Abstract
本发明涉及一种用于通过检测产妇的样本中的微生物群落的变化来诊断早产风险的组合物、诊断试剂盒和诊断方法。更具体地,本发明涉一种及用于通过从产妇的样本中检测魏斯氏菌微生物来诊断早产风险的组合物、诊断试剂盒和诊断方法。
Description
技术领域
本发明涉及用于通过检测产妇样本中微生物群落的变化来诊断早产风险的组合物、诊断试剂盒和诊断方法。更具体地,本发明涉及用于通过从母亲的样本中检测魏斯氏菌(Weissella)微生物来诊断早产风险的组合物、诊断试剂盒和诊断方法。
背景技术
早产(PTB;在妊娠37周之前出生)通常是指20周之后且37周之前经行分娩。早产的新生儿有很高的死亡风险,约占所有婴儿死亡的60%。此外,即存活下来的婴儿也会由于神经系统发育障碍、呼吸系统并发症和产后发育延迟而需要新生儿重症监护,并且,长期和短期疾病的发病率极高。大约70%的早产是由于各种病理过程(如子宫内感染和炎症)所导致的早产阵痛(PTL)和胎膜早破(PPROM)引起的。然而,与早产有关的机制尚未明确,并且通常与自然阵痛有关的危险因素包括生殖器感染、多胎妊娠、妊娠中期和妊娠晚期出血、以及有早产史等。
已知,早产儿存活已提高为最低妊娠周数27周,出生体重0.9kg,并且与出生体重相比,新生儿的患病率和死亡率主要受妊娠周数,即成熟度的影响。因此,在早期周数内出现早产迹象之时,通过适当的治疗使分娩延迟一周来增加新生儿的成熟度,是影响产妇和新生儿的健康、生活质量和成本的重要因素。
迄今为止,对早产的治疗是通过对出现早产阵痛的产妇使用子宫收缩抑制剂、抗生素治疗、类固醇和孕酮来抑制阵痛并使分娩延迟。但是,在已经出现早产阵痛和胎膜早破的情况下,抗生素对子宫内感染和炎症的治疗作用是有限的,并且不能防止早产。此外,由于患有宫颈功能不全的产妇有极高的感染和流产的风险,因此虽然已经可以通过进行宫颈环扎术来作为治疗和预防早产的措施,但这种方法只有助于短暂地延长妊娠,而无法进行根本性的治疗。
因此,相比于出现胎膜早破或早产症状后才进行治疗,更需要通过提前预测早产的风险来甄别有早产风险的产妇组,从而通过对其进行适当的护理来预防早产或开发有效的早产治疗方法。另外,早产的时间越提前,给新生儿留下后遗症的可能性就越大,并且后遗症也越严重,因此,如果可以预测早产,则有望大幅度降低早产儿以及因此导致的残疾儿童的出现率。
通常用于预测早产的方法是通过妇产科学、人口统计学和各种综合症来确认需要特别关注的女性群体,但是该方法存在不具有敏感性和特异性的问题。为了克服该问题,已经进行了许多研究以寻找用于预测突发性早产或胎膜早破的生化标志物,然而,虽然已提出血浆雌二醇-17β(plasma estradiol-17beta),孕酮(progesterone)和C反应蛋白(C-reactive protein)等物质作为候选,但也已经证明这些物质不太准确。因此,有必要开发更易于检测,并且具有更高灵敏度和特异性的标志物。
妇女的阴道微生物群落(microbiota)是细菌和病原体的屏障,对妇女的健康起着重要的作用,微生物群落的生化学和免疫学特征可以作为阴道环境的健康指标。正常的微生物群落可以保护妇女免受如酵母菌感染、性传播感染、尿路感染和人类免疫缺陷病毒(HIV)感染等的泌尿生殖系统疾病的侵害。据报道,在被诊断为细菌性阴道病(BV)的妇女中,早产和晚期流产的风险分别高出两倍和六倍。
近期,虽然随着基因测序技术的发展,使得研究人体各个部位的人类微生物群落成为可能,但迄今为止,大多数关于阴道微生物群落的研究都集中在健康妇女、或患有细菌性阴道炎妇女的阴道微生物群落的构成上,而几乎没有以查明阴道微生物群与早产风险之间关系,并将其运用于诊断早产风险性为目的的研究。尤其,尚未对出现早产阵痛的产妇和胎膜早破的韩国产妇进行过阴道微生物特征的比较研究。
因此,本发明的发明者通过对出现早产的主要原因的早产阵痛或胎膜早破的产妇的阴道微生物进行表征,并使用高通量基因测序鉴定来确认预测早产的阴道微生物参数,从而完成了本发明。

在本发明中,通过对产妇阴道内微生物群落进行分析来发掘可暗示早产危险性的早产预测标志物,从而为早产的预防作出贡献。
为此,在一个实施例中,提供一种用于诊断产妇早产风险的组合物,所述组合物包括能够检测魏斯氏菌微生物的试剂。
在另一个实施例中,提供了一种包含所述组合物的用于诊断产妇早产风险的试剂盒。
在另一个实施例中,提供了一种提供诊断早产风险所必需的信息的方法,该方法包括从产妇样本中检测魏斯氏菌微生物。
技术方案
一方面,本发明涉及用于诊断产妇早产风险的组合物,其包含能够检测魏斯氏菌微生物的试剂。
优选地,本发明的用于诊断的组合物可以进一步包括能够检测选自乳杆菌(Lactobacillus)、拟杆菌(Bacteroides)和普氏菌(Prevotella)的微生物的试剂。
优选地,在本发明中,能够检测所述微生物的试剂可以是对微生物具有特异性的引物、探针、反义寡核苷酸、适体或抗体。
更优选地,所述引物可以是能够扩增微生物的16S rRNA(16S rDNA)的引物。
在另一个方面,本发明涉及一种包含所述组合物的用于诊断产妇早产风险的试剂盒。
在又另一个方面,本发明涉及一种通过从产妇样本中检测魏斯氏菌微生物来提供诊断早产风险所必需的信息的方法。
此外,本发明涉及一种用于提供诊断早产风险所必需的信息的方法,该方法包括从产妇样本中检测魏斯氏菌微生物的步骤。
优选地,所述方法可以进一步包括检测选自乳杆菌、拟杆菌和普氏菌的微生物的步骤。

(b)使提取的基因组DNA与对魏斯氏菌微生物具有特异性的引物进行反应的步骤,以及

有益效果
本发明提供了一种用于诊断早产风险的微生物生物标志物,可通过利用该微生物生物标志物,和进行简单的采样来快速检测早产风险,并且还可以开发与其相关的快速诊断试剂盒。

图3示出了韩国人的阴道CST和诊断。根据微生物种数据的ward连锁聚类(wardlinkage clustering)对样本进行CST分类(CST I,橙色(I);CST II,红色(II);CST III,蓝色(III);CST IV,紫色(IV);CST IA,浅蓝色(A);CST IVA,绿色(B);早产阵痛,灰色(PTL);胎膜早破,黑色(PPROM);其他,白色(其他))。
图4示出了有关从产妇(PTL(n=36)或PPROM(n=21))收集的阴道粘液样本(n=57)的阴道内微生物的upgma聚类的条形图;正常(>90%乳杆菌),中级(30-90%乳杆菌)或失调(dysbiotic)(<30%乳杆菌)。
图5a-5d示出了阴道微生物群落与早产之间的关系。在根据乳杆菌比率分类的正常组、中级组和失调组中,示出了比较乳杆菌(图5a)、拟杆菌(图5b)、普氏菌(图5C)和魏斯氏菌(图5D)的比率的结果。曼-惠特尼U(Mann Whitney U)检验,p<0.05。
具体实施方式
以下,将详述本发明。
在本发明中,通过分析因早产阵痛和胎膜早破而入院的产妇阴道样本中微生物群落的组成对早产的影响,结果确认乳杆菌(Lactobacillus)的减少,拟杆菌(Bacteroides)和普氏菌(Prevotella)的增加、魏斯氏菌(Weissella)的减少会对早产产生影响。特别地,乳杆菌的减少和拟杆菌的增加与本发明人已发现的早产标记IL-7有关(参见韩国专利注册号10-1559975)。因此,这些微生物的分布特征在于提供用于预测早产的生物标记。尤其,乳杆菌的减少和拟杆菌的增加与本发明人发现的早产预测标志物IL-7有关(参见韩国专利号10-1559975)。因此,这些微生物的分布变化的特征在于提供用于预测早产的生物标志物。
因此,在本发明中,可以通过从产妇的样本中检测魏斯氏菌微生物来诊断产妇早产的风险,为此,提供了用于检测魏斯氏菌微生物的组合物、试剂盒和方法。优选地,本发明还能够通过进一步对乳杆菌、拟杆菌和/或普氏菌微生物进行检测来诊断产妇早产的风险,并提供与其相关的组合物、试剂盒和方法。
在本发明中,术语“风险诊断”或“风险预测”是对指产妇是否有在孕周不到37周时发生早产的可能性、在不到37周的时间发生早产的可能性是否相对较高、或者在不到37周的时间内是否有出现早产迹象的可能性的判断。本发明针对产妇,通过对具有高风险的在不到37周发生早产的母亲进行适当的特殊护理来延迟或预防早产。此外,本发明可通过进行小于37周的早产的早期诊断,来选择最合适的治疗方案,从而在临床上用于做出治疗决定。
在本发明中,术语“诊断用标志物,用于诊断的标志物或诊断标志物”或“标志物”是指过用作预测和区分早产产妇(例如,在小于37周时,发生早产的产妇)和正常分娩母亲(例如,在37周或以上进行分娩的产妇)的标准物质,与正常分娩产妇,在被预测为早产的产妇样本中的所显示出明显差异的生物体有机分子。在本发明的目的上,是指在被预测为早产的产妇样本中表现出特异性分布变化的魏斯氏菌微生物或群落,进一步的,是指选自乳杆菌、拟杆菌和普氏菌中一种或多种的微生物或群落

在本说明书中,术语“可以检测的试剂”是指可用于检测样本中存在作为早产风险诊断标志物的魏斯氏菌、乳杆菌、拟杆菌和/或普氏菌的物质。例如,可以是能够对特异性地存在于魏斯氏菌、乳杆菌、拟杆菌和/或普氏菌中的蛋白质、核酸、脂质、糖脂质、糖蛋白或糖(单糖、二糖、寡糖等)等有机生物体分子的引物、探针、反义寡核苷酸、适体、抗体等。
优选地,在本发明中,可检测的试剂可以是能够检测乳杆菌、拟杆菌和/或普氏菌的引物。优选地,用所述引物对魏斯氏菌、乳杆菌、拟杆菌和/或普氏菌的基因组序列进行特异性检测,并且所述引物不与其他微生物的基因组序列进行特异性结合。更优选地,所述引物可以是能够使选自魏斯氏菌、乳杆菌、拟杆菌和/或普氏菌中的一种或多种微生物扩增的引物。
在本说明书中,术语“引物”是指可以形成与模板链互补的碱基对,并用作模板链复制起点的七至五十个核酸序列。虽然引物通常是合成的,但也可用于天然存在的核酸中。引物的序列不必一定与模板的序列完全相同,互补性足够也能够与模板进行杂交。可以并入不改变引物本性的其他特征。可以并入的附加特征的例子有甲基化、加帽(capping)、用同源物取代一个或多个核酸、以及核酸之间的修饰等,但不限于此。优选地,本发明的引物可以是能够对魏斯氏菌,乳杆菌、拟杆菌和/或普氏菌微生物的16S rRNA进行扩增的引物。
在本说明书中,术语“16S rRNA”是指构成原核生物核糖体的30S亚基的、所有物种共有的保守区和能够对特定物种进行分类的高变区的rRNA,因此可以通过碱基序列分析来鉴定微生物的存在。特别地,由于同源物种之间几乎没有多样性,而不同物种之间则具有多样性,因此可以通过比较16S rRNA的序列来有效地鉴定原核生物。此外,由于16S rDNA是编码16S rRNA的基因,因此也可以使用16S rDNA来鉴定微生物。
在一个优选的实施例中,本发明中的引物可用于扩增魏斯氏菌,乳杆菌、拟杆菌和/或普氏菌的具有特异性的16S rRNA(或16S rDNA)序列,通过序列扩增的结果是否生成所需产物来检测魏斯氏菌、乳杆菌、拟杆菌和/或普氏菌的存在。利用引物来扩增序列的方法可以使用本领域已知的各种方法。例如可以使用聚合酶链反应(PCR)、逆转录聚合酶链反应(RT-PCR)、多重PCR(Multiplex-PCR)、降落PCR(touchdown PCR)、热启动(hot start)PCR、巢式(nested)PCR、增效(booster)PCR、实时(real-time)PCR、差异显示PCR(differential display PCR:DD-PCR)、cDNA末端快速扩增、反向PCR、小载体(Torrent)PCR、交错式热不对称PCR(TAIL-PCR)、连接酶链反应、修复型链反应、转录介导的扩增、自我维持碱基序列复制、目标碱基序列的选择性扩增反应,但不限于此。
此外,在本发明中,微生物检测试剂可以是抗体,并且可以使用基于抗原-抗体反应的免疫学方法来检测所述微生物。为此,分析方法有免疫印迹、酶联免疫吸附测定(ELISA)、放射免疫分析(RIA)、放射免疫扩散法(radioimmunodiffusion)、奥氏扩散法(Ouchterlony)、和火箭(rocket)免疫电泳法、组织免疫染色、免疫沉淀法(Immunoprecipitation assay)、补体结合分析法(Complement Fixation Assay)、荧光激活细胞分类器(FACS)或蛋白质芯片等,不限于此。
另外,本领域广泛使用的分子和免疫学方法可用于检测本发明的微生物。
包含本发明的微生物检测试剂的用于诊断的组合物可以以试剂盒的形式实现。本发明的试剂盒不仅包含引物、探针、反义寡核苷酸、适体或抗体等用于检测所述微生物的检测试剂,并且还包括一种或多种适用于分析方法的其他成分组合物、溶液或装置。
在一个具体实施例中,在本发明中,包含对魏斯氏菌、乳杆菌、拟杆菌和普氏菌具有特异性的引物的试剂盒可以是含有用于进行PCR等的扩增反应的基本元件的试剂盒。例如,用于PCR的试剂盒可以包括试管或其他合适的容器、反应缓冲液、三磷酸碱基脱氧核苷酸(dNTP)、酶(如Taq聚合酶和逆转录酶)、脱氧核糖核酸酶(DNase)、核糖核酸酶(RNAse)抑试剂、DEPC-水、或无菌水等。
此外,本发明还提供一种通过从产妇样本中检测魏斯氏菌微生物来诊断产妇早产风险的方法。
换言之,本发明提供一种用于提供诊断产妇早产风险所必需的信息的从产妇样本中检测魏斯氏菌微生物的方法。或者,提供一种用于提供诊断早产风险所必需的信息的方法,该方法包括从产妇样本中检测魏斯氏菌微生物的步骤。
优选地,所述方法可以进一步包括检测乳杆菌、拟杆菌和普氏菌微生物的步骤。
在一个优选的实施例中,所述方法可以通过包括以下步骤来实现:
(a)从产妇的样本中提取基因组DNA的步骤,
(b)使提取的基因组DNA与对魏斯氏菌微生物具有特异性的引物进行反应的步骤,以及
(c)扩增所述反应物的步骤。
在所述步骤(a)中,“产妇样本”是指取自妇女的身体的,用于诊断早产风险的物质,优选地,可以是从产妇阴道中收集的细胞样本、组织样本或体液样本,例如,可以是阴道样本、阴道粘液样本。
在所述步骤(b)中,可以通过应用本领域已知的常规技术从产妇样本中提取基因组DNA,对魏斯氏菌、乳杆菌、拟杆菌和普氏菌微生物具有特异性的引物如上所述。

在所述步骤(c)中,可以进一步进行将反应物的扩增产物的量与正常产妇样本的扩增产物的量、或者截止值(cut-off)进行比较的步骤,当通过与正常产妇样本的扩增产物的量、或者截止值(cut-off)进行比较,判断产妇样本中魏斯氏菌的扩增产物显著减少时,则可以确定该产妇具有较高的早产风险。
此外,所述方法可以进一步包括检测乳杆菌、拟杆菌和普氏菌的微生物的步骤,并将其与正常产妇样本的扩增产物或截止值(cut-off)进行比较,当产妇样本中的乳杆菌的扩增产物显著减少,或者拟杆菌或普氏菌的扩增产物显著增加,则可能认为产妇具有较高的风险。
实施例
以下,将通过以下实施例对本发明进行更详细的说明。但是,这些实施例仅用于说明一个或多个具体示例,本发明的范围不限于此。
实施例1.材料和方法
本研究的实验的所有程序和过程均已被梨花女子大学木洞医院(Ewha WomansUniversity Mokdong Hospital)(证书编号:ECT 06-127-7)、三星医学中心(SamsungMedical Center)(SMC 2014-06-094-003)、建国大学医学中心(Konkuk UniversityMedical Center)(KUH1040034)、首尔圣玛丽医院(Seoul ST.Mary’s Hospital)(KC14TIMI0591)和SMG-SNU博拉麦医学中心(SMG-SNU Boramae Medical Center)(16 201494)批准,并按照医学伦理委员会的指南进行。所有参与者均提供了书面同意。
实施例2.研究设计
在首尔地区的5所大学医院(梨花女子大学木洞医院、三星医学中心、建国大学医学中心、首尔圣玛丽医院和SSMG-SNU博拉麦医学中心医学中心)中,以在妊娠22周+0至36周+6天时由于出现早产阵痛(PTL)和/或胎膜早破(PPROM)的症状而住院的65名单胎产妇为对象,收集阴道粘液。根据子宫收缩的频率和强度来诊断早产镇痛,子宫收缩的频率和强度可能会影响在妊娠20到37周之间宫颈渐进性展平(progressive effacement)和扩张。通过阴道积液、阳性硝嗪试验(pH阳性)和羊齿试验来确认胎膜早破。使用棉棒在后穹窿收集阴道粘液样本,并将其存储在两个试管中用于ELISA和16S rRNA基因测序。收集的样本在30分钟以内存储在-80℃下。排除多胎,新生儿有出生缺陷和患有妊娠并发症的女性。由最后一次月经的第一天和超声检查决定来确定胎龄。子宫颈管长度定义为宫颈内口到宫颈外口之间的距离。根据胎盘病理学诊断出生后的组织病理性绒毛膜羊膜炎。从患者图表中收集人口统计和临床特征,新生儿出生数据包括Apgar评分、出生体重、性别和进入新生儿重症监护病房。
实施例3.胎儿纤连蛋白和细胞因子的分析
在冰上使阴道粘液样本融化,并以3000×g进行2分钟的离心分离,然后将上清液收集在新微量离心管中以除去细胞碎片。使用人胎儿纤连蛋白试剂盒(Cusabio,武汉,湖北,中国)在无细胞上清液中测量人胎儿纤连蛋白的浓度。使用680型微孔板读数器(Bio-Rad Laboratories Inc.,Hercules公司,加利福尼亚,美国)读取450nm的吸光度。使用Human Magnetic Luminex Screening Assay(Luminex Co.,Austin公司,德克萨斯州,美国)和Bio-Plex200系统(Bio-Rad Laboratories Inc.)来测量无细胞上清液中的细胞因子水平。然后对13种白介素(IL)-1β、IL-4、IL-5、IL-6、IL-7、IL-8、IL-10、IL-13、IL-17α、G-CSF、TNF-α、MIP-1α和MIP-1β进行Luminex筛选检测。在Luminex Human Premixed检测试剂盒规定的标准范围内,通过使用96孔板,在2种优化后的稀释液(1:1和1:50)中对样本进行分析来进行分析物检测。
实施例4. 16S rRNA测序和分类法分配(Taxonomy Assignment)
根据制造商的说明,使用PowerMax Soil DNA Isolation试剂盒(MOBIO,Carlsbad公司,加利福尼亚州,美国)从样本中提取细菌DNA。根据Illumina 16S宏基因组测序文库协议(Illumina,Waltham公司,加利福尼亚,美国)对细菌基因组DNA中的16S rRNA基因V3-V4高变区(519F-816R)进行扩增。用于扩增的条形码融合引物序列为519F(5′CCTACGGGNGGCWGCAG 3′)和806R(5′GACTACHVGGTATCTAATCC 3′)。根据qPCR定量方案指南(用于Illumina测序平台的KAPA文库定量试剂盒)对最终纯化的产物进行qPCR扩增,并使用LabChip GX HT DNA高灵敏度试剂盒(PerkinElmer,Waltham,马塞诸塞,美国)进行验证。Macrogen通过使用MiSeqTM平台(Illumina)进行了配对末端(2×300bp)测序。在97%的序列相似性水平下进行开放参考下的操作分类单位(OTU)挑选,并使用StrainInfo的类型修改数据库将OUT序列分配给分类组。通过使用QIIME软件包(第1.8节)计算α和β多样性来分析序列数据。
实施例5.统计分析
通过Fisher's精确检验分类变量和Kruskal-Wallis检验连续变量,分析PTL组、PPROM组和医学指征(MI)组产妇的临床特征和结果。通过使用QIIME进行曼-惠特尼U测试来比较阴道样本中的菌种和繁殖数量。通过Kruskal-Wallis检验和Spearman之间的相关性来评价乳杆菌丰度((Lactobacillus abundance)对细菌属的影响、菌种数量以及多样性。使用21.0版本的SPSS软件(IBM,Armonk,NY,USA)进行统计分析。所有分析均为双边分析,p<0.05的值被认为具有统计学意义。
实验结果
1.受试者的一般特征
表1示出了65名单胎产妇的临床特征。被诊断为PTL(n=36),PPROM(n=21)或MI(n=8)的产妇在怀孕22周+0至36周+6天之间入院。MI组包括先兆子痫(preeclampsia)(3例)、宫颈功能不全(2例)、子宫颈短(2例)或血小板减少综合征(HELLP)(1例)。PTL、PPROM和MI组的平均年龄分别为31.7岁、33.6岁和30.6岁。虽然三组的胎龄在入院时无显著差异,但出生时的胎龄、从住院到分娩的时间间隔、以及出生体重都有显著差异(p<0.05)。绒毛膜羊膜炎的患病率在三组之间没有显著差异。

研究对象的临床特征
在上述表1中,总数据表示为平均值±标准差(SD),其他数据表示为中位数和象限(IQR),分类数据表示为n(%)。基于χ2检验的Kruskal-Wallis检验和名义尺度(nominalscale),分析数据之间的显著差异(p<0.05)。PTL表示早产镇痛,PPROM表示胎膜早破。
2.有早产风险的产妇阴道内的微生物群落多样性
分析除意外患有并发症外的57名产妇的阴道样本,以对用于评估产妇早产风险的阴道微生物的组成进行表征。本研究中的数据集由阴道粘液样本中的1,055,829个基因序列组成,每个样本平均包含16,243个读数(范围:3,216-28,407)。
根据诊断(PTL、PPROM和MI)对阴道样本的序列数据进行Chao 1的主要坐标分析和测量(图1A-1C)。计算Shannon指数和Simpson指数的平均值,以评估PTL和PPROM样本中的细菌多样性。具有PPROM的妇女的阴道样本的Shannon指数(微生物群落丰度指标)和Simpson指数(微生物多样性指标)显著高于PTL的指数(p<0.05,表2)。在PPROM产妇中,患有PTL产妇的乳杆菌的优势显著增加,并且与PTL产妇相比,在PPROM产妇中、拟杆菌、普氏菌、鞘氨醇单胞菌、魏斯氏菌、链球菌、根瘤菌、脲原体、丙酸杆菌、梭杆菌的数量显著增加(p<0.05)。但是,PTL产妇和PPROM产妇的奇异菌、加德纳菌、支原体和斯尼思菌的数量没有差异。

在以上表2中,总数据表示为平均值±标准差(SD),其他数据表示为中位数和象限(IQR)。数据之间的显著差异是通过Mann-Whitney U检验进行分析(p<0.05)。
3.有早产风险的产妇的阴道内的微生物谱
根据乳杆菌属(Lactobacillus spp.),将阴道生物谱分类为分别与CST(群落状态类型)I、II和III相对应的卷曲乳杆菌(L.crispatus,n=12)、格氏乳杆菌(L.gasseri,n=2)和惰性乳杆菌(L.iners,n=13)(图2)。CST IV的IVA亚组主要由双歧杆菌、小杆菌、链球菌和拟杆菌组成(n=21)。CST I的亚组IA主要为卷曲乳杆菌和拟杆菌(n=9)。有趣的是,具有CST I微生物的产妇没有在妊娠28周之前分娩的,在属于CST IVA的产妇而中,阴道微生物组中含有9.4%的魏斯氏菌的5名产妇5正常分娩,属于CST IVA的产妇全部早产(图3)。
当乳杆菌的优势度用基于细菌总数的百分比表示时,正常(>90%乳杆菌,n=10)、中等(30-90%乳杆菌,n=23)或菌群失调(30%乳杆菌,n=24)。图4示出了住院时主要的微生物群落(micobiota)的丰度情况。十名患有PTL的产妇的微生物群落正常(表3)。23名被诊断患有PTL或PPROM的产妇出现了微生物群落显示为中等或失调。随着乳杆菌的减少,拟杆菌、加德纳菌、普氏菌、鞘氨醇单胞菌、魏斯氏菌、链球菌、根瘤菌、丙酸杆菌、斯尼思菌和梭杆菌显著增加(p<0.01)(图4)。有趣的是,与阴道微生物群落正常或中等的产妇相比,阴道微生物群落失调的产妇更多会患绒毛膜羊膜炎(表3)。

在上述表3中,根据乳杆菌比率分为三组。总数据表示为平均值±标准差(SD),其他数据表示为中位数和象限(IQR),分类数据表示为n(%)。基于χ2检验的Kruskal-Wallis检验和名义尺度(nominal scale),分析数据之间的显著差异(p<0.05)。
4.阴道微生物与细胞因子浓度的关系
对入院时阴道细菌群与出生时妊娠周数之间的关联性进行分析。在乳杆菌正常的组中,与正常分娩的产妇相比,早产产妇的阴道粘液的拟杆菌和普氏菌的含量较高,且魏斯氏菌较低(图5a-5d)。在乳杆菌的中等组中,早产产妇的阴道粘液中的拟杆菌和普氏菌的含量较高,而早产产妇的乳杆菌则较低(p<0.05)。在乳杆菌失调组中,早产产妇的阴道粘液中乳杆杆菌含量较高,而魏斯氏菌的含量较低(p<0.05)。此外,高比例的普氏菌(r2=0.331,p<0.05)还与低比例的乳杆菌(r2=-0.503,p<0.05)的绒毛膜羊膜炎的发病有关。
通过评价13种炎性细胞因子的浓度与魏斯氏菌、乳杆菌、拟杆菌和/或普氏菌的优势度之间的相关性来确定与早产有关的微生物指标。IL-1β水平(n=55,平均816.7±1483.4pg/ml)与普氏菌比率呈明显的负相关(r2=-0.388,p<0.05)。IL-7水平(n=55,平均7.7±5.6pg/ml)与乳杆菌的优势度(r2=-0.404,p<0.01)和胎龄(r2=-0.318,p<0.05)呈负相关,与拟杆菌的优势度呈正相关(r2=0.307,p<0.05。阴道微生物群落与其他细胞因子水平之间没有相关性。
5.结论
在本发明中,对因早产阵痛或胎膜早破而住院的韩国产妇的阴道粘液中的微生物组进行了比较。被诊断为胎膜早破的产妇的阴道微生物种类相比被诊断为早产阵痛的产妇的阴道微生物种类更为多样。此外,有早产风险的产妇阴道粘液中的拟杆菌、普氏菌和魏斯氏菌的比例会根据乳杆菌的优势度发生大幅变化。尤其,在全部的按乳杆菌优势度分类的三个组中,排名前20位的主要微生物组中的拟杆菌、普氏菌和魏斯氏菌与早产密切相关。
乳杆菌和魏斯氏菌的优势度的增加与正常分娩有关,而拟杆菌和普氏菌的优势度的增加与早产有关。尤其,乳杆菌的耗竭与病原菌的存在、绒毛膜羊膜炎的发作、分娩周数低有关,在阴道内菌种异常的产妇中,高比例的拟杆菌与分娩延迟有关。此外,IL-7的浓度与低的乳杆菌比例和高的拟杆菌比例有关。本研究进一步表明,乳杆菌的减少和拟杆菌的增加与早产标志物IL-7相关,魏斯氏菌的减少与IL-1β相关,因此可以将其用作为早产标志物。

1.一种用于检测产妇的早产风险的组合物,所述组合物包含能够检测魏斯氏菌微生物的试剂。
2.根据权利要求1所述的组合物,其特征在于,
还包含能够检测选自乳杆菌、拟杆菌、普氏菌中的微生物的试剂。
3.根据权利要求1所述的组合物,其特征在于,
能够检测所述微生物的所述试剂为对微生物具有特异性的引物、探针、反义寡核苷酸、适体或抗体。
4.根据权利要求3所述的组合物,其特征在于,
所述引物为能够扩增微生物的16S rRNA的引物。
5.一种用于诊断早产风险的试剂盒,所述试剂盒包含权利要求1-4中任一项所述的组合物。
6.一种提供诊断早产所必需的信息的方法,所述方法包括从产妇样本中检测魏斯氏菌微生物的步骤。
7.根据权利要求6所述的方法,其特征在于,
进一步包括检测选自乳杆菌、拟杆菌和普氏菌中的微生物的步骤。
8.根据权利要求6所述的方法,其特征在于,包括:
(a)从产妇的样本中提取基因组DNA的步骤,
(b)使提取的基因组DNA与对魏斯氏菌微生物具有特异性的引物进行反应的步骤,以及
(c)对所述反应物进行扩增的步骤。
9.根据权利要求8所述方法,其特征在于,
所述步骤(c)通过聚合酶反应进行。
10.根据权利要求8所述的方法,其特征在于,
还包括将所述步骤(c)的扩增产物的量与正常产妇样本的扩增产物进行比较的步骤。
