Abstract

Description
本申请主张11月14申请的美国临时专利申请案62/080,038的优先权,其全文在此一并纳入参照。
技术领域
本发明内容是与测定脑部含氧量(brain oxygen level)及检测脑血肿(brainhematoma)有关。更具体来说,本发明内容是关于利用近红外光(near infrared,NIR)波长来测量个体不同部位的相对血红素饱和度,藉以测定脑部含氧量及/或检测脑血肿的装置及方法。
背景技术
对头部创伤病患进行初步复苏的首要任务是确认及手术清除创伤性颅内血肿。血肿主要是因出血或外伤造成血液堆积于血管外所致。由于持续扩大的血块会压迫到脑干致使个体死亡或造成脑部缺血性损伤,因此手术清除需尽早进行。一般来说,延迟治疗(即脑部损伤与血肿清除的时间间距超过4小时)会增加伤者的死亡率及伤势的恶化程度。
目前已有数种检测方法可用以确认创伤性颅内血肿及其位置,例如计算机断层扫描(computed tomography,CT)、核磁共振影像(magnetic resonance imaging,MRI)、X辐射或X光(X-radiation,X-ray)、脑部血肿检测仪(infrascan)及微脉冲雷达(micropowerimpulse radar,MIR)。然而,在实际检测脑血肿时,这些技术皆有不同的限制条件。
CT扫描可提供受伤部位的三维立体空间等向性影像,能同时显影受损伤的骨骼、软组织及血管。然而,一般CT的功效往往受限于:(1)准备及检测时间过长;(2)无法正确辨别血肿及脑水肿(hydrocephalus);(3)无法持续性监测;(4)无法排除潜在性损伤,例如缺血及缺氧;(5)于脑干及后颅窝(posterior fossa)具有较差的显影力;(6)辐射曝露;以及(7)显影剂可能引发过敏或肾脏病变。这些限制往往局限了CT在临床上的相关应用。
MRI是一种不需暴露于电离辐射且具有高敏感度的显影技术,可藉由检测组织中水分子的移动来检测及评估脑干损伤及早期中风。然而,临床上MRI的应用可能受限于下列情况。第一,个体体内的移植物或其他金属物质会造成MRI无法清晰显影。第二,由于牵引装置及其他生命维持仪器必须远离显影区域,急性损伤的病患并不适合进行MRI显影分析。第三,MRI显影所需时间较长,且无法立即得知结果。此外,与CT显影相似,显影剂的使用可能会导致病患产生过敏反应或肾脏病变。
X光是一种电磁辐射,仅需数秒即可完成X光影像扫描。X光的最大限制为其扫描时无法改变影像平面,因此无法提供详细的立体信息。此外,X光也会破坏DNA,有可能会造成基因缺损及/或疾病。
脑部血肿检测仪是一种利用NIR扫描头部创伤病患的技术,其是依据头部二侧的光学密度对比数据来进行诊断及分析。因此,当头部二侧同时产生血肿时,该项检测技术即无法有效地诊断分析。
MIR技术是利用可穿透人体、且持续时间极短的电磁脉冲来进行诊断。与超声波或其他电磁技术不同的是,MIR可穿透头骨来分析脑内及硬膜外血肿。然而,与脑部血肿检测仪相似,MIR运作准确与否取决于受测者先天头部二侧是否对称,并且需要基准值或受伤前扫描方能进行微差分析(differential analysis),以确认脑部是否有异常。因此,MIR仅可用来检测头部单侧血肿,而无法检测双侧血肿。
有鉴于此,相关领域亟需一种改良的装置及/或方法,藉由持续监测血氧浓度(blood oxygen level)的变化,来早期检测脑血肿(特别是双侧脑血肿)。

本发明内容是关于一种用以测量个体的脑部含氧量及/或监测个体脑血肿的装置及其用途。
在第一方面,本发明内容是关于一种用以测量个体的脑部含氧量及/或监测个体的脑血肿的装置。该装置包含:
一探头(probe),包含第一、第二及第三光源,可分别发散第一、第二及第三之可穿透个体脑部的NIR波长,其中该第一、第二及第三NIR波长分别约为790-810奈米、650-790奈米及810-1,000奈米;
一检测机构(detecting means),包含第一、第二及第三NIR检测器(NIRdetector),其中各NIR检测器可分别检测该第一、第二及第三NIR波长在穿透个体脑部后的第一、第二及第三强度;以及
一与该探头及该检测机构连接的处理器(processor),可藉由该检测机构所检测的第一、第二及第三强度来计算脑部含氧量或分析脑血肿。
依据本发明内容的一种实施方式,第一NIR波长约为808奈米,第二NIR波长约为780奈米,且第三NIR波长约为850奈米。
依据本发明内容另一种实施方式,第一、第二及第三光源中的各光源可以是雷射二极管(laser diode,LD)或发光二极管(light emitting diode,LED)。
依据本发明内容一种实施方式,探头具有一管状(tube)结构,且第一、第二及第三光源是设置于管状结构的一端。依据本发明内容另一种实施方式,探头及检测机构是分别配置为垫片形式(pad)。依据本发明内容再一种实施方式,探头及检测机构是分别配置于一头戴装置(headset)上,且彼此间隔角度为180度。在该实施例中,配置于头戴装置上的探头及检测机构是分别与一驱动装置(driving means)链接,该驱动装置可驱动探头及检测机构于所述头戴装置上同步进行圆周运动。
相较于传统检测及显影技术,本发明装置的优势在于探头及检测机构可依应用需求分别放置于不同位置(例如个体的头部、鼻腔、耳道及口腔),藉以改善测量的准确性及功效。
本发明内容的第二方面是关于一种利用本发明装置来测定一个体的脑部含氧量的方法。该方法包含:

其中I0及I分别表示由光源发射的NIR波长的强度及由NIR检测器测得的NIR波长的强度,R是I及I0比例的对数,λ为NIR的波长,∈为血红素(hemoglobin,Hb)或氧合血红素(Oxydative Hemoglobin,HbO2)的消光系数(extinction coefficient),L为NIR波长的光路径长度,且G为吸收系数(absorption coefficient)。
脑部含氧量可提供用以评估脑血肿的血块大小及发生位置的信息。或者是,脑血肿的发生位置可直接由三种NIR波长的讯号值分析测定,其中这三种NIR波长可分别穿透三条具有不同深度及吸亮度的路径。因此,本发明内容的第三方面是关于一种用以监测或检测一个体的脑血肿的方法。该方法包含:
(a)分别将探头及检测机构放置于第一及第二位置,其中第一及第二位置系分别选自由个体的头部、鼻腔、耳道及口腔所组成的群组;
(b)利用检测机构的第一、第二及第三NIR检测器来分别测量第一、第二及第三NIR波长在穿透个体脑部后的第一、第二及第三强度;以及
(c)将步骤(b)测得的第一、第二及第三NIR波长的第一、第二及第三强度与一健康个体的相对值(即第一、第二及第三NIR波长在穿透健康个体脑部后的第一、第二及第三强度)进行比较,若所测得的第一、第二及第三NIR波长的第一、第二及第三强度与健康个体的相对值不同,则代表该个体具有脑血肿。
为提供准确的测量结果,本发明探头及检测机构可分别放置于不同的位置。依据本发明内容一种实施方式,探头及检测机构是分别放置于个体的鼻腔及头部。依据本发明内容另一种实施方式,探头及检测机构是分别放置于个体的耳道及头部。依据本发明内容再一种实施方式,探头及检测机构是分别放置于个体的口腔及头部。

“个体”(subject)一词在本说明书是指一种包含人类的哺乳类动物,其可以本发明装置及/或方法进行检测。除非另有定义,否则“个体”(subject)一词在本说明书中同时包含男性及女性。
“近红外线”(near infrared,NIR)一词在本说明书中是指一波长介于约650到3,000奈米间的光线;更优选是介于约650到1,400奈米的光线;最优选是介于约650到1,000奈米的光线。
红血球是生物体中主要的氧气载体,因此,可藉由血红素(hemoglobin)的何种饱和程度、血中的氧分子,或氧合血红素(oxy-hemoglobin,HbO2)与脱氧合血红素(deoxy-hemoglobin,Hb)的比例来检测血氧浓度。上述二种血红素(即HbO2及Hb)对于穿透组织的近红外光光波具有不同的吸收特性。因此,可基于其NIR特性,利用由比尔-朗伯定律(Beer-Lambert law)衍生的公式来计算HbO2及Hb的浓度,进而得到血氧浓度。组织中的血氧浓度可作为一指数,以辨定组织中是否具有损伤。
比尔-朗伯定律是一种将光吸收联系至光穿透物质特性的关系表示法。有数种方式可以表示比尔-朗伯定律。光对一介质的穿透度(transmittance,T)是指光线进入介质时的光强度(IO)与离开介质时的光强度(II)的比例,可表示为:其中λ为光的波长,k为消光系数。光的吸光度(absorbance,A)为且A=α·l·c,其中l为光穿透介质的距离(即路径长度),c为介质中吸收物质的浓度,而α则为介质的吸收系数或莫耳吸光度(molar absorptivity)。
本发明是关于一种装置,可准确且有效地测量个体的脑部含氧量。本发明装置的操作是基于HbO2及Hb对NIR的吸收特性,以多种NIR波长穿透个体后,于多个位置(例如至少三种不同的位置)来测量该些NIR。更具体来说,在本发明装置中所使用的三种NIR,其中一种NIR波长是HbO2及Hb的等消光点(isosbestic point),另外二种波长则可以是近红外光光谱中的任何波长(约为650奈米至1,000奈米);二种波长中的一种波长必需比等消光点波长来得短,另一种波长则是比等消光点波长来得长。将三种波长在三种不同位置测得的NIR强度套入由比尔-朗伯定律所衍生的算法中,即可得到组织的氧合饱和度。医师可由组织的氧合饱和度来辨别是否有脑血肿,优选是,辨别出发生脑血肿的位置。
据此,本发明内容的第一方面是关于一种用以测量一具有或疑似具有脑血肿的个体其脑部含氧量的装置。
图1是依据本发明内容一种实施方式所绘示的一种用以测量脑部含氧量的装置100的示意图。此装置100包含一探头110及一检测机构120,其分别与一处理器130连接。探头110包含一个可间歇性发射出能穿透个体脑部的第一、第二及第三NIR波长的第一光源115;其中第一NIR波长是HbO2及Hb的等消光波长,第二NIR波长短于第一NIR波长,而第三NIR波长则长于第一NIR波长。检测机构120包含第一NIR检测器121、第二NIR检测器122及第三NIR检测器123;其中三个NIR检测器中,各NIR检测器皆可个别检测第一、第二及第三NIR波长在穿透个体脑部后的第一、第二及第三强度。在一例示性实施例中,探头110及检测机构120分别以光导(light guide,140,150)与处理器130连接,其中该光导是包覆于一种塑料材质中,例如聚氯乙烯(polyvinyl chloride,PVC)包覆线圈。或者是,探头110及检测机构120可以无线连接方式与处理器130连接,例如蓝牙(Bluetooth)、无线保真(wirelessfidelity,WiFi)、红外线(infrared)及超宽带连接(ultra-wideband connection)等。
图2绘示了本发明内容另一实施方式。在该实施方式中,装置200与图1绘示的装置100具有相同的组件配置,不同之处在于装置200包含三个光源,而装置100仅包含一个光源。例示性的装置200包含一探头210及一检测机构220,其分别与一处理器230连接。装置200的特色在于包含第一、第二及第三光源(215a,215b,215c)的探头210可同时发射出能穿透个体脑部的第一、第二及第三NIR波长。检测机构220包含第一、第二及第三NIR检测器(221,222,223),其中各NIR检测器(221,222,223)可分别测量第一、第二及第三NIR波长在穿透个体组织(例如头部)后的第一、第二及第三强度。与图1所示相似,探头210及检测机构220可利用包覆于塑料(例如PVC)线圈的光导(240,250)与处理器230连接,或是以无线方式(包含,但不限于蓝牙、无线保真、红外线及超宽带连接)与处理器230连接。
在本发明内容实施方式中,第一NIR波长介于约790奈米到810奈米间,第二NIR波长介于约650奈米到790奈米间,而第三NIR波长则介于约810奈米到1,000奈米间。在一特定实施例中,第一NIR波长约为808奈米,第二NIR波长约为780奈米,而第三NIR波长约为850奈米。
适用于本发明装置的光源可以是雷射二极管(laser diode,LD)或发光二极管(light emitting diode,LED),二种二极管皆可发散出NIR波长的光线。在本发明内容一优选实施方式中,是使用能发散出约650奈米至1,000奈米波长光线的LD当作光源。依据本发明内容实施方式,各光源的输出功率约为5毫瓦(milliwatt,mW)。
在操作上,探头210及检测机构220可分别放置于个体的头部、鼻腔、耳道或口腔。一般来说,探头210及检测机构220是分别放置于个体的不同位置,例如将探头210放置于鼻腔中,而将检测机构220放置于头部。或者是,可将探头210及检测机构220放置于相同位置(例如皆放置于头部),二者以预定距离相隔。举例来说,若将探头210及检测机构220放置于个体的头上,二者的相隔角度约为10到180度。
在一特定实施方式中,设置于探头210的光源(215a,215b,215c)在启动后即会发射出可穿透个体脑部的第一、第二及第三NIR波长。穿透组织后的三种NIR波长可被设置于检测机构220的NIR检测器(221,222,223)所接收。接着再由内建于处理器230中、由比尔-朗伯定律所衍生的算法来处理所测得NIR强度,据以计算个体脑部的含氧量。依据本发明内容的实施方式,是利用公式(1)及(2)来计算脑部含氧量:
其中I0及I分别表示由光源发射的NIR波长的强度及由NIR检测器测得的NIR波长的强度,R是I及I0比例的对数,λ为NIR波长,∈为血红素(hemoglobin,Hb)或氧合血红素(Oxydative Hemoglobin,HbO2)的消光系数,L为NIR波长的光路径长度,且G为吸收系数。
若欲将本发明装置探头210放置于一具有管道状结构的位置,例如鼻腔、口腔及耳道,则需要一特定构型的探头来实现本发明。图3A绘示了一种适用于鼻腔、口腔及耳道的探头310。探头310具有一管状结构,其一端设置有三个光源(315a,315b,315c)。可利用具可挠性的塑料弹性材质来制备探头310,例如橡胶、聚氯乙烯(polyvinyl chloride,PVC)、聚酰胺(polyamide,PA)、聚酯(polyester,PES)、聚对酞酸乙二酯(polyethyleneterephthalate,PET)、聚乙烯(polyethylene,PE)、高密度聚乙烯(high-densitypolyethylene,HDPE)、聚二氯亚乙烯(polyvinylidene chloride,PVDC)、低密度聚乙烯(low-density polyethylene,LDPE)、聚丙烯(polypropylene,PP)、聚苯乙烯(polystyrene,PS)、耐冲击性聚苯乙烯(high impact polystyrene,HIPS)及聚氨酯(polyurehtanes,PU)等。或者是,探头310可由二氧化硅(silica)来制备,例如熔融硅石玻璃(fused silica glass)、钠钙硅玻璃(soda-lime-silica glass)、硼硅酸钠玻璃(sodiumborosilicate glass)、氧化铅玻璃(lead-oxide glass)及铝硅玻璃(aluminosilicateglass)。探头310可以更包含一间隔组件(spacer element,330),其设置于管状结构外表面的一侧,且靠近设有光源(315a,315b,315c)的一端。在操作上,是将探头310放置于鼻腔、口腔或耳道,间隔组件330可提供一额外的空间,避免管道阻塞。探头310的长度足以使探头310放置至特定位置,例如鼻腔、口腔及耳道。一般来说,探头310的长度约为5到30公分,例如5公分、10公分、15公分、20公分、25公分或30公分。
依据本发明内容一种实施方式,探头310可还包含一照相机350。在该实施例中,光源(315a,315b,315c)及照相机350是设置于探头310的同一端。依据本发明内容另一实施方式,在探头310设置照相机350及光源(315a,315b,315c)的一端具有一倾斜面,藉以使探头310较易放置入管道结构中。在该实施方式中,该倾斜面与探头310的纵轴方向具有一约45±10度的夹角θ。
图3B提供一种实施例,其中探头360及检测机构370是分别设置为垫片形式。各垫片由上至下的组成为一分离层(releasing film)、一粘着层(adhesive layer)及一支撑基质(supporting substrate),其中探头360或检测机构370是藉由粘着层固定于垫片的支撑基质上。操作时,使用者先撕下分离层,曝露出利用粘着层固定于支撑基质的三个光源(365a,365b,365c)或三个NIR检测器(375,376,377),再通过按压使粘着层贴附于特定位置,使垫片固定于特定位置(例如头部)。依据本发明内容实施方式,可利用任何常规材料来制备本发明垫片,优选是利用弹性聚氨酯、天然或合成橡胶或织物来制备本发明垫片。
在其他实施方式中,是将本发明装置(100或200)的探头及检测机构配置在一可戴于个体头部的头戴装置上。图4绘示了头戴装置400,其系包含一轨道状结构401以在其上架设探头410及检测机构420,其中探头410及检测机构420彼此相隔角度为180度;以及一驱动装置430与探头410及检测机构420相连接,藉以驱动探头410及检测机构420在轨道状结构401上同步进行圆周运动。可利用塑料或金属材料来制备头戴装置400。优选的情况是,头戴装置400本身为一中空结构,以减轻头戴装置的重量。非必要性地,头戴装置400更包含一固定机构(securing means)450,藉以使头戴装置400可依照用户需求来重设大小。固定机构450可以是一由弹性材料制成的条带、钩夹或环扣(例如子母沾,VelcroTM)。
由于血块会影响NIR光波的穿透,本发明装置亦可用以监测脑血肿。具体来说,是通过分析穿透脑部的第一、第二及第三NIR强度的信号值来检测脑血肿。据此,本发明内容的另一方面是关于一种利用本发明装置来监测或检测个体脑血肿的方法。该方法包含:
(a)分别将探头及检测机构放置于第一及第二位置,其中第一及第二位置系分别选自由个体的头部、鼻腔、耳道及口部所组成的群组;
(b)利用检测机构的第一、第二及第三NIR检测器来测量第一、第二及第三NIR波长在穿透个体脑部后的第一、第二及第三强度;以及
(c)将步骤(b)测得的第一、第二及第三NIR波长的第一、第二及第三强度与一健康个体的相对值(即第一、第二及第三NIR波长在穿透健康个体脑部后的第一、第二及第三强度)进行比较,若所测得的第一、第二及第三NIR波长的第一、第二及第三强度与健康个体的相对值不同,则代表该个体具有脑血肿。
临床医师及/或操作人员可依据个体的年龄、病历及近期病况来决定适合放置本发明装置的探头及检测机构的第一及第二位置。如图5所示,可将探头510及检测机构520分别放置于个体的额头及头部前方,以检测邻近运动前区(premotor area)、初级运动皮质(primary motor cortex)或初级躯体感觉皮质(primary somesthetic cortex)的脑血肿(brain hematoma,BH)。
在图6所示的实施例中,是将配置为管状结构的探头610放置于个体的鼻腔,并将设置为垫片的检测机构620放置于个体的头部前方,以检测邻近布格卡皮质区(Broca’sarea)的脑血肿BH。在再一实施例中,是将探头610放置于个体的鼻腔,并将检测机构620放置于个体的头部后方,以检测邻近躯体感觉相关区域(somesthetic association area)或威尼克氏区(Wernicke’s area)的脑血肿BH。
相似地,如图7所示,可将设置为管状结构的探头710放置于个体的耳道,并将设置为垫片的检测机构720放置于个体的头部后方,以检测位于主要听觉皮质区(primaryauditory cortex)或味道区域(taste area)的脑血肿BH。
在再一实施方式中,如图8所示,是将设置为管状结构的探头810放置于个体的口腔,并将设置为垫片的检测机构820放置于个体的头枕部,以检测邻近视觉相关区域(visual association area)或视觉皮质(visual cortex)的脑血肿BH。
下文提出多个实验例来说明本发明的某些方面,以利本领域技术人员实施本发明,且不应将这些实验例视为对本发明范围的限制。
实施例
1.1以包埋墨条作为脑血肿仿体
在本实施例中,是将墨条(1立方公分:0.65公分×0.9公分×1.7公分;2.5立方公分:1.63公分×0.9公分×1.7公分;或5立方公分:3.25公分×0.9公分×1.7公分)包埋于一个由二氧化钛(titanium oxide)及聚酯树脂(polyester resin)制成的塑料对象中(墨条与塑料对象表面的距离分别为0.5公分、1.7公分或2.5公分),藉以模拟不同情况的脑血肿(具有不同的血块大小及发生位置)。探头可发射分别具有808奈米、780奈米及850奈米波长的NIR。操作时,分别将探头及检测机构放置在塑料对象(即脑血肿仿体)的相同侧(如传统检测方法一般,将光源及检测器配置于邻近位置,彼此相邻近地作用)或是相对两侧。图9及图10分别阐述置于相同侧及相对两侧的检测结果。
当探头与检测机构的位置相近时,对照组(即不包含墨条的塑料对象,标记为基本含量;图9A)与仿体组(即将墨条包埋于特定深度的塑料对象;图9B到9D)之间并不具有显著的差异。相较之下,当将探头及检测机构放置于塑料对象(即仿真脑血肿位置)的相对两侧时,可观察到相较于对照组(标记为基本含量;图10A),信号值会随着墨条包埋深度的不同而有所差异(图10B到10D)。这些结果显示,相较于传统的检测方法(测量会受限于光源及检测器的配置位置),用户可以不同的间隔距离来独立操作本发明探头及检测机构,因此,本发明装置及方法可更准确地测量发生于不同深度的脑血肿。
检测到的NIR强度经计算分析后可表示为含氧量,结果总结于表1,其中探头及检测机构是置于塑料对象的相对两侧。塑料对象(具有不同的血块大小及发生位置)的平均含氧量约为86.74%-91.05%。

1.2检测猪头中的墨条仿体
在此实施例中,是将实施例1的墨条(1立方公分、2.5立方公分或5立方公分)放置于猪头的颞叶中,据以模拟个体的脑血肿。将图3A所述的探头310放置于猪头的鼻腔,并将图3B所述的检测机构370放置于猪头的头部。设定测量波长为808奈米、780奈米及850奈米。如图11所示,含有0立方公分(图11A,作为健康对照组)、1立方公分(图11B)、2.5立方公分(图11C)或5立方公分(图11D)的猪头的脑部含氧量约为56-63%。
总结上述,本发明内容提供了一种用以检测个体的脑部含氧量或监测个体的脑血肿的装置及方法。相较于传统检测及显影技术,本发明装置的优势在于光源及检测器可独立操作,依据不同的情况分别放置于适当的位置来检测脑血肿。据此,本发明内容提供一种更为准确及有效的方法来检测脑部含氧量及/或脑血肿;藉此提供有需要的病患更适当且及时的治疗。
虽然上文实施方式中揭露了本发明的具体实施例,然其并非用以限定本发明,本技术领域技术人员在不悖离本发明的原理与精神的情形下,当可对其进行各种更动与修饰,因此本发明的保护范围当以附随申请专利范围所界定者为准。
Claims (5)
1.一种用以测定一个体脑部含氧量及/或监测脑血肿的装置,包含
一探头,其包含一第一、一第二及一第三光源,可分别同时发射出可穿透个体脑部的一第一、一第二及一第三近红外光(near infrared,NIR)波长,其中所述第一、第二及第三NIR波长分别为790-810奈米、650-790奈米及810-1,000奈米;
一检测机构,其包含一第一、一第二及一第三NIR检测器,其中各NIR检测器可分别检测所述第一、第二及第三NIR波长在穿透所述个体脑部后的第一、第二及第三强度;
一处理器,与所述探头及所述检测机构连接,可通过所述检测机构所检测到的所述第一、第二及第三强度来计算脑部含氧量或分析脑血肿;
一环形头戴装置,可戴于所述个体头部,包含,
一环形轨道状结构,可将所述探头及所述检测机构架设于其上,其中所述探头及所述检测机构设置在所述轨道状结构的对立点,从而彼此相隔180度;以及
一驱动装置,其设置于所述环形轨道状结构上,且与所述探头及所述检测机构连接,以驱动所述探头及所述检测机构于所述环形轨道状结构上,以与所述环形头戴装置平行的方向同步进行圆周运动。
2.如权利要求1所述的装置,其中所述第一、第二及第三NIR波长分别为808奈米、780奈米及850奈米。
3.如权利要求1所述的装置,其中各所述第一、第二及第三光源是一雷射二极管(laserdiode,LD)或一发光二极管(light emitting diode,LED)。
5.如权利要求1所述的装置,其中所述头戴装置为可调整以符合所述个体的头部大小。
