In the process of examining or operating on the nose, various auxiliary tools are needed due to the complexity and the particularity of the construction of the nose, for example, a nasal endoscope and the like are commonly used in the functional nasal endoscope operation.

On the basis of the narrow surgical field range of the nasal operation, two persons, namely an operator and an assistant, are required to cooperate and operate in the limited surgical space, so that the problems of insufficient operating space, complex operation and time waste of the operator exist.
In addition, nasal surgery is not a major operation, and in particular, in recent years, more and more people receive functional nasal plastic surgery (the so-called functional nasal plastic surgery is the plastic surgery of the nasal shape and the nasal septum in synchronization, so as to achieve the synchronous improvement of the shape and the function, and the surgery is composed of two parts of the nasal septum plastic surgery and the external nasal plastic surgery), and sometimes, one operator alone can independently complete the surgeries.
However, due to the special features of the nasal operation site and structure, an assistant is required to be provided to pull open the nasal septum mucosa and the nasal dorsal flap by using other auxiliary tools such as dissectors, small hooks, and nasal hooks in order to expose the operation field and to complete the operation smoothly. In this case, depending on the experience of the assistant and the degree of the default with the surgeon, the assistant may not be required by the surgeon, which may not only affect the smooth operation of the operation, but also may cause the accuracy of the operation to be degraded because the assistant does not recognize the intention of the surgeon. In addition, not only is there a great waste in the arrangement of personnel, but also the accuracy of the operation is affected, and time is sometimes wasted without effect.

Disclosure of Invention
The present invention has been made in view of the above problems, and it is an object of the present invention to provide a nasal endoscope retractor which can be operated by a single operator, illuminate an operation field with a nasal endoscope, and perform a nasal operation at the same time. The invention relates to an intranasal endoscope retractor, which comprises: a retractor body portion having a connection end and an open end; an insertion part which is arranged at the connecting end of the draw hook main body part and can be detached relative to the draw hook main body part; and an endoscope fixing member which is arranged on the bottom surface of the draw hook main body part and can be detached relative to the draw hook main body part.
Preferably, the retractor main body portion is formed in a hollow box shape as a whole, a catching groove portion extending in a longitudinal direction of the retractor main body portion is provided in a bottom surface of the retractor main body portion, and the endoscope fixing member is caught by the catching groove portion and is slidable in a reciprocating manner in the longitudinal direction of the catching groove portion in a state of being caught by the catching groove portion.
Preferably, the endoscope fixing member has an endoscope fixing portion for fixing the nasal endoscope and a engaging leg portion for being engaged with the engaging groove portion.

Effects of the invention
According to the nose endoscope retractor, an operator can simultaneously perform the retractor, nose endoscope illumination and observation and nasal operation by one-man operation, so that the operation is simple and easy, an assistant is omitted, and the operator can perform the operation accurately due to various operations of the operator himself, so that the operation can be performed more smoothly. Hereinafter, specific embodiments of the present invention will be described with reference to the drawings.

As shown in fig. 1 and 2, the endoscopic sinus retractor (S) of the present invention mainly comprises a retractor main body 1, an insert portion 2 provided at one end of the retractor main body 1 and detachable from the retractor main body 1, and an endoscopic attachment 3 provided at one surface side of the retractor main body 1 and detachable from the retractor main body 1.
As shown in fig. 4A, the retractor main body 1 of the present invention is generally shaped like a hollow box with openings at both sides. Here, for convenience of explanation, in the hook main body 1, one surface on which the endoscope fixing tool 3 is located is a bottom surface of the hook main body 1, the other surface opposite to the bottom surface is a surface of the hook main body 1, one end on which the insert portion 2 is located is a connection end, and the other end opposite to the connection end is an open end.
Fig. 4A is a perspective view of the retractor body 1 of the endoscopic sinus retractor S of the present invention, showing a perspective view of the retractor body 1 when viewed from the open end side and the bottom surface of the retractor body 1 is visible. As shown in fig. 5A, the endoscope fixing member 3 of the present invention has a substantially semi-closed cylindrical shape with both ends open, and further, as shown in fig. 5B, has a substantially semi-closed circular ring shape in cross section. Specifically, as shown in fig. 5A and 5B, the endoscope fixing member 3 has an endoscope fixing portion 3-1 and two engaging legs 3-2, wherein the endoscope fixing portion 3-1 is substantially cylindrical and in a semi-closed state, and the two engaging legs 3-2 extend from the semi-closed edge of the endoscope fixing portion 3-1 in opposite directions along a direction substantially tangential to a circular ring surface of a cylindrical cross section, that is, as shown in fig. 5A, the cross section of the endoscope fixing member 3 is substantially "Ω" shaped.
Further, as shown in fig. 1, 2 and 4A, the retractor main body 1 of the endoscopic sinus retractor S of the present invention has a substantially hollow box shape (box shape), preferably a flat hollow box shape, and has a substantially rectangular shape in plan view of the surface and the bottom thereof, and both ends of the retractor main body 1 are open. A substantially rectangular notch portion 4 extending along the long side of the retractor main body 1 is provided on the bottom surface of the retractor main body 1 of the endoscopic sinus retractor S of the present invention, and the notch portion 4 is closed on the side (left side in the drawing) of the bottom surface of the retractor main body 1 near the connection end and is opened on the side (right side in the drawing) of the bottom surface of the retractor main body 1 near the open end, so that the two engagement leg portions 3-2 of the endoscopic sinus guide 3 can be inserted from the open end side and can slide along the notch portion 4 to the closed side of the connection end.
In the present invention, as shown in fig. 4A and 4B, it is preferable that the cross section of the connection end of the hook body 1 is a substantially completely closed rectangular frame, and the cross section of the open end is a substantially semi-closed rectangular frame (the open portion is a portion forming the notch portion 4).
In the invention, the components such as the nasal endoscope and the like can be fixed on the endoscope fixing part 3-1 of the endoscope fixing part 3, so that the nasal endoscope can slide back and forth between the open end and the connecting end (the closed side of the clamping groove part 4) along the length direction of the clamping groove part 4 along with the endoscope fixing part 3 through the clamping structure of the clamping foot part 3-2 and the clamping groove part 4, thereby meeting the requirement of an operator for adjusting the visual field in the operation. In addition, in the present invention, it is preferable that the material of the endoscope fixing member 3 has a certain hardness and elasticity, so that it can be more easily and smoothly engaged with the notch portion 4, more smoothly slide in the notch portion 4, and more firmly fasten the nasal endoscope and the like.
Although not shown, a notch portion may be provided at the bottom of the hook body 1 so that both ends of the notch portion are completely closed. At this time, the endoscope fixing piece can be forcibly clamped in the clamping groove part by utilizing the characteristic that the endoscope fixing piece has elasticity.
Next, the plug-in unit 2 will be explained. First, the plug-in unit 2 will be described as an example of a flat plug-in unit. Fig. 3A is a perspective view of the plug portion 2 of the endoscopic sinus retractor S according to the present invention, showing a state in which the plug portion 2 is detached from the retractor main body 1. Fig. 3B is a partial perspective view of the insert portion 2 of the endoscopic sinus retractor S of the present invention. As shown in fig. 3A and 3B, the insert part 2 of the present invention has an insert main body part 2-1, a raised part 2-2, and an insertion part 2-3 that can be inserted into the connection end of the hook main body part 1, and preferably, the insertion part 2-3 is formed slightly thinner than the insert main body part 2-1 and the raised part 2-2. When the rhinoscope retractor S is used, the retractor structure can be formed only by inserting the insertion part 2-3 of the insertion part 2 into the connecting end of the retractor main body part 1.
In this case, in order to fix the plug-in unit 2 more favorably, it is preferable that a screw hole n is provided on the side of the hook body 1 close to the connection end, and the plug-in unit 2 is fixedly connected to the connection end of the hook body 1 by screwing a screw or the like into the screw hole and making contact with the insertion unit 2-3 with a screw. In the present invention, the screw and the screw hole are not limited to be fixed, and for example, a pin may be inserted into a pin hole to be fixedly connected by providing the pin hole. Further, a screw hole, a pin hole, or the like may be provided at a position corresponding to the screw hole n of the hook body 1 in the insertion portion 2-3 of the insertion portion 2 for fixation. At this time, the plug-in part 2 can be assembled and disassembled only by loosening or fastening a screw (pin shaft), so that the use process in actual operation is very convenient, and redundant plug-in part carriers are omitted.
As shown in fig. 6A to 6C, a hook-shaped insert portion 2' may be provided instead of the plate-shaped insert portion 2. With this hook-shaped insert part 2', the structure may be substantially the same as the plate-shaped insert part 2 except that the upturned part 2-2 is replaced with the hook-shaped part g. Specifically, the hook-shaped plug-in part 2 'includes a plug-in body part 2' -1, an insertion part 2 '-3, and a hook-shaped part g provided at the tip of the plug-in body part 2' -1, and one end of the hook-shaped part g connected to the plug-in body part 2 '-1 may be set narrower than the plug-in body part 2' -1. The structures of the insert body 2 ' -1 and the insert portion 2 ' -3 may be substantially the same as those of the insert body 2-1 and the insert portion 2-3, and the coupling manner of the insert portion 2 ' -3 and the hook body 1 may be the coupling manner of the insert portion 2-3 and the hook body 1. That is, only the upwarping portion 2-1 is replaced with the hook g for pulling open the nasal tip flap to expose the nasal tip and columella tissue structures. In addition, it is preferable that the distal end of the hook be tapered to facilitate hooking tissue.
Therefore, the types of the plug-in parts connected with the retractor main body part 1 can be replaced according to the functions of the plug-in parts and the requirements of the operation, and different plug-in parts are distributed by using one retractor main body part to form the retractor with different functions. For example, the plate-shaped plug part and the hook-shaped plug part are switched, so that complicated hook pulling tools can be reduced.
Next, specific examples of parameters of the structure of each part of the endoscopic sinus retractor S according to the present invention will be described. Specifically, the length of the retractor main body portion 1 is about 3-5 cm, the width thereof is about 1.2cm, the thickness thereof is about 1.7mm, the thickness of the hollow portion thereof is about 0.7mm, the width thereof is about 1.1cm, and the connecting end thereof can accommodate the insertion of a plug-in unit (i.e., an insertion portion of the plug-in unit) having the thickness of about 0.5mm and the width thereof of 1 cm. Further, the width of the pocket portion 4 is approximately 3 mm. Further, as shown in a cross-sectional view of the endoscope fixing member 3 with reference to fig. 5B, the endoscope fixing member 3 having a semi-closed cylindrical shape has an outer diameter of about 6mm and an inner diameter of about 5 mm. As shown in the perspective view of the endoscope fixing member 3 with reference to fig. 5A, the semi-closed cylindrical endoscope fixing member 3 has a cylindrical length of about 1cm and a cylindrical thickness of about 0.5 mm. Preferably, the width of the plate-shaped retractor insertion part is about 1cm and the thickness of the plate-shaped retractor insertion part is about 0.5mm, the length of the insertion part main body part of the plate-shaped retractor insertion part is about 1-2 cm, and the length of the upturned part is about 2 mm. Further, as shown in fig. 1 and 2, the rising angle α of the rising portion 2-2 with respect to the plug main body portion 2-1 is preferably 160 °, and it is also preferable that the angle β formed by the plug main body portion 2-1 and the hook main body portion 1 is 160 ° when the insertion portion 2-3 is inserted into the connection end of the hook main body portion 1 (i.e., in a state where the plug portion 2 is attached to the hook main body portion 1). Furthermore, when the insert portion is a hook-like insert portion, it is preferred that the hook-like insert portion has an overall length of 2cm, a length adjacent the main body section of the retractor of about 1cm, a width of about 1cm and a thickness of about 0.5 mm. Furthermore, it is preferred that the hook flat section of the hook insert portion is about 5mm in length and about 2mm in diameter.
Next, the operation of the endoscopic sinus retractor S of the present invention will be explained. When the nose endoscope retractor S is used, the nose endoscope retractor S is installed on a nose endoscope through an endoscope fixing part 3, the nose septum mucous membrane or the nose dorsal flap of an operative field is pulled open by using the retractor S, the operative field is exposed to illuminate the operative field, therefore, the retractor operation which must be performed by an assistant before can be omitted in a functional nose plastic operation, an operator only needs to hold the nose endoscope with the retractor S in a hand, the operative field mucosa flap or the nose flap is pulled open by the retractor S sleeved on the nose endoscope, the nose endoscope illuminates the anatomical structure can be clearly distinguished, and meanwhile, the operation can be performed by the right hand, so that the operation is more convenient for the operator. In addition, on the premise of not obstructing the operation, because the operator himself/herself carries out the hook pulling operation, unnecessary trouble can be reduced, and the problem caused by the experience of the assistant and the degree of understanding with the assistant does not exist.
In addition, the hook body part is used for distributing different plug-in parts so as to become a hook with different functions, thereby reducing the complicated hook tools. For example, the plate-shaped insert part and the hook main body part can be combined to form a flat plate type nasoscope hook S which can be used for pulling apart the nasal septum mucous membrane in the nasal septum plasty to expose the nasal septum cartilage and bone, or used for pulling apart the nasal back skin flap to expose the nasal bone and the nasal back cartilage in the external rhinoplasty; the hook-shaped plug part and the hook main body part can be combined to form a hook-shaped nose endoscope hook S which can be used for pulling a nose tip skin flap to expose a nose tip and a columella tissue structure.
Industrial applicability
In the invention, the plug-in part (drag hook) is fixed at the lens end of the nasal endoscope by the endoscope fixing part arranged on the drag hook main body part, an operator holds the endoscope at the left hand, pulls open the mucosa flap or the skin flap of the operation field by the drag hook in front of and below the lens to expose the operation field, and simultaneously illuminates the operation field by the nasal endoscope, and performs the operation by the right hand. Therefore, the operation can be simultaneously carried out by a single operator, the drag hook, the nose endoscope illumination and observation and the operation of the nose are carried out, so that the operation is simple and easy, an assistant is not needed, and the operation can be accurately carried out due to various operations carried out by the operator himself, so that the operation can be carried out more smoothly. In addition, the operation space of the operation can be saved, and the complicated operation process can be reduced. Furthermore, there is also an advantage in reducing waste of personnel/resources.
