血管支架对于解决血管堵塞是必要的。
对于缺血性下肢目前也有许多研究正积极研发血管新生的疗法,如与血管新生信号传递相关的重组生长因子,如刺激血管新生的VEGF和FGF。已经发现血小板衍生生长因子(PDGF)可在发育组织或成体组织中刺激间叶细胞增生、迁移分化,并且应用于促进患者骨髓释放内皮衍生细胞,达到血管增生功效。也可以由与血管生成信号间接相关的物质刺激人类脐静脉内皮细胞(HUVEC)以刺激血管生成。利用组织纤溶酶原激活物(tPA)和HUVEC给予治疗,以增加内皮前趋细胞从骨髓迁移至血管的数量,以促进血管内皮新生,达到治疗效果。
糖尿病造成的高血糖的生理情况会使血管内皮生长因子的分泌减少,使得在血管病变严重时可能导致截肢。目前大多数的治疗方式与血管生长因子相关,其在临床使用上或医学伦理上都充满着种种困难。如今,仍然没有有效的治疗方法来再生缺血性组织,以及从截肢中拯救肢体。因此,发展出适合大多数病患缺血组织的治疗性组合物是需要解决的重要问题。
发明内容
在一方面,本文描述了用于治疗缺血性组织的药物组合物,包含核心成分和基质成分,其中核心成分包括溶栓药物,并且基质成分包括透明质酸或其衍生物,该药物组合物具有大于10mPa·s的黏度。在一些实施方案中,黏度为10至10000mPa·s。在一些实施方案中,药物组合物含有1mg/ml至100mg/ml的透明质酸。
在一些实施方案中,透明质酸的平均分子量为100kDa至5000kDa。例如,透明质酸可具有700kDa至2000kDa的平均分子量。
在一些实施方案中,药物组合物的黏度在3至10mg/ml的平均分子量为700至2000kDa的透明质酸的黏度范围内。在一些实施方案中,黏度与5mg/ml的平均分子量为1560kDa的透明质酸的黏度相同。透明质酸的平均分子量可以为700至2000kDa,透明质酸的浓度可以为3至10mg/ml。在一些实施方案中,透明质酸的平均分子量为1560kDa,透明质酸的浓度为5mg/ml。
在一些实施方案中,药物组合物中的基质成分还包含胶原蛋白、细胞外基质因子、蛋白质或多糖。
药物组合物中的溶栓药物可选自由噻氯匹定(ticlopidine)、华法林(warfarin)、组织纤溶酶原激活物(tissue plasminogen activator)、阿尼普酶(eminase)、瑞替普酶(retavase)、链激酶(streptase)、组织纤溶酶原激活物、替奈普酶(tenecteplase)、阿伯激酶(abbokinase)、尿激酶(kinlytic)、尿激酶(urokinase)、尿激酶原(prourokinase)、茴香酰化纯化链激酶活化剂复合物(APSAC),纤维蛋白(fibrin)和纤溶酶(plasmin)组成的组。
在一些实施方案中,药物组合物还包含血管生成物(例如,血管内皮生长因子)。
另一方面,本文提供了治疗缺血性组织的方法。该方法包括将本文所述的药物组合物直接给予至受试者的缺血性组织,条件是该药物组合物是非经由静脉内给予。
在一些实施方案中,缺血性组织是溃疡,或在受试者的心脏或肢体中。缺血性组织可以是肌肉。在一些实施方案中,受试者患有糖尿病。
在附图和以下描述中阐述了一个或多个实施例的细节。根据说明书和附图以及权利要求,实施例的其它特征、目的和优点将显而易见。

具体实施方式
于意外中发现含有具有一定黏度的透明质酸和溶栓药物的药物组合物对治疗缺血性组织是有效的。
药物组合物
因此,本文描述了用于治疗缺血性组织的药物组合物。药物组合物包含核心成分和基质成分,核心成分包括溶栓药物和基质成分包括透明质酸或其衍生物。药物组合物的黏度大于10mPa·s。根据选择用于测量黏度的参数(例如,转子和旋转速度),组合物的黏度可以为10至10000mPa·s的范围内(例如,10-100、50-150、100-200、150-250、250-500、500-1000、1000-1500、1500-2000、2000-2500、2500-3000、3000-3500、3500-5000、5000-6000、6000-7000、7000-8000、8000-9000、或9000-10000)。
药物组合物的黏度可以在3至10mg/ml(例如,3、3.5、4、4.5、5、5.5、6、6.5、7、7.5、8、8.5、9、9.5或10mg/ml)的平均分子量为700至2000kDa(例如,700、800、900、1000、1500、1600、1700、1800、1900或2000)的透明质酸的黏度范围内。见下表2至表5。在一些实施方案中,组合物的黏度与5mg/ml的平均分子量为1560kDa的透明质酸的黏度相同。例如,下面描述的数据显示4mg/ml的2000kDa透明质酸、5mg/ml的1,560kDa透明质酸和6.5mg/ml的700kDa透明质酸具有大约相同的黏度。
药物组合物中透明质酸的分子量可以为4kDa至5000kDa的范围内(例如,4至20、20至100、100至500、500至1000、1000至2000、2000至2500、2500至5000、5、10、50、100、200、300、400、500、750、1000、1500、1800、2000、2500、3000、3500、4000、4500或5000kDa)。药物组合物中透明质酸的浓度可以是1至100mg/ml(例如,1、1.5、2、2.5、3、3.5、4、4.5、5、5.5、6、6.5、7、7.5、8、8.5、9、9.5、10、20、30、40、50、60、70、80、90或100mg/ml)。具体地,如果使用平均分子量为700至2000kDa的透明质酸,药物组合物中透明质酸的浓度可以是3至10mg/ml(例如,3、3.5、4、4.5、5、5.5、6、6.5、7、7.5、8、8.5、9、9.5、或10mg/ml)。熟练的从业者将能够选择分子量和浓度的适当组合以获得具有所需黏度的组合物。熟练的从业者将还能够使用本领域已知的方法和市售仪器测定组合物的黏度。
术语“透明质酸”是指天然存在阴离子型的非硫酸化糖胺聚糖,包括N-乙酰葡糖胺和D-葡糖醛酸的重复性二糖单元及其衍生物。天然存在的透明质酸(hyaluronan)(也称为透明质酸(hyaluronic acid)或透明质酸盐(hyaluronate))可以通过常规方法从其天然来源例如链球菌(Streptococci)孢蒴、公鸡鸡冠、软骨、滑膜关节液、脐带、皮肤组织和眼玻璃体中分离。参照文献例如,Guillermo Lago等人,Carbohydrate Polymers 62(4):321-326,2005;和Ichika Amagai等人,Fisheries Science 75(3):805-810,2009。或者,它可以从商业供货商处(例如Genzyme Corporation、Lifecore Biomedical、LLC和Hyaluron ContractManufacturing)购买。天然存在的透明质酸的衍生物包括但不限于透明质酸酯、己二酸二酰肼改性的透明质酸、透明质酸酰胺产物、交联的透明质酸、琥珀酸半酯或其重金属盐透明质酸、透明质酸的部分或全部酯类、硫酸化透明质酸、N-硫酸化透明质酸和胺或二胺改性的透明质酸。它们可以通过化学改性透明质酸的一个或多个官能团(例如,羧酸基团、羟基、还原性端基、N-乙酰基)来获得。羧基可以通过酯化反应或由碳二亚胺和双酰肼介导的反应进行改性。羟基改性包括但不限于硫酸化、酯化、异脲偶联、溴化氰活化和高碘酸盐氧化。还原性端基可以通过还原胺化来改性。它还可以与磷脂、染料(例如荧光团或发色团)或适于制备亲和基质的试剂连接。天然存在的透明质酸的衍生物也可以通过使用交联剂(例如,双环氧化物、二乙烯砜、双碳二亚胺、小的同双功能连接分子、甲醛、环己基异氰化物和赖氨酸乙酯、金属阳离子、酰肼或其混合物)交联,或者通过内部酯化、光交联或表面等离子体处理来获得。为了制备透明质酸溶液,可以将透明质酸溶解在磷酸盐缓冲溶液(例如,pH 7±1,<0.05M)和/或NaCl(例如,<0.9%)中。
基质成分可含有一种或多种其它基质分子,只要组合物的黏度保持在所需范围内。基质分子可包括明胶、胶原蛋白、透明质酸、纤连蛋白、弹性蛋白、细胞粘合素、层粘连蛋白、玻连蛋白、多肽、硫酸乙酰肝素、软骨素、硫酸软骨素、角质素、硫酸角质素、硫酸皮肤素、卡拉胶、肝素、甲壳素、几丁聚糖、海藻酸盐、琼脂糖、琼脂、纤维素、甲基纤维素、羧甲基纤维素、肝糖及其衍生物。此外,基质成分可包括纤维蛋白、纤维蛋白原、凝血酶、聚谷氨酸、合成聚合物(例如,丙烯酸酯、聚乳酸、聚乙醇酸或聚(乳酸-共-羟基乙酸))或交联剂(例如,京尼平、戊二醛、甲醛或环氧化物)。
溶栓药物可以是噻氯匹定、华法林、组织纤溶酶原激活物(t-PA)、阿尼普酶(eminase,anistreplase)、瑞替普酶(retavase,reteplase)、链激酶(streptase,streptokinase,kabikinase)、阿替普酶(activase)、替奈普酶(tenecteplase,TNKase)、尿激酶(abbokinase)、尿激酶(kinlytic,rokinase)、尿激酶(urokinase)、尿激酶原(prourokinase)、茴香酰化纤溶酶原链激酶活化复合物(APSAC)、纤维蛋白、纤溶酶。药物组合物可包括一种或多种溶栓药物。药物组合物可含有与推荐的临床剂量相似或更低剂量的溶栓药物。
药物组合物可进一步包括血管生成物,例如血管内皮生长因子(VEGF)。
治疗方法
可以将有效量的药物组合物给予至患者以治疗缺血性组织。它可以被直接给予(例如,注射或敷用)到缺血性组织(例如肌肉)或附近。凝胶状或黏稠的组合物是非经静脉内给予的。
可依据需要将组合物给予至受试者,例如每天1至5次、每周1至5次、每月1至5次,持续合适的治疗期,例如1至4周、1至12个月、或1至3年。优选的是在缺血或缺血性损伤发生后(例如,在0至48小时或1-7天内)尽快给药。
给予的药物组合物的量应足以提供治疗性化合物(例如,溶栓药物)的有效剂量。有效剂量可以是例如受试者每克体重0.00001至10μg(例如,0.00001至0.001、0.001至0.005、0.005至0.01、0.05至0.1、0.1至0.5、0.5至1、0.00001、0.0001、0.0005、0.001、0.005、0.01、0.02、0.03、0.04、0.05、0.06、0.07、0.08、0.09、0.1、0.2、0.3、0.4、0.5、0.6、0.7、0.8、0.9、1、2、3、4、5、6、7、8、9或10μg),取决于溶栓药物的功效。
“治疗”是指向患有疾病或有患病风险的受试者给予药物组合物,目的是治愈、缓解、减轻、补救、推迟疾病的发作,预防或改善疾病的疾病、疾病症状、继发于疾病的疾病状态、或对疾病的倾向。“有效量”是指能够在治疗的受试者中产生医学上期望的结果的组合物的量。治疗方法可以单独进行或与其它药物或疗法联合进行。待治疗的受试者可以是人或实验动物或家畜动物。
以下具体实施例应被解释为仅是说明性的,并且不以任何方式限制本公开的其余部分。无需进一步详细说明,相信本领域技术人员基于本文的描述可以最充分地利用本公开信息。本文引用的所有出版物均通过引用整体并入本文。
实施例1:糖尿病下肢缺血小鼠模型
C57BL/6品系雄鼠来源为成功大学(NCKU)的实验动物中心或乐斯科生物科技公司(BioLASCO Taiwan Co.,Ltd.)。小鼠饲养于成功大学生物科技研究所的动物房至少一周,以便它们适应环境后才进行实验。所有进行的实验由NCKU的实验动物管理委员会(IACUC)预先批准。
建立这种实验动物模型用于研究下肢缺血的治疗方法。将6月龄或更大的小鼠以50mg/kg体重的链脲佐菌素(STZ)溶液处理以诱导I型糖尿病,以便展示高年龄与不易再生的受损糖尿病组织的特性。由于小鼠的低血糖水平会干扰结果,因此将血糖水平在400mg/dl至550mg/dl范围内的小鼠用于实验,并可能向小鼠施用微量胰岛素以避免威胁生命的高血糖水平。为了避免自我再生新血管形成的可能性,阻断小鼠下肢的股动脉及其外周血管。该模式使血管再生的可能性最小化,能够更加确切评价测试药物的血管新生能力。
为诱导下肢缺血,将剃毛处理的糖尿病小鼠置于气体麻醉箱中,通气使每分钟每公升气体含有1~3%的异氟烷。待小鼠昏迷后,将其移至手术台上并维持气体麻醉。在利用透气胶带将小鼠四肢固定后,以37℃加热垫维持小鼠体温恒定。对小鼠的下腹部与肢体进行消毒后,将肢体的皮肤从左脚踝处的小开口切割至大腿。以手术缝线将小鼠小腿肌肉背侧的两个侧向血管的上下游都打结,并移除血管以阻断背侧血管血流。接着阻断腹侧股动脉分支与主要血管。利用手术缝线将踝关节附近的动脉末端及其周围血管打结,以确保股动脉和外周血流完全被阻塞。
截断血管后,将待测试的用于测试缺血疗效的待测药物组合物直接施用于组织或注射于腓肠肌的8个位点处,并对开创伤口处进行缝合。同时向小鼠皮下注射1mg/kg体重的酮咯酸止痛剂及盐酸利多卡因局部麻醉剂并且还给予1ml生理食盐水,减缓伤痛与补充水分。必要时则给予葡萄糖液以维持体力。
在术后第0、1、2、3、4、5、6、7、14、21和28天,使用表1中所示的评分系统和激光多普勒血流仪(laser Doppler flowmetry)分别评估小鼠下肢的表现外观和血流量。ROI计算为术后左下肢的血流信号与未治疗的右下肢的血流信号的比值,并且以手术前采集的血流信号为基准,将该比值以百分比标准化。

实施例2:透明质酸黏度测试
制备含有不同浓度的分子量不同的透明质酸的组合物。
根据手册采用DV2TRV黏度计(DV2TRV Viscometer)(Brookfield,USA)检测药物组合物的黏度。依照黏度选取适当的转子(CPE40或CPE52)。在测量前,对机器进行校正,并且设定为在室温25℃以20rpm运转1分钟。以黏度吸管将500μl的每个样品转移至样品盘上,按下运转键开始检测样品黏度。测定5mg/ml的分子量分别为1560kDa、700kDa和2,000kDa的透明质酸的黏度,结果如表2所示。将5mg/ml的平均分子量为1,560kDa的透明质酸的粘度作为参考,并且测量了各种浓度的平均浓度为700kDa和2,000kDa的透明质酸的粘度,如表3和表4所示。然后计算黏度接近于参考黏度的平均分子量为700kDa和2000kDa透明质酸的浓度,并分别调节至6.5和4mg/ml。
如表5所示,值得注意的是,当测量参数改变时,具有相同浓度和分子量范围的透明质酸的黏度发生变化。

实施例3:含有血管内皮生长因子(VEGF)的药物组合物
向小鼠给予含有5mg/ml的平均分子量为1,560kDa的透明质酸和VEGF的组合物(DIV),并且如上文实施例1所述来评价组合物对下肢和血流量的影响。将术后未以组合物治疗的糖尿病小鼠用作对照。VEGF药物在文献中已经描述具有血管生成作用。根据体重将对于人体VEGF最大和最小有效剂量转换为小鼠剂量。
外观评分显示在图1。给予100μl DIV相当于向小鼠施用3.125ng/g体重的VEGF。与对照组相比,给小鼠给予3.125ng/g的VEGF(DIV2)导致更高的外观评分。在VEGF的剂量降至0.3ng/g(DIV1)时,所观察到的外观评分低于对照组。在VEGF的剂量增加至15ng/g(DIV3)时,所观察到的下肢缺血且坏疽恶化。
血流测量结果显示在图2中。与对照组相比,在术后第14天及以后,仅3.125ng/g的VEGF显著增加血流量。结果显示VEGF仅在0.3ng/g和15ng/g之间的特定剂量下有效。
实施例4:含有噻氯匹定的药物组合物
向小鼠给予含有5mg/ml的分子量在1000至1800kDa范围的透明质酸和噻氯匹定的组合物(DIT),并如上文实施例1中所述来评价组合物对于下肢和血流量的影响。将术后未用组合物治疗的糖尿病小鼠用作对照。根据体重将对于人体的噻氯匹定最大和最小有效剂量转换为小鼠剂量。
术后外观评分如图3所示。给予100μl的DIT相当于向小鼠施用0.7μg/g体重的噻氯匹定(DIT2)。在该剂量下,从第2天至第28天外观评分与对照组的结果显著不同(P<0.05)。当给予0.07μg/g噻氯匹定(DIT1)时,外观评分也与对照组中的显著不同。
然而,当剂量增加至7μg/g体重(DIT3)或110μg/g体重(DIT4)时,在观察期间的外观评分与对照组的外观评分没有显著差异。结果还表明,噻氯匹定在低剂量下能够有效缓解缺血引起的坏疽,但当剂量低于一定阀值时,效果降低。
血流测量结果如图4所示。结果显示,在0.07μg/g(DIT1)和0.7μg/g(DIT2)体重的噻氯匹定时,从术后第7天缺血下肢的血流信号与对照组相比明显增加(P<0.05)。然而,在较高剂量(DIT3和DIT4)下,与对照组相比,血流量没有显著差异。
实施例5:含有华法林的药物组合物
向小鼠给予含有5mg/ml的平均分子量为1,560kDa的透明质酸和华法林的组合物(DIW),并如上文实施例1中所述来评价组合物对于下肢和血流量的影响。将术后未用组合物治疗的糖尿病小鼠作为对照。根据体重将对于人体的华法林最大和最小有效剂量转换为小鼠剂量。
术后外观评分如图5所示。给予100μl DIW相当于向小鼠施用70ng/g体重的华法林。在该剂量(DIW2)下,从第2天至第28天外观评分与对照组的结果显著不同(P<0.05)。此外,当剂量增加两倍至140ng/g体重(DIW4)时,从第5天到第28天的外观评分也与对照组的结果显著不同(P<0.001-0.05)。另一方面,如果剂量增加3倍至210ng/g(DIW5),则外观评分与对照组的结果没有显著差异。结果表明,DIW的最佳剂量为70ng/g体重的华法林,在缺血性窘迫的情况下,可以保持下肢的外观完整性并避免组织坏疽。
血流量测量结果如图6所示。结果显示,当华法林剂量为70ng/g体重(DIW2)时,从术后第7天开始下肢血流信号与对照组显著不同。当剂量为35ng/g(DIW1)、105ng/g(DIW3)、140ng/g(DIW4)或210ng/g(DIW5)时,从术后第14天开始,下肢血流信号显著不同于对照组。然而,在210ng/g的华法林下,在术后第28天时与对照组相比没有显著差异。
比较用DIV(实施例3)、DIT(实施例4)和DIW获得的结果,给予70ng/g体重华法林(DIW2)的DIW似乎是最有效的。观察到,在DIW2组中,在术后观察期间仅下肢脚趾的远端略微变黑。相比之下,在对照组中,在术后第3天左下肢有变黑的外观。同样,在术后第7天可以观察到一些组织脱落,并且在术后第14天可以观察到下肢坏疽。因此,通过DIW2给药可以显著减轻由缺血引起的坏疽出现。
另外,通过激光多普勒血流仪检测的血流信号分布显示,DIW2组中的血流信号在术后第14天逐渐增加。相反,在对照组中未观察到血流信号的增加。另外,由于下肢坏疽,激光多普勒成像仪无法检测到对照组下肢的血流量。结果进一步显示,与对照组相比,DIW2组中下肢坏疽出现显著减少。
使用血氧仪进行对照组和DIW2组术后下肢血流变化的进一步分析。如图7所示,对照组和DIW2组的下肢血流量在术后立即开始下降。DIW2组在术后第7天开始恢复血流量,并且在观察期间血流量达到约100-200AU。对照组在28天观察期内没有显示任何血流量恢复,反之血流量呈现轻微下降。
实施例6:功能性评估用含有华法林的药物组合物治疗的小鼠
对DIW2组和对照组的小鼠进行功能性评估。于术后第35天进行评估。
将每只小鼠置于平台上,以固定高度约5cm处拉其尾巴,观察其站立抓地的姿势。观察到DIW2组小鼠其站立姿势仍无法与正常小鼠一样可以抓地。然而,发现DIW组小鼠的步距和步幅距离与对照组相比显著增加(P<0.001)且与正常小鼠可比较。参照图8(A)和(B)。结果显示了由于DIW2组小鼠下肢在术后开始萎缩,因而造成其步距无法如同正常组别,但相对于对照组显著更佳。
此外,将小鼠放在跑道上,根据脚印分析它们的步态。观察到,DIW2组和正常小鼠在5rpm下能够维持于机台上不掉落的时间相当,然而对照组小鼠在显著更短时间后会掉落。参见图8(C)。当增加转速至10rpm时,观察到DIW2组小鼠比正常小鼠在更短的时间掉落,但比对照组小鼠维持显著更长的时间。参见图8(D)。
实施例7:治疗时机效果
如实施例1中所述制备下肢缺血的糖尿病小鼠。用含有华法林和5mg/ml透明质酸的组合物治疗小鼠,所述透明质酸的分子量范围为1000至1800kDa,华法林剂量为70ng/g体重,如上所描述的DIW2组中的小鼠一样,但是在术后的不同时间点。如图9所示,观察到如果在术后延迟治疗,下肢外观评分的减少以取决于延迟长短的方式。尽管如此,即使术后迟至48小时进行治疗,术后第28天的外观评分仍为约9分,表明部分下肢仍然只有到脚趾坏死。如果延迟治疗到72小时,则无法挽救下肢坏死。
实施例8:不同分子量的透明质酸对于治疗效果的影响
研究药物组合物中透明质酸的不同分子量是否影响治疗效果。
制备含有华法林和5mg/ml透明质酸的药物组合物,透明质酸具有不同的平均分子量,即74kDa、357kDa、700kDa、1560kDa、2000kDa和2590kDa。将该组合物以70ng/g体重华法林的剂量给予于患有下肢缺血的糖尿病小鼠,并如实施例1中所述进行评价。
如图10所示,含有平均分子量为1,560kDa的透明质酸的组合物表现出最佳的治疗效果。随着平均分子量增加到2,000kDa,外观评分虽然优于对照组,但低于1,560kDa透明质酸组。另外,含平均分子量低于1,560kDa的透明质酸组倾向于降低外观评分。术后28天,与对照组相比,357kDa组和74kDa组的外观评分均恶化。
如图11所示,与对照组相比,术后第14天,2000kDa组和1560kDa组的血流信号显著增加。
实施例9:黏度对于治疗效果的影响
研究黏度是否影响药物组合物的治疗效果。
制备各自含有4mg/ml的平均分子量2000kDa的透明质酸、5mg/ml的平均分子量1560kDa的透明质酸或6.5mg/ml的平均分子量700kDa的透明质酸的组合物。选择透明质酸浓度以便使三者具有相似的黏度。参见上面的表2、3和4。每种都与华法林混合以产生凝胶状组合物。将该组合物以70ng/g体重华法林的剂量给予至患有下肢缺血的糖尿病小鼠,并如实施例1中所述进行评价。
如图12所示,在观察期间,用含有不同分子量的透明质酸的组合物治疗的小鼠中下肢外观评分没有较大差异。因此,透明质酸的黏度似乎比其分子量更重要。如图10和11所示,含5mg/ml的平均分子量1560kDa的透明质酸组和含5mg/ml的平均分子量2000kDa的透明质酸组比含相同浓度下更高或更低分子量的透明质酸组显著地更有效。这些结果还表明一定范围的黏度是最佳的。
其它实施例
本说明书中公开的所有特征可以任何组合进行组合。本说明书中公开的每个特征可以由用于相同,等同或类似目的的替代特征代替。因此,除非另有明确说明,否则所公开的每个特征仅是一系列等效或类似特征的示例。
根据以上描述,本领域技术人员可以容易地确定所描述的实施例的基本特征,并且在不脱离其精神和范围的情况下,可以对实施例进行各种改变和修改以使其适应各种用途和条件。因此,其它实施例也在权利要求内。
Claims (32)
1.一种用于治疗缺血性组织的药物组合物,包含核心成分和基质成分,
其中所述核心成分包括溶栓药物和所述基质成分包括透明质酸或其衍生物,
所述药物组合物具有大于10mPa·s的黏度。
2.根据权利要求1所述的药物组合物,其中所述黏度为10至10000mPa·s。
3.根据权利要求2所述的药物组合物,其中所述透明质酸的平均分子量为100kDa至5000kDa。
4.根据权利要求3所述的药物组合物,其中所述透明质酸的平均分子量为700kDa至2000kDa。
5.根据权利要求3所述的药物组合物,其中所述药物组合物含有1mg/ml至100mg/ml的所述透明质酸。
6.根据权利要求1所述的药物组合物,其中所述黏度在3至10mg/ml的平均分子量为700至2000kDa的所述透明质酸的黏度范围内。
7.根据权利要求1所述的药物组合物,其中所述黏度与5mg/ml的平均分子量为1560kDa的所述透明质酸的黏度相同。
8.根据权利要求6所述的药物组合物,其中所述透明质酸的平均分子量为700至2000kDa且所述透明质酸的浓度为3至10mg/ml。
9.根据权利要求7所述的药物组合物,其中所述透明质酸的平均分子量为1560kDa且所述透明质酸的浓度为5mg/ml。
10.根据权利要求1所述的药物组合物,其中所述基质成分进一步包括胶原蛋白、细胞外基质因子、蛋白质或多糖。
11.根据权利要求1-10中任一项所述的药物组合物,其中所述溶栓药物是选自由噻氯匹定、华法林、组织纤溶酶原激活物、阿尼普酶、瑞替普酶、链激酶、组织纤溶酶原激活物、替奈普酶、阿伯激酶、尿激酶(kinlytic)、尿激酶(urokinase)、尿激酶原、茴香酰化纯化链激酶活化复合物(APSAC)、纤维蛋白和纤溶酶所组成的组。
12.根据权利要求1-11中任一项所述的药物组合物,进一步包含血管生成物。
13.根据权利要求12所述的药物组合物,其中所述血管生成物是血管内皮生长因子(VEGF)。
14.根据权利要求1-13中任一项所述的药物组合物,其中所述药物组合物被直接给予至缺血性组织,但非经由静脉内给予。
15.根据权利要求14所述的药物组合物,其中所述缺血性组织是溃疡,或在受试者的心脏或肢体中。
16.根据权利要求15所述的药物组合物,其中所述缺血性组织是肌肉。
17.一种治疗受试者中缺血性组织的方法,包括:将药物组合物直接给予至缺血性组织,条件是所述药物组合物非经由静脉内给予;
其中所述药物组合物含有核心成分和基质成分,所述核心成分包括溶栓药物且所述基质成分包括透明质酸或其衍生物,和其中所述药物组合物具有大于10mPa·s的黏度。
18.根据权利要求17所述的方法,其中所述缺血性组织是溃疡,或在所述受试者的心脏或肢体中。
19.根据权利要求18所述的方法,其中所述缺血性组织是肌肉。
20.根据权利要求17所述的方法,其中所述受试者患有糖尿病。
21.根据权利要求17-20中任一项所述的方法,其中所述黏度为10至10000mPa·s。
22.根据权利要求21所述的方法,其中所述透明质酸的平均分子量为100kDa至5000kDa。
23.根据权利要求22所述的方法,其中所述透明质酸的平均分子量为700kDa至2000kDa。
24.根据权利要求21所述的方法,其中所述药物组合物含有1mg/ml至100mg/ml的所述透明质酸。
25.根据权利要求21所述的方法,其中所述黏度在3至10mg/ml的平均分子量为700至2000kDa的所述透明质酸的黏度范围内。
26.根据权利要求21所述的方法,其中所述黏度与5mg/ml的平均分子量为1560kDa的所述透明质酸的黏度相同。
27.根据权利要求25所述的方法,其中所述透明质酸的平均分子量为700至2000kDa且所述透明质酸的浓度为3至10mg/ml。
28.根据权利要求26所述的方法,其中所述透明质酸的平均分子量是1560kDa且所述透明质酸的浓度是5mg/ml。
29.根据权利要求17-28中任一项所述的方法,其中所述基质成分进一步包含胶原蛋白、细胞外基质因子、蛋白质或多糖。
30.根据权利要求17-28中任一项所述的方法,其中所述溶栓药物是选自由噻氯匹定、华法林、组织纤溶酶原激活物、阿尼普酶、瑞替普酶、链激酶、组织纤溶酶原激活物、替奈普酶、阿伯激酶、尿激酶(kinlytic)、尿激酶(urokinase)、尿激酶原、茴香酰化纯化的链激酶活化复合物(APSAC)、纤维蛋白和纤溶酶所组成的组。
31.根据权利要求17-28中任一项所述的方法,进一步包含血管生成物。
32.根据权利要求31所述的方法,其中所述血管生成物是血管内皮生长因子(VEGF)。
