Abstract

本发明是一种用于固定角膜片的眼科手术器械。现有的角膜固定装置需用缝线固定角膜片,操作繁琐,固定不牢,费力费时,易损伤角膜组织。本发明由带有尖齿突起的固定环、固定螺丝、主体和调节螺丝构成。该装置结构简单,重量轻,固定角膜容易、牢固,分离角膜片十分方便。本发明可用于角膜板层移植手术或其它实验研究。

Description

本发明属于一种眼科手术器械。在施行眼科板层角膜移植手术时,需从离体的角膜片上要分离切取角膜板层植片并将其移植于受体眼球上。由于离体角膜片体积小又较薄,在其上进行分离切取板层操作不方便。因此,临床上常用线将角膜片缝在纱布球的表面或者用Furness角膜夹或用自制的义眼座固定器固定离体角膜,进行分离切取板层角膜的操作(王守敬等《角膜移植术》P63,1980年4月第1版西安)。
现有的固定器由义眼座主体、义眼、内套、外套构成。操作时将义眼座固定于手术台上,先将角膜片表皮向上置于义眼上,在角膜边缘3、6、9、12点处穿上缝线,分别固定在内套上,然后将外套套上拧紧螺丝,即可将角膜片固定紧进行板层角膜的分离切取。
现有技术在固定角膜片时要穿过数根缝线,然后通过固定缝线来固定角膜片,并且固定器需固定在手术台上操作。因此,操作繁琐、十分不便,而且常固定不牢或不易固定,费时费力又易损伤角膜组织。为了使角膜片固定牢固而且操作方便简捷、快速、防止损伤角膜组织特设计本装置《角膜固定器》。
本发明由主体、角膜托、固定环、固定螺丝和调节螺丝构成。角膜托由聚四氟乙烯或其他材料制成园柱形,长20~30mn,直径15~20mn,其中心为中央小孔与上下端面相通,内径15~3mn,其上下端面为园形突起,突起的曲率半径为8~14mn。固定器的主体为园柱形,高25~35mn,外径30~38mn,其上部外径较小,最上端外径18~25mn,其下部位外径25~35mn,有外螺纹。主体内腔为角膜托槽,上口内径15~20mn,下口内径10~15mn,有内螺纹。固定环为园形环,上口小、下口大,上口内侧有4~8个对称分布的尖齿,尖齿内径12~16mn,尖齿向内或向下突出,环的下端向外周突出。固定环下口内径大小以套在主体最上端外部为宜。固定环与主体之间设置有制动销或其他制动方式,使二者只能顺轴向移动,而不能相互作旋转移动,以防止固定环随固定螺丝的拧动作旋转移动而损伤角膜。固定螺丝为中空园形,上口大小能套在固定环外周压在固定环下端向外周的突起上。固定螺丝下口较大,有内螺纹,可拧在主体上部外螺纹上,拧紧时可使固定环向下移动与主体紧密接触。调节螺丝为园柱体,上部外径较小约10~15mn做成外螺纹,可从主体的下口旋进主体内腔角膜托槽中。拧动调节螺丝可调节角膜托位置的高低,调整角膜片固定的牢固程度。调节螺丝下部外径较大25~35mn,既可方便地拿在手中操作,又可平稳地放在台上操作。
本发明由于设计了园形的固定环,且环的上口内侧有数个尖齿,能直接将角膜片固定在角膜托上,省去了挂置缝线的步骤,因而达到了快速简捷,固定牢固的效果,显著地方便了操作,缩短了操作时间。
同时,在主体和固定环之间设置了制动销,限制固定环与主体之间同轴心的旋转移动,但可以顺轴向方便地套入或拨出,从而当拧紧固定螺丝时不会带动固定环的旋转移动,防止了损伤角膜片。
调节螺丝的设置,方便了调节角膜托位置的高低,确保了角膜片固定松紧适度。
由于配置了尖齿内径大小不同的固定环和曲率半径不同的角膜托,满足了不同大小角膜片的需要,保证了分离切取的角膜片平整、损伤轻。因而提高了分离板层角膜植片的质量。

实施例:
参见附图1、2,角膜托(5)用聚四氟乙烯材料做成直径17mn,长25mn的园柱体6个,中央小孔(8)直径为2mn,两端为园形突起,曲率半径分别为10~12mn。
固定器主体(1)上角膜托槽(9)的内径17mn,角膜托槽(9)下口内径11mn。主体(1)上端外径21mn,主体外螺纹外径30mn,主体(1)外径35mn。固定环(3)为园形环,上口内侧有尖齿(7)8个,尖端向下,按尖齿不同内径12、13、14、16mn分别做成4个固定环。固定环下口内径21mn,下口向外周突出部外径28mn。固定螺丝(2)为中空园形,外径35mn,上口内径26mn,下部内径28.5mn。调节螺丝(4)为园柱形,上部外径12mn,有外螺纹,下部外径28mn。在主体和固定环之间设置了制动销(6)。
使用时,先将调节螺丝(4)的上部旋进主体(1)的下口螺孔内。然后按需选用适当曲率半径的角膜托(5)放入主体的角膜托槽(9)内。再将角膜片表皮向上置于角膜托(5)上。此后,选用适当尖齿内径的固定环(3)套在主体上端,最后将固定螺丝套在固定环上,并拧在主体外螺纹上,此时固定环向下移动,尖齿将角膜片牢固地固定在角膜托上,此时即可方便地进行角膜片的分离切取等操作。

Claims (6)

1、一种用于固定角膜片的眼科手术器械,由主体、角膜托、固定环、固定螺丝、调节螺丝构成。其特征是:固定环的上口内侧有4~8个尖齿,尖齿的内径为12~16mm。
2、如权利要求1中所述的固定环尖齿,其尖端向内或向下或者向内下。
3、如权利要求1中所述的主体与固定环,其间设置有制动销或其他制动方式,使二者间只能有顺轴向移动,而不能互相旋转移动。
4、如权利要求1中所述的主体其内腔开有下口,有内螺纹。
5、如权利要求1中所述的角膜托为园柱形两端为园形突起,突起的曲率半径为10~12mn,中心的中央小孔与上下端面相通。
6、如权利要求5中所述角膜托由聚四氟乙烯材料制成。
