It can be understood that the absence of the change condition parameter information means that the change condition parameter has no specific value or is in an effective state, that is, the change condition parameter has no specific value as described above, for example, cognitive impairment/dementia does not exist, mental disease does not exist, … … does not occur, and the determined preliminary level of the ability of the elderly person may be directly determined as the final level of the ability of the elderly person of the target object.

In one embodiment, referring to fig. 5, before determining the care level, the care plan basic information of the target object may be entered. In one embodiment, the following manner may be used in determining the care level matching the final aged person ability level according to the final aged person ability level.
Firstly, according to the final old people ability grade and the matching relation between the parameter values of the grade index grading parameters, the care plan grade matched with the final old people ability grade is determined.
The matching relationship can be determined in various possible ways. For example, in one embodiment, the combined relationship between the final levels of geriatric competency and the parameter values of the primary index rating parameters may be exhausted and a care plan rating may be assigned for each combined relationship. It is understood that one care plan level may correspond to one combination relationship, or may correspond to a plurality of combination relationships of two or more.
In another embodiment, the determining of the care plan level matching the final level of the elderly ability may be determined by setting a care plan determination map for the elderly and combining the care plan determination map for the elderly, and specifically may include:
acquiring an old people care plan judgment diagram, wherein the old people care plan judgment diagram comprises a capability grade item and each score option of each grade index grading parameter;
determining the current ability grade item of the final ability grade of the old people in the old people care plan judgment map according to the final ability grade of the old people;
determining each current score item of each primary index grading parameter in the old people care plan judgment chart according to the parameter value of each primary index grading parameter;
and determining the care plan grade based on the corresponding relation between the current ability grade items and the current score items in the care plan judgment graph of the old people.
The determination diagram of the nursing plan for the elderly in one embodiment is shown in fig. 7, and in fig. 7, different color blocks represent different nursing service plans, which relate to a nursing service plan package with specific nursing items in the package. Specifically, as shown in fig. 7, the care plan for the care of the aged service is divided into the following 8 levels: health preserving, primary care 1, primary care 2, middle care 1, middle care 2, high care 1, high care 2, and high care 3.
In fig. 7, the elderly are ranked as "activities of daily living" 1, "mental state" 0, "sensory perception and communication" 0, "social participation" 1, and the final ranking of the elderly's ability is "mild disability". And judging according to the judgment chart of the care plan of the old people, wherein the recommended care plan grade is 'primary care 1'.
And secondly, determining a care level matched with the care plan level based on the care plan level.
The care plan level of the endowment service determined as described above is divided into 8 levels, and service items included therein are generally provided by an endowment organization. The determined 8 levels and corresponding service items can be configured as standard care service items at each care institution. However, each care-giver may set a care level less than or more than the 8 care plan levels by itself based on the difference of each care-giver, and at this time, it is necessary to intelligently match the care level of the care-giver itself.
In one embodiment, determining a care level matching the care plan level based on the care plan level comprises:
acquiring a current endowment institution identification and a matching relation between a care plan grade and a care grade corresponding to the current endowment institution identification;
and determining the care level matched with the care plan level based on the matching relation between the care plan level and the care level.
Referring to the care level rating of the nursing home institution shown in fig. 8, the nursing home institution care level is classified into 6: general care, secondary care, primary care, special care A, special care B and special care, and provides nursing care service according to grades. Wherein, the ordinary care of the nursing institution corresponds to the health care in the care plan level, the primary care of the nursing institution corresponds to the middle care 1 and the middle care 2 in the care plan level, the rest can be referred to fig. 8, and the nursing care plan level of the determined nursing service of the elderly person is "primary care 1", so that the care level of the corresponding nursing institution is "secondary care".
It is understood that, if the care institution does not set its own care level hierarchy and directly adopts the 8-level care plan level as described above, when determining a care level matching the care plan level based on the care plan level, the determined care plan level may be directly determined as a matching care level.
Step S104: and determining a care plan of the target object according to the old people capability evaluation result and the care level.
In one embodiment, determining the care plan of the target subject according to the elderly capability assessment result and the care level may be performed by:
acquiring each service class corresponding to the care level;
determining each service item under each service category and service requirements corresponding to each service item according to the old people ability evaluation result of the target object;
the care plan of the target object comprises the service categories, the service items under the service categories and the service requirements corresponding to the service items.
Therefore, after the care level is determined, the service categories corresponding to the care level can be determined according to the determined care level, and each service item under each service category and the service requirement corresponding to each service item are determined according to the capability evaluation result of the old people, so that the final care plan of the target object is obtained.
It is to be understood that, in the case that the special service requirement is entered when the care plan basic information is entered, the care plan of the target object may further include the special service requirement.
At this time, the determining the care plan of the target subject according to the elderly capability assessment result and the care level further includes: and acquiring the special service requirement information of the target object. At this time, the care plan of the target object further includes the special service requirement information.
In some embodiments, the endowment institution may also recommend some matching recommended care service items to the target object based on the actual situation of the target object, so that the target object or its family members can select the items.
Accordingly, in some embodiments, after determining the care plan of the target subject, the following steps are further included:
first, a recommended care service item for a target object is determined.
Referring to fig. 9, the determined recommended care service items may be a daily care service, a safety protection service, an elderly rehabilitation service, and the like, and may include content of care items that are provided by the elderly care institution autonomously and must be executed (possibly based on laws and regulations, or based on the final level of abilities of the elderly, or determined by the elderly care institution itself), such as the safety protection service shown in fig. 9, a nutrition recipe service, a washing service, a sanitation service, and the like in the daily care service, and may also include content of care service items that are recommended by some elderly care institutions but not necessarily executed by the elderly care institution, such as the elderly rehabilitation service shown in fig. 9. The staff of the care institution may check specific content of the recommended care item by clicking a specific option of the recommended care item, as shown in fig. 10, the specific content may include a service description and a charging condition of the recommended care item, and may further include information necessary for different care levels, so that the staff of the care institution may add, delete, and modify categories, prices, whether to put on shelf, names, descriptions, and types of care services of the specific service item. Wherein the adjustment of the care service type may affect the display of the "recommended/necessary" related care item content shown in fig. 9. For example, if the selection of general care in the care service type corresponding to a certain service item in fig. 10 is "null" and the care plan level of the target subject is "general care", the item is not displayed in the service menu in fig. 9. Assuming that the general care is selected as "necessary" in the care service type column, when the care plan level of a certain target object is "general care", the item is displayed in the service menu of fig. 9 and the "care service type" is "necessary".
And secondly, receiving an item adding and deleting instruction, and acquiring a newly added care service item which is added or deleted on the recommended care service item.
Based on the recommended care service items, the elderly or their family members as the target object may communicate with the care institution staff, and the care institution staff may use all the recommended care service items as new care service items, or may add or delete the recommended care service items based on the recommended care service items, for example, reduce some unnecessary care items, add some targeted care items, and the like, as shown in fig. 11, the corresponding recommended care service items may be deleted by deleting a control (e.g., a recycle bin flag shown in fig. 11), or add new care items by clicking a control such as "add service items"; on the other hand, the service requirement of each recommended care service item can be added, and the service times of each recommended care item can be displayed in the interface so as to be referred to by the old or the family members of the old as the target object. After the addition or the deletion is finished, a determination instruction can be sent by clicking controls such as 'save', 'submit' and the like to determine the care plan.
And then, adding the newly added care items into a care plan to obtain an updated care plan of the target object.
Therefore, the finally obtained care plan of the target object comprises the care plan matched with the old people capability evaluation result and the care level, the special service requirement information and the newly added care service item, and the accuracy and the completeness of the finally determined care plan are realized.
It should be understood that, although the steps in the flowchart of fig. 1 are shown in order as indicated by the arrows, the steps are not necessarily performed in order as indicated by the arrows. The steps are not performed in the exact order shown and described, and may be performed in other orders, unless explicitly stated otherwise. Moreover, at least a portion of the steps in fig. 1 may include multiple sub-steps or multiple stages that are not necessarily performed at the same time, but may be performed at different times, and the order of performance of the sub-steps or stages is not necessarily sequential, but may be performed in turn or alternately with other steps or at least a portion of the sub-steps or stages of other steps.
After the care plan of the target subject is determined, a care service plan implementation scheme of the target subject may be further generated according to the care plan of the target subject, where the generated care service plan implementation scheme includes: date information, time information, service item content determined from the care plan associated with the date information and time information, and shift information.
In one embodiment, generating the care service plan implementation solution of the target object according to the care solution of the target object may be performed in the following manner.
Firstly, according to the care scheme of the target object, generating a care service plan implementation table of the target object, wherein the care service plan implementation table comprises: the planning implementation setting control comprises a date option, a time option and planning implementation setting controls of the positions of the sub-tables corresponding to the date option and the time option; the date option and the time option are arranged in a row and column.
A schematic diagram of a blank care service plan implementation table generated in a specific example is shown in fig. 12, and as shown in fig. 12, a date option (for example, monday, tuesday … … weekday shown in fig. 12, it is understood that, in other embodiments, a date option such as 1 day, 2 day … …, or a date option determined by using another time period) and a time option (for example, 6:00, 8:00 … … 16:00 shown in fig. 12) are provided in the care service plan implementation table, where the date option and the time option are arranged in a row, that is, when the date option is a row, the time option is a column, when the date option is a column, the time option is a row, and in fig. 12, the time option is illustrated as a column. The target plan implementation setting control is a control that needs to add a service implementation plan to the date and time corresponding to the control in the actual operation process, that is, any plan implementation setting control in the care service plan implementation table may be used as the target plan implementation setting control. The user clicks the control in the sub-table corresponding to the date "monday" and the time "12: 00", which is a service implementation plan setting interface as shown in fig. 13.
As shown in fig. 13, in the service implementation plan setting interface, a date option "monday" and a time option "12: 00" corresponding to the control may be displayed, so that the operator can check the selected date and time without error, and a service selection item may be provided, through which the content of the service item determined in the care plan, for example, the "nutrition recipe service" shown in fig. 13, may be selected and determined. In the embodiment shown in fig. 13, the service item content is selected by a drop-down list selection, it can be understood that in other embodiments, the service item content may also be selected by other manners, for example, when the selectable service item content is not large, each service item content is displayed, and each service item content corresponds to a selection control, and the corresponding service item content is selected by the selection control. It is understood that in other embodiments, other options may be used.
On the other hand, as shown in fig. 13, a shift selection item may be further provided in the service implementation plan setting interface, the shift selection item is provided in correspondence with the service selection item, and if there are a plurality of shifts of staff in the same day, a specific shift may be designated by the shift selection item to be responsible, and as shown in fig. 13, "nutritional catering service" may be selected to be responsible for "white shift".

Generating a care service plan implementation scheme of the target object according to the care scheme of the target object, further comprising:

Therefore, the daily living habits can be filled in the page to be noticed by the working staff through the sub-form positions corresponding to the daily living habits options and the time options. Based on the same idea of the method as described above, referring to fig. 15, an apparatus for determining an elderly care plan according to an embodiment of the present application includes:

In one embodiment, the level matching module obtains a weight of each level of index classification parameter, or a weight of each parameter value of each level of index classification parameter; based on the weight of each first-level index grading parameter or the weight of each parameter value of each first-level index grading parameter, carrying out weighted summation on the parameter values of each first-level index grading parameter to obtain a weighted calculation result; and determining the primary grade of the ability of the old people matched with the evaluation result of the ability of the old people based on the weighted calculation result interval where the weighted calculation result is located, wherein different primary grades of the ability of the old people correspond to different weighted calculation result intervals.

Accordingly, in an embodiment, there is provided a computer device comprising a memory and a processor, the memory having stored therein a computer program, the processor when executing the computer program implementing the method of determining an elderly care plan as in any of the embodiments described above.
It will be understood by those skilled in the art that all or part of the processes of the methods of the embodiments described above can be implemented by hardware instructions of a computer program, which can be stored in a non-volatile computer-readable storage medium, and when executed, can include the processes of the embodiments of the methods described above. Any reference to memory, storage, database, or other medium used in the embodiments provided herein may include non-volatile and/or volatile memory, among others. Non-volatile memory can include read-only memory (ROM), Programmable ROM (PROM), Electrically Programmable ROM (EPROM), Electrically Erasable Programmable ROM (EEPROM), or flash memory. Volatile memory can include Random Access Memory (RAM) or external cache memory. By way of illustration and not limitation, RAM is available in a variety of forms such as Static RAM (SRAM), Dynamic RAM (DRAM), Synchronous DRAM (SDRAM), Double Data Rate SDRAM (DDRSDRAM), Enhanced SDRAM (ESDRAM), Synchronous Link DRAM (SLDRAM), Rambus Direct RAM (RDRAM), direct bus dynamic RAM (DRDRAM), and memory bus dynamic RAM (RDRAM).
The technical features of the above embodiments can be arbitrarily combined, and for the sake of brevity, all possible combinations of the technical features in the above embodiments are not described, but should be considered as the scope of the present specification as long as there is no contradiction between the combinations of the technical features.
The above-mentioned embodiments only express several embodiments of the present application, and the description thereof is more specific and detailed, but not construed as limiting the scope of the invention. It should be noted that, for a person skilled in the art, several variations and modifications can be made without departing from the concept of the present application, which falls within the scope of protection of the present application. Therefore, the protection scope of the present patent shall be subject to the appended claims.

Claims (13)

1. A method of determining an elderly care regimen, the method comprising:
acquiring an old people ability evaluation result of a target object;
determining the final aged ability grade of the target object according to the aged ability evaluation result;
determining a care level matched with the final aged ability level according to the final aged ability level;
and determining the care scheme of the target object according to the old people capability evaluation result and the care level.
2. The method of claim 1, wherein determining a care level matching the final aged person competence level according to the final aged person competence level comprises:
determining a care plan grade matched with the final grade of the ability of the old people according to the matching relation between the final grade of the ability of the old people and the parameter values of the grading parameters of the first-grade indexes;
and determining a care level matched with the care plan level based on the care plan level.
3. The method of claim 2, wherein determining a care plan level matching the final aged person competence level according to the final aged person competence level and a matching relationship between parameter values of each of the primary index ranking parameters comprises:
acquiring an old people care plan judgment diagram, wherein the old people care plan judgment diagram comprises an old people ability final grade item and each score option of each grade index grading parameter;
determining the current ability grade item of the final ability grade of the old people in the old people care plan judgment map according to the final ability grade of the old people;
determining each current score item of each primary index grading parameter in the old people care plan judgment chart according to the parameter value of each primary index grading parameter;
and determining the care plan grade based on the corresponding relation between the current ability grade items and the current score items in the care plan judgment graph of the old people.
4. The method of claim 3, wherein determining a care level matching the care plan level based on the care plan level comprises:
acquiring a current endowment institution identification and a matching relation between a care plan grade and a care grade corresponding to the current endowment institution identification;
and determining the care level matched with the care plan level based on the matching relation between the care plan level and the care level.
5. The method of claim 1, wherein determining a care plan for the target subject based on the elderly capability assessment results and the care level comprises:
acquiring each service class corresponding to the care level;
determining each service item under each service category and service requirements corresponding to each service item according to the old people capability evaluation result;
the care plan of the target object comprises each service category, each service item under each service category and the service requirement corresponding to each service item.
6. The method of claim 5, wherein:
determining a care plan of the target object according to the old people ability evaluation result and the care level, and further comprising: acquiring special service requirement information of the target object;
the care plan of the target object also comprises the special service requirement information.
7. The method according to claim 5 or 6, further comprising, after determining the care plan of the target subject, the steps of:
determining a recommended care item for the target object;
receiving an item adding and deleting instruction, and acquiring a new nursing item after adding or deleting the recommended nursing item;
and adding the newly added care items into the care scheme to obtain an updated care scheme of the target object.
8. The method of claim 1, further comprising, after determining the care plan for the target subject, the steps of:
generating a care service plan implementation scheme of the target object according to the care scheme of the target object, wherein the care service plan implementation scheme comprises the following steps: date information, time information, service item content determined from the care plan associated with the date information and time information, and shift information.
9. The method of claim 8, wherein generating a care service plan implementation for the target subject based on the care plan for the target subject comprises:
generating a care service plan implementation table of the target object according to the care scheme of the target object, wherein the care service plan implementation table comprises: the planning implementation setting control comprises a date option, a time option and planning implementation setting controls of the positions of the sub-tables corresponding to the date option and the time option; the date option and the time option are arranged in a row and column;
when an adding instruction is received through a target plan implementation setting control in the plan implementation setting controls, a service implementation plan setting interface is obtained and displayed, and service item content determined from the care plan and shift information corresponding to the service item content are obtained through the service implementation plan setting interface;
and when a determination instruction is received through the service implementation plan setting interface, returning to the care service implementation plan table interface, and displaying the determined service item content and the shift information corresponding to the service item content at the sub-table corresponding to the target plan implementation setting control.
10. The method of claim 8, wherein the care service plan implementation table further comprises: a daily lifestyle habit option; the daily life habit option and the date option are positioned in the same row or the same column;
generating a care service plan implementation scheme of the target object according to the care scheme of the target object, further comprising:
and receiving the input daily care note information at a sub-table position corresponding to the daily life habit option and the time option, and displaying the daily care note information at the sub-table position.
11. An apparatus for determining an elderly care regimen, the apparatus comprising:
the evaluation result acquisition module is used for acquiring the evaluation result of the ability of the old people of the target object;
the ability grade determining module is used for determining the final grade of the ability of the old people of the target object according to the ability evaluation result of the old people;
the care level determining module is used for determining a care level matched with the final old people ability level according to the final old people ability level;
and the care scheme determining module is used for determining the care scheme of the target object according to the old people capability evaluation result and the care level.
12. A computer device comprising a memory and a processor, the memory storing a computer program, characterized in that the processor realizes the steps of the method of any one of claims 1 to 10 when executing the computer program.
13. A computer-readable storage medium, on which a computer program is stored, which, when being executed by a processor, carries out the steps of the method of any one of claims 1 to 10.
