技术领域
本公开涉及微创手术。更确切地说,本公开涉及用于在微创手术程序期间对脉管进行成像的方法和设备。
背景技术
已经开发了手术技术和器械,这些手术技术和器械允许外科医生执行越来越多的对患者的皮肤和身体组织的切口最小的手术程序。微创手术已经在许多医学专业中被广泛接受,经常取代传统的开放手术。不同于需要长切口的开放手术,例如内窥镜检查或腹腔镜检查等微创程序是通过一个或多个短切口来执行,对身体的创伤少得多。

在微创手术中,外科医生不能直接看见手术区域,且因此微创技术与对应的开放手术技术相比需要专门技能。虽然微创技术变化很大,但外科医生通常依赖于内窥镜的尖端处的照明相机来查看手术部位,其中监视器显示所述部位的放大版本以供外科医生在手术程序期间用作参考。外科医生随后在监视器上观看程序的同时执行手术。在需要识别和解剖脉管的手术程序期间,外科医生即使借助于照明相机也经常难以看见脉管。因此,需要一种在微创手术期间看见脉管的改进装置。
发明内容
在本公开的一个方面中,提供一种手术成像系统且其包含内窥镜、处理器和显示器。所述内窥镜包含具有远侧末端部分的伸长轴杆、附接到所述远侧末端部分且被配置成捕获组织的图像的成像器、附接到所述轴杆的所述远侧末端部分以用于向组织发射红外光的红外发射器,以及附接到所述轴杆的所述远侧末端部分以用于接收由所述组织反射的红外光的红外接收器。处理器与所述红外发射器和所述红外接收器通信,且被配置成基于从所述红外接收器接收的数据而识别所述组织中的脉管且产生所述脉管的数字图像。显示器被配置成显示由成像器捕获的组织的图像和由处理器产生的脉管的数字图像。脉管的数字图像覆盖在组织的图像上。

在一些方面中,脉管的图像可以与脉管的实际颜色不同的颜色显示。
在其它方面中,脉管的数字图像的颜色可以基于脉管的温度而改变。
在本公开的另一方面中,提供一种在手术程序期间对脉管进行成像的方法且其包含:将内窥镜的远侧末端部分定位成邻近于手术部位的组织,用附接到内窥镜的远侧末端部分的成像器捕获组织的图像,从附接到轴杆的远侧末端部分的红外发射器向组织发射红外光,接收由组织反射到附接到轴杆的远侧末端部分的红外接收器中的红外光,使用来自红外接收器的数据产生组织中的脉管的数字图像,在显示器上显示由成像器捕获的组织的图像,且在显示器上在组织的图像上显示脉管的数字图像。

如下文将详细地描述,提供一种包含内窥镜的手术成像系统,所述内窥镜具有用于捕获手术部位的组织的图像的相机以及用于对安置于组织的表面下方的脉管进行检测和成像的红外光发射器和传感器。内窥镜与处理器通信或包含处理器,所述处理器使用由红外传感器搜集的数据产生脉管(例如,静脉)的数字图像。处理器与显示器通信,所述显示器被配置成显示由相机捕获的组织和脉管的实际图像且将脉管的数字图像叠加于脉管的实际图像上。
本公开利用近红外(near-infrared,NIR)光以在组织内成像或可视化达到各种深度,例如几厘米。静脉含有脱氧血红蛋白,其具有在约760nm处的近红外吸收峰以及在800到950nm范围内的较小、较宽的吸收平线区。在650与900nm之间的近红外区中存在波长窗,其中光子能够穿透组织足够远以照射超过1cm深度的较深结构。本公开的手术成像系统通过使用近似880到890nm的近红外波长用于对组织中的皮下静脉进行成像而利用此现象。

在操作中,微创手术程序可能需要对组织“T”下方的脉管“V”的位置的了解,以避开所述脉管或者因为所述脉管是操作的目标。为了定位和查看脉管“V”,内窥镜100通过端口组合件105以将内窥镜100的远侧末端部分122定位成邻近于组织“T”。内窥镜100的成像器124捕获组织“T”的图像(例如,视频或静态图像),且在显示器130上显示组织“T”的图像,如图3A所示。由于安置于组织下方的大多数脉管是至少部分可见的,因此在大多数情况下,显示器130上的组织的图像还将示出脉管“V”。
在用成像器124捕获组织“T”的图像的同时,内窥镜100的红外发射器126朝向组织“T”发射红外光。由于组织(例如,肌肉、皮肤、脂肪)与脉管之间的红外吸收能力的差异,指向无脉管“V”的组织“T”的大部分红外光朝向内窥镜100反射回,而指向下方安置有脉管“V”的组织“T”的大部分红外光被脉管“V”吸收。被组织“T”反射的红外光由红外接收器128接收,红外接收器128将数据传送到处理器“P”。
处理器“P”使用从红外接收器126接收的数据来定位/识别脉管“V”且产生脉管“V数字”的数字图像。处理器“P”将脉管“V数字”的数字图像中继到显示器130,由此显示器130将脉管“V数字”的数字图像叠加在由成像器124捕获的脉管“V”的实际图像上。现在具有脉管“V”的较好可视化的临床医生可以更有效地在脉管“V”周围导航或取决于正执行的手术程序来处理脉管“V”。

在实施例中,在显示器130上显示的脉管“V数字”的数字图像可以是与脉管“V”的实际颜色不同的颜色。举例来说,脉管“V数字”的数字图像可以黄色、绿色、蓝色或任何合适的颜色显示。
在实施例中,手术成像系统1可以包含用于确定脉管“V”的温度的温度传感器(未示出)。处理器“P”或显示器130可以利用由温度传感器确定的脉管“V”的温度以基于脉管“V”的温度调整脉管“V数字”的数字图像的颜色。举例来说,如果脉管“V”的温度低于已知基线温度(例如,98.6°F)或温度范围(例如,95°F-99°F),那么脉管“V数字”的数字图像可以显示为蓝色,而如果脉管“V”的温度高于已知基线温度或温度范围,那么脉管“V数字”的数字图像可以显示为黄色。
在实施例中,系统1的组件(例如,内窥镜100、处理器“P”和显示器130)可以并入到便携式手持装置中。
应理解,可以对本文中公开的实施例作出各种修改。因此,以上描述不应解释为限制性的,而仅仅是作为各种实施例的例证。所属领域的技术人员将设想在本文所附的权利要求书的范围和精神内的其它修改。
Claims (9)
1.一种手术成像系统,其包括:
内窥镜,其包含:
伸长轴杆,其具有远侧末端部分;
成像器,其附接到所述远侧末端部分且被配置成捕获组织的图像;
红外发射器,其附接到所述轴杆的所述远侧末端部分以用于向组织发射红外光;以及
红外接收器,其附接到所述轴杆的所述远侧末端部分以用于接收由所述组织反射的红外光;
处理器,其与所述红外发射器和所述红外接收器通信,所述处理器被配置成基于从所述红外接收器接收的数据而识别所述组织中的脉管且产生所述脉管的数字图像;以及
显示器,其被配置成显示由所述成像器捕获的所述组织的所述图像和由所述处理器产生的所述脉管的所述数字图像,其中所述脉管的所述数字图像覆盖在所述组织的所述图像上。
2.根据权利要求1所述的手术成像系统,其中由所述成像器捕获的所述组织的所述图像包含所述脉管的图像,所述显示器被配置成将由所述处理器产生的所述脉管的所述数字图像叠加在由所述成像器捕获的所述脉管的所述图像上。
3.根据权利要求1所述的手术成像系统,其中所述脉管的所述图像是以与所述脉管的实际颜色不同的颜色显示。
4.根据权利要求3所述的手术成像系统,其中所述脉管的所述数字图像的所述颜色基于所述脉管的温度而改变。
5.一种在手术程序期间对脉管进行成像的方法,其包括:
将内窥镜的远侧末端部分定位成邻近于手术部位的组织;
用附接到所述内窥镜的所述远侧末端部分的成像器捕获所述组织的图像;
从附接到轴杆的所述远侧末端部分的红外发射器向所述组织发射红外光;以及
接收由所述组织反射到红外接收器中的所述红外光,所述红外接收器附接到所述轴杆的所述远侧末端部分;
使用来自所述红外接收器的数据产生所述组织中的脉管的数字图像;
在显示器上显示由所述成像器捕获的所述组织的所述图像;以及
在所述显示器上在所述组织的所述图像上显示所述脉管的所述数字图像。
6.根据权利要求5所述的方法,还包括用处理器识别所述脉管的位置,其中所述处理器使用来自所述红外接收器的所述数据产生所述脉管的所述数字图像。
7.根据权利要求5所述的方法,其中由所述成像器捕获的所述组织的所述图像包含所述脉管的图像,所述显示器被配置成将产生的所述脉管的所述数字图像叠加在由所述成像器捕获的所述脉管的所述图像上。
8.根据权利要求5所述的方法,还包括在所述显示器上以与所述脉管的实际颜色不同的颜色显示所述脉管的所述数字图像。
9.根据权利要求8所述的方法,其中所述脉管的所述数字图像的所述颜色基于所述脉管的温度而改变。
