技术领域
本发明涉及扫描成像技术领域,尤其涉及一种低剂量CT灌注的扫描成像系统。
背景技术
淋巴结转移是胃癌最常见的转移方式,因此很大比例的胃癌患者在最初被诊断为胃癌时会出现胃周淋巴结增大转移。胃周淋巴结转移的存在对胃癌的治疗及预后有很大影响。如果广泛切除胃周围淋巴结,可能会影响身体的免疫功能,并增加手术的创伤。所以,在胃癌术前准确诊断淋巴结有无转移至关重要。目前,根据传统的影像诊断标准,当胃周淋巴结的短径超过10毫米时,被认为是转移。但是,诊断正确率不高,有很高的假阴性和假阳性。因为,胃周淋巴结的增大可能是由于癌细胞的扩散引起,也可能是由于炎性反应性增生导致淋巴结肿大。从理论上讲,前者由肿瘤浸润和肿瘤血管生成因子产生肿瘤新生血管,会改变和破坏转移性淋巴结内部的血管形态结构(包括局部淋巴结内的血流量和血管通透性的明显增大);而后者炎症反应性淋巴结增生是由于局部体液因素引起淋巴结内的血管扩张(相比前者局部淋巴结内的血流量和血管通透性增大程度要低)。如果这两种不同的变化能被一种可靠的检查方法探测到,胃癌术前就能准确区分转移性和炎症反应性胃周围淋巴结。避免胃癌周围炎症反应性增生淋巴结的不必要清扫。
然而,目前还没有一种可靠的准确的影像模式或技术来诊断胃周围淋巴结转移。CT灌注成像技术,是一种功能成像,用于评估局部血流情况和血液动力学,它可以提供四个灌注参数值和相应功能图,包括血流量和血管通透性参数。可为常规的淋巴结形态学CT图像增加一些血流功能方面的诊断信息,提高对转移性淋巴结的诊断准确率。本研究同时采用低剂量(低管电流:100mAs)联合靶扫描(小扫描野SFOV:20cm×20cm)技术,旨在减少CT灌注检查带来的辐射,同时提高靶病灶图像的空间分辨率。
发明内容
本发明的目的是为了解决现有技术中存在的缺点,而提出的一种低剂量CT灌注的扫描成像系统。
为了实现上述目的,本发明采用了如下技术方案:
一种低剂量CT灌注的扫描成像系统,包括移动台、扫描成像仪和设置在其内部的CT灌注成像扫描系统,所述CT灌注成像扫描系统通讯连接有图像处理系统;
所述CT灌注成像扫描系统包括有主控模块,所述主控模块通讯连接有螺旋CT扫描模块和CT灌注成像扫描模块,所述螺旋CT扫描模块通讯连接有射线发生器、探测器和扫描器,且所述射线发生器、所述探测器和所述扫描器通讯连接至图像采集处理模块,所述图像采集处理模块通讯连接有图像重建模块,所述图像重建模块通讯连接至图像处理系统;
所述射线发生器、所述探测器和所述扫描器通过编码器将扫描图像传输至所述图像处理系统。
优选的,所述成像扫描仪的仪器壳体内腔中安装有金属环,且金属环上安装有螺旋运转的碳刷,碳刷与X线球管连接。
优选的,所述成像扫描仪的仪器壳体前端设置有移动台。
优选的,所述X线球管扫描参数:层厚和层间距均为5毫米、管电压120KV、管电流100mAs、矩阵512×512、螺距0.984:1、扫描野30cm×30cm。
优选的,所述探测器选择型号为FPD1M的平板探测器,探测器运行参数:有效入射野尺寸43cm×43cm、图像分辨率3064×3072、小可检测剂量<100nGy、大线性剂>60μGy、极限分辨率≥3.4lp/mm。
优选的,所述CT灌注成像扫描模块运行前进行CT灌注成像扫描前的对比剂注射,且采用全自动高压注射器经肘部静脉留置针注入造影剂碘佛醇50毫升,注射流速5毫升/秒,注射造影剂后延时7秒开始CT灌注扫描。

本发明的有益效果是:
1、本发明选择胃肿瘤和胃周淋巴结的层面,进行小剂量CT灌注成像靶扫描,通过全自动高压注射器经肘部静脉留置针注入造影剂碘佛醇50毫升,然后利用螺旋CT扫描仪对患者进行灌注扫描,提高了扫描的跨度。
2、本发明采用低剂量(低管电流:100mAs)联合靶扫描(小扫描野SFOV:20cm×20cm)技术,旨在减少CT灌注检查带来的辐射,同时提高靶病灶图像的空间分辨率。
综上所述,低剂量CT灌注成像靶扫描技术能提供胃癌周围淋巴结的血流动力学方面的功能信息,在术前鉴别诊断胃周转移性和反应增生性淋巴结方面,灌注参数BF值是比参数PS或淋巴结大小更有效的标志物,这有助于胃癌的术前精准分期,可减少术中对反应增生性淋巴结的不必要清扫,而改善患者预后。

实施例:一种低剂量CT灌注的扫描成像系统,包括移动台2、扫描成像仪1和设置在其内部的CT灌注成像扫描系统,CT灌注成像扫描系统包括有主控模块,主控模块通过控制开关启动,供电模块用于CT灌注成像扫描仪的电量供应,通讯模块用于扫描数据的有线或无线传输,成像扫描仪的仪器壳体前端设置有移动台2,患者躺在移动台2上,控制移动台2的运转,带动患者向靠近扫描仪的位置侧移动,利用成像扫描仪进行常规腹部CT平扫,成像扫描仪64层螺旋CT扫描仪;
其中,主控模块通讯连接有螺旋CT扫描模块和CT灌注成像扫描模块,螺旋CT扫描模块通讯连接有射线发生器(射线发生器选择X线球管5)、探测器3和扫描器,且射线发生器、探测器3和扫描器通讯连接至图像采集处理模块,图像采集处理模块通讯连接有图像重建模块,图像重建模块通讯连接至图像处理系统;

具体的,在对患者进行CT灌注扫描前10min,肌肉注射抗胆碱能药物654-2 20mg,以抑制胃蠕动,并且在CT灌注检查前5分钟,要求喝800–1200毫升的温水来扩张胃,以更好地观察胃肿瘤,在肘部正中静脉放置18G型号的静脉留置针,便于后续进行对比剂的低剂量注射;
进一步的,在碳刷4沿金属环6运转的带动下,使X线球管5进行螺旋运转扫描,探测器3对X线球管5运转的轨迹进行计数,利用螺旋CT扫描的方式对腹部情况进行扫描后的图像,经编码器采集处理后,传输至图像处理系统,对扫描的图像进行再处理后显示。
其中,CT灌注成像扫描模块运行前进行CT灌注成像扫描前的对比剂注射,且采用全自动高压注射器经肘部静脉留置针注入造影剂碘佛醇(300mgI/ml)50毫升,注射流速5毫升/秒,注射造影剂后延时7秒开始CT灌注扫描,CT灌注成像扫描模块的运行扫描参数:管电压120KV、低管电流100mAs、矩阵512×512、螺距0.984:1、小扫描野(SFOV)20cm×20cm。
具体的,对患者进行腹部CT平扫后,选择胃肿瘤和胃周淋巴结的层面,进行小剂量CT灌注成像靶扫描,首先通过全自动高压注射器经肘部静脉留置针注入造影剂碘佛醇(300mgI/ml)50毫升,然后利用螺旋CT扫描仪对患者进行灌注扫描,根据64层螺旋CT扫描的工作原理,采用滑环技术,将电源电缆和一些信号线与固定机架内不同金属环6相连运动的X线球管5和探测器3滑动电刷与金属环6导联,球管和探测器3不受电缆长度限制,沿人体长轴连续匀速旋转,扫描床同步匀速递进,扫描轨迹呈螺旋状前进,可快速、不间断地完成容积扫描;
进一步的,利用螺旋CT扫描后的图像数据,经编码器(将信号(如比特流)或数据进行编制、转换为可用以通讯、传输和存储的信号形式的设备)将数据传输至图像处理系统,利用图像处理系统处理后得到CT灌注图像;
再进一步的,利用螺旋CT扫描后的图像数据,经过图像采集处理模块采集的图像,对扫描的图像进行集中对比整合,利用图像重建模块进行图像重建,保证扫描出图像的完整性。
其中,CT灌注成像扫描系统通讯连接有图像处理系统,射线发生器、探测器3和扫描器通过编码器将扫描图像传输至图像处理系统;
图像处理系统选择为西门子后处理工作站系统,并选择腹部肿瘤灌注协议的CT灌注3.0软件用于接收的图像数据处理,以腹主动脉作为输入动脉,通过后处理工作站得到时间-密度曲线和伪彩CT灌注图像。
具体的,把低剂量CT灌注靶扫描的数据传输到西门子后处理工作站系统(ADW4.0),选择腹部肿瘤灌注协议的CT灌注3.0软件(去卷积法)用于数据处理,以腹主动脉作为输入动脉,通过后处理工作站得到时间-密度曲线和伪彩CT灌注图像,伪彩由灰白到黑白,灰白色代表局部高血流量及高通透性,相反黑白色代表低血流量及低通透性。然后在胃癌肿块及胃周淋巴结上放置感兴趣区(ROI),ROI尽可能大,以减少噪声,从而获得相应的CT灌注参数值,即血流量(BF)和血管表面通透性(PS)值;
进一步的,根据利用低剂量CT灌注扫描成像系统,对证实胃镜活检病理为胃癌的患者进行病例记载:常规ct检查发现有胃周淋巴结,无远处转移。病例排除标准:对比剂过敏,肾功能受损,严重的CT灌注伪影,患者例情况如下记载:男性38人,女性22人(中位年龄57岁,年龄范围:36-74岁)。根据胃肿瘤的位置,28例位于胃贲门区,12例位于胃体区,20例位于胃窦区。根据肿瘤细胞分化的程度,10例为高分化胃癌,27例为中分化胃癌,23例为低分化胃癌。根据肿瘤组织的病理类型,包括管状腺癌38例、印戒细胞癌10例、粘液腺癌12例。
其中,60例胃癌患者共115个胃周淋巴结,分为转移性淋巴结组(N=65)和炎性反应性淋巴结组(N=50);转移性淋巴结的短径从0.6厘米到2.5厘米(平均1.3厘米),而炎性反应性淋巴结的短径从0.4厘米到1.8厘米(平均1.1厘米)。所有病例在ct灌注检查后一周内进行胃肿瘤切除和胃周围淋巴结清扫术。术后标本送至病理科,得到病理诊断结果作为金标准,与CT灌注参数值进行对照分析。

本申请旨在评估低剂量CT灌注成像靶扫描技术在鉴别胃癌周围转移性和反应增生性淋巴结方面的诊断效能。
方法:通过胃镜检查病理确诊的60例胃癌患者共115个胃周淋巴结进行手术前低剂量CT灌注成像靶扫描。然后通过体部CT灌注自带的商业软件对扫描数据进行后处理,以计算包括血流量(BF)和表面通透性(PS)两个灌注参数值,并测量胃周淋巴结的大小、灌注参数值以及胃肿瘤的灌注参数值。根据手术后淋巴结的病理结果作为金标准,将胃周淋巴结分为两组:转移性和反应增生性淋巴结。通过独立样本t检验比较两组淋巴结的灌注参数值和大小,并使用受试者特性操作曲线(ROC)比较各个参数的诊断效能,并确定各个参数的最佳诊断截值、灵敏度、特异性和曲线下面积(AUC)。同时,运用spearman检验分析胃周淋巴结的灌注参数值与胃肿瘤的微血管密度(MVD),Ki-67和血管内皮生长因子(VEGF)表达的相关性。
结果:115个胃癌周围淋巴结中,转移性65枚(56.52%),反应增生性50枚(43.48%)。转移性和反应增生性两组淋巴结的灌注参数平均值分别为,BF:90.05vs.79.31ml/100mg/min(p<0.01),PS:42.19vs.35.89ml/100mg/min(p<0.01)。两组淋巴结大小平均值为,1.34cm vs.1.16cm(p>0.05)。灌注参数BF鉴别诊断两组淋巴结的最佳截值为80.76ml/100mg/min,敏感性为81.5%,特异性为66.0%,AUC为0.784,优于参数PS或淋巴结大小(P<0.05)。胃周淋巴结BF值与胃肿瘤的MVD相关,相关系数为0.853(p<0.01);同样,胃周淋巴结的PS值与胃肿瘤的VEGF相关(p<0.01)。而胃肿瘤的Ki-67水平与胃周淋巴结的灌注参数及大小均无相关(p>0.05)。
结论:低剂量CT灌注成像靶扫描技术能提供胃癌周围淋巴结的血流动力学方面的功能信息,在术前鉴别诊断胃周转移性和反应增生性淋巴结方面,灌注参数BF值是比参数PS或淋巴结大小更有效的标志物,这有助于胃癌的术前精准分期,可减少术中对反应增生性淋巴结的不必要清扫,从而改善患者预后。

Claims (8)
1.一种低剂量CT灌注的扫描成像系统,包括移动台(2)、扫描成像仪(1)和设置在其内部的CT灌注成像扫描系统,其特征在于,所述CT灌注成像扫描系统通讯连接有图像处理系统;
所述CT灌注成像扫描系统包括有主控模块,所述主控模块通讯连接有螺旋CT扫描模块和CT灌注成像扫描模块,所述螺旋CT扫描模块通讯连接有射线发生器、探测器(3)和扫描器,且所述射线发生器、所述探测器(3)和所述扫描器通讯连接至图像采集处理模块,所述图像采集处理模块通讯连接有图像重建模块,所述图像重建模块通讯连接至图像处理系统;
所述射线发生器、所述探测器(3)和所述扫描器通过编码器将扫描图像传输至所述图像处理系统。
2.根据权利要求1所述的一种低剂量CT灌注的扫描成像系统,其特征在于,所述成像扫描仪的仪器壳体内腔中安装有金属环(6),且金属环(6)上安装有螺旋运转的碳刷(4),碳刷(4)与X线球管(5)连接。
3.根据权利要求2所述的一种低剂量CT灌注的扫描成像系统,其特征在于,所述成像扫描仪的仪器壳体前端设置有移动台(2)。
4.根据权利要求2所述的一种低剂量CT灌注的扫描成像系统,其特征在于,所述X线球管(5)扫描参数:层厚和层间距均为5毫米、管电压120KV、管电流100mAs、矩阵512×512、螺距0.984:1、扫描野(FOV)30cm×30cm。
5.根据权利要求1所述的一种低剂量CT灌注的扫描成像系统,其特征在于,所述探测器(3)选择型号为FPD1M的平板探测器(3),探测器(3)运行参数:有效入射野尺寸43cm×43cm、图像分辨率3064×3072、小可检测剂量<100nGy、大线性剂>60μGy、极限分辨率≥3.4lp/mm。
6.根据权利要求1所述的一种低剂量CT灌注的扫描成像系统,其特征在于,所述CT灌注成像扫描模块运行前进行CT灌注成像扫描前的对比剂注射,且采用全自动高压注射器经肘部静脉留置针注入造影剂碘佛醇50毫升,注射流速5毫升/秒,注射造影剂后延时7秒开始CT灌注扫描。
7.根据权利要求6所述的一种低剂量CT灌注的扫描成像系统,其特征在于,所述CT灌注成像扫描模块的运行扫描参数:管电压120KV、低管电流100mAs、矩阵512×512、螺距0.984:1、小扫描野20cm×20cm。
8.根据权利要求1所述的一种低剂量CT灌注的扫描成像系统,其特征在于,所述图像处理系统选择为西门子后处理工作站系统,并选择腹部肿瘤灌注协议的CT灌注3.0软件用于接收的图像数据处理,以腹主动脉作为输入动脉,通过后处理工作站得到时间-密度曲线和伪彩CT灌注图像。
