Technical Field
The invention relates to the field of circumcision, in particular to a C-shaped everting type circumcision stitching instrument.
Background
Too long prepuce or phimosis is one of the sources of urinary tract infections in men and sexually transmitted diseases. Too long prepuce or phimosis can cause urinary tract infection to cause chronic prostatitis, and a series of symptoms such as soreness of waist and back, impotence and premature ejaculation can occur, so that the removal of too long prepuce is one of good measures for preventing the symptoms.
Traditionally, phimosis or overlong foreskin is removed by surgical operation, and the technical points are to cut redundant foreskin, stop bleeding and incised edge skin alignment suture. Patients can not walk after operation, and the patients can not bear huge pain when taking out stitches finally after changing one medicine, and in addition, the prepuce hematoma can be caused by incomplete ligation hemostasis, so that the patients need to be treated by operation again. In addition, since the circumcision and the hemostatic process are separately performed, the operation time is prolonged, and the panic mind of the patient is aggravated.
Laser and high frequency electrotome techniques have been developed for circumcision treatments that, although replacing scissors cuts, can coagulate bleeding sites. However, this treatment burns the patient's tissue and is susceptible to infection.
In the prior art, the foreskin incision stitching instrument is used for stitching the foreskin incision once, but most of the foreskin and the penis of the traditional built-in foreskin incision stitching instrument adopted in the prior art are exposed outside in the operation process, so that the foreskin incision stitching instrument is easy to be damaged or infected, the foreskin clamping part of the built-in foreskin incision stitching instrument is positioned outside the foreskin incision stitching instrument, the stitching position of the U-shaped nail is positioned at the top end of the foreskin incision stitching instrument, and when bleeding is caused by instrument failure or misoperation, the foreskin incision stitching instrument is easy to be infected. The ring cutter of the built-in prepuce cutting suture instrument is built-in, adopts an internal cutting outer Zhou Dading, and is easy to cause pain and risk caused by stacking cutting under the conditions of overlong prepuce and the like. In addition, if only the upper cover (the upper comprehensive cover) and the lower cover (the lower comprehensive cover/the connecting cover) are matched with each other and the inner ring (the foreskin fixing hoop) and the inner comprehensive cover (the staple top ring and/or the thimble guide block) are matched with each other, the circular cutting and the staple hemostasis are carried out after the clamping in place, so that foreskin blood vessels cannot be accurately positioned due to the fact that the positions of the built-in staple grooves are invisible, the staples in the staple grooves cannot be accurately corresponding to the foreskin blood vessels during operation, the operation hemostasis effect is poor, even the fact that the staple heads are just driven into the blood vessels, and the operation risk cannot be caused by timely hemostasis occurs. In addition, the prior art directly nails the staples into the foreskin, and in the process of waiting for necrosis and falling off of the foreskin and removing the staples after the operation, adhesion between the staples and the foreskin and even incapability of timely removing the staples after the foreskin is deep inside the foreskin easily occur, so that great pain is caused to a patient.
Disclosure of Invention
Aiming at the problems in the prior art, the invention aims to provide an everting type foreskin cutting stitching instrument, the structure of the everting type foreskin cutting stitching instrument can realize that the front end of a penis is mostly contained in the foreskin cutting stitching instrument in the operation process, the clamping and ring cutting parts are all internally arranged, the situations of bleeding infection and the like are avoided, the external ring cutter and the blood vessel butting device reduce the risk of the operation, and the arrangement of a U-shaped nail pad further avoids the problem of falling failure of a U-shaped nail in the later stage, and the specific technical scheme is as follows:
a C-shaped everting type foreskin cutting stitching instrument comprises an upper comprehensive cover, a lower comprehensive cover, a foreskin fixing hoop and a U-shaped nail device, wherein,
the upper comprehensive cover and the lower comprehensive cover are in releasable matching connection;
the foreskin fixing hoop is wholly or partially arranged in the upper comprehensive cover;
the U-shaped nail device is wholly or partially arranged in the lower comprehensive cover;
the foreskin fixing hoop can be positioned in the upper comprehensive cover and matched with the upper comprehensive cover to clamp the foreskin, and/or the lower end of the foreskin fixing hoop can be matched with the U-shaped device to clamp the foreskin.
Further, the staple device comprises a staple ejector ring and a staple ejector guide block, wherein the staple ejector block is used for accommodating staples for stitching and/or guiding movement of the staples for stitching and/or positioning and/or guiding a staple ejector of the staple ejector ring, and the staple ejector ring is used for being matched with the staple ejector block to eject the staples in the staple ejector ring.
Further, the ejector pin guide block also comprises a U-shaped nail gasket which is an annular gasket arranged at the top end of the ejector pin guide block.
Further, the U-shaped gasket is also provided with a gasket positioning table perpendicular to and/or angled with respect to the gasket plane, and the gasket positioning table is one or more bulges and/or complete annular bulges arranged at the inner periphery and/or the outer periphery and/or the middle of the gasket plane of the U-shaped gasket.
Further, a first through hole is formed in the middle of the thimble guide block, the thimble guide block comprises an annular inner wall and/or an annular outer wall and/or a positioning table accommodating groove, the annular inner wall is used for being mutually matched and positioned with one or more bulges and/or complete annular bulges arranged on the inner periphery of a gasket plane of the U-shaped gasket, and/or the annular outer wall is used for being mutually matched and positioned with one or more bulges and/or complete annular bulges arranged on the outer periphery of the gasket plane of the U-shaped gasket, and/or the positioning table accommodating groove is arranged at the top end of the thimble guide block and is used for accommodating one or more bulges and/or complete annular bulges arranged in the middle of the gasket plane of the U-shaped gasket.
Further, the thimble guide block is provided with a plurality of thimble guide grooves, the thimble guide grooves are annularly arranged on the annular thimble guide block, and the part or the whole part of the U-shaped nail gasket covers the thimble guide grooves.
Further, the lower end face of the upper comprehensive cover is annular, a convex positioning table and/or a concave positioning groove is formed along the annular end face, the upper end face of the lower comprehensive cover is annular, a concave positioning groove and/or a convex positioning table is formed along the annular end face, and the positioning groove and the positioning table are in concave-convex fit with each other to position the upper comprehensive cover and the lower comprehensive cover; or,
the connecting cover is arranged between the upper comprehensive cover and the lower comprehensive cover, the upper comprehensive cover is detachably matched and connected to the lower comprehensive cover through the connecting cover, and preferably, the U-shaped device is wholly or partially arranged in the connecting cover; or,
the connecting cover is arranged between the upper comprehensive cover and the lower comprehensive cover, the lower end face of the upper comprehensive cover is annular, a convex positioning table and/or a concave positioning groove are arranged along the annular end face, the upper end face of the connecting cover is annular, a concave positioning groove and/or a convex positioning table are arranged along the annular end face, and the positioning groove and the positioning table are in concave-convex fit with each other to position the upper comprehensive cover and the connecting cover.
Further, the upper end face of the connecting cover is annular, a convex positioning table is arranged along the annular end face, and the positioning table is matched with a positioning groove arranged on the lower end face of the upper comprehensive cover for positioning; the lower part of the connecting cover is provided with one or more clamping hooks, the upper part of the lower comprehensive cover is provided with one or more clamping hooks, and the clamping hooks between the connecting cover and the lower comprehensive cover are mutually matched for positioning.
Further, the inner ring comprises a locking ring, and the surface of the inner ring of the locking ring is provided with an internal thread; the outer surface of the lower end of the upper comprehensive cover is provided with an external thread and/or the outer surface of the upper end of the connecting cover is provided with an external thread and/or the outer surface of the upper end of the lower comprehensive cover is provided with an external thread, and the internal thread and the external thread can be matched with each other to finish positioning of the upper comprehensive cover, the connecting cover, the lower comprehensive cover and/or the locking ring.
Further, a second through hole is formed in the middle of the upper comprehensive cover, the upper comprehensive cover is provided with an inner cavity for accommodating and/or positioning a foreskin fixing hoop, the foreskin fixing hoop is a circular ring with a third through hole in the middle, and the third through hole corresponds to the second through hole; and/or the inner cavity of the upper comprehensive cover is provided with one or more positioning grooves, the outer surface of the foreskin fixing hoop is provided with one or more positioning tables, and the positioning grooves and the positioning tables are matched with each other to perform positioning; and/or, the device further comprises a ring cutter, wherein the ring cutter is sleeved outside the thimble guide block and/or the U-shaped nail ejector ring; preferably, the loop cutter is driven and moved by the staple top ring; and/or, the prepuce fixing hoop also comprises a blade gasket which is arranged on the lower end face of the prepuce fixing hoop and corresponds to the position of the annular cutter; and/or the U-shaped device is partially arranged in the lower comprehensive cover and partially arranged in the connecting cover; and/or, further comprising a connecting block, the upper end of which is connected to the lower end of the lower integrated cover; and/or, the device also comprises a push rod, wherein the upper end of the push rod is inserted into the lower end of the connecting block; and/or, the push rod further comprises a plug which is arranged at the lower end of the push rod; and/or further comprising a handle drivingly connected to the connection block and/or the push rod and for driving the push rod and/or the connection block and/or an internal transmission of the push rod and/or an internal transmission of the connection block to thereby drive the staple top ring and/or the ring cutter to move upwards; and/or the outer surface of the lower end of the foreskin fixing hoop is provided with a circle of concave/convex structures for vessel butt joint.
Compared with the prior art, the foreskin cutting stitching instrument can accommodate front foreskin and most of penis in the operation process, avoid injury or infection, adopt the structure of inner ring (foreskin fixing hoop) and inner integrated cover (U nail top ring and/or thimble guide block) to enable the foreskin clamping part to be positioned in the foreskin cutting stitching instrument, and the inner integrated cover (U nail top ring and/or thimble guide block) is internally provided with a U nail groove in a lower ring to enable the U nail stitching position to be positioned in the foreskin cutting stitching instrument, thereby avoiding easy infection to occur operation accidents when bleeding is caused by instrument failure or operation error. The external annular cutter of the prepuce cutting stitching instrument adopts external cutting and internal nailing, and can not cause pain and risk caused by stacking cutting under the conditions of overlong prepuce and the like. Through the blood vessel butt joint device arranged on the inner ring (foreskin fixing hoop), when the upper cover (upper comprehensive cover) and the lower cover (lower comprehensive cover/connecting cover) are mutually matched and the inner ring (foreskin fixing hoop) and the inner comprehensive cover (U-shaped nail top ring and/or thimble guide block) are mutually matched, the corresponding condition of foreskin blood vessels and the U-shaped nail can be accurately judged, so that after the blood vessels are accurately positioned, the final positioning of the inner ring (foreskin fixing hoop) and the inner comprehensive cover (U-shaped nail top ring and/or thimble guide block) and the upper cover (upper comprehensive cover) and the lower cover (lower comprehensive cover/connecting cover) is carried out, and the effect of operation hemostasis is greatly improved. More preferably, the vessel butt joint groove can be flexibly arranged to be in one-to-one correspondence with the U nail groove, the U nails in the U nail groove, the space between the U nail grooves, the width and the like, so that different circular cutting devices and different operation demands are met, and finally the main surface of the U nail is closed between the U nail and the vessel, thereby having good hemostasis during operation and accelerating necrosis process after operation. More preferably, the U-shaped nail pad is arranged to ensure that the pad is arranged between the U-shaped nail and the foreskin when the U-shaped nail is driven into the foreskin, so that the pain of a patient caused by the failure of later-stage nail removal due to the adhesion between the U-shaped nail and the foreskin is avoided.

Detailed Description
The present invention will be described in detail below with reference to the accompanying drawings, which are one preferred example of various embodiments of the present invention.

The staple device comprises a staple ejector ring 11 and a thimble guide block 14, wherein the thimble guide block 14 is used for accommodating staples 12 for stitching, guiding movement of the staples 12 for stitching, positioning and guiding thimbles of the staple ejector ring 11, and the staple ejector ring 11 is used for being matched with the thimble guide block 14 to eject the staples 12. Also included is a staple spacer 15 which is an annular spacer disposed at the top of the ejector pin guide 14. The U-shaped gasket 15 is also provided with a gasket positioning table 15-1 perpendicular to the gasket plane, and the gasket positioning table 15-1 is a complete annular bulge arranged on the inner periphery or the outer periphery or the middle of the gasket plane of the U-shaped gasket 15. The thimble guide block 14 is formed with first through-hole in the middle, and thimble guide block 14 includes annular inner wall, annular outer wall and locating table holding tank, and annular inner wall is used for mutually supporting the location with the annular bulge that sets up in the gasket planar inner periphery of staple gasket 15, and annular outer wall is used for mutually supporting the location with the annular bulge that sets up in the gasket planar outer periphery of staple gasket 15, and locating table holding tank sets up in thimble guide block 14 top and is used for holding the annular bulge that sets up in the middle of the gasket planar of staple gasket 15. The thimble guide block 14 is provided with a plurality of thimble guide grooves 14-1, the thimble guide groove 14-1 is annularly arranged on the annular thimble guide block 14, and the thimble guide groove 14-1 is entirely covered by the U-shaped nail pad 15.
The connecting cover 10 is arranged between the upper comprehensive cover 18 and the lower comprehensive cover 9, the upper comprehensive cover 18 is detachably matched and connected to the lower comprehensive cover 9 through the connecting cover 10, the lower end face of the upper comprehensive cover 18 is annular, a concave positioning groove is formed along the annular end face, the upper end face of the connecting cover 10 is annular, a convex positioning boss 10-3 is arranged along the annular end face, and the positioning groove and the positioning boss 10-3 are mutually matched in a concave-convex mode to position the upper comprehensive cover 18 and the connecting cover 10. The upper end surface of the connecting cover 10 is annular, a convex positioning boss 10-3 is arranged along the annular end surface, and the positioning boss 10-3 is matched with a positioning groove arranged on the lower end surface of the upper comprehensive cover 18 for positioning; the lower part of the connecting cover 10 is provided with a plurality of hooks 10-1, the upper part of the lower comprehensive cover 9 is provided with a plurality of hooks 9-1, and the hooks between the connecting cover 10 and the lower comprehensive cover 9 are mutually matched for positioning.
The locking ring 8 is also included, and the inner ring surface of the locking ring 8 is provided with an internal thread 8-1 and/or a clamping ring 8-2; the outer surface of the lower end of the upper comprehensive cover 18 is provided with external threads, the outer surface of the upper end of the connecting cover 10 is provided with external threads or a clamping ring 10-2, and the internal threads 8-1 and the external threads and/or the clamping ring 8-2/10-2 can be matched with each other to finish positioning of the upper comprehensive cover 18, the connecting cover 10, the lower comprehensive cover 9 and the locking ring 8.
The middle of the upper comprehensive cover 18 is provided with a second through hole, the upper comprehensive cover 18 is provided with an inner cavity for accommodating and positioning the foreskin fixing hoop 17, the foreskin fixing hoop 17 is a circular ring with a third through hole in the middle, and the third through hole corresponds to the second through hole; the inner cavity of the upper comprehensive cover 18 is provided with a plurality of positioning grooves, the outer surface of the foreskin fixing hoop 17 is provided with a plurality of positioning tables 17-2, and the positioning grooves 18-2 are matched with the positioning tables 17-2 for positioning; the device also comprises a ring cutter 13, wherein the ring cutter 13 is sleeved outside the thimble guide block 14 and the staple ejector ring 11, and the ring cutter 13 can be driven by the staple ejector ring 11 to move; the cutter gasket 16 is arranged on the lower end surface of the foreskin fixing hoop 17 and corresponds to the position of the annular cutter 13; the U-shaped device is partially arranged in the lower comprehensive cover 9 and partially arranged in the connecting cover 10; further comprises a connecting block 3, the upper end of which is connected to the lower end of the lower comprehensive cover 9; the push rod is also included, and the upper end of the push rod is inserted into the lower end of the connecting block 3; the plug 1 is arranged at the lower end of the push rod; also included is a handle 6 drivingly connected to the connecting block 3 and the push rod 2 and adapted to drive the push rod 2, the transmission of the connecting block 3 and thereby the staple top ring 11 and the loop cutter 13 to move upwardly. The outer surface of the lower end of the foreskin fixing hoop is provided with a circle of blood vessel butt joint groove 17-1 for butt joint of blood vessels; also included is an opening knob 4 which is fitted over the connecting block 3 and serves to lock or unlock the handle and/or the components driven by the handle. Referring to fig. 9 and 10, which are enlarged views of a staple washer and an opening knob, respectively, the opening knob 4 is provided at an upper end thereof with a pointer 4-1 and in a middle thereof with a circular hole 4-2, and a guide block 4-3 is provided at an inner wall thereof to be engaged with a guide groove 3-1 provided on the connection block 3 so as to lock or unlock a handle or other driving member, and a washer positioning table 15-1 is provided at one side of the staple washer 15 to position it on the ejector pin guide block 14.
Referring to fig. 8, which is a front view of an everting circumcision stapler, and which is a detailed description thereof for clarity, reference is made to fig. 2,5-7, which are each a perspective view of the everting circumcision stapler. Referring to fig. 3 and 4, in the preferred embodiment, the eversion foreskin cutting stapler comprises a handle 1, a plug 2, a small connecting rod 3, a pin 4, a push rod 5, a connecting block 6, an opening knob 7, a spring 8, a lower integrated cover 9,U, a nail top ring 10, an upper integrated cover 11, a threaded locking ring 12, a ring cutter 13, a staple backing ring 14, a ring cutter gasket 15, the handle 1 is connected to the push rod 5 and the connecting block 6 through the small connecting rod 3 and the pin 4 in a driving way, the opening knob 7 is sleeved outside the connecting block 6, the plug 2 is arranged at the lower end of the push rod 5, the upper integrated cover 11 and the lower integrated cover 9 are directly connected or connected through a connecting cover, the spring 8 is arranged in the inner cavity of the connecting block 6, the staple top ring 10 is wholly or partly arranged in the lower integrated cover 9, or wholly or partly arranged in the connecting cover, the other part of the lower integrated cover 9 is arranged in the locking ring 12 is locked on the upper integrated cover 11 and/or the lower integrated cover 9 and/or the periphery of the connecting cover, the ring 13 is arranged on the periphery of the nail top ring 10 and the periphery of the guide block 14, and the staples are not used for being punched into contact with the foreskin when the staple 14 is not punched on one side of the foreskin. The principle of other relevant driving components, actuating components and ring cutting components is the same as that of the previous embodiment, and the description thereof is omitted.
