Description
Technical Field
The invention relates to a puncture outfit for laparoscopic surgery, which is a surgical instrument matched with a laparoscope and is suitable for establishing a working channel in the process of laparoscope examination and surgery.
Background
With the development of medicine and the improvement of health consciousness of people, the laparoscopic surgery plays more and more important roles in minimally invasive surgery with the advantages of small trauma, quick recovery, light postoperative pain, short hospitalization time and the like, so the clinical application of the minimally invasive surgery advocated by the laparoscopy is more and more extensive.
The principle of the laparoscopic surgery is to use carbon dioxide gas to make artificial pneumoperitoneum as an observation and operation space; puncturing a plurality of small holes of 0.5-1.0 cm on the abdominal wall by using a puncture outfit to establish a passage between the abdominal cavity and the outside; the puncture outfit is inserted with a video system, a light source and other surgical instruments, the conditions inside the abdominal cavity are transmitted to the display through the CCD camera, and a doctor observes in real time through the images on the screen and performs surgical operation.
The puncture outfit generally comprises a puncture casing pipe, a sealing assembly, a puncture rod assembly and an air inlet assembly, wherein the air inlet assembly is connected with the puncture casing pipe, the sealing assembly is arranged on the puncture casing pipe, and the puncture rod assembly penetrates through the sealing assembly to be inserted into the puncture casing pipe when in use. In order to avoid leakage of carbon dioxide gas, the sealing assembly is composed of multi-stage sealing silica gel components to achieve the sealing effect. However, surgical instruments often experience resistance from the seal assembly when accessing the puncture channel, resulting in greater force required by the surgeon to operate the instrument and discomfort to the surgeon's hand over time. Accordingly, the seal assembly may be deformed and not restored in time when subjected to a relatively large tensile force for a long period of time, thereby causing a failure of the seal.
Patent CN204049782 discloses a sealing assembly of a puncture outfit, which is composed of a sealing lower cover, a sealing ring, a sealing upper cover, a sealing lower gland, an elastic sealing element, a supporting element and a sealing upper gland, wherein the elastic sealing element is composed of 5 identical conical elements with notches, the circumferential angle of each element is 225 degrees, and the overlapping angle of every two elements is 153 degrees. The multistage sealing assembly in the puncture outfit brings resistance to the puncture needle and other surgical instruments in the repeated conversion process due to excessive number of sheets and overlarge overlapping area, so that the operation of a doctor is not smooth.
To solve the resistance problem, there are sealing assemblies that are modified and there are sealing assemblies or piercing cannulas that have substances added to them to reduce resistance, but these methods have their own disadvantages to varying degrees.
Patent CN207152631 discloses a small-resistance sealing ring, which is composed of a sealing ring formed by more than two segments and more than two supporting pieces respectively arranged on each segment of the sealing ring. The sealing ring has magnetic strips or magnetic powder on its segments, and the support sheet is made of hard material, such as plastic or metal. In clinical application, the puncture rod is usually made of metal materials, the puncture rod cannot smoothly pass through a sealing ring with magnetic strips or magnetic powder, and the support sheet made of hard materials can also bring resistance to puncture of the puncture rod.
Patent CN105943131 discloses a self-lubricating puncture outfit, which is composed of a puncture cannula, a core rod, a cannula holder, a conversion cap, a core rod holder and a lubricating component. The lubricating assembly comprises a microporous bag and lubricating grease positioned in the microporous bag, the surgical instrument or the core rod penetrates through the working channel, the lubricating grease can be extruded by the microporous bag when the microporous bag is extruded, and the lubricating grease is in contact with the surgical instrument or the core rod to achieve a lubricating effect. In clinical application, the front end of the puncture core rod is mostly of a sharp structure, when the puncture core rod penetrates through the microporous bag, the microporous bag is easily punctured instead of extruded, and lubricating substances can be released in a large amount and flow into a human body to cause harm.
Patent CN202665587 discloses a self-lubricating puncture outfit, which is composed of a puncture rod and a sheath tube, wherein the sheath tube is provided with a medical elastic porous material and a medical lubricant which can absorb, store and release the medical lubricant. When the puncture rod contacts with the lubricating component, the sharp structure of the front section of the puncture rod can damage the medical elastic porous material, especially the medical elastic porous material is damaged more seriously after the puncture rod or the surgical instrument is punctured for many times, the damaged porous material is easy to generate fragments, and the fragments enter the human body and are harmful. In addition, the medical silicone oil is selected as the lubricant, so that the sealing component made of the silicone material is dissolved with the silicone oil, and the sealing performance of the puncture outfit is damaged.
It can be seen that the above-disclosed patents do not address the resistance problem well and continued improvement is needed.
