Abstract
The invention discloses a puncture sheath with a suture function, which is characterized in that: it comprises a sheath body and a peritoneal suture device inserted into the sheath body. The rod-shaped suture device body of the peritoneal suture device stretches into the laparoscopic operation wound through the puncture sheath, the suture needle on the suture device body or the head valve passes through the peritoneum, the suture thread head is reserved on the head valve or the suture device body, and the thread head is carried out when the suture device body is pulled out, so that the suture of the wound is realized. The spread head valve can play a role in positioning abdominal wall body tissues and preventing leakage, and can also prevent other viscera and tissues from being damaged by suturing. The invention has simple operation and good suture effect, can effectively prevent secondary injury during wound suture and reduce incidence of incision hernia.

Background
Laparoscopic surgery is a newly developed minimally invasive surgical procedure, so that many of the past open surgery has now been replaced by endoluminal surgery, greatly increasing the surgical options. With the development of laparoscopic techniques, laparoscopic surgery is accepted by more and more doctors and patients. The laparoscopic surgery must be performed by creating a working channel through a puncture sheath (trocar) in the abdominal wall and then using a specially-made elongated surgical instrument to perform the same procedure as the open surgery under television monitoring to achieve the same surgical result. At present, the puncture sheath has a specification of 5-12mm in diameter, and generally, for the puncture hole of the abdominal wall larger than 5mm, the peritoneum and fascia layers must be sutured to prevent incision bleeding and reduce the incidence rate of incision hernia. While conventional arcuate needle suturing methods cannot be performed for such small incisions, particularly for obese patients, straight needle suturing methods cannot correspond to the tissue on both sides of the small incision due to the lack of necessary guiding and positioning means, and it is difficult to achieve the desired suturing result.
Disclosure of Invention
The invention aims at solving the defect that the existing suture instrument is difficult to suture a laparoscopic operation wound, and provides a puncture sheath with a suture function, which is convenient for suture the wound and reduces the occurrence rate of incision hernia.
The invention adopts the following technical scheme to realize the aim:

It is further characterized by: the two head petals are connected to the head part of the stitching instrument body through a middle pivot, and the tail end of the stitching instrument body is connected with the push rod head part through a pivot; the suture device is characterized in that suture needle grooves which incline downwards are formed in two side walls of the suture device body, suture needles are arranged in the suture grooves, and two inclined planes are arranged on the push rod and are abutted against the tail parts of the suture needles.
Preferably: the suture needle groove is a stepped hole, the tail of the suture needle is provided with a step, and a reset spring is arranged between the suture needle step and the suture needle groove step.
And the valve thread head indwelling area is filled with silica gel.
The diameter of the suture instrument body is 5-12mm, and the head part of the combined head valve is arc-shaped or conical.

Detailed Description
Embodiment one:
as shown in fig. 1-3, a puncture sheath with suturing function comprises a sheath body 1 and a peritoneal stapler. The peritoneal suture device comprises a rod-shaped hollow suture device body 11 with the diameter of 5-12mm and a push rod 12 arranged in the suture device body 11; the stapler body 11 has a detachable head piece 13 at the head. The number of the head petals 13 is two, and the head part of the head petals 13 is circular arc-shaped or conical after being combined. The middle part of the head flap 13 is pivotally connected to the head of the stitching instrument body 11, and the tail end shares a pivot to be connected with the head of the push rod 12; the suture device body 11 is provided with suture needle grooves 16 which incline downwards in the two side walls, and the suture needles 14 are arranged in the suture grooves 16. The suture needle groove 16 is a stepped hole, the tail of the suture needle 14 is provided with a step, and a reset spring 17 is arranged between the step of the suture needle 14 and the step of the suture needle groove 16. The push rod 12 is provided with two inclined planes which are propped against the tail of the suture needle 14. The valve thread head indwelling area is filled with silica gel 15.
As shown in fig. 4, in use, the suture device 11 is inserted into a laparoscopic surgical wound through the sheath 1, and the push rod 12 presses the opening head flap 13 downward to press the peritoneum. The two slopes on the push rod 12 descend to push the suture needle 14 out of the suture needle slot 16, across the peritoneum, and into the silicone 15 of the valve head 13. The push rod 12 moves upwards, the suture needle 14 is retracted under the action of the return spring 17, and the thread ends are left in the silica gel 15 of the head flap 13. Simultaneously, the push rod 12 moves upwards to drive the head flap 13 to fold, and when the suture device body 11 is pulled out, two thread ends of the suture thread are taken out together, and the suture thread is tightened, so that the peritoneal suture can be realized.
Embodiment two:
as shown in fig. 1, 5 and 6, a puncture sheath with suture function comprises a sheath body 1 and a peritoneal suture device. The peritoneal stapler comprises a hollow stapler body 21 with a rod shape and a diameter of 5-12mm, and a push rod arranged in the stapler body 21. The stapler body 21 has a detachable head 24 at the head. The number of the head petals 24 is two, and the head part of the head petals 24 is circular arc-shaped or conical after being combined; the middle part of the head flap 24 is pivotally connected to the head of the stitching instrument body 21, and the tail end of the head flap shares a pivot for connecting with the head of the push rod. The push rod comprises a push rod sleeve 22 and a push rod core 23, and the push rod core 23 is sleeved and inserted in the push rod sleeve 22; the head of the push rod core 23 is pivotally connected to the head flap 24. A return spring 29 is provided between the upper surface of the tail portion and the top of the inner cavity of the push rod sleeve 22. The head valve 24 is internally provided with an upward circular arc-shaped suture needle groove 26, a suture needle 25 which is matched with the radian of the suture needle groove 26 is arranged in the suture needle groove 26, the tail part of the suture needle 25 is connected with an elastic steel wire 28, and the elastic steel wire 28 is fixedly arranged on the push rod sleeve 22 of the push rod. The push rod core 23 presses the head flap 24 downwards to press the peritoneum; the push rod sleeve 22 is pushed down to drive the suture needle 25 to be inserted into the suture body 21 to leave the suture thread end in the side wall of the suture body 21. The suture body 21 is filled with silicone 27 in the thread end retention area.
As shown in fig. 6, when the suture device body 21 is inserted into a laparoscopic operation wound through the puncture sheath 1 in use, the push rod sleeve 22 presses the elastic steel wire 27 downwards to push the suture needle 25 to extend out along the circular arc-shaped suture needle groove 26, and meanwhile, the push rod sleeve 22 overcomes the spring force of the return spring 29 to drive the push rod core 23 to press the head flap 24 downwards to press the peritoneum. The suture needle 25 is pushed out from the suture needle groove 26, passes through the peritoneum, and is inserted into the silica gel on the side wall of the suture instrument body 21. The push rod moves upwards, the suture needle 25 is retracted under the drive of the elastic steel wire 27, and the thread ends are remained in the silica gel 27 of the suture device body 21. Simultaneously, the push rod core 23 moves upwards to drive the head valve 24 to fold, and when the suture device body 21 is pulled out, two thread ends of the suture thread are taken out together, and the suture thread is tightened, so that the peritoneal suture can be realized.

the two head petals are connected to the head part of the stitching instrument body through a middle pivot, and the tail end of the stitching instrument body is connected with the push rod head part through a pivot; the suture device is characterized in that suture needle grooves which incline downwards are formed in two side walls of the suture device body, suture needles are arranged in the suture grooves, and two inclined planes are arranged on the push rod and are abutted against the tail parts of the suture needles.
