本发明提供一种胆汁引流与装载放射性粒子条套件,由内管,侧孔,外管,引流管头端,外管壁螺纹,外接螺丝帽螺纹,外接螺丝帽壁,外接螺丝帽斜面和弹簧圈组成,侧孔设置在外管头端,内管插入外管内,外管壁螺纹设置在外管尾端外侧,外接螺丝帽螺纹、外接螺丝帽壁、外接螺丝帽斜面构成外接螺丝帽,外接螺丝帽螺纹通过外管壁螺纹进行固定。本发明设计合理,制作简单,操作方便,它兼具胆汁引流及装载放射性粒子条实施管腔内内放疗的作用,适用于胆管恶性肿瘤的治疗,以提高胆管恶性肿瘤患者的临床治疗效果,延长生存期。本发明由介入医生在x线下采用可实施梗阻胆管的引流,并同时在受肿瘤侵犯的胆管内置入放射性粒子实施肿瘤的内放射治疗。

Description

一种胆管引流与装载放射性粒子条套件
技术领域
本发明器械属医疗器材领域,涉及一种胆汁引流与装载放射性粒子条套件,用于胆管恶性肿瘤的介入治疗,由介入医生在x线下采用本器械实施梗阻胆管的引流,并同时在受肿瘤侵犯的胆管内置入放射性粒子实施肿瘤的内放射治疗。
背景技术
胆管恶性肿瘤常常引起梗阻性黄疸,导致患者肝功能受损,消化功能障碍。由于大多数胆管恶性肿瘤患者疾病发现较晚,同时由于部位的特殊性,常常失去外科手术的机会。临床上为解除患者的黄疸,多采用胆汁引流术。梗阻胆管内的胆汁被引流出体外或内引流到十二指肠。随着内支架技术的发展,也可在胆管狭窄处放置内支架,使胆管被动打开,恢复胆汁的排出。但所有这些引流术并没有对导致胆管狭窄闭塞的原发肿瘤进行治疗。临床上胆管癌的治疗疗效较差,化疗常常不敏感,体外放疗尽管经常采用,但由于受周围脏器的放射剂量限制,也不能取得较好的治疗效果。随着放射性粒子植入技术的临床应用,在肿瘤的局部产生的放射剂量是常规外放疗的几十倍,提高了肿瘤治疗的效果,而对周围脏器几乎不产生明显的损害。因而,设计一种既能引流胆汁,又能在局部置入放射性粒子,并能调整和补充粒子的数量,能根据临床需要灵活固定或调整装该置位置的一套胆汁引流兼具装载粒子的专用套件将具有十分重要的意义。
发明内容
本发明的目的是提供一种胆汁引流与装载放射性粒子条套件,主要由内管,侧孔,外管,引流管头端,外管壁螺纹,外接螺丝帽螺纹,外接螺丝帽壁,外接螺丝帽斜面和弹簧圈组成,侧孔设置在外管头端,外管头端开口为引流管头端,内管插入外管内,外管壁螺纹设置在外管尾端外侧,外接螺丝帽螺纹设置在外接螺丝帽壁内测的外接螺丝帽斜面上,外接螺丝帽螺纹通过旋转外管壁螺纹进行固定。弹簧圈至于内管内,用于固定填装的粒子。
外接螺丝帽螺纹、外接螺丝帽壁、外接螺丝帽斜面构成外接螺丝帽,为一个独立的末端有孔洞的中空盖帽,通过与外管的外管壁螺纹旋转吻合后,既可连接在一起,中空的部位可以引流胆汁。
内管直径0.9-1mm,头端封闭,尾端开口。弹簧圈为直线型的,长1~2mm。
使用时,外管主要起到引流胆汁和固定位置的功能,其头部与现在常用的引流管一样,由塑料材料构成,长度30-50cm,前段有许多侧孔,其尾部外侧带有外管壁螺纹,外管中间部位为中空的管状圆管;内管为中空的塑料管,管径比外管小,可以抽出和插入到外管内,为独立的管道,内管1的功能为装载粒子,内腔粗细刚好装入粒子,长度35-55cm,可根据需要调整长度,内管头端为封闭状,将粒子依次装载入内管后,在管内放入一枚1~2mm直线型的弹簧圈,通过直导丝推其至内管头部,与粒子接触,随着直导丝的继续推送,直线型的弹簧圈受力后会被压缩成弯曲状态,并于粒子紧密接触,固定粒子在内管头部,防止粒子移位,完成固定后,抽出直导丝,直导丝的直径比内管小一点,在介入治疗领域很常见,有不同的规格可选择。然后将装载粒子条的内管插入到外管内,在X光的透视下,确定好内管插入的深度,从而也确定了粒子位于外管内的位置,将内管1的尾端超出外管出口2cm处剪去多余的部分,然后通过外接螺丝帽螺纹通与外管壁螺纹连接固定。即成为所述胆汁引流与装载放射性粒子条的专用套件。
本发明是一种用于胆管恶性肿瘤治疗的医疗器具,用于胆管恶性肿瘤的介入治疗,由介入医生在x线下采用本器械实施梗阻胆管的引流,并同时在受肿瘤侵犯的胆管内置入放射性粒子实施肿瘤的内放射治疗。本发明设计合理,制作简单,操作方便,它兼具胆汁引流及装载放射性粒子条实施管腔内内放疗的作用,适用于胆管恶性肿瘤的治疗,以提高胆管恶性肿瘤患者的临床治疗效果,延长生存期。

具体实施方式
本发明内容结合附图和具体实施例作进一步的说明。
实施例1
参见图1-图4,本发明的目的是提供一种胆汁引流与装载放射性粒子条套件,由外管、内管、螺丝帽三部分构成,主要由内管1,侧孔2,外管3,引流管头端4,外管壁螺纹5,外接螺丝帽螺纹6,外接螺丝帽壁7,外接螺丝帽斜面8和弹簧圈10组成,侧孔2设置在外管3头端,外管3头端开口为引流管头端4,内管1直径0.9-1mm,头端封闭,尾端开口,内管1通过头端插入外管3内,外管壁螺纹5设置在外管尾端外侧,外接螺丝帽螺纹6设置在外接螺丝帽壁7内测的外接螺丝帽斜面8上,外接螺丝帽螺纹6通过旋转外管壁螺纹5进行固定。弹簧圈10至于内管1内,用于固定填装的粒子9。
外接螺丝帽由外接螺丝帽螺纹6,外接螺丝帽壁7,外接螺丝帽斜面8组成,为一个独立的有孔洞的中空盖帽,与外管3的外管壁螺纹5旋转吻合后,既可连接在一起,外接螺丝帽末端有孔洞(图中未示出),中空的部位可以引流胆汁。
外管3主要起到引流胆汁和固定位置的功能,其头部与现在常用的引流管一样,由塑料材料构成,长度30-50cm,前段有许多侧孔2,其尾部外侧带有外管壁螺纹5,外管3中间部位为中空的管状圆管;内管1为中空的塑料管,管径比外管小,可以抽出和插入到外管内,为独立的管道,内管1的功能为装载粒子9,内腔粗细刚好装入粒子9,长度35-55cm,可根据需要调整长度,内管1头端为封闭状,将粒子依次装载入内管1后,在管内放入一枚1~2mm的直线型弹簧圈10,通过直导丝推其至内管1头部,与粒子9接触,随着直导丝的继续推送,直线型的弹簧圈10受力后会被压缩成弯曲状态,并于粒子紧密接触,固定粒子9在内管1头部,防止粒子9移位,完成固定后,抽出直导丝,直导丝的直径比内管小一点,在介入治疗领域很常见,有不同的规格可选择。然后将装载粒子条的内管1插入到外管3内,在 X光的透视下,确定好内管插入的深度,从而也确定了粒子位于外管3内的位置,将内管1 的尾端超出外管出口2cm处剪去多余的部分,然后通过外接螺丝帽螺纹6通与外管壁螺纹5 连接固定。即成为所述胆汁引流与装载放射性粒子条的专用套件。
实施例2
参见图1-图4,经皮肝胆道引流术(percutaneous transhepatic cholangialdrainage,PTCD),是在医学影像设备引导下(X线)通过穿刺针直接穿刺肝内扩张胆管,见胆汁外溢后,在X 线下注入造影剂造影,显示肝内胆管,将引流管放置到胆管内,置入的引流管通过狭窄段(肿瘤侵犯段),引流管的侧孔2仍在阻塞胆管内,固定好引流管,从而达到消除黄疸,恢复肝胆功能的目的。本发明套件对以往的引流管进行了改进,增加了内管及其他附件,内管管径刚好装载粒子9,内管1直径0.9-1mm,头端封闭,另一端保持开放,使用时先装载放射粒子9成粒子条(粒子数量按临床需要),粒子9装载后管腔内送入一枚1-2m的直线型的弹簧圈10以防止粒子脱出。在X光透视的条件下,将内管1送至特定位置,粒子条全程覆盖肿瘤侵犯段胆管,之后在末端用剪刀剪断,预留3厘米,进行近180度的方向扭转,用手指把剪断一端的末端压在外管3壁的外管壁螺纹5前,通过外接螺丝帽螺纹6与外管壁螺纹5旋转固定,同时外接螺丝帽斜面8也会随着外接螺丝帽螺纹6向外管壁螺纹5靠近,会对内管1起到压紧固定的作用。外接螺丝帽有孔洞,可以引流液体。

1.一种胆汁引流与装载放射性粒子条套件,其特征在于,由内管(1),侧孔(2),外管(3),引流管头端(4),外管壁螺纹(5),外接螺丝帽螺纹(6),外接螺丝帽壁(7),外接螺丝帽斜面(8)和弹簧圈10组成,侧孔(2)设置在外管(3)头端,外管(3)头端开口为引流管头端(4),内管(1)插入外管(3)内,外管壁螺纹(5)设置在外管(3)尾端外侧,外接螺丝帽螺纹(6)设置在外接螺丝帽壁(7)内测的外接螺丝帽斜面(8)上,外接螺丝帽螺纹(6)通过旋转与外管壁螺纹(5)进行固定,弹簧圈(10)至于内管(1)内用于固定粒子(9)。
2.根据权利要求1所述的一种胆汁引流与装载放射性粒子条套件,其特征在于,外接螺丝帽螺纹6,外接螺丝帽壁7,外接螺丝帽斜面8组成外接螺丝帽,为一个独立的有孔洞的中空盖帽,其孔洞用于引流胆汁。
3.根据权利要求1所述的一种胆汁引流与装载放射性粒子条套件,其特征在于,内管(1)直径0.9-1mm,头端封闭,尾端开口。
4.根据权利要求1所述的一种胆汁引流与装载放射性粒子条套件,其特征在于,弹簧圈(10)为直线型,长1-2mm。
