Abstract

本发明公开了一种半月板体部和前角撕裂的缝合方法,具体包括以下步骤:步骤1,观察半月板损伤处创口;步骤2,在半月板损伤处置入缝合套管,缝合线两端均连接有缝合针,两个缝合针均位于缝合套管外侧;步骤3,将缝合针分别对应穿过半月板损伤处的两侧并穿出关节腔;步骤4,将每个缝合针后的缝合线均剪断,并将两个缝合线在关节外打结,缝合结束。本发明的缝合方法通过采用缝合套管和缝合针配合,对半月板体部和前角的位置损伤进行缝合,通过本发明的缝合方法,能恢复半月板的正常功能,更有利于保存正常的生理状态及功能,操作步骤简单,使用成本较低,有很好的医学和社会应用价值。

一种半月板体部和前角撕裂的缝合方法
技术领域
本发明属于半月板修复方法技术领域,具体涉及一种半月板体部和前角撕裂的缝合方法。
背景技术
半月板是两个月牙形的纤维软骨,位于胫骨平台内侧和外侧的关节面,有润滑关节、分散负荷、吸收震荡和维持膝关节稳定等作用,其横断面呈三角形,外厚内薄,上面稍呈凹形,以便与股骨髁相吻合,下面平整,与胫骨平台相接。这样的结构恰好使股骨髁在胫骨平台上形成一较深的凹陷,从而使球形的股骨髁与胫骨平台的稳定性增加。半月板的前后端分别附着在胫骨平台中间部非关节面的部位,在髁间棘前方和后方,这个部位又可称做半月板的前角和后角。
目前缝合半月板后角撕裂的缝合系统有两种:FAST-FIX系统和Rapidloc系统,两者使用方便,但均不适合用于半月板体部和前角的缝合;缝合半月板前角有MM2缝合系统,但使用过程复杂且极不方便,浪费时间,不能在手术者想要的精确地方缝合,价格昂贵,在使用过程中存在一定的问题。
发明内容
本发明的目的是提供一种半月板体部和前角撕裂的缝合方法,解决了现有半月板缝合系统操作复杂、使用成本较高的问题。
本发明所采用的技术方案是,一种半月板体部和前角撕裂的缝合方法,包括以下步骤:
步骤1,在关节镜观测下,处理半月板损伤处的情况,当半月板损伤处创面呈新鲜化时,准备缝合;
步骤2,在关节镜的继续观测下,在步骤1处理后的半月板损伤处置入弧形的缝合套管,缝合套管内设置有缝合线,缝合线两端均连接有缝合针,两个缝合针均位于缝合套管外侧;
步骤3,将步骤2中两个缝合针分别对应穿过半月板损伤处的两侧,并穿出关节腔;
步骤4,将步骤3每个缝合针后的缝合线均剪断,并将两个缝合线在关节外打结,取出缝合针和缝合套管,缝合结束。
本发明的特征还在于,
步骤1中,当半月板损伤处创面陈旧时,使用刨刀刨削创面,使创面新鲜化后再进行缝合。
步骤2中缝合套管包括一组相同的管a和管b,管a和管b相对处均设置有豁口,豁口之间通过通道连接,管a和管b的一端设置有支板,管a和管b的另一端设置有手柄。
缝合线依次通过管a和管b,且连接缝合针的缝合线两端均位于靠近支板处。
支板呈刀尖状。
缝合针的长度不小于管a的长度。
本发明的有益效果是:本发明一种半月板体部和前角撕裂的缝合方法通过采用缝合套管和缝合线配合,对半月板体部和前角的位置损伤进行缝合,通过本发明的缝合方法,能恢复半月板的正常功能,更有利于保存正常的生理状态及功能,操作步骤简单,使用成本较低,有很好的应用价值。

具体实施方式
下面结合附图和具体实施方式对本发明进行详细说明。
本发明一种半月板体部和前角撕裂的缝合方法,如图1所示,具体包括以下步骤:
步骤1,在关节镜观测下,处理半月板损伤处的情况,当半月板损伤处创面呈新鲜化时,准备缝合;
步骤2,在关节镜的继续观测下,在步骤1处理后的半月板损伤处置入弧形的缝合套管1,缝合套管1内设置有缝合线2,缝合线2两端均连接有缝合针3,两个缝合针3均位于缝合套管1外侧;
步骤3,将步骤2中两个缝合针3分别对应穿过半月板损伤处的两侧,并穿出关节腔;
步骤4,将步骤3每个缝合针3后的缝合线2均剪断,并将两个缝合线2在关节外打结,取出缝合针3和缝合套管1,缝合结束。
步骤1中,当半月板损伤处创面陈旧时,使用刨刀刨削创面,使创面新鲜化后再进行缝合。
步骤2中缝合套管1包括一组相同的管a5和管b6,管a5和管b6相对处均设置有豁口9,豁口9之间通过通道4连接,管a5和管b6的一端设置有支板7,管a5和管b6的另一端设置有手柄8,手柄8方便缝合操作的进行。
缝合线2依次通过管a5和管b6,且连接缝合针3的缝合线2两端均位于靠近支板7处。
支板7呈刀尖状,方便缝合针3找准扎入半月板的位置。
缝合针3的长度不小于管a5的长度。
实施例
a患者症状:患者半月板前角和体部损伤。
b缝合过程:采用蛛网膜下腔阻滞麻醉或蛛网膜下腔-硬膜外腔联合阻滞麻醉,患侧大腿上气囊止血带。采用膝前内侧和膝前外侧入路,置入并连接好关节镜器械后,用探针按照髌上囊髌股关节内侧间隙-髁间窝外侧间隙的顺序探查滑膜、软骨、交叉韧带和半月板情况;首先,患者半月板损伤处创口新鲜化,准备缝合;其次,在关节镜的继续观测下,在半月板损伤处置入缝合套管1,缝合套管1内设置有缝合线2,缝合线2两端均连接有缝合针3,两个缝合针3均位于缝合套管1外侧;再次,将两个缝合针3分别对应穿过半月板损伤处的两侧,并穿出关节腔;最后,将每个缝合针3后的缝合线2均剪断,并将两个缝合线2在关节外打结,取出缝合针3和缝合套管1,缝合结束。
c缝合后效果:患者缝合后第三天在病床上,活动患肢,屈伸膝关节;一周后在支具的保护下,下地行走;一月后正常行走;45天左右恢复正常,能正常行走、运动,且未出现疼痛等症状。
本发明的缝合方法通过采用缝合套管1和缝合线2配合,对半月板体部和前角的位置损伤进行缝合,通过本发明的缝合方法,能恢复半月板的正常功能,更有利于保存正常的生理状态及功能,操作步骤简单,使用成本较低,有很好的应用价值。

Claims (6)

1.一种半月板体部和前角撕裂的缝合方法,其特征在于,具体包括以下步骤:
步骤1,在关节镜观测下,处理半月板损伤处的情况,当半月板损伤处创面呈新鲜化时,准备缝合;
步骤2,在关节镜的继续观测下,在所述的步骤1处理后的半月板损伤处置入弧形的缝合套管(1),所述的缝合套管(1)内设置有缝合线(2),缝合线(2)两端均连接有缝合针(3),两个缝合针(3)均位于缝合套管(1)外侧;
步骤3,将步骤2中所述的两个缝合针(3)分别对应穿过半月板损伤处的两侧,并穿出关节腔;
步骤4,将步骤3所述的每个缝合针(3)后的缝合线(2)均剪断,并将两个缝合线(2)在关节外打结,取出缝合针(3)和缝合套管(1),缝合结束。
2.根据权利要求1所述的一种半月板体部和前角撕裂的缝合方法,其特征在于,所述的步骤1中,当半月板损伤处创面陈旧时,使用刨刀刨削创面,使创面新鲜化后再进行缝合。
3.根据权利要求1所述的一种半月板体部和前角撕裂的缝合方法,其特征在于,所述的步骤2中缝合套管(1)包括一组相同的管a(5)和管b(6),管a(5)和管b(6)相对处均设置有豁口(9),豁口(9)之间通过通道(4)连接,管a(5)和管b(6)的一端设置有支板(7),管a(5)和管b(6)的另一端设置有手柄(8)。
4.根据权利要求3所述的一种半月板体部和前角撕裂的缝合方法,其特征在于,所述的缝合线(2)依次通过管a(5)和管b(6),且连接缝合针(3)的缝合线(2)两端均位于靠近支板(7)处。
5.根据权利要求3所述的一种半月板体部和前角撕裂的缝合方法,其特征在于,所述的支板(7)呈刀尖状。
6.根据权利要求3所述的一种半月板体部和前角撕裂的缝合方法,其特征在于,所述的缝合针(3)的长度不小于管a(5)的长度。
