本发明提供一种用于输尿管镜钬激光碎石术的持续负压吸引装置。用于输尿管镜钬激光碎石术的持续负压吸引装置,包括:三通输液器;吸液软管,所述吸液软管套设在所述三通输液器的一端;输尿管导管,所述输尿管导管固定安装在所述三通输液器的一端,且所述输尿管导管与所述三通输液器相连通;第一密封帽,所述第一密封帽密封套设在所述三通输液器和所述输尿管导管上;第二密封帽。本发明提供的具有成本低廉,使用方便,可保持良好视野,提高碎石效率,缩短手术时间,防止结石上移,提高结石清除率,降低肾盂压力,减少感染风险,减少由于拉扯吸液软管掉落的可能,保障手术的顺利进行等优点。

Description

一种用于输尿管镜钬激光碎石术的持续负压吸引装置
技术领域
本发明涉及医疗技术领域,尤其涉及一种用于输尿管镜钬激光碎石术的持续负压吸引装置。
背景技术
输尿管镜钬激光碎石术因其效果确切、恢复快、并发症少,是输尿管结石的一线治疗方法。术中结石上移至肾脏是输尿管镜钬激光碎石术失败的主要原因之一,也是引起医疗纠纷的重要隐患。文献报道,输尿管下段结石行输尿管镜钬激光碎石术时结石上移发生率3%~15%,而输尿管上段结石发生率高达28%~48%。结石上移导致手术失败或更改手术方式,如改成软镜碎石术或经皮肾镜术,增加了医疗费用和手术风险。
术中防止结石上移一直以来是泌尿外科临床研究的课题。国内外已经开发了许多装置防止结石上移,包括XenXTM、Stone ConeIM、LithocatchTM、PassportTM和NtrapTM等。虽然这些装置效果良好,但存在不足。这些装置需要一根导线或导管留在输尿管内,对手术视野和术中激光碎石有一定影响。此外,激光对装置有损伤风险,从而导致装置不能关闭和移除困难。另一类防止结石上移装置包括BackStopTM(反向热敏聚合物)和凝胶。即在激光碎石前将凝胶通过导管注入到输尿管结石近端阻塞输尿管,防止结石上移。然而,在插入导管过程中结石有上移可能,碎石过程中凝胶也有可能被冲散,影响手术视野及防堵效果。而且上述装置均为一次性使用产品,价格高昂。
也有研究提示,输尿管镜碎石术中使用输尿管鞘有利于输尿管镜的进出,使视野更清晰,降低肾盂压力,提高清石率,但使用输尿管鞘可能对输尿管造成损伤,导致术后持续血尿、尿外渗甚至输尿管狭窄,且部分患者并不能成功置鞘,因此,临床应用较少。
此外,激光碎石过程中冲洗液灌注不足致输尿管局部温度过高,引起输尿管热损伤,易导致后期输尿管狭窄;冲洗液灌注过高,增加肾盂压力和术中术后感染概率,甚至肾破裂。
如何有效解决输尿管镜钬激光碎石术结石上移、提高结石清除率、降低术中肾盂压力、减少术后感染,以及防止后期输尿管狭窄等一直都是国内外泌尿外科学科平台关注和研究的热点,目前国内外解决输尿管镜钬激光碎石术中结石上移的方法主要是采用封堵装置,应用效果良好,但费用高昂,难以常规使用和在基层医院推广。传统输尿管镜钬激光碎石过程中,冲洗液灌注不足局部温度过高,易致输尿管热损伤,冲洗液灌注过多增加肾盂压力及感染风险,粘膜出血、碎石粉末影响术野和碎石效率,因此有必要提供一种用于输尿管镜钬激光碎石术的持续负压吸引装置解决上述技术问题。
发明内容
本发明解决的技术问题是提供一种成本低廉,使用方便,可保持良好视野,提高碎石效率,缩短手术时间,防止结石上移,提高结石清除率,降低肾盂压力,减少感染风险,减少由于拉扯吸液软管掉落的可能,保障手术顺利进行的用于输尿管镜钬激光碎石术的持续负压吸引装置。
为解决上述技术问题,本发明提供的用于输尿管镜钬激光碎石术的持续负压吸引装置,包括:三通输液器;吸液软管,所述吸液软管套设在所述三通输液器的一端;输尿管导管,所述输尿管导管固定安装在所述三通输液器的一端,且所述输尿管导管与所述三通输液器相连通;第一密封帽,所述第一密封帽密封套设在所述三通输液器和所述输尿管导管上;第二密封帽,所述第二密封帽密封套设在所述三通输液器远离所述第一密封帽的一端。
优选的,所述三通输液器上设有阀门。

本发明提供一种用于输尿管镜钬激光碎石术的持续负压吸引装置,该装置成本低廉,使用方便,无需额外培训,便于推广,可常规使用,利用负压通过负压吸引输尿管镜钬激光碎石,术中持续灌注和负压吸引,可保持良好视野,提高碎石效率,缩短手术时间,防止结石上移,提高结石清除率,降低肾盂压力,减少感染风险,避免“水煮”输尿管所致远期输尿管狭窄,转动把手带动两个弧形防滑垫相互靠近,直至将吸液软管加紧,防止在吸液过程中吸液软管滑落,减少由于拉扯吸液软管掉落的可能,保障手术的顺利进行。

第一实施例
请结合参阅图1,在本发明的第一实施例中,用于输尿管镜钬激光碎石术的持续负压吸引装置包括:三通输液器1;吸液软管2,所述吸液软管2套设在所述三通输液器1的一端;输尿管导管3,所述输尿管导管3固定安装在所述三通输液器1的一端,且所述输尿管导管3与所述三通输液器1相连通;第一密封帽4,所述第一密封帽4密封套设在所述三通输液器1和所述输尿管导管3上;第二密封帽4,所述第二密封帽4密封套设在所述三通输液器1远离所述第一密封帽4的一端。
所述三通输液器1上设有阀门6。
本发明提供的用于输尿管镜钬激光碎石术的持续负压吸引装置的工作原理如下:
将钬激光光纤与输尿管导管3一并插入输尿管镜中进行负压吸引的方式,选择200μm钬激光细光纤从输尿管导管管腔内插入,术中可以来回抽动光纤,有效防止碎石片堵塞输尿管导管3而影响负压吸引效果。该装置成本低廉,使用方便,无需额外培训,便于推广,可常规使用,利用负压通过负压吸引输尿管镜钬激光碎石,术中持续灌注和负压吸引,可保持良好视野,提高碎石效率,缩短手术时间,防止结石上移,提高结石清除率,降低肾盂压力,减少感染风险,避免“水煮”输尿管所致远期输尿管狭窄。
与相关技术相比较,本发明提供的用于输尿管镜钬激光碎石术的持续负压吸引装置具有如下有益效果:

基于本申请的第一实施例提供的用于输尿管镜钬激光碎石术的持续负压吸引装置,本申请的第二实施例提出另一种用于输尿管镜钬激光碎石术的持续负压吸引装置。第二实施例仅仅是第一实施例的优选的方式,第二实施例的实施对第一实施例的单独实施不会造成影响。
下面结合附图和实施方式对本发明的第二实施例作进一步说明。
请结合参阅图2-3,用于输尿管镜钬激光碎石术的持续负压吸引装置还包括两个固定块8,两个所述固定块8对称固定安装在所述三通输液器1上,两个所述固定块8的顶部均固定安装有支撑块9,两个所述支撑块9上转动安装有同一个螺纹杆10,所述螺纹杆10上对称螺纹安装有两个连接块11,两个所述连接块11的底端均固定安装有衔接块12,两个所述衔接块12相互靠近的一侧均固定安装有支撑柱13,两个所述支撑柱13相互靠近的一侧均固定安装有安装块14,两个所述安装块14相互靠近的一侧均固定安装有弧形防滑垫15,且两个所述弧形防滑垫15与所述吸液软管2相接触。
两个所述支撑块9相互靠近的一侧均开设有转动孔,两个所述转动孔内均设有轴承,两个所述轴承的外圈分别与两个所述转动孔的内壁固定连接,两个所述轴承的内圈均固定套设在所述螺纹杆10上。
所述螺纹杆10上对称开设有两段旋向相反的外螺纹,两个所述连接块11相互靠近的一侧均开设有螺纹通孔,所述螺纹杆1贯穿两个所述螺纹通孔并与两个所述螺纹通孔的内壁相旋合。
两个所述支撑块9相互靠近的一侧固定安装有同一个滑杆,两个所述连接块11均与所述滑杆滑动连接。
所述弧形防滑垫15的材质为橡胶材质。
插好吸液软管2至三通输液器1上后,转动把手7,把手7带动螺纹杆10转动,螺纹杆10带动两个连接块11相互靠近,两个连接块11带动两个衔接块12相互靠近,两个衔接块12带动两个支撑柱13相互靠近,两个支撑柱13带动两个安装块14相互靠近,两个安装块14带动两个弧形防滑垫15相互靠近,直至将吸液软管2加紧,防止在吸液过程中吸液软管2滑落,减少由于拉扯吸液软管2掉落的可能,保障手术的顺利进行。

三通输液器;
吸液软管,所述吸液软管套设在所述三通输液器的一端;
输尿管导管,所述输尿管导管固定安装在所述三通输液器的一端,且所述输尿管导管与所述三通输液器相连通;
第一密封帽,所述第一密封帽密封套设在所述三通输液器和所述输尿管导管上;
第二密封帽,所述第二密封帽密封套设在所述三通输液器远离所述第一密封帽的一端。
2.根据权利要求1所述的用于输尿管镜钬激光碎石术的持续负压吸引装置,其特征在于,所述三通输液器上设有阀门。
3.根据权利要求1所述的用于输尿管镜钬激光碎石术的持续负压吸引装置,其特征在于,所述三通输液器上对称固定安装有两个固定块,两个所述固定块的顶部均固定安装有支撑块,两个所述支撑块上转动安装有同一个螺纹杆,所述螺纹杆上对称螺纹安装有两个连接块,两个所述连接块的底端均固定安装有衔接块,两个所述衔接块相互靠近的一侧均固定安装有支撑柱,两个所述支撑柱相互靠近的一侧均固定安装有安装块,两个所述安装块相互靠近的一侧均固定安装有弧形防滑垫,且两个所述弧形防滑垫与所述吸液软管相接触。
4.根据权利要求3所述的用于输尿管镜钬激光碎石术的持续负压吸引装置,其特征在于,两个所述支撑块相互靠近的一侧均开设有转动孔,两个所述转动孔内均设有轴承,两个所述轴承的外圈分别与两个所述转动孔的内壁固定连接,两个所述轴承的内圈均固定套设在所述螺纹杆上。
5.根据权利要求3所述的用于输尿管镜钬激光碎石术的持续负压吸引装置,其特征在于,所述螺纹杆上对称开设有两段旋向相反的外螺纹,两个所述连接块相互靠近的一侧均开设有螺纹通孔,所述螺纹杆贯穿两个所述螺纹通孔并与两个所述螺纹通孔的内壁相旋合。
