The operation of the circumcision anastomat according to one embodiment of the invention is briefly described as follows:
in the present embodiment, the glans 20 is detachably connected to the distal end of the housing 10, and in the initial state, the glans 20 is in an openable state, and the target tissue (foreskin) to be operated can be placed between the nail bin 41 and the anvil assembly 45.
When the circumcision anastomat/nail bin assembly 40 is switched from the initial state to the closed state, the adjusting knob 50 is rotated, the screw rod 60 moves towards the near end under the action of the adjusting knob 50, the screw rod 60 drives the nail head seat 20 to move close to the machine shell 10 until the anvil assembly 45 and the nail bin 41 are closed, the target physiological tissue can be clamped between the nail head seat and the nail bin, the nail pushing sheet 43 and the cutter 44 do not move and are still located at the near end of the nail bin assembly 40.
When the circumcision anastomat/nail bin assembly 40 is switched from a closed state to a fired state, the anvil assembly 45 and the nail bin 41 are still kept closed, the safety block 80 is driven to be separated from the hole 101, the firing handle 30 is pressed to drive the ejector block 70 to move towards the far end, the ejector block 70 drives the nail pushing sheet 43 and the cutter 44 to move from the near end to the far end of the nail bin assembly 40, and the cutter 44 and the nail pushing sheet 43 cut and sew the clamped target physiological tissue and can be limited at the far end of the nail bin assembly 40 to stop moving.
When the circumcision anastomat/nail bin assembly 40 is switched from the firing state to the firing completion state, the firing handle 30 is released, the adjusting knob 50 is rotated in the opposite direction, the screw rod 60 drives the nail head seat 20 to move away from the machine shell 10, the nail pushing sheet 43 and the cutter 44 return to the initial position, the anvil assembly 45 and the nail bin 41 are opened, and the cut and sutured target physiological tissues can be separated from the nail head seat 20 at the moment.
In this embodiment, at least one tab 90 is further disposed between staple cartridge 41 and anvil assembly 45, and tab 90 cooperates with staples (not shown). The auxiliary tablet 90 can reduce post-operative erectile pain, reduce edema, and assist in hemostasis.
Referring to fig. 3 and 4, in the present embodiment, a flap fastener 412 is provided on a distal end surface of the magazine 41, and the flap fastener 412 can fix a flap 90 in a direction toward an edge at the center of the cartridge 42.
Specifically, the auxiliary plate fixing member 412 is a convex pillar protruding from the distal end surface of the nail bin 41 to the distal end, the convex pillar corresponds to the hole on the auxiliary plate 90, and the convex pillar is arranged to intersect with the nail outlet 413 on the nail bin 41.
Further, in this embodiment, a staple cartridge fixing structure 421 is disposed on the staple cartridge 42, a fitting member 411 that fits the staple cartridge fixing structure 421 is disposed on the staple cartridge 41, and the fitting member 411 and the staple cartridge fixing structure 421 fit each other, so that when the staple cartridge 41 is fitted to the staple cartridge 42, the staple cartridge 42 and the staple cartridge 41 are relatively stationary.
Specifically, the nail barrel 42 includes a distal end surface, a proximal end surface and a side wall 422 connecting the distal end surface and the proximal end surface, the side wall 422 encloses a chamber capable of accommodating the nail bin 21, the nail bin fixing structure 421 is located in the chamber, that is, the nail bin fixing structure 421 is located on the inner surface of the side wall 422, and the nail bin fixing structure 421 is close to the distal end surface of the nail barrel 42.
In this embodiment, the staple cartridge fixing structure 421 is two notches symmetrically distributed on the inner surface of the side wall 422, and the fitting element 411 is two protrusions protruding out of the outer surface of the staple cartridge 41, and the notches limit the protrusions. But not limited thereto, for example, the protrusion may be located at the staple cartridge 42 and the notch may be located at the staple cartridge 41.
An indicating groove 423 extending along the axial direction of the nail barrel 42 is formed at the outer surface of the side wall 422 of the nail barrel 42, and the indicating groove 423 is used for aligning the lace direction when assembling.
Further, in the present embodiment, with reference to fig. 5 to 7, the cutting knife 44 may be assembled with the nail-pushing piece 43, and at least when the cutting knife 44 is assembled to the nail-pushing piece 43, a distal end surface of the cutting knife 44 has an oval shape.
Here, the cutter 44 with the elliptical distal end surface during cutting can make the structure of the cutter 44 and the structure of the whole circumcision anastomat more suitable for the shapes of the glans and the coronary sulcus of the patient, so that the cutting line after the operation is more beautiful and more suitable for the natural growth shape of the human body.
Further, in the present embodiment, the plurality of nail outlets 413 have an oval profile.
When the staples at the staple outlet 413 are pushed out by the staple pushing sheet 43, the formed contour of the plurality of staples is oval.
It will be appreciated that the profile of the staple forming portion (not labeled) on the anvil 451 is also oval in order to accommodate the configuration of the plurality of staple outlets 413.
Here, when the anvil 452 is a "zigzag" shaped anvil, a thin cutting wall (not shown) corresponding to the cutting blade 44 is formed on the anvil 452, and the outline of the thin cutting wall is also formed in an elliptical shape.
It should be noted that the nail bin 41, the anvil 451 and the knife anvil 452 are only required to be arranged to ensure that the finally formed cutting line and the contour of the suturing nail are elliptical, and at this time, the contour of the cutting line and the contour of the suturing nail are both matched with the shape of the glans penis coronary sulcus.
That is, it is only necessary to ensure that the contour of the staple outlet 413, the contour of the staple forming portion, and the contour of the cut thin wall are elliptical, and other portions of the staple cartridge 41, the anvil 451, and the knife anvil 452 may not be limited.
In addition, other parts of the circumcision anastomat of the embodiment can be adjusted correspondingly.
For example, the distal end region of the driver blade 43 is an elliptical ring structure, the distal end region of the barrel 42 that mates with the cutter blade 44 is an elliptical ring structure, the anvil assembly mounting surface (or annular cavity) of the glans 20 is an elliptical ring structure, and the auxiliary blade 90 is an elliptical ring structure.
Of course, a partial region of the other member may have an elliptical structure.
For example, only the distal inner surface area of the ejector pin piece 43 matching the cutter 44 is of an elliptical ring structure, the distal outer surface area of the ejector pin piece 43 is of a ring structure, both the inner surface and the outer surface of the nail barrel 42 are of ring structures, the outer contour of the anvil assembly mounting surface of the glans 20 is circular, only the inner ring area of the sub-piece 90 is elliptical, and the outer ring area of the sub-piece 90 is circular.
The above description is only a specific example, and the specific form of each component of the circumcision anastomat of the present invention is not limited, and it is only required to ensure that the contour of the finally formed cutting line and suturing nail is elliptical, and at this time, the contour of the cutting line and suturing nail is matched with the shape of the glans penis coronary sulcus.
In the present embodiment, the profiles of the nail outlets 41 are located outside the distal end of the cutting knife 44, and when the profiles of the nail outlets 41 and the distal end surface of the cutting knife 44 are located on the same plane, a first distance (not shown) is provided between any point on the profiles of the nail outlets 41 and any point on the distal end surface of the cutting knife 44 in a direction from the center of the nail barrel 42 to the edge, and the first distance is kept constant in the circumferential direction of the cutting knife 44.
That is, at this time, the profiles of the nail outlets 41 and the major and minor axes of the two ellipses formed by the distal end surfaces of the cutters 44 are all on the same straight line, and the difference between the major axes is consistent with the difference between the minor axes.
Further, in the present embodiment, the nail-pushing piece 43 is provided with a knife-placing member 431, and the cutter 44 is detachably connected to the nail-pushing piece 43 through the knife-placing member 431.
In a specific example, the cutting knife 44 itself is an elliptical ring-shaped cutting knife, and when the cutting knife 44 is connected with the nail pushing sheet 43 through the knife placing piece 431, the distal end face of the cutting knife 44 is kept elliptical.
In another specific example, the initial state of the cutting knife 44 is non-elliptical, for example, the cutting knife 44 itself is a circular ring-shaped cutting knife or another cutting knife, the body of the cutting knife 44 has elasticity, and when the cutting knife 44 is connected to the nail-pushing sheet 43 through the knife-placing piece 431, the cross section of the knife-placing piece 431 driving the cutting knife 44 along the center of the cutting knife 44 toward the edge direction is elliptical.
Here, the knife holder 431 is a groove 431, the groove 431 has an elliptical ring shape, when the cutting knife 44 is placed in the groove 431, the groove 431 limits the cutting knife 44, and the groove 431 may drive the cutting knife 44, which is not elliptical in the initial state, to deform.
It should be noted that the knife placing member 431 is not limited to the structure of the groove body 431 as described above, and the groove body 431 is not limited to the elliptical ring shape as described above, and the knife placing member 431 only needs to ensure that the cutting knife 44 can be positioned and the state of the cutting knife 44 after being placed is elliptical.
In the present embodiment, referring to fig. 7, the groove 431 is located inside the nail pushing sheet 43, when the cutting knife 44 is fitted to the groove 431, the cutting knife 44 has a first width W in the axial direction of the nail barrel 42, and the groove 431 has a first depth D in the axial direction of the nail barrel 42, and the first depth D is not less than one-half of the first width W of the corresponding region. The width between the two side walls of the groove body 431 in axial contact with the cutting knife 44 is slightly larger than the thickness of the cutting knife 44, so that the cutting knife 44 can be stably placed in the groove body 431.
Here, particularly in the case where the initial state of the cutting knife 44 is a non-elliptical shape, the groove body 431 needs to provide a sufficient force to the cutting knife 44 to stably maintain the elliptical shape, and the present embodiment controls the first depth D of the groove body 431 to be equal to or more than one half of the first width W of the cutting knife 44, so that the overlapping area of the groove body 431 and the cutting knife 44 is sufficiently large to ensure that the shape of the cutting knife 44 after deformation is stably maintained.
Further, in this embodiment, a first fixing structure 441 is disposed on the cutting knife 44, a second fixing structure 432 engaged with the first fixing structure 441 is disposed on the groove body 431, and when the cutting knife 44 is assembled to the groove body 431, the first fixing structure 441 and the second fixing structure 432 are engaged with each other, so that the cutting knife 44 and the groove body 431 are fixed relatively stationary.
Here, the first fixing structure 441 is a hook protruding toward the inner side of the cutter 44, and the first fixing structure 441 may be implemented by bending a partial region of the cutter 44.
The second fixing structure 432 is a recess dug toward one side of the groove 431, and the second fixing structure 432 may be implemented by cutting a partial region of the groove 431.
The opening width of the groove body 431 is approximately the same as the thickness of the cutting knife 44 except for the second fixing structure 432, when the cutting knife 44 is placed in the groove body 431, the first fixing structure 441 is in a state of being attached to the other area of the cutting knife 44, so that the cutting knife 44 can smoothly enter the groove body 431, and when the first fixing structure 441 reaches the second fixing structure 432, the first fixing structure 441 is unfolded towards the second fixing structure 432, so that the first fixing structure 441 and the second fixing structure 432 are mutually clamped.
The groove body 431 and the cutter 44 of the present embodiment are tightly fitted, the groove body 431 provides a better limiting effect for the cutter 44, and the cutter 44 deforms under the action of the groove body 431 and can stably maintain the deformed elliptical state.
In this embodiment, the second fixing structure 432 includes at least two fixing members 4321, and when the groove 431 is an elliptical groove, the two fixing members 4321 are symmetrically distributed on a major axis or a minor axis of the groove 431.
That is, the two fixing members 4321 are located at approximately two long axis vertices, or the two fixing members 4321 are located at approximately two short axis vertices.
It should be understood that although the present description refers to embodiments, not every embodiment contains only a single technical solution, and such description is for clarity only, and those skilled in the art should make the description as a whole, and the technical solutions in the embodiments can also be combined appropriately to form other embodiments understood by those skilled in the art.
The above-listed detailed description is only a specific description of a possible embodiment of the present invention, and they are not intended to limit the scope of the present invention, and equivalent embodiments or modifications made without departing from the technical spirit of the present invention should be included in the scope of the present invention.
