本发明涉及一种基于18F‑FDG的模拟脑血流灌注参数成像方法,先确定原始PET影像的各像素点的像素值C对应的单位质量组织内的流量f,再以f的数值作为PET影像的像素值C绘制新的PET影像;f是通过查表确定的,表的建立过程为:首先确定p的取值范围,然后得到给定p值下f和C/k的关系曲线,最后从关系曲线上选取多个点,建立可根据p和C/k查询f的表;f的确定过程为:首先确定p和k,然后遍历原始PET影像中的所有像素值C,对不同的C计算C/k,最后判断C/k是否出现在表中并对应p,如果是,则从表中查找C/k对应的f;反之,则根据表中的数据计算C/k对应的f的近似值。本发明大大缩短了绘制影像所需的时间。

技术领域
本发明属医学影像技术领域,涉及一种基于18F-FDG的模拟脑血流灌注参数成像方法。
背景技术
灌注成像是一种成像方法,其用于捕获对应于流体(例如血液、淋巴液等)通过所灌注的解剖组织(例如大脑、肌肉或心脏区等等)的信息,并且使该信息量化,以便于确定该组织或其它组织的健康状况,对临床诊断及治疗均有重要参考价值。灌注成像常用的成像模式包括计算机断层扫描(CT)、正电子发射断层摄影(PET)等。
PET是反映病变的基因、分子、代谢及功能状态的显像设备。它是利用正电子核素标记葡萄糖等人体代谢物作为显像剂,通过病灶对显像剂的摄取来反映其代谢变化,从而为临床提供疾病的生物代谢信息。是当今生命科学、医学影像技术发展的新里程碑。PET利用正电子发射体的核素标记一些生理需要的化合物或代谢底物如葡萄糖、脂肪酸、氨基酸、受体的配体及水等,引入体内后,应用正电子扫描机而获得的体内化学影像。它以其能显示脏器或组织的代谢活性及受体的功能与分布而受到临床广泛的重视,也称之为“活体生化显像”。可以说,PET的出现使得医学影像技术达到了一个崭新的水平,使无创伤性的、动态的、定量评价活体组织或器官在生理状态下及疾病过程中细胞代谢活动的生理、生化改变,获得分子水平的信息成为可能,这是目前其他任何方法所无法实现的。因此,在发达国家,PET广泛应用于临床,已成为肿瘤、冠心病和脑部疾病这三大威胁人类生命疾病诊断和指导治疗的最有效手段。目前最常用的PET显像剂为18F标记的FDG(18F-FDG氟化脱氧葡萄糖),是一种葡萄糖的类似物,已被用作以下医疗领域:
1)癫痫定位:对脑癫痫病灶准确定位,为外科手术或伽玛刀切除癫痫病灶提供依据;
2)脑肿瘤定性和复发判断:脑肿瘤的良恶性定性、恶性胶质瘤边界的确定、肿瘤治疗后放射性坏死与复发的鉴别、肿瘤活检部位的选择等;
3)痴呆早期诊断:早老性痴呆的早期诊断、分期并与其他类型痴呆如血管性痴呆进行鉴别;
4)脑受体研究:帕金森病的脑受体分析,进行疾病的诊断和指导治疗;
5)脑血管疾病:PET可以敏感地捕捉到脑缺血发作引起的脑代谢变化,因此可以对一过性脑缺血发作(TIA)和脑梗死进行早期诊断和定位,并进行疗效评估和预后判断;
6)药物研究:进行神经精神药物的药理学评价和指导用药,观察强迫症等患者脑葡萄糖代谢的变化情况,为立体定向手术治疗提供术前的依据和术后疗效随访等;
7)高级健康体检:早期肿瘤是可以得到治愈的,但大部分肿瘤发现时已经是中晚期了,故肿瘤的常规筛查不可忽视,PET/CT简便,安全,全面,准确,是人群健康体检的最佳手段。
由于烟雾病等脑血管疾病需要对脑组织血流灌注情况进行分析,而常规PET图像只能显示代谢功能,因而需要脑血流灌注的模拟,模拟过程即根据原始PET影像绘制新的PET影像的过程,模拟过程具体为:先确定原始PET影像的各像素点的像素值C对应的单位质量组织内的流量f,再以f的数值作为PET影像的像素值C绘制新的PET影像,其中,f是通过以下方程计算得到的:
f*exp(-f/p)=C/k;
式中,C是PET测量的局部组织活性值,p和k是常数;
f的计算过程等价于在给定C、p、k的情况下求解以上方程式的根的过程,因为C由测量得到,在PET影像中随像素位置不同而不同,所以实际计算时只需给定p和k。
然而,由于计算f所用的方程是一个超越方程,求解速度很慢,而PET影像往往包含200×200×200以上的像素,直接求解每个像素对应的f所需的时间往往要以小时计算,极大地影响了临床应用的效率。
因此,研究一种能有效缩短计算时间的模拟脑血流灌注成像的方法具有十分重要的意义。
发明内容
本发明的目的是解决现有技术中模拟脑血流灌注成像计算时间较长的问题,提供一种基于18F-FDG的模拟脑血流灌注参数成像方法。本发明通过查表法大大加快了模拟灌注影像的计算过程,并能够很好地应用于实际临床工作。
为达到上述目的,本发明采用的方案如下:
基于18F-FDG的模拟脑血流灌注参数成像方法,先确定原始PET影像的各像素点的像素值C对应的单位质量组织内的流量f,再以f的数值作为PET影像的像素值C绘制新的PET影像,即实现模拟灌注影像;
原始PET影像对应的显像剂为18F-FDG;PET影像中的每个像素点的像素值代表的是局部组织活性;
f是通过查表确定的,表的建立过程为:首先根据方程(I)确定p的取值范围,然后根据方程(II)得到给定p值下f和C/k的关系曲线,最后从关系曲线上选取多个点,建立可根据p和C/k查询f的表,表的数量与p值的个数相同,一个p值对应一张表,各表反应的是给定p值下f和C/k的对应关系,例如为一张两行多列的表,第一行中各列为C/k值,第二行中各列为与第一行各列对应的f值;
p和k是由方程(I)中的时间t1和t2确定的积分常数,方程(I)的表达式如下:
式中,C是PET测量的局部组织活性,Ci(t)是该区域的放射性示踪剂浓度在时间t的值,可以从断层的定量放射自显影推导获得,CA(t)*是动脉血放射性浓度测定的时间函数,λ是脑组织与血液平衡分配系数的示踪,t1和t2为造影剂注射后的任意两个时间点;
方程(II)的表达式如下:
f*exp(-f/p)=C/k;
f的确定过程为:首先随机取值确定p和k(随机取值时二者的取值范围根据方程(I)确定),然后遍历原始PET影像的各像素点的像素值C,对不同的C计算C/k,最后判断C/k是否出现在表中并对应p,如果是,则从表中查找C/k对应的f;反之,则根据表中的数据计算所述计算得到的C/k对应的f的近似值。
作为优选的方案:
如上所述的基于18F-FDG的模拟脑血流灌注参数成像方法,所述关系曲线为横坐标为C/k且纵坐标为f的曲线。
如上所述的基于18F-FDG的模拟脑血流灌注参数成像方法,从关系曲线上选取的多个点中,相邻两点的横坐标值的差值为0.01。
如上所述的基于18F-FDG的模拟脑血流灌注参数成像方法,从关系曲线上选取的多个点分布在关系曲线的整段上。建表时从关系曲线的整段上选取多个点,并控制相邻两点的横坐标值的差值为0.01,使得表中的数据充实,便于后续查找应用。
如上所述的基于18F-FDG的模拟脑血流灌注参数成像方法,所述计算得到的C/k对应的f的近似值的计算过程为:首先确定a和b,a为对应p、小于所述计算得到的C/k且出现在表中的C/k中的最大值,b为对应p、大于所述计算得到的C/k且出现在表中的C/k中的最小值,然后从表中查出a和b对应的f值,最后求取所述计算得到的C/k对应的f的近似值。
如上所述的基于18F-FDG的模拟脑血流灌注参数成像方法,求取采用插值法,求取采用的方法不限于此,其他能够计算得到所述计算得到的C/k对应的f的近似值的方法都可适用于本发明。
如上所述的基于18F-FDG的模拟脑血流灌注参数成像方法,插值法为线性插值法,还可以为多项式插值法等等,由于线性插值法相对于其他插值法具有简单、计算速度快等优点,因而作为本发明优选的方法。
如上所述的基于18F-FDG的模拟脑血流灌注参数成像方法,在表已经建立的前提下,绘制新的PET影像所需的时间为0.122844秒,相对于现有技术绘制新的PET影像需要数小时,本发明显著提高了临床应用的效率。
如上所述的基于18F-FDG的模拟脑血流灌注参数成像方法,绘制新的PET影像采用CoreTM i7-7700HQ CPU,具体采用的软件不限于此,此处仅列举一种可用的软件。
如上所述的基于18F-FDG的模拟脑血流灌注参数成像方法,新的PET影像的分辨率为400×400×148。
有益效果:
(1)本发明的基于18F-FDG的模拟脑血流灌注参数成像方法,利用查表法加速超越方程的求解,从而大大缩短了绘制影像所需的时间;
(2)本发明的基于18F-FDG的模拟脑血流灌注参数成像方法,可以实际应用于PET影像的临床分析,极具推广价值。

假设葡萄糖是自由扩散,局部脑血流(rCBF)单位质量组织内的流量f通过求解方程获得;在脑中,水的示踪分配系数的灰质和白质值平均为0.95,f的计算等价于在给定C、p、k的情况下求解方程(II)的根,方程(II)的表达式如下:
f*exp(-f/p)=C/k;
因为C由测量得到,在PET影像中随像素位置不同而不同,所以实际计算时只需给定p和k即可,如图1是给定p=28.5时f和C/k的关系曲线。
由方程的根f代替原始PET的像素值C而绘制的新的PET影像,称为模拟灌注影像。由于方程(II)是一个超越方程,求解速度很慢,而PET影像往往包含200×200×200以上的像素,直接求解每个像素对应的f所需的时间往往要以小时计算,极大的影响了临床应用的效率。
由于解的曲线是一个单调递增曲线,因此本发明采用了查表法来优化对实际PET影像的每个像素点求解此方程的过程,先将给定p值的(f,C/k)的对应关系一次性计算出来并以C/k为索引建立可查询的表,再遍历PET影像的各像素点的像素值C,对不同的C先计算C/k,再通过查表确定C/k对应的f。
下面将具体介绍本发明的基于18F-FDG的模拟脑血流灌注参数成像方法:
基于18F-FDG的模拟脑血流灌注参数成像方法,具体过程为:先确定原始PET影像的各像素点的像素值C对应的单位质量组织内的流量f,再以f的数值作为PET影像的像素值后采用CoreTM i7-7700HQ CPU绘制新的PET影像,即实现模拟脑血流灌注参数成像;
原始PET影像对应的显像剂为18F-FDG;PET影像中的每个像素点的像素值代表的是局部组织活性;

根据计算时间可知,本发明通过查表法大大加快了模拟灌注影像的计算,使其可以真正应用于实际临床工作。

1.基于18F-FDG的模拟脑血流灌注参数成像方法,其特征是:先确定原始PET影像的各像素点的像素值C对应的单位质量组织内的流量f,再以f的数值作为PET影像的像素值C绘制新的PET影像,即实现模拟脑血流灌注参数成像;
原始PET影像对应的显像剂为18F-FDG;PET影像中的每个像素点的像素值代表的是局部组织活性;

p和k是由方程(I)中的时间t1和t2确定的积分常数,方程(I)的表达式如下:
式中,C是PET测量的局部组织活性,Ci(t)是该区域的放射性示踪剂浓度在时间t的值,CA(t)*是动脉血放射性浓度测定的时间函数,λ是脑组织与血液平衡分配系数的示踪,t1和t2为造影剂注射后的任意两个时间点;
方程(II)的表达式如下:
f*exp(-f/p)=C/k;
f的确定过程为:首先随机取值确定p和k,然后遍历原始PET影像的各像素点的像素值C,对不同的C计算C/k,最后判断计算得到的C/k是否出现在表中并对应p,如果是,则从表中查找所述计算得到的C/k对应的f;反之,则根据表中的数据计算所述计算得到的C/k对应的f的近似值。
2.根据权利要求1所述的基于18F-FDG的模拟脑血流灌注参数成像方法,其特征在于,所述关系曲线为横坐标为C/k且纵坐标为f的曲线。
3.根据权利要求2所述的基于18F-FDG的模拟脑血流灌注参数成像方法,其特征在于,从关系曲线上选取的多个点中,相邻两点的横坐标值的差值为0.01。
4.根据权利要求3所述的基于18F-FDG的模拟脑血流灌注参数成像方法,其特征在于,从关系曲线上选取的多个点分布在关系曲线的整段上。
5.根据权利要求1所述的基于18F-FDG的模拟脑血流灌注参数成像方法,其特征在于,所述计算得到的C/k对应的f的近似值的计算过程为:首先确定a和b,a为对应p、小于所述计算得到的C/k且出现在表中的C/k中的最大值,b为对应p、大于所述计算得到的C/k且出现在表中的C/k中的最小值,然后从表中查出a和b对应的f值,最后求取所述计算得到的C/k对应的f的近似值。
6.根据权利要求5所述的基于18F-FDG的模拟脑血流灌注参数成像方法,其特征在于,求取采用插值法。
7.根据权利要求6所述的基于18F-FDG的模拟脑血流灌注参数成像方法,其特征在于,插值法为线性插值法。
8.根据权利要求1所述的基于18F-FDG的模拟脑血流灌注参数成像方法,其特征在于,在表已经建立的前提下,绘制新的PET影像所需的时间为0.122844秒。
9.根据权利要求8所述的基于18F-FDG的模拟脑血流灌注参数成像方法,其特征在于,绘制新的PET影像采用CoreTMi7-7700HQ CPU。
