在过去的几十年中,已经证明由塑料制成的植入物(特别是由UHMWPE制成的植入物)不应与骨头直接接触。UHMWPE是一种疏水性材料,并且因此为骨组织生长到植入物的表面中提供了非常不利的条件。因此,在骨组织与植入物的表面之间的边界面处持续发生有微运动,这可能导致结缔组织变形并且导致植入物上的塑料的退化。为了避免这个问题,完全由塑料制成的膝关节内置假体的胫骨部件用骨水泥进行固定。因此,在植入物的骨接触面和骨组织之间分别形成有水泥外壳或水泥层。因此,骨水泥一方面用于将植入物分别固定在骨头中和骨头上,并且同时还用作塑料与骨组织之间的分离层。
完全由塑料制成的胫骨部件的柄部的粘合的问题在于它可能导致柄部的端部处的应力峰值。这些不会发生在柄部区域中没有进行粘合的胫骨部件的植入中。在最坏的情况下,所描述的应力峰值可能导致胫骨断裂,即,包围柄部的骨头断裂。
发明内容
因此,本发明的一个目的是改进开头所述类型的植入物或关节植入物,使得可以至少部分地省去骨水泥固定。
根据本发明,该目的在开头所述类型的植入物中实现,其中,骨接触面设置有或涂覆有第一生物相容的骨接触层或承载有生物相容的骨接触层。
所提出的、对已知植入物的进一步发展特别是能够在不使用骨水泥的情况下将植入物固定在骨头上或骨头中。特别地是,柄部(柄部的所述至少一个骨接触面设置有或涂覆有第一生物相容的骨接触层或承载有第一生物相容的骨接触层)直接与骨头或骨组织接触,而不会发生上述不期望的后果。因此,特别地是,可以省去柄部的粘合,使得也不会发生上述应力峰值,并且显着降低了胫骨断裂的风险。当然,植入物也可以具有可以与骨接触的面。如果这些面未设置有生物相容的骨接触层,则这些面可以例如通过骨水泥层与骨组织分离。特别地是,胫骨板因此可以借助于骨水泥固定在胫骨的面向股骨方向的已适当备好的骨面上。
植入物的柄部有利地是意图在没有骨水泥的情况下被锚固在骨腔中。“意图”和“意图目的”分别表示这样的情况:柄部由能够与骨组织直接接触的材料制成或涂覆有能够与骨组织直接接触的材料,而不会发生上述不期望的副作用,如这些副作用是从塑料与骨组织直接接触的塑料植入物已知的。因此,由于生物相容的骨接触层,以这种方式形成的柄部可以在没有骨水泥的情况下被锚固在骨腔中,即,没有可能发生结缔组织变形或由塑料制成的柄部的局部损坏的风险。
有利的是:植入物包括基体以及柄部远离基体的下侧延伸。基体可以被构造成例如具有滑动面的胫骨板的形式,该滑动面用于邻接抵靠股骨部件的相对应的滑动面。柄部远离基体延伸用于将植入物优化地锚固在骨头上。
为了进一步改进植入物在骨头上的固定,特别地是可以规定:基体具有基体骨接触面,以用于邻接抵靠已备好的骨面。因此,换句话说,不仅柄部可以与骨组织相接触,而且基体或植入物的另一部分也可以与骨组织相接触。如果基体骨接触面未设置有生物相容的骨接触层,则可以借助于骨水泥将植入物固定在骨头上,以避免塑料与骨组织接触的缺点。
可以更有利的是:基体骨接触面设置有第二生物相容的骨接触层或承载第二生物相容的骨接触层。这种进一步的发展特别地是允许在完全没有骨水泥的情况下将植入物锚固在骨头上。因此,可以以简单的方式避免塑料与骨组织接触的上述不期望的缺点。
此外,有利的是:基体意图是在没有骨水泥的情况下被锚固在骨头上。“意图”和“意图目的”在这里分别也指基体由能够与骨组织直接接触的材料制成或涂覆有能够与骨组织直接接触的材料,而不会发生上述不期望的副作用,如这些副作用是从塑料与骨组织直接接触的塑料植入物已知的。
如果第一骨接触层和第二骨接触层具有相同的构造,则可以特别简单地形成植入物。特别地是,例如,如果植入物被形成为单件,特别是整体形成,则骨接触层可以在一个制造步骤中涂敷。
特别有利的是:第一骨接触层和/或第二骨接触层的厚度在约1μm至约1000μm的范围内。特别地是,它们的厚度可以在约5μm至约700μm的范围内。将骨接触层的厚度在指定范围内形成具有以下优点:不管植入物具有有利的性质,即,能够在没有骨水泥的情况下被植入,可以特别容易地和/或轻重量地形成植入物。第一骨接触层和/或第二骨接触层优选地至少厚得以使得可以可靠地避免制造植入物的塑料与骨组织之间的直接骨接触。
为了分别改进骨组织的向内生长以及骨组织生长到植入物上,有利的是:第一骨接触层和/或第二骨接触层形成为粗糙的。它们的粗糙度优选在约0.5μm至约10μm的范围内。
为了能够形成具有期望的粗糙度的第一骨接触层和/或第二骨接触层,并且同时简单地制造植入物,有利的是:骨接触面和/或基体骨接触面的粗糙度在约0.5μm至约10μm的范围内。植入物的相应表面(其意图目的是与骨组织相接触)然后可以以简单的方式涂覆有薄的骨接触层,其中,骨接触面和/或基体骨接触面的粗糙度可以被保持或者基本上被保持。这可以例如通过借助于冷气体喷射和/或通过物理和/或化学气相沉积来涂敷骨接触层而进行。
有利的是:第一骨接触层和/或第二骨接触层由至少一种金属制成和/或由生物相容的塑料制成和/或由陶瓷制成和/或由至少一种骨矿物质制成。取决于制造植入物的塑料,可以制造合适的材料,以形成第一骨接触层和/或第二骨接触层。
所述至少一种金属优选为钛或包含钛。因此,它特别地也可以是包含钛和其它生物相容的金属的合金。
生物相容的塑料有利地是聚醚醚酮(PEEK)或包含聚醚醚酮(PEEK)。PEEK最佳地适合于形成轻重量但仍然是生物相容的植入物,特别是还要涂覆生物不相容的塑料。
可以更有利的是:骨矿物质是羟基磷灰石。羟基磷灰石出现在生物中,并且特别是在人体内通过生物矿化出现。它以约40%的比例包含在骨头中。因此,可以形成骨头与植入物之间的特别骨相容的接触。
如果通过冷气体喷射和/或通过物理和/或化学气相沉积而形成第一骨接触层和/或第二骨接触层,则可以以简单的方式实现由塑料制成的植入物的涂层。特别地是,因此可以形成例如在上面指定的厚度范围内的薄的骨接触层。
第一骨接触层和/或第二骨接触层优选地形成为完全闭合的。以这种方式,特别地是,可以确保骨组织不与制造植入物的塑料相接触。
可以更有利的是:除第一骨接触层和/或第二骨接触层之外,植入物完全由生物不相容的塑料制成。这特别地能够使植入物和限定该植入物的植入物基体分别由塑料生产,所述塑料具有形成植入物所需的期望的机械性能。特别地是,不必考虑所述塑料是否是生物相容的,因为由于第一骨接触层和/或第二骨接触层而可以避免生物不相容的塑料与患者的骨组织之间的直接接触。
特别地是,为了在关节植入物中形成滑动面,有利的是:非生物相容的塑料是聚乙烯或超高分子量聚乙烯(UHMWPE)。特别地是,滑动面因此可以与植入物一体地或整体地形成。因此,可以以简单的方式最小化内置假体所需零件的数目。
植入物有利地是形成为单件。特别地是,它可以整体形成。植入物在此特别地应被理解为具有第一骨接触层和/或第二骨接触层的植入物基体。
有利的是:植入物被构造为人造关节或者关节植入物的第一关节部件的形式并且具有至少一个关节面。利用对应形成的第二关节部件,因此可以以简单的方式形成人造关节。特别地是,所述至少一个关节面(其也可以是两个、三个或更多个)分别与植入物和植入物基体一体地或整体地形成。
所述至少一个关节面优选地由生物不相容的塑料形成。该塑料可以特别地进行选择,使得其特性可以以有利的方式可用于形成尽可能低摩擦和低磨损的关节面。特别地是,这里可以使用UHMWPE。
为了形成膝关节内置假体,特别有利的是:第一关节部件被构造为胫骨部件的形式。可替代地是,它也可以被构造为股骨部件的形式。因此,其它关节也可以进行人工生产,例如人造肩关节、人造踝关节、或人造髋关节。
根据本发明,在开头所述类型的关节植入物中进一步实现了开头所述的目的,其中,第一关节部件和/或所述至少一个第二关节部件被构造为上述有利的植入物之一的形式。于是,结合植入物的优选实施例,整个关节植入物至少部分地也具有上述优点。
有利的是:关节植入物被构造为膝关节内置假体的形式,第一关节部件被构造为胫骨部件的形式,并且第二关节部件被构造为股骨部件的形式。因此,可以以简单的方式形成膝关节内置假体,其包括至少一个(特别是两个)关节部件,这些关节部件被构造为上述有利的植入物的形式。这种膝关节内置假体是长期稳定的并且是最佳生物相容的。此外,它们可以具有最小的重量,这改进了患者习惯人造关节的过程。

具体实施方式
例如在图1中示意性地描绘了以膝关节内置假体12形式的关节植入物,其用附图标记10表示。
关节植入物10包括以胫骨部件16形式的第一关节部件14以及以股骨部件20形式的第二关节部件18。
第一关节部件14具有两个关节面22,这两个关节面与第二关节部件18的两个关节面24相对应地形成。关节面22和24意图是彼此邻接抵靠并且可选地在彼此上滑动和/或在彼此上滚动,以用于形成人造关节。
两个关节部件14和18随后也被简称为植入物26。
在植入物26上分别设置关节面22和24不是强制性的,至多提供两个协作配合的植入物26,以用于形成关节植入物10。因此也可以设想到植入一个单独的植入物26作为人体或动物体内的骨头的局部替换。
结合图1至图3,更详细地讨论了以胫骨部件16形式的植入物26的结构。
植入物26包括柄部30,该柄部能够插入骨腔28中并且由塑料制成。此外,植入物26包括基体32,柄部30从该基体的下侧34延伸离开。
关节面22形成在基体32的上侧36上。
柄部30的外表面38限定骨接触面40。该骨接触面设置有或涂覆有第一生物相容的骨接触层42或承载这种骨接触层42。
基体32还具有基体骨接触面44,以用于邻接抵靠已备好的骨面46。例如,如图1中示意性描绘的骨面46可以涉及在部分切除胫骨48之后形成的骨面的平面表面。
基体骨接触面44形成下侧34的、包围柄部30的剩余区域。
植入物26总体上形成为单件,并且该植入物包括:即,塑料的植入物基体50,其例如通过注射成型而整体形成。
塑料特别地是可以涉及生物不相容的塑料,该生物不相容的塑料在骨接触的情况下导致开头所述的不期望的后果。非生物相容的塑料特别地是可以涉及聚乙烯或超高分子量聚乙烯(UHMWPE)。
如上所述,整体式植入物基体50仅在柄部30的区域中设置有生物相容的骨接触层42。所述层的厚度在约1μm至约1000μm的范围内。特别地是,所述层的厚度可以在约5μm至约700μm的范围内。
此外,第一骨接触层42具有粗糙度。该粗糙度可以在约0.5μm至约10μm的范围内。
骨接触面40(即,植入物基体50的外表面的一部分)也可以是粗糙的并且具有在约0.5μm至约10μm范围内的粗糙度。
第一骨接触层42可以例如由金属制成,特别是由钛或者含钛的合金制成或由生物相容的塑料(像例如聚醚醚酮)制成。可替代地是,骨接触层42也可以由陶瓷制成或由骨矿物质制成。骨矿物质特别地是可以是羟基磷灰石。
第一骨接触层42特别地是可以通过冷气体喷射和/或通过物理和/或通过化学气相沉积而形成。
为了避免植入物基体50和植入物基体50的骨接触面40之间与骨组织的接触,第一骨接触层42形成为完全闭合的。
基体骨接触面44是未涂覆的。为了避免与骨组织接触,这里可以使用骨水泥来固定植入物26。骨水泥然后在基体骨接触面44与骨组织之间形成分离层,并且还用于将植入物26固定在骨头上。
植入物基体50的整体式构造具有以下优点:关节面22可以由特别的生物不相容的塑料制成,植入物基体50由该塑料制成。
例如在图1至图3中所描绘的植入物26的实施例被构造为使得柄部30意图在没有骨水泥的情况下被锚固在骨腔28中。这能够通过不影响骨组织的骨接触层42来实现。
在图4中示意性地描绘了以第一关节部件14形式的植入物26的另一实施例,该植入物被构造为胫骨部件16。该实施例与图1至图3中所描绘的植入物26的实施例的区别仅在于:基体骨接触面44也被涂覆,即涂覆有第二骨接触层52。
第二骨接触层52可以特别地具有与第一骨接触层42相同的构造。为了避免重复,参考上面描述的第一骨接触层42的性质,如粗糙度和生产的方式。
第二骨接触层52也可以由上述材料制成,该材料结合对骨接触层42的描述进行详细说明。
提供第二骨接触层52使得能够特别地是在没有骨水泥的情况下将基体32按意图的那样锚固在骨头上(即,特别是骨面46)。
在植入物26的两个所述实施例中,柄部30还可以可选地被构造为使得它根据需要而可替代地或另外地用骨水泥锚固在骨头中。
股骨部件20可以以与胫骨部件16类似的方式具有柄部,以用于锚固在股骨的骨腔中。在这里,柄部和(根据具体情况)股骨部件20的基体也可以设置有一个或多个骨接触层,以便在不使用骨水泥的情况下将股骨部件植入股骨上时,避免制造股骨部件的植入物基体的材料与周围的骨组织之间的直接接触。

Claims (25)

1.植入物(26),所述植入物(26)具有柄部(30),所述柄部(30)能够插入骨腔(28)中,所述柄部(30)由塑料制成,特别是由生物不相容的塑料制成,并且所述柄部(30)限定至少一个骨接触面(40),其特征在于,所述骨接触面(40)设置有或涂覆有第一生物相容的骨接触层(42)或者承载生物相容的骨接触层(42)。
2.根据权利要求1所述的植入物,其特征在于,所述植入物(26)的所述柄部(30)意图是在没有骨水泥的情况下被锚固在所述骨腔(28)中。
3.根据前述权利要求中的任一项所述的植入物,其特征在于,所述植入物(26)包括基体(32),并且所述柄部(30)远离所述基体(32)的下侧(34)延伸。
4.根据权利要求3所述的植入物,其特征在于,所述基体(32)具有基体骨接触面(44),以用于邻接抵靠已备好的骨面(46)。

13.根据权利要求12所述的植入物,其特征在于,所述至少一种金属是钛或包含钛。
14.根据权利要求12或13所述的植入物,其特征在于,所述生物相容的塑料是聚醚醚酮(PEEK)或包含聚醚醚酮(PEEK)。
15.根据权利要求12-14中任一项所述的植入物,其特征在于,所述骨矿物质是羟基磷灰石。
16.根据前述权利要求中的任一项所述的植入物,其特征在于,所述第一骨接触层和/或所述第二骨接触层(42)通过冷气体喷射和/或通过物理和/或化学气相沉积而形成。
17.根据前述权利要求中的任一项所述的植入物,其特征在于,所述第一骨接触层和/或所述第二骨接触层(42)被形成为完全闭合的。
18.根据前述权利要求中的任一项所述的植入物,其特征在于,除了所述第一骨接触层和/或所述第二骨接触层之外,所述植入物(26)完全由所述生物不相容的塑料制成。
19.根据前述权利要求中的任一项所述的植入物,其特征在于,所述生物不相容的塑料是聚乙烯或超高分子量聚乙烯。
20.根据前述权利要求中的任一项所述的植入物,其特征在于,所述植入物(26)形成为单件,特别是整体形成。
21.根据前述权利要求中的任一项所述的植入物,其特征在于,所述植入物(26)被构造为人造关节植入物(10)的第一关节部件(14;18)的形式并且具有至少一个关节面(22;24)。
22.根据权利要求21所述的植入物,其特征在于,所述至少一个关节面(22;24)由所述生物不相容的塑料形成。
23.根据权利要求21或22所述的植入物,其特征在于,所述第一关节部件(14)被构造为膝关节内置假体(12)的胫骨部件(16)的形式。
24.关节植入物(10),所述关节植入物(10)具有第一关节部件(14)以及至少一个第二关节部件(18),所述至少一个第二关节部件(18)与所述第一关节部件(14)协作配合,其特征在于,所述第一关节部件和/或所述至少一个第二关节部件(14、18)被构造为根据前述权利要求中的任一项所述的植入物(26)的形式。
25.根据权利要求24所述的关节植入物,其特征在于,所述关节植入物(10)被构造为膝关节内置假体(12)的形式,并且所述第一关节部件(14)被构造为胫骨部件(16)的形式,并且所述第二关节部件(18)被构造为股骨部件(20)的形式。
