Abstract
透视下气道肿瘤内药物注射装置,包括外鞘管和内推杆,内推杆同轴线插设在外鞘管内,内推杆的后端部设置有拇指扣,外鞘管的后端部设置有食指扣,内推杆的长度大于外鞘管,内推杆的前端固定有安装环,安装环为六边形,安装环上安装有注药组件;本发明通过控制食指将注药针头向前推出外鞘管,通过注药撑开气囊将注药针头膨开并穿刺进肿瘤组织内,注药针头均匀分布,可以将化疗药物均匀地输入到肿瘤组织内,化疗效果好,且避免对肿瘤组织多次穿刺,减少操作步骤,避免多次穿刺导致的肿瘤扩散。
Description
技术领域
本发明属于医疗技术领域,具体涉及一种透视下气道肿瘤内药物注射装置。
背景技术
呼吸道又称气道,是肺呼吸时气流所经过的通道,以环状软骨下缘为界,将鼻、咽、喉合称上呼吸道;气管、支气管和肺部器官,合称为下呼吸道。气道内肿瘤分为原发性肿瘤和继发性肿瘤,原发性气管恶性肿瘤大多生长于软骨环与膜部交界处。鳞状上皮细胞癌可呈现为突入气管腔的肿块或溃破形成溃疡,有时癌变可浸润整段气管。囊性腺样癌一般生长较为缓慢,较晚发生转移,有时呈现长段粘膜下浸润或向纵隔内生长,有的肿瘤呈哑铃状,小部分突入气管腔,大部分位于纵隔内,晚期病例可侵入纵隔和支气管。 原发性气管良性肿瘤种类多,形态不一,生长缓慢,表面光滑,粘膜完整,常有瘤蒂,不发生转移,但如切除不彻底易复发。乳头状瘤多发生于气管膜部,突入气管腔底部,常有细蒂,大小自数毫米至2cm。有时为多发性,表面呈疣状,质软而脆易脱落,破裂时出血。
继发性肿瘤中,胸部肿瘤(如肺癌、食管癌)晚期极易侵犯或转移至气管,形成气管(环状软骨以下,隆突以上)的新生物, 一旦发生气道受侵或转移,治疗方法有限,手术切除难度大、风险高,且患者一般情况差,往往不能耐受手术治疗。支架置入治疗可以暂时改善气道通气,但随着肿瘤组织浸润、再生长以及肉芽组织增生均可至支架阻塞。气道内肿瘤可以直接采用肿瘤内化疗药物注射的方式,尤其是对于不能手术切除或者放置支架后肿瘤突入支架内的患者。
现在向肿瘤内注入药物的方式为:将一个头端带有钩状针头、尾端连接有注药管的注射装置,其外部设置套筒,然后将该装置插入到患者气道内,再将针头推出外鞘并插入到肿瘤组织内进行注射药物,该装置在进行注射时只有一个注药针头,对于环绕气道内生长的肿瘤需要进行多点穿刺均匀给药,以保证注药的效果,此时再用一个注药针头进行多次穿刺时难以保证均匀穿刺气道周壁,使注药效果下降,而且多次穿刺时其针头内带有肿瘤细胞,相当于人工将肿瘤细胞种植到其他部位,导致肿瘤扩散。
发明内容
本发明为了解决现有技术中的不足之处,提供一种避免多次穿刺注射药物、可以同时进行多点穿刺均匀给药的透视下气道肿瘤内药物注射装置。
为解决上述技术问题,本发明采用如下技术方案:透视下气道肿瘤内药物注射装置,包括外鞘管和内推杆,内推杆同轴线插设在外鞘管内,内推杆的后端部设置有拇指扣,外鞘管的后端部设置有食指扣,内推杆的长度大于外鞘管,内推杆的前端固定有安装环,安装环为六边形,安装环上安装有注药组件;
注药组件包括注药撑开气囊和四组注药件,四组注药件均布在安装环的四条棱上,每组注药件包括连动杆和注药针头,连动杆的后端通过铰套铰接在安装环的棱上,注药针头固定在连动杆的前端部外侧,注药针头向后延伸设置且注药针头与连动杆的夹角为45°-60°;注药撑开气囊为环形气囊,注药撑开气囊连接在四组注药件的连动杆前端部内侧之间;
所述的内推杆的后端部还安装有注药接头和充气接头,充气接头通过充气软管与注药撑开气囊连接,注药接头通过注药软管分别与四组注药件的注药针头连接,所述的充气软管内置于内推杆内,注药软管内置于内推杆和连动杆内。
注药针头的长度为2.3-2.8mm。
外鞘管为14F导管,内推杆为12F导管。
采用上述技术方案,本发明具有以下有益效果:本发明通过控制食指将注药针头向前推出外鞘管,通过注药撑开气囊将注药针头膨开并穿刺进肿瘤组织内,注药针头均匀分布,可以将化疗药物均匀地输入到肿瘤组织内,化疗效果好,且避免对肿瘤组织多次穿刺,减少操作步骤,避免多次穿刺导致的肿瘤扩散。

具体实施方式
如图1-4所示,本发明的透视下气道肿瘤内药物注射装置,包括外鞘管1和内推杆2,内推杆2同轴线插设在外鞘管1内,内推杆2的后端部设置有拇指扣3,外鞘管1的后端部设置有食指扣4,内推杆2的长度大于外鞘管1,内推杆2的前端固定有安装环5,安装环5为六边形,安装环5上安装有注药组件;
注药组件包括注药撑开气囊6和四组注药件,四组注药件均布在安装环5的四条棱上,每组注药件包括连动杆7和注药针头8,连动杆7的后端通过铰套9铰接在安装环5的棱上,注药针头8的长度为2.3-2.8mm,注药针头8固定在连动杆7的前端部外侧,注药针头8向后延伸设置且注药针头8与连动杆7的夹角为45°-60°;注药撑开气囊6为环形气囊,注药撑开气囊6连接在四组注药件的连动杆7前端部内侧之间;
所述的内推杆2的后端部还安装有注药接头10和充气接头11,充气接头11通过充气软管与注药撑开气囊6连接,注药接头10通过注药软管分别与四组注药件的注药针头8连接,所述的充气软管内置于内推杆2内,注药软管内置于内推杆2和连动杆7内。
外鞘管1为14F导管,内推杆2为12F导管,装置整体为软质导管,具有较好的顺应性,方便插入。
本发明在使用时,操作者将拇指插入内推杆2后端部的拇指扣3内,食指扣4入到食指扣4内,将装置整体放入气道内,然后通过向后拉动食指扣4从而将外鞘管1向后移动,使得注药组件脱离外鞘管1内腔后暴露在外侧,然后通过充气接口向注药撑开气囊6内注入气体,注药撑开气囊6充气后逐渐膨胀,注药撑开气囊6膨胀并向四周撑开四组注药件的连动杆7,连动杆7通过铰套9绕安装环5的棱转动,从而连动杆7前端部外侧的注药针头8向后运动并刺入肿瘤组织内,通过透视观察注药针头8刺入肿瘤组织内深度是否合适,直至合适后,停止注入气体,完成穿刺工作;紧接着通过注药接口向注药软管内注药,药物经注药软管流向四个注药针头8,最终由注药针头8注入到肿瘤组织内,即可进行化疗;
待药物注射完毕时,排出注药撑开气囊6内的气体,向后拉动拇指扣3,四根连动杆7即可收回至外鞘管1内。

Claims (3)
1.透视下气道肿瘤内药物注射装置,其特征在于:包括外鞘管和内推杆,内推杆同轴线插设在外鞘管内,内推杆的后端部设置有拇指扣,外鞘管的后端部设置有食指扣,内推杆的长度大于外鞘管,内推杆的前端固定有安装环,安装环为六边形,安装环上安装有注药组件;
注药组件包括注药撑开气囊和四组注药件,四组注药件均布在安装环的四条棱上,每组注药件包括连动杆和注药针头,连动杆的后端通过铰套铰接在安装环的棱上,注药针头固定在连动杆的前端部外侧,注药针头向后延伸设置且注药针头与连动杆的夹角为45°-60°;注药撑开气囊为环形气囊,注药撑开气囊连接在四组注药件的连动杆前端部内侧之间;
所述的内推杆的后端部还安装有注药接头和充气接头,充气接头通过充气软管与注药撑开气囊连接,注药接头通过注药软管分别与四组注药件的注药针头连接,所述的充气软管内置于内推杆内,注药软管内置于内推杆和连动杆内。
2.根据权利要求1所述的透视下气道肿瘤内药物注射装置,其特征在于:注药针头的长度为2.3-2.8mm。
3.根据权利要求1所述的透视下气道肿瘤内药物注射装置,其特征在于:外鞘管为14F导管,内推杆为12F导管。
