Abstract
本发明涉及VEGF单抗联合纳米紫杉醇在制备治疗癌性腹腔粘连药物中的应用以及一种VEGF单抗联合纳米紫杉醇组合物,将VEGF单抗与纳米紫杉醇进行联合,形成复合物,在治疗癌性腹腔粘连方面具有意想不到的技术效果,与单独施用VEGF单抗进行治疗相比,腹腔粘连、肠梗阻等症状改善更为明显,同时毒副作用较小。另外,采用壳聚糖纳米包裹强化后的VEGF单抗联合纳米紫杉醇,可以使作用持续时间长并且可以调控作用时间和作用强度,疗效进一步增强的同时,进一步降低毒副作用,降低并发症发生概率。

技术领域
本发明属于药物制剂的应用领域,特别涉及VEGF单抗联合纳米紫杉醇在制备治疗癌性腹腔粘连药物中的应用以及一种VEGF单抗联合纳米紫杉醇组合物。
背景技术
腹腔粘连的治疗一直是临床医生尚未解决的重要难题,然而,直至目前尚无标准规范治疗方案。根据国外近年的试验研究和临床观察的Mate分析显示:目前尚无一种手术措施可以完全的预防腹腔粘连形成,并在临床广泛应用。近年来很多学者从手术因素着手,研究控制和改善由于手术导致的腹腔粘连的药物,有了很大的进展,例如减轻炎症反应的药物(非甾体类抗炎药、壳聚糖或称几丁糖)、促纤维蛋白溶解(尿激酶、组织纤维酶原激活剂等)、防止纤维蛋白沉积(奥曲肽、抑肽酶)、抗粘连物(血管内皮生长因子(VEGF)单抗、右旋糖酐、胶原材料、纤维素等)、美蓝、卡铂复合液等。但是从肿瘤所致腹腔粘连的研究甚少,癌性腹腔粘连的治疗仍处于探索阶段。
Epstein JC等发现VEGF可致肿瘤浸润腹膜组织通透性增高,产生纤维蛋白原等粘连物漏出,在腹膜腔内沉积、机化,形成粘连索带,并有大量血管活性的新生血管包绕,导致癌性腹腔粘。Karanlik H等发现腹腔灌注贝伐单抗对结肠癌吻合术后粘连形成有临床意义,常规剂量贝伐珠(5mg/Kg)可能增加吻合口破裂压力,但有效地减少粘连的形成,比对照组显著降低粘附分数;如果贝伐减半至2.5mg/Kg,没有伤口裂开,既没有影响吻合口及伤口愈合,还有效地减少了形成粘连,针对抗血管生成疗法预防腹腔粘连,认为粘连形成是血管发生依赖性的并且是部分地通过VEGFR信号传导通路。采用舒尼替尼(VEGFR-2拮抗剂)可以显著降低在鼠模型粘连形成,提示抗VEGF抗体在治疗腹膜粘连中发挥了举足轻重的作用,但是目前只施用VEGF单抗以治疗癌性腹腔粘连疗效还较差,且静脉注射VEGF单抗会引发更多并发症并增加死亡率,包括出血(23.5%)和胃肠道穿孔(7.1%)。
紫杉醇作为抗癌药物,在卵巢癌中具有明显的抗肿瘤和抗癌作用,并且在宽范围的肿瘤模型中也表现了优异的抗肿瘤活性,但目前未见报道其具有抗腹腔粘连之功效。
发明内容
本发明提供VEGF单抗联合纳米紫杉醇在制备治疗癌性腹腔粘连药物中的应用,将VEGF单抗与纳米紫杉醇进行联合,形成的复合物,对于癌性腹腔粘连具有显著的疗效,明显优于单独施用VEGF单抗的疗效。

优选地,壳聚糖纳米包裹强化的VEGF单抗联合纳米紫杉醇中,所述VEGF单抗联合纳米紫杉醇与壳聚糖的重量份数比为:1:7:40。
优选地,所述治疗癌性腹腔粘连药物为腹腔注射用药。
本发明另一方面提供了一种壳聚糖纳米包裹强化VEGF单抗联合纳米紫杉醇复合物,所述复合物主要由如下重量份的原料制备而成:壳聚糖45-224、纳米紫杉醇8-13、VEGF单抗1-3。
另一方面,本发明还提供所述壳聚糖纳米包裹强化VEGF单抗联合纳米紫杉醇复合物在制备治疗癌性腹腔粘连的药物中的应用。
本发明将VEGF单抗与纳米紫杉醇进行联合,形成复合物,在治疗癌性腹腔粘连方面具有意想不到的技术效果,与单独施用VEGF单抗进行治疗相比,腹腔粘连、肠梗阻等症状改善更为明显,同时毒副作用较小。
壳聚糖纳米包裹强化后的VEGF单抗联合纳米紫杉醇,疗效进一步增强、药效持续时间延长的同时,进一步降低毒副作用,降低并发症发生概率,同时可以通过调节壳聚糖纳米包裹量,来灵活调节VEGF单抗联合纳米紫杉醇的药效作用时间与作用强度。
具体实施方式
下面结合实施例对本发明作进一步的描述,下列实施例仅用于解释本发明的发明内容,不用于限定本发明的保护范围。
实施例1 VEGF单抗联合纳米紫杉醇在小鼠术后腹腔粘连的治疗作用
1.1动物:昆明小鼠,雌雄各半
1.2腹腔损伤模型的建立
术前动物称重,按照38mg/kg的用量给药戊巴比妥钠,完全麻醉后,无菌条件下,取下腹正中进行腹部切口,找到盲肠及升结肠后,将其轻轻提起,在肠管外周各侧面用纱布反复摩擦30次,中线右侧的壁层腹膜用止血钳多出反复钳夹30次,然后用可吸收缝合线缝合腹膜,最后缝合线结扎前将治疗药物缓慢注入腹腔内,然后结扎缝合,逐层关腹。术后禁食12h,分笼标准喂养,不用任何药物。
1.3粘连评价标准
术后20天后,处死小鼠,在元切口上方左右肋缘下斜切口检查上次腹部正中切口的完整性和腹腔内粘连情况,通过对粘连程度,位置及类型的综合评价,得出各组粘连分数,粘连分级标准参考郑振华的分级方法进行三级分类:0级:无粘连;1级:轻度粘连,粘连容易分离;2级:中度粘连,粘连难以分离,但局限于一个区域;3级:重度粘连,呈纤维性粘连增生,可见多条粘连待,肠粘连或肠管固定于腹壁构成团块状。
1.4分组情况
取昆明小鼠50只,随机5组,每组10只,雌雄各半,对照组治疗药物为生理盐水;试验1组治疗药物为兰尼单抗;试验2组为纳米紫杉醇;试验3组为兰尼单抗联合纳米紫杉醇(兰尼单抗与纳米紫杉醇重量比为3:8);试验4组为壳聚糖纳米包裹强化的VEGF单抗联合纳米紫杉醇(兰尼单抗、纳米紫杉醇和壳聚糖重量比为3:8:45);其中对照组生理盐水浓度为150mmol/L,试验1-4组治疗药物在试验前均用浓度为150mmol/L生理盐水按照1:100的体积稀释。
VEGF单抗联合纳米紫杉醇的制备方法,以及壳聚糖对VEGF单抗联合纳米紫杉醇进行纳米包裹强化的工艺,按照常规白蛋白联合纳米紫杉醇的制备方法以及常规壳聚糖纳米包裹方法制备即可,工艺成熟,本领域技术人员无需创造性劳动即可制备,因此本发明不做详述。
1.5试验结果
表1 VEGF单抗联合纳米紫杉醇在小鼠术后腹腔粘连的治疗作用
由上述试验结果可知,对照组小鼠粘连发生率在100%,即在该处理条件下,小鼠会发生中度以上的腹腔粘连症状;单纯兰尼单抗对小鼠腹腔粘连具有一定的治疗作用,将粘连率降至70%;单纯纳米紫杉醇与对照组相比,纳米紫杉醇组的重度腹腔粘连比重增加,对小鼠腹腔粘连症状无治疗作用的情况下,有一定的恶化反作用;而将兰尼单抗与纳米紫杉醇进行联合后形成的复合物,大幅提高了小鼠腹腔粘连的治疗效果,具有意想不到的技术效果,而壳聚糖包裹后,治疗效果更佳。
实施例2不同VEGF单抗形成的VEGF单抗联合纳米紫杉醇在小鼠术后腹腔粘连的治疗作用
本实施例所用的试验动物、粘连评价标准以及腹腔损伤模型的建立与实施例1相同。
2.1分组情况
取昆明小鼠72只,随机6组,每组12只,雌雄各半,试验1组治疗药物为贝伐单抗联合纳米紫杉醇;试验2组治疗药物为兰尼单抗联合纳米紫杉醇;试验3组治疗药物为舒尼替尼联合纳米紫杉醇;试验4组治疗药物为雷珠单抗联合纳米紫杉醇;试验5组治疗药物为哌加他尼联合纳米紫杉醇;试验6组治疗药物为VEGF兔IgG多抗联合纳米紫杉醇;VEGF抗体与纳米紫杉醇重量比为3:13,治疗药物在试验前均用浓度为150mmol/L生理盐水按照1:100的体积稀释。
2.2试验结果
表2不同VEGF单抗对VEGF单抗联合纳米紫杉醇在小鼠术后腹腔粘连的治疗作用的影响
由上述试验结果可知,不同的VEGF单抗联合相同的纳米紫杉醇,对小鼠的腹腔粘连均有一定的治疗效果,但是疗效并不相同,经试验发现兰尼单抗和雷珠单抗与纳米紫杉醇相联合而得到的复合物对小鼠腹腔粘连治疗效果较佳。
实施例3不同重量份数比的VEGF单抗与纳米紫杉醇制备而成的VEGF单抗联合纳米紫杉醇在小鼠术后腹腔粘连的治疗作用
本实施例所用的试验动物、粘连评价标准以及腹腔损伤模型的建立与实施例1相同。
3.1分组情况
取昆明小鼠80只,随机8组,每组10只,雌雄各半,选用雷珠单抗与纳米紫杉醇进行联合,考察两个原料不同配比得到的复合物针对腹腔粘连的治疗效果,治疗药物在试验前均用浓度为150mmol/L生理盐水按照1:100的体积稀释。
3.2试验结果
表3不同重量份数比的VEGF单抗与纳米紫杉醇对VEGF单抗联合纳米紫杉醇在小鼠术后腹腔粘连的治疗作用的影响
由上述试验结果可知,不同重量份数比的VEGF单抗与纳米紫杉醇经联合制备的VEGF单抗联合纳米紫杉醇对小鼠腹腔粘连的治疗效果差异较大,一般来说VEGF单抗具有一定的抗腹腔粘连的作用,而纳米紫杉醇没有该功效,将VEGF单抗与纳米紫杉醇进行联合,将有效成分VEGF单抗的含量大于纳米紫杉醇,例如VEGF单抗重量为纳米紫杉醇重量的2倍,预期疗效将优于VEGF单抗与纳米紫杉醇重量相同的试验例,但试验结果与预期相反,随着纳米紫杉醇重量份数的增加,疗效反而增强,当VEGF单抗与纳米紫杉醇重量份数比由3:7升至3:8后,粘连率由60%下降一半,到30%,当VEGF单抗与纳米紫杉醇重量份数比为3:21时,疗效最佳,腹腔粘连率下降至10%,当VEGF单抗与纳米紫杉醇重量份数比为3:36之后,随着纳米紫杉醇重量份数继续增加疗效显著降低,因此得到优选的VEGF单抗与纳米紫杉醇的重量份数比例,具有意想不到的技术效果。
实施例4不同重量份数比的VEGF单抗联合纳米紫杉醇与壳聚糖制备而成的壳聚糖纳米包裹强化VEGF单抗联合纳米紫杉醇在小鼠术后腹腔粘连的治疗作用
本实施例所用的试验动物、粘连评价标准以及腹腔损伤模型的建立与实施例1相同。
4.1分组情况
取昆明小鼠96只,随机8组,每组12只,雌雄各半,雷珠单抗联合纳米紫杉醇与壳聚糖进行包裹形成壳聚糖纳米包裹强化VEGF单抗联合纳米紫杉醇,考察两个原料不同配比得到的复合物针对腹腔粘连的治疗效果,治疗药物在试验前均用浓度为150mmol/L生理盐水按照1:100的体积稀释。
4.2试验结果
表4不同壳聚糖纳米包裹强化VEGF单抗联合纳米紫杉醇在小鼠术后腹腔粘连的治疗作用
由上述试验结果可知,壳聚糖纳米包裹VEGF单抗联合纳米紫杉醇之后,疗效的增强也与壳聚糖与VEGF单抗联合纳米紫杉醇的重量份数比具有较密切的关系,当壳聚糖与VEGF单抗联合纳米紫杉醇的重量份数比为1:5时,可将粘连发生率降低至0%,疗效最佳。

1.VEGF单抗联合纳米紫杉醇在制备治疗癌性腹腔粘连药物中的应用,所述VEGF单抗与所述纳米紫杉醇的重量份数比为:1-3:8-13。
2.如权利要求1所述的VEGF单抗联合纳米紫杉醇在制备治疗癌性腹腔粘连药物中的应用,其特征在于,所述VEGF单抗为兰尼单抗。
3.如权利要求1所述的VEGF单抗联合纳米紫杉醇在制备治疗癌性腹腔粘连药物中的应用,其特征在于,所述VEGF单抗为雷珠单抗。
4.如权利要求1所述的单抗联合纳米紫杉醇在制备治疗癌性腹腔粘连药物中的应用,其特征在于,所述治疗癌性腹腔粘连药物为腹腔注射用药。
5.VEGF单抗联合纳米紫杉醇在制备治疗癌性腹腔粘连药物中的应用,其特征在于,所述VEGF单抗联合纳米紫杉醇为壳聚糖纳米包裹强化的VEGF单抗联合纳米紫杉醇。
6.如权利要求5所述VEGF单抗联合纳米紫杉醇在制备治疗癌性腹腔粘连药物中的应用,其特征在于,所述VEGF单抗联合纳米紫杉醇与壳聚糖的重量份数比为:1-2:5-7。
7.如权利要求5所述的VEGF单抗联合纳米紫杉醇在制备治疗癌性腹腔粘连药物中的应用,其特征在于,所述VEGF单抗、纳米紫杉醇和壳聚糖的重量份数比为:1:7:40。
8.一种壳聚糖纳米包裹强化VEGF单抗联合纳米紫杉醇复合物,其特征在于,所述复合物主要由如下重量份的原料制备而成:壳聚糖45-224、纳米紫杉醇8-13、VEGF单抗1-3。
9.如权利要求8所述的一种壳聚糖纳米包裹强化VEGF单抗联合纳米紫杉醇复合物在制备治疗癌性腹腔粘连的药物中的应用。
