当针对特定器官(例如汗腺、皮脂腺、毛囊...)时,目标是仅在靶向器官中诱导组织破坏过程,同时使周围组织保持完好,即将靶向器官温度升高至开始坏死过程的水平,同时保持周围组织温度在安全水平。有几种方法可以实现这一目标。
一种方法是利用吸收光谱的差异以及使用一种或多种特定的光波波段,该波段比其周围组织更好地被靶向器官吸收。
通过以脉冲模式使用能量源也可以获得所需的温差。如果设计正确,热松弛时间(TRT)的差异会导致靶向组织长时间保持其升高的温度,从而增加发生坏死过程的可能性。
在某些情况下,也可以利用使用连续波型激光或其他来源的整体加热。如上所述,坏死和/或凋亡过程是复杂的,并且不同的器官将需要不同的温度水平来启动这些过程。如果组织损伤过程在低于周围组织的温度下发生在特定器官中,则将组织整体加热至该温度足够长的时间最终将导致靶向器官坏死过程,并可能对周围组织造成潜在的最小影响。这可能是使用整体加热脱毛时发生的情况。
汗腺
所有人都会出汗。这是身体消除废物本身以及冷却身体的方式。在许多人中,考虑到除臭剂的可用性和止汗剂的可用性以基本阻止汗液的排泄、过量,烦人的出汗不是问题。然而,对于其他人而言,出汗可能是一个主要问题,或者出现局部化妆水无法解决的尴尬局面。
汗腺是皮肤附属物,如毛囊或皮脂腺
汗腺是卷曲的管状系统,并且通常落入两种类型:外泌汗腺和顶泌汗腺,如下所述。虽然他们通常被认为是不同的单一实体,但有证据表明两个外泌汗腺的线圈也可能在彼此内部发生卷曲;因此,这些结构可能很容易被误认为是一个具有两个或多个导管的大型线圈。
在外泌腺和顶泌腺之间存在区别,它们的形态和含量(汗成分)彼此不同。图1示出了这些腺体的示意图。在图1中,参考数字100代表皮肤组织的表面。汗孔102被显示为到达皮肤组织的表面,使得分泌物可以被释放到皮肤组织的外部。外泌汗腺类型的汗腺106以及围绕毛囊干104的顶泌型汗腺108也被示出。通常,外泌汗腺发生在大部分身体上,并直接打开到皮肤表面,而顶泌汗腺通向毛囊,如图所示,到达皮肤表面。顶泌汗腺发育于毛囊丰富的区域,如头皮,腋窝和腹股沟区。
外泌腺由单个细管组成,总长约4至8毫米。汗腺的表皮内部称为末端汗管(acrosyringium),随之是真皮管,带有一个直线和一个卷曲部分,以及一个卷曲的分泌部分,位于深部真皮层中,如图1所示。外泌汗液含量的99%是水,其余是盐和蛋白质。
顶泌腺通常局限于多毛的身体区域,因为它们打开并分泌到发根管中。出于这个原因,顶泌腺只能在腋窝、乳房、会阴和生殖器区域找到。顶泌管非常短,可以在毛囊附近找到,也可以在图1中看到。
有些人的汗腺不能分类为顶泌汗腺或外泌汗腺,而是具有两者的特征;这样的腺体被称为apoeccrine(顶-外泌汗腺)。(参见维基百科wikipedia.org关于“Sweat Gland(汗腺)”的描述。)
顶-外泌腺体大于外泌汗腺,但小于顶泌汗腺;它们的分泌部分具有与外泌汗腺中的分泌线圈类似的狭窄部分,而其张大的部分使人想起顶泌汗腺。在腋窝和肛周区域发现的顶-外泌腺体具有通向皮肤表面的通道。据推测,它们是在青春期从外泌汗腺发育而来,并且可以占所有腋窝腺体的50%。顶-外泌汗腺分泌比外泌汗腺和顶泌汗腺更多的汗液,因此在腋窝出汗中起到很大的作用。
被发现的汗腺至少部分位于脂肪岛内,这些脂肪岛被同化在真皮层中,以及真皮-皮下组织边界和上层皮下组织。因此,大部分汗腺被脂肪层包围。图2示出了脂肪细胞和汗腺之间的这种关系,其中在真皮200里显示出包括在真皮中的脂肪袋202内发现的汗腺204。
多汗症
有几种方法可以治疗大量出汗(多汗症)。最简单也可能是最常用的方法是在皮肤组织表面涂抹止汗剂(其中很多含有铝盐),其形成阻塞汗液排泄物的“塞子”。很多人每天使用这种止汗剂。
在止汗剂不能解决多余出汗问题的情况下,有几种药物治疗可以纠正这种过度出汗:
离子电渗疗法:在这种治疗过程中,低电流流过浅水区,病人在其中浸泡手或脚(或两者)。每次治疗需要大约20到30分钟,患者必须每周至少重复几次。没有人确切知道这种治疗方法的作用,但专家认为它可以阻止汗液进入皮肤表面。
肉毒杆菌毒素:大量出汗的另一种治疗选择是注射肉毒杆菌毒素A(肉毒杆菌毒素)。这是一种纯天然的纯化蛋白质,当小剂量注射到皮肤中时,可阻断供给外泌汗腺的神经;这可以防止腺体产生汗水。通过阻断或中断这种化学信使,肉毒杆菌毒素“关闭”已注射部位的出汗。肉毒杆菌注射非常浅,这意味着材料被注射并保持到皮肤表面下方。肉毒杆菌毒素是FDA批准用于治疗腋下过度出汗的药物,并且已显示其导致出汗减少82-87%。一些医生也可以在手掌和脚掌上使用肉毒杆菌毒素,其效果为80%-90%。这种治疗不能治愈多汗症-它只能提供暂时缓解。它需要每三到六个月重复一次以获得最大效果。
抗胆碱能药物:有阻止汗腺激活的口服抗胆碱能药物。由于视力模糊,心悸和泌尿问题等严重副作用,通常不推荐使用。
手术:这可能是那些对其他治疗没有反应的严重多汗症病例的唯一选择。在手术过程中,医生可能会切割、刮擦或抽出汗腺。一种手术选择是内窥镜胸交感神经切除术(ETS),其中外科医生做出非常小的切口并切割通常激活汗腺的腋窝中的神经。这个程序非常有效,但它仅作为尝试过所有其他治疗的人的最后的手段。ETS不可逆转,并且可能留下疤痕。几乎所有接受ETS治疗的人都必须面对的一个副作用是补偿性出汗。当身体在一个区域停止出汗时,可能会在另一个区域(例如脸部或胸部)开始出汗以补偿。
几家供应商如Fotona(XP2)、Deka(Smartlipo)和Cynosure(SideLaze)提供激光治疗多汗症的微创解决方案。这些公司销售的系统使用激光通过光热加热汗腺组织,对周围组织造成最小的损伤,从而永久性地摧毁腋窝汗腺。在某些情况下,激光通过光纤引入皮下组织。在这些情况下,局部麻醉剂可能会在治疗前注入待治疗的区域,例如腋窝。相同的供应商声称使用声称被腋下皮下组织选择性吸收的特定波长。以上是由Fontona和Deka提供的,它们提供工作在1064nm的装置,以及提供在1440nm工作的装置的Cynosure。一家名为Miramar Labs的公司使用非侵入式手持设备(miraDry)将精确控制的微波能量输送到腋下皮肤下的汗腺所在的特定区域,从而导致汗腺热解(热分解)。在这种治疗过程中,皮肤的顶层同时被冷却和保护。治疗后不相信汗腺会长出来,所以效果几乎可以立即看出来,结果可能会持久。有些患者可能只需要一次治疗,但为了获得最佳效果,可能推荐相隔三个月的两次手术。
本发明所针对的是通过破坏汗腺的排泄来减少出汗。
发明内容
在一个方面,治疗人体以减少多汗症的美容方法包括将激光能量施加到人体的一个或多个部分,其中激光能量近似为约930nm、约970nm和约1060nm中的一个或多个。所述激光能量可以作为以下之一传递:连续或脉冲。
在另一方面,脉搏率可以是1Hz或者可以选自1/2Hz到3Hz之间。所施加的脉冲可以是短脉冲,例如在2016年7月28日提交的标题为“Apparatus and Cosmetic Method forUsing Short Pulses for Dermatological Treatments(用于皮肤病治疗的短脉冲装置和美容方法)”的美国申请号15/221,623中所描述的,其全部公开内容在此通过引用并入本文。
在另一方面,所述方法可以包括在施加激光能量附近冷却人体的步骤。
另一方面,脉冲之间的时间延迟可以在脂肪细胞的TRT和汗腺的TRT之间。因此,在图6A和6B中,可以看到汗腺的温度升高到比脂肪组分更高的水平,从而导致汗腺的选择性损伤。也可以使用不同的脉冲,例如在治疗开始时以较高水平的脉冲将汗腺细胞的温度升高至靶向温度或至少接近靶向温度,然后将脉冲降至一个较低的水平,以保持汗腺细胞的稳定温度,以允许汗腺中的生化过程受控发展,最终导致汗腺损伤。
在另一方面,美容方法可以进一步包括提供人体皮肤表面的整体加热的步骤。整体加热可以作为主要的治疗方式递送,如下所述,也可以与选择性靶向治疗联合递送。当整体加热与选择性靶向治疗一起递送时,可以在施用选择性靶向治疗之前、之中和之后的一个或多个中进行。整体加热可以通过以下一种或多种来提供:激光能量,IPL光能量,RF能量,微波能量或超声波能量。可以在大约400nm至大约650nm处施加整体加热。

具体实施方式
在本发明中,通过外部激光源完成多汗症的非侵入性治疗。目标是引起汗腺的选择性热诱导坏死和/或凋亡,同时保持周围组织完整。如上所述,有几种方法可以实现这一目标。适当的选择性治疗的关键因素是各种组织组件对电磁辐射的相对吸收。图3分别给出了脂肪和水的光吸收光谱行为的曲线A和B。如图3所示,在波长650nm-660nm附近,在脂肪和水之间存在等吸光点300。在区域301中,在比等吸收波长短的波长处,脂肪吸收比水吸收高。在区域302中,在比等吸收波长更长的波长处,水吸收高于脂肪吸收。如下文将进行更加详细的解释,根据本发明的一个方面,区域301中的选择性汗腺处理可以通过整体加热处理模式来实现。
可以通过使用脉冲或连续波激光器来实现组织整体加热。在这种治疗模式下,整个组织体将被均匀加热至大约相同的温度,同时由于表征汗腺与周围脂肪的损伤温度阈值(DTT)的差异,可以实现治疗选择性。由于汗腺的DTT低于脂肪,选择性可以通过将体温升高至高于汗腺的DTT但低于脂肪的DTT的水平来实现。
通过使用区域301中的辐射波长,瞄准了通过加热周围脂肪层而被脂肪组织包围的汗腺。因此,可以瞄准脂肪组织并将其温度增加至一定水平,从而启动嵌入其中的汗腺的坏死和/或凋亡过程,同时使真皮组织处于较低温度。
用图3的区域301中的波长照射组织会使脂肪温度比汗腺升高的更快。然而,由于汗腺被脂肪组织包裹,热量将从该组织扩散到腺体并增加其温度。通过采用适当的照射方案,整体温度可升高至高于汗腺DTT但低于脂肪细胞的DTT的点,从而导致对汗腺的选择性治疗。
可以使用与水相比有利于脂肪吸收的波段,例如400nm-650nm。在这种情况下,可以将具有相对小的峰值功率和实质上高的重复率的低功率连续波(CW)或连续振荡(或脉冲式)辐射施加到处理区域。为了逐渐均匀地增加靶向区域的温度并将其保持在一定水平,在宽区域来回滑动激光束产生头部是公知的,并且也可以用于治疗汗腺。如前所述,对表皮层施加冷却降低了不利影响的风险。
相反,在区域302中,汗腺被直接瞄准以将其温度升高到足够的水平并持续足够的时间。因为在这部分光谱中,水中光的吸收比脂肪高,所以汗腺的温度将比周围的脂肪层升高的快。这使得基于靶向组织的非均匀加热和直接靶向汗腺的选择性治疗成为可能。另外,围绕基于汗腺的水的脂肪层将其与环境热隔离并有效地增加其热弛豫时间(TRT)。热弛豫时间是靶向发色团内吸收的光能产生的热量在激光曝光后立即耗散到其值的50%所需的时间。更长的TRT增强了治疗的选择性,因为汗腺将长时间保持其升高的温度,增加引发凋亡和/或坏死过程的可能性。
基于上面概述的事实,当选择要治疗的区域302内的特定波长时,期望可以是使用最大化脂肪和水之间的吸收差异的波长。另一方面,对水中吸收的期望不要太高。这可能导致表皮和上真皮层对辐射的高吸收(小的穿透深度),很少能量穿透真皮深层,并且不足以影响汗腺。
由此看来这种处理的最佳波长可能在930nm-1030nm波段,最好在970nm左右。因此,根据一个实施方案,基于上述分析,用于治疗汗腺的一种建议的程序是在大约970nm处的二极管激光器。与它们在脂肪中的吸收相比,这些波长的能量在水中的吸收优先。这在图3A中示出,其显示出当比较例如930nm或1060nm时,970nm在脂肪和水之间产生更大的差异吸收(汗腺含量)。970nm能够深入皮下层并影响途中的水溶性组分,如汗腺。由于如上所述的事实,汗腺被热隔离组织(在这种情况下为脂肪)包围,其在这个波长处具有较低的吸收特性,因此可以实现汗腺的相对选择性坏死。
因此,使用大约970nm可以被看作相对于周围组织来说,选择性地瞄准汗腺本身。然而,正如下面将要讨论的那样,差别吸收本身并不是灵丹妙药,因此甚至更小的差异可能用于损害汗腺,因为来自嵌入脂肪层的分泌细胞的水的形态结构,例如,整体加热。
区域302中的另一个可选波段是1030nm-1200nm,但可能预计其功效比970nm低。然而,现在转向图6A和图6B中,可以看出,在1060nm处,虽然脂肪组织中该波长处的吸收大于970nm处所示的吸收,但它并不过高,并且汗腺中的吸收与970nm所示的吸收相等,在这部分光谱中也可以实现选择性。
680nm-910nm波段是区域301中的另一个波段,其可以应用于汗腺处理,但是如本文所讨论的,该范围可能预期在皮肤组织的整体加热中更有效。
通过提供脂肪和汗腺的选择性加热,坏死过程可以仅在汗腺中诱导,同时使周围组织保持完整。使用表1中列出的参数的各种脉冲序列用波长为970nm的激光器进行测试,并实际导致对汗腺的非常选择性的损伤。
在图4A和4B的绘图序列400中,图4A通常说明了治疗一周后这种脉冲序列对猪的皮肤的影响,以及图4B为处理后一个月的效果。现在转到图4A,该图显示了图4A照片中心部分的毛囊和出血性伤口404(HT)。图4A也显示由浅色脂肪细胞403(在照片中)包围的坏死深色(在照片中)的汗腺细胞402(N)。现在转到图4B的照片,一个月后,该图显示毛囊408(HF),其被亚慢性炎症包围(由箭头410和412示出),但值得注意的是在区域中没有汗腺组分的残留,而他们以前存在。因此,图4A和4B的这个序列400说明一周后可观察到汗腺中的选择性炎症效应(表明坏死过程),并且一个月后汗腺消失。

为了降低表皮烧伤等不良反应对黑色素吸收辐射的风险,可在治疗过程中采用局部冷却。这种冷却可以使用,例如由在以色列的Lumenis Ltd.在其和产品系列中制造的一些产品中的“冷冻尖端(chilled tip)”技术提供的内置冷却机构来实现。其他的冷却方法也可以考虑。前后冷却可进一步减少副作用,例如水肿或疼痛,可提供部分麻醉,并可降低黑色素丰富表皮的温度升高。
可以连续或间歇地提供冷却,并且可以在治疗期间或者在治疗和后期治疗期间的结合中施用,以最小化不希望的皮肤反应,例如红斑,水肿,紫癜,而不干扰汗腺中的热积聚机制。在向组织输送能量期间或之后局部冷却皮肤表面可以避免热积聚,即使在真皮层上面,有助于程序的安全性并且如果需要的话允许更多的能量被传递到组织。
通过对治疗区域施加真空来递送治疗时可以获得治疗功效的进一步提高。图5所示的手持件是一种提供激光能量的装置;所述手持件包括开口尖端和真空源,并且使用它的过程包括在施加激光能量之前将皮肤组织拉入开口尖端的步骤,使得汗腺可被拉动更接近包含在手持件内的激光器光源。图5的手持件是在US2008/0119830中描述的,其被转让给本发明的同一受让人,并且其全部内容通过引用并入本文,或者也可以是由同一受让人制造的Lightsheer HS激光设备。对组织施加真空的另一个优点可能是血管的血管收缩,从而将血液从靶向区域推出,并进一步增强从激光能量源到汗腺的光能吸收而不是被竞争发色团吸收。
虽然前面已经讨论了用于整体加热的区域301中的波长和用于更有针对性的加热的区域302中的波长,但是波长不需要局限于上述波长或光谱之一,也可以是根据影响汗腺和/或脂肪组分的计划治疗,从相同区域301或302或从两个区域“混合和匹配”上述两种或更多种波长或光谱。此外,整体加热和适当的脉冲处理可以组合以实现损害汗腺的期望效果。可编程的控制器可以用来控制和排序这些不同的治疗参数。
尽管前面的一般性讨论针对治疗腋下汗腺,但应当理解的是,其他身体部位也可以用本发明治疗,例如脚底、手掌、前额、背部、胸部和腹股沟,以及在人体各个部位治疗臭汗症(脚臭)和腋臭(大汗臭味)。
通常情况下,对于冷冻尖端手柄结构,根据所使用的波形,手柄将使用“拾取和放置”或“滑动”或“绘画(painting)”进行定位,同时在整个治疗过程中保持牢固的压缩。
在“拾取和放置”方法中,尖端在脉冲之后立即从皮肤上拾取,移动到下一个处理位置,然后降低到皮肤上。在“滑动”技术中,尖端与皮肤保持连续接触,并在激光脉冲后立即移动到下一个治疗位置,确保蓝宝石尖端与皮肤紧贴或紧密接触时激光被施加。在“绘画”技术中,操作人员可以在机体表面来回移动手持件。
