Background
Pathological diagnosis is a morphological observation method, and generally, a diagnostician observes morphological structure changes of cells and tissues by observing pathological sections made of pathological tissue specimens to determine pathological nature, so that the pathological diagnosis is made, has certain subjectivity and has higher requirements on the diagnostician. In some cases, the accuracy of pathological diagnosis is affected because the diagnosis personnel may have insufficient ability and experience to perform diagnosis missing or over-diagnosis. In order to improve the accuracy of pathological diagnosis and reduce the number of wrong pathological diagnosis, quality control personnel are required to review and verify the diagnosis report of some specific lesions (such as malignant tumors, precancerous lesions, and the like).
The existing quality control mode of the diagnosis report adopts manual screening and then quality control judgment on the diagnosis report on a related system, and the mode has low efficiency and low accuracy. For example, some relatively simple cases may also cause diagnostic defects due to personnel ability problems, and if only a specific lesion is used for screening, some cases may be missed, resulting in incomplete screening and low accuracy of quality control diagnostic reports.

Detailed Description
The technical solutions in the embodiments of the present invention will be clearly and completely described below with reference to the drawings in the embodiments of the present invention, and it is obvious that the described embodiments are only a part of the embodiments of the present invention, and not all of the embodiments. All other embodiments, which can be derived by a person skilled in the art from the embodiments given herein without making any creative effort, shall fall within the protection scope of the present invention.
In one embodiment, a quality control method for diagnostic reports is provided, which can automatically screen diagnostic reports requiring quality control for further quality control, and obtain corresponding quality control results. The method can be applied to a pathological diagnosis report management system in places such as hospitals.
In one embodiment, as shown in FIG. 1, a method of quality control of diagnostic reports is provided. The method specifically comprises the following steps S102-S108:
step S102, at least one diagnostic report to be determined is obtained.
Specifically, the diagnostic report to be determined is a diagnostic report in the database to be retrieved, where the diagnostic report may be a pathological diagnosis report or a physical examination report, and the diagnostic report may record patient information, diagnostician information, pathological tissue or specimen information of diagnosis, scanned image information, and/or diagnosis opinions, as shown in fig. 2 as an example. The diagnosis opinion is a diagnosis result in a diagnosis report, and may include disease analysis, pathological image analysis, etc., and may be a diagnosis opinion part of the diagnosis report shown in fig. 2, which is a core part of the diagnosis report and is a basis for determining whether there is an error in the diagnosis report.
And step S104, acquiring at least one diagnostic report to be determined from the at least one diagnostic report to be determined according to preset quality control conditions to serve as a target diagnostic report.
Specifically, the quality control condition is a retrieval condition for retrieving a diagnostic report of which quality is required from at least one diagnostic report to be determined. For example, the quality control condition specifies that a diagnostic report having the feature a requires quality control, and a diagnostic report having the feature B does not require quality control. The characteristics of the report needing quality control can be determined by setting the quality control conditions, and the diagnosis report obtained by the quality control conditions is a target diagnosis report needing further quality control.
The target diagnosis report is obtained by screening the quality control conditions, namely the diagnosis report which needs to be subjected to quality control, and an experienced expert is needed to further recheck whether the diagnosis opinions in the target diagnosis report are wrong or not, judge whether the corresponding diagnosis opinions have problems or not, and whether the diagnosis opinions are agreed.
In one embodiment, the quality control conditions comprise positive keywords, negative spot check proportion and/or a preset list; the step of obtaining at least one diagnostic report to be determined as a target diagnostic report from the at least one diagnostic report to be determined according to a preset quality control condition further includes: and searching for a diagnosis report meeting at least one of the quality control conditions in at least one to-be-determined diagnosis report, and taking the searched diagnosis report as a target diagnosis report.
The positive keywords refer to words and phrases in the diagnosis report showing pathological conditions, such as cancer, sarcoma, etc., within the quality control range. If the diagnosis opinions contain the positive keywords, the diagnosis report determines a serious disease, and the corresponding diagnosis report needs further quality control, so that the error diagnosis of the serious disease is avoided.
The negative spot check proportion is the proportion of the diagnosis report of the random spot check to the total number of the diagnosis reports, and can be 10%, for example, or the diagnosis report of a certain proportion of the random spot check. If the diagnostic opinions do not contain the positive keywords, the diagnostic report is determined that no serious diseases appear, however, the diseases can not be diagnosed due to insufficient experience or misoperation, so that the corresponding diagnostic report needs to be further controlled, and the accurate diagnosis of serious diseases is avoided.
The preset list is a list of doctors who issue diagnosis reports, some doctors have the problem of special diagnosis and may make a mistake in the diagnosis of some serious diseases, which results in the wrong diagnosis of the serious diseases, and the diagnosis reports corresponding to the doctors in the preset list need to be further quality-controlled, so that the wrong diagnosis of the serious diseases is avoided.
In the diagnostic report to be determined, the diagnostic report corresponding to at least one quality control condition is searched as a target diagnostic report, that is, the diagnostic report can be retrieved when meeting one quality control condition, thereby ensuring the comprehensiveness of the retrieval of the diagnostic report.
In other embodiments, the number of the quality control conditions that need to be satisfied may be set arbitrarily according to the application scenario.
Generally, if a word such as "cancer not found" appears in the diagnostic opinions of the diagnostic reports, it indicates that the diagnostic reports do not confirm a serious disease, and the diagnostic reports do not need to be controlled. Therefore, in this embodiment, it is also necessary to search for a report that false positive data may occur using a positive keyword (for example, cancer) and to extract a report corresponding to the false positive data from a report that requires quality control. That is, in the quality control conditions, certain screening conditions are also required to be set to exclude these diagnostic reports.
In one embodiment, the quality control conditions further comprise at least one screening condition, the screening condition comprising: not including the preset screening keywords; after the step of searching for a diagnostic report satisfying at least one of the quality control conditions, the method further comprises: and deleting the diagnosis report corresponding to the screening keyword from the diagnosis reports corresponding to the positive keywords, and taking the deleted diagnosis report as a target diagnosis report.
Specifically, the screening condition is reversed, and the diagnostic report including the preset screening keyword needs to be removed, that is, the diagnostic report including the screening keyword in the diagnostic report corresponding to the positive keyword is removed.
In one embodiment, the screening condition is a preset screening keyword, and is a quality control condition mainly used for eliminating a false positive diagnosis report retrieved by a positive keyword, for example, the screening keyword may be a word without cancer, tumor or malignancy. And deleting the diagnosis report corresponding to the screening keyword from the diagnosis reports corresponding to the positive keywords in the quality control conditions, and taking the diagnosis report left after deleting the diagnosis report corresponding to the screening keyword as a target diagnosis report, so that the screening accuracy of the diagnosis report is improved.
The positive keywords and the negative spot check proportion are used for comprehensively searching and spot checking at least one diagnostic report to be determined. The preset list is for doctors who issue diagnosis reports, and doctors may have problems of specialized diagnosis and may have diagnosis errors in some disease categories. To avoid loss of diagnosis errors, some doctors should review and verify their corresponding diagnosis reports.
In another embodiment, doctors with different qualifications have different accuracies, or hospitals or diseases, and different quality control conditions can be set for different application scenarios, so that on the premise of ensuring accuracy, workload is further reduced, and efficiency is improved.
In one embodiment, the step of obtaining at least one diagnostic report to be determined from the at least one diagnostic report to be determined as a target diagnostic report according to a preset quality control condition further includes: acquiring quality control sub-conditions corresponding to different doctors in the quality control conditions; and searching a diagnosis report meeting the quality control sub-condition corresponding to the doctor as a target diagnosis report aiming at the diagnosis reports corresponding to different doctors in the to-be-determined diagnosis reports.
The quality control sub-condition is a retrieval condition for retrieving a part or all of the determined diagnosis reports of a certain doctor, and the quality control sub-condition can be a positive keyword, a negative spot check proportion and the like. Doctors may have some disease types with diagnosis problems, and therefore, the quality control of the diagnosis report corresponding to the specific disease type of the doctor is required. For inexperienced physicians, it is necessary to have control over the entire diagnostic report for that physician. Therefore, a part of or all diagnosis reports corresponding to a certain doctor are determined first, and then the diagnosis report corresponding to the quality control sub-condition is retrieved from the diagnosis report to be determined. The diagnosis report is searched according to the doctor condition, so that the comprehensiveness and the accuracy of the searched diagnosis report are improved.
Step S106, obtaining the diagnosis data corresponding to the target diagnosis report, wherein the diagnosis data at least comprises diagnosis opinions.
Specifically, the diagnosis data refers to the contents of the disease condition in the diagnosis report, and at least includes the diagnosis opinions. May include textual descriptions of the tissue or specimen, scanned images of the tissue or specimen, as shown in fig. 2. And the system can also comprise storage position information of the tissue or the sample, and the tissue or the sample is obtained according to the position information for rechecking and verification.
In one embodiment, a quality control list can be generated according to the diagnostic data corresponding to the target diagnostic report, and the diagnostic data is represented by the quality control list in a table form, so that the diagnostic data of the target diagnostic report can be displayed more intuitively.
And step S108, respectively acquiring quality control opinions input aiming at the diagnosis data, and generating a quality control result corresponding to the at least one target diagnosis report.
Specifically, the quality control opinion is a diagnostic opinion obtained by rechecking and verifying diagnostic data in a target diagnostic report. The quality control opinions are the result of the review made aiming at the target diagnosis report, and the content of the quality control opinions needs to be specified so as to accurately express the review result of the target diagnosis report. Wherein the quality control opinion comprises at least one of a quality control conclusion, a re-diagnosis result and/or a quality control abnormality analysis.
The quality control result is the final quality control result generated according to the diagnosis opinions. And respectively rechecking and verifying the diagnostic data of each target diagnostic report, giving a quality control opinion, and respectively generating a quality control result corresponding to each target diagnostic report.
The quality control conclusion refers to whether the diagnosis opinions of the target diagnosis report are agreed. The result of the double diagnosis refers to the diagnostic opinion given again to the target diagnostic report. The quality control abnormal analysis refers to the analysis of error reasons made on the result of the repeated diagnosis and the original diagnosis opinions, so that a diagnosis report doctor learns and improves according to the quality control abnormal analysis. The content of the quality control opinions is regulated so as to accurately express the rechecking result of the target diagnosis report and ensure the rationality of the content of the quality control opinions.
Under the condition that the quality control conclusion is abnormal, the information of the diagnosis report with unqualified quality control can be notified to the doctor corresponding to the target diagnosis report, so that the doctor can be helped to quickly obtain the quality control result and improve the quality control result in time.
In one embodiment, the quality control conclusion is obtained and comprises approval diagnosis opinions or disapproval diagnosis opinions; and under the condition that the quality control conclusion is that the diagnosis opinions are not approved, acquiring a target diagnosis report corresponding to the quality control opinions, and sending the diagnosis opinions to doctors corresponding to the target diagnosis report.
The doctor corresponding to the target diagnosis report is the doctor who issues the target diagnosis report. And under the condition that the quality control conclusion is that the diagnosis opinions are not agreed, the diagnosis opinions can be sent to doctors corresponding to the target diagnosis report through a system notification function of short messages and mails. And under the condition that the quality control conclusion is that the diagnosis opinions are agreed, the doctor corresponding to the target diagnosis report can not be informed, and the doctor corresponding to the target diagnosis report can be informed that the doctor can make a normal return visit. And informing the corresponding pathological doctors of the quality control result to help the pathological doctors to quickly obtain the quality control result and improve the quality control result in time.
The diagnosis opinions and the diagnosis results are made for a certain target diagnosis report, and if the quality control condition is needed to be known, the diagnosis opinions or the diagnosis results can be counted.
In one embodiment, a corresponding quality control report is generated according to a quality control result of the at least one target diagnosis report, and the quality control report includes at least one of the number of diagnosis reports, the number of diagnosis abnormal opinion reports, and/or diagnosis reports for diagnosis abnormal opinion of a preset list.
The quality control report is data for counting all or part of the quality control results, wherein the quality control results in a certain time period can be counted, the quality control results in a periodic time can be counted, all the quality control results can be counted, and the quality control results in a certain time period, a certain period or all the quality control results can be counted for some doctors. The number of diagnosis reports is the number of diagnosis reports searched and retested. The report number of the abnormal diagnosis opinions means the number of target diagnosis reports different in quality control opinions and diagnosis opinions in the target diagnosis reports. The diagnosis report of abnormal diagnosis opinions aiming at the preset list refers to a target diagnosis report of which the diagnosis opinions aiming at a specific doctor are different from the quality control opinions. The data of quality control work can be accurately and quickly counted by generating the quality control report, so that the condition of quality control is reflected.
In one embodiment, if the ratio of the number of abnormal diagnostic reports to the total number of target diagnostic reports is greater than a certain ratio, for example, 80%, the negative spot check ratio in the quality control condition is increased, for example, the negative spot check ratio is increased from 10% to 15%. The quality control conditions are modified so that the number of retrieved diagnostic reports is a reasonable number.
