本发明涉及脊柱手术用医疗器械技术领域,详细讲是一种相互配合使用时,连接简便、快捷,连接可靠性高、不易松脱,易拆卸、压棒操作速度快、效率高的椎弓根螺钉及压棒装置。
背景技术
我们知道,压棒是脊柱矫形和脊柱后路微创手术中经常需要使用的术中操作,其方法是通过工具或者长尾椎弓根螺钉将连接棒压入螺钉的U形槽中。对于多节段的胸腰段后路手术,当椎弓根螺钉植入病人椎体后,由于椎弓根螺钉的钉座位置在多数情况下不可能在一条直线上,也不能完全贴合固定棒的轴线,将连接棒放入其中某几个螺钉的钉座后,连接棒就无法轻易放入剩余的螺钉钉座中,这时候就需要将棒压入这些钉座里。对于脊柱矫形手术,医生需要根据术后理想的胸腰椎生理曲线预先弯曲连接棒,医生在植入椎弓根螺钉后,螺钉的位置由于病人脊柱畸形,无法贴合预弯的连接棒,因此就需要在螺钉座上放置延长套筒,将预弯的连接棒穿入延长套筒的侧槽,然后将棒逐一压入所有螺钉的钉座中,由于连接棒有一定的刚性,而病人的脊柱也具有一定的弹性,这样就将椎体沿着连接棒的形状固定下来,从而达到脊柱矫形的目的。
国家知识产权局公开的专利号为:201220604550.0、名称为:椎弓根螺钉及椎弓根螺钉连接器的实用新型专利记载了一种安装方便的压棒套筒及与之配合的椎弓根螺钉,其特征在于使用一个长尾的椎弓根螺钉和一个U形开口的螺钉延长套管,延长套管外有一个压棒套管和一个螺母。在实际使用中有如下缺点:一、螺钉尾部较长,不仅加工复杂成本高,而且需要在手术完成时执行额外的折断操作;二、手术完成后需要将螺母从延长套管底部的旋出,由于在矫形手术中使用的螺钉和套管数量很多,因此医生需要花大量的时间将螺母逐一取出,非常费时,不仅延长了手术时间,同时还增加了病人的出血和感染的风险;三、在矫形的过程中当棒对延长套管施加侧向力时,会出现延长套管张开而发生松脱;四、在拆卸延长套管时其侧面的圆柱状凸起容易卡在螺钉座的侧壁凹槽里,需要左右反复摇晃才能取出,大大影响了手术的顺利进行。现有的其他的压棒装置或椎弓根螺钉的尾部连接装置均存在连接困难,拆卸更困难,拆卸时间长、效率低等缺点。
发明内容
本发明的目的是解决上述现有技术的不足,提供一种相互配合使用时,连接简便、快捷,连接可靠性高、不易松脱,易拆卸、压棒操作速度快、效率高的椎弓根螺钉及压棒装置。
一种椎弓根螺钉,包括钉体和螺钉座,螺钉座上设有 U 形置棒槽,U 形置棒槽将螺钉座分为相互对称的横截面呈弧形的左板体和右板体,左板体左侧和右板体右侧上分别设有定位凹槽,其特征在于左板体和右板体上端部的前侧和后侧分别纵向设有上端敞口的定位滑槽。
本发明中所述的螺钉座前后两侧加工为扁平面,定位滑槽设在左板体和右板体上端部的扁平面上。
一种压棒装置,其特征在于包括延长套筒、压棒套筒和压棒螺母,延长套筒下部纵向设有下端敞口的压棒槽,压棒槽左右两侧的延长套筒侧壁上轴向开设有两个相对的长条口,长条口内设有定位弹片,延长套筒下部的长条口内的两个定位弹片下端部内侧设有定位凸起;延长套筒下部内壁上设有与定位滑槽相配合的、向内凸起的定位滑块,压棒套筒套设在延长套筒上,压棒套筒上方的延长套筒上部经螺纹连接有压棒螺母。
本发明中所述的延长套筒内插设有拆卸装置,所述的拆卸装置包括拆卸套筒和拆卸顶杆,拆卸套筒下部的侧壁上轴向开设有两个相对的长条口,长条口内设有顶撑弹片,顶撑弹片内侧下端部设有向内凸起的扩张凸块,扩张凸块的上侧面呈由外至内、由上至下的倾斜面,拆卸顶杆可自由滑动的插设在拆卸套筒内。
本发明中所述的拆卸顶杆下端部设有与扩张凸块上侧的倾斜面相配合的顶撑斜面。顶撑斜面由上至下向内倾斜。当拆卸套筒固定不动而拆卸顶杆继续向下运动时,顶撑斜面与扩张凸块上侧的倾斜面配合将扩张凸块顶动,带动顶撑弹片向外做扩展运动。
本发明中所述的拆卸套筒上部设有与延长套筒上端相配合的限位凸台。当拆卸套筒插入延长套筒内、顶撑弹片下端与定位弹片相对时,限位凸台卡在延长套筒的上端部、限制拆卸套筒继续向延长套筒内插入,位置掌控精准,拆卸便利。
本发明中所述的拆卸套筒上部设有径向贯穿拆卸套筒的定位销,拆卸顶杆上轴向设有与定位销相配合的定位滑槽。定位滑槽与定位销配合、限定拆卸顶杆在拆卸套筒内只能轴向、定距离的滑动。使用方便、便于掌握拆卸套筒上顶撑弹片的方向及顶起程度。

使用时,首先将延长套筒下端口对准螺钉座,在往下压的过程中,延长套筒两侧的定位弹片上的定位凸起碰到螺钉座侧壁而向外张开,当延长套筒往下压至合适的距离时,弹性片的凸起就会嵌入钉座侧壁的定位凹槽里,将延长套管锁至螺钉座上,并防止延长套筒在操作过程中向上脱出。延长套筒下部的四个定位滑块滑入螺钉座侧壁上的定位滑槽内,有效防止了延长套筒在术中操作时向外侧张开而导致松脱。将连接棒穿入延长套筒的压棒槽后,将压棒套筒从上方套到延长套筒外侧,将压棒螺母沿着延长套筒向下拧的过程中下压压棒套筒,实现压棒操作。当压棒动作完成时,压棒螺母和压棒套筒已经压至底部,此时便可以将锁紧螺塞通过延长套筒的中心孔旋入螺钉座。螺塞拧紧后便可以拆卸所有套筒。将拆卸装置插入延长套筒内,向下按压拆卸顶杆,就会将顶撑弹片向外顶起,顶动定位弹片从螺钉座侧壁的定位凹槽内弹出,定位凸起从螺钉座侧壁的定位凹槽内脱出。由于在压棒套筒下端部设有定位弹片扩张槽,定位弹片可以在压紧位置时自由向外张开,从而将整个压棒装置从螺钉座上拆除,而无需将螺母逐一拧出延长套筒,大大简化了手术操作。本发明具有连接简便、快捷,连接可靠性高、不易松脱,易拆卸、压棒操作速度快、效率高等优点。

本发明进一步改进,所述的定位凸起11上方的延长套筒下部内设有用于阻挡螺钉座的限位凸起21。当限位凸起碰到螺钉座侧壁的上表面时,定位弹片上的定位凸起嵌入螺钉座侧壁上的凹槽内。所述的压棒螺母为圆柱形,压棒螺母外壁上均布有防滑凹槽。以便用手或工具将螺母沿纵向旋入或旋出。
使用时,首先将延长套筒下端口对准螺钉座,在往下压的过程中,延长套筒两侧的定位弹片上的定位凸起碰到螺钉座侧壁而向外张开,当延长套筒往下压至合适的距离时,弹性片的凸起就会嵌入钉座侧壁的定位凹槽里,将延长套管锁至螺钉座上,并防止延长套筒在操作过程中向上脱出。延长套筒下部的四个定位滑块滑入螺钉座侧壁上的定位滑槽内,有效防止了延长套筒在术中操作时向外侧张开而导致松脱。将连接棒穿入延长套筒的压棒槽后,将压棒套筒从上方套到延长套筒外侧,将压棒螺母沿着延长套筒向下拧的过程中下压压棒套筒,实现压棒操作。当压棒动作完成时,压棒螺母和压棒套筒已经压至底部,此时便可以将锁紧螺塞通过延长套筒的中心孔旋入螺钉座。螺塞拧紧后便可以拆卸所有套筒。将拆卸装置插入延长套筒内,向下按压拆卸顶杆,就会将顶撑弹片向外顶起,顶动定位弹片从螺钉座侧壁的定位凹槽内弹出,定位凸起从螺钉座侧壁的定位凹槽内脱出。由于在压棒套筒下端部设有定位弹片扩张槽,定位弹片可以在压紧位置时自由向外张开,从而将整个压棒装置从螺钉座上拆除,而无需将螺母逐一拧出延长套筒,大大简化了手术操作。本发明具有连接简便、快捷,连接可靠性高、不易松脱,易拆卸、压棒操作速度快、效率高等优点。

Claims (9)

1.一种椎弓根螺钉,包括钉体和螺钉座,螺钉座上设有 U 形置棒槽,U 形置棒槽将螺钉座分为相互对称的横截面呈弧形的左板体和右板体,左板体左侧和右板体右侧上分别设有定位凹槽,左板体和右板体上端部的前侧和后侧分别纵向设有上端敞口的定位滑槽;其特征在于:椎弓根螺钉专用压棒装置包括延长套筒、压棒套筒和压棒螺母,延长套筒下部纵向设有下端敞口的压棒槽,压棒槽左右两侧的延长套筒侧壁上轴向开设有两个相对的长条口,长条口内设有定位弹片,延长套筒下部的长条口内的两个定位弹片下端部内侧设有定位凸起;延长套筒下部内壁上设有与定位滑槽相配合的、向内凸起的定位滑块,压棒套筒套设在延长套筒上,压棒套筒上方的延长套筒上部经螺纹连接有压棒螺母。
2.根据权利要求1 所述的椎弓根螺钉,其特征在于所述的螺钉座前后两侧加工为扁平面,定位滑槽设在左板体和右板体上端部的扁平面上。
3.根据权利要求2所述的椎弓根螺钉,其特征在于所述的延长套筒内插设有拆卸装置,所述的拆卸装置包括拆卸套筒和拆卸顶杆,拆卸套筒下部的侧壁上轴向开设有两个相对的长条口,长条口内设有顶撑弹片,顶撑弹片内侧下端部设有向内凸起的扩张凸块,扩张凸块的上侧面呈由外至内、由上至下的倾斜面,拆卸顶杆可自由滑动的插设在拆卸套筒内。
4.根据权利要求3所述的椎弓根螺钉,其特征在于所述的拆卸顶杆下端部设有与扩张凸块上侧的倾斜面相配合的顶撑斜面。
5.根据权利要求3 或4所述的椎弓根螺钉,其特征在于所述的拆卸套筒上部设有径向贯穿拆卸套筒的定位销,拆卸顶杆上轴向设有与定位销相配合的定位滑槽。
6.根据权利要求5 所述的椎弓根螺钉,其特征在于所述的定位凸起上方的延长套筒下部内设有限位凸起。
7.根据权利要求5 所述的椎弓根螺钉,其特征在于所述的压棒套筒下部的侧壁上与定位弹片相对侧设有的定位弹片扩张槽。
8.根据权利要求5 所述的椎弓根螺钉,其特征在于所述的定位弹片和顶撑弹片呈长条片状,定位弹片的上端与延长套筒上的长条口上部的延长套筒相连、顶撑弹片与拆卸套筒上的长条口上部的拆卸套筒相连。
9.根据权利要求5 所述的椎弓根螺钉,其特征在于所述的拆卸套筒上部设有与延长套筒上端相配合的限位凸台。
