背景技术
基于能量的组织治疗在本领域中是众所周知的。各种类型的能量(例如,电、超声、微波、低温、热、激光等)被用来治疗组织。用来治疗组织的这样一种类型的能量是电外科能量。电外科是在外科手术期间应用电和/或电磁能量来切割、解剖、消融、凝结、烧灼、愈合或以别的方式治疗生物组织。一般而言,电外科使用可操作成输出能量的电外科发生器以及经电缆组件以电的方式连接到发生器的有源和返回电极。
在双极电外科中,手持式器械的一个电极充当有源电极并且该手持式器械的另一个电极充当返回电极。返回电极放成紧靠有源电极,使得电路在两个电极之间形成(例如,电外科钳)。双极电外科技术和器械可以用来凝结血管或组织,例如,软组织结构,诸如肺、脑和肠。外科医生可以通过控制在电极之间并且通过组织施加的电外科能量的强度、频率和持续时间来烧灼、凝结、干燥组织,或者简单地减少或减慢流血。为了在不在手术部位造成不想要的组织炭化或对相邻组织造成间接损害,例如热扩散,的情况下实现期望的外科效果,来自电外科发生器的输出被控制,例如功率、波形、电压、电流、脉冲速率等。
在单极电外科中,有源电极通常是外科医生持有的、应用到要治疗的组织的外科器械的一部分。具有一个或多个返回电极的患者返回垫放成远离有源电极,以便把电外科能量带回发生器并且安全地分散 由有源电极施加的电流。返回电极通常具有大的患者接触表面积,以最小化那个部位的发热。发热是由直接依赖于表面积的高电流密度造成的。越大的表面接触面积导致越低的局部化热强度。返回电极的尺寸通常是基于在特定外科手术期间使用的最大电流和发生器的占空比(即,发生器开启的时间的百分比)的假设来选择的。
已经为各种用途和应用开发出了利用电和/或电磁能量的电外科设备。一种类型的基于能量的组织治疗是基于微波能量的治疗。通常,在外科手术中使用的微波装置包括充当能量源的微波发生器,及具有用于把能量指向目标组织的天线组件的微波外科器械(例如,微波消融探针)。微波发生器和外科器械通常由具有多个导体的电缆组件操作耦合,这多个导体用于把微波能量从发生器发送到器械,并且用于在器械和发生器之间传送控制、反馈和识别信号。

发明内容

图1是根据本公开内容实施例的双极和单极电外科系统1的示意性说明。系统1包括外科发生器20。根据本公开内容的外科发生器20可以执行基于能量的外科手术。在这里所公开的一些实施例中,外科发生器20被描述为电外科发生器;但是,应当理解,在这里所公开的各种实施例中,微波外科发生器或超声外科发生器或者其它合适的发生器都可以在适当的时候代替电外科发生器使用。

发生器20配置为以各种模式操作。在一种实施例中,发生器20可以输出各种模式,例如,切割、混合、利用止血法的组织分割、电气烧灼和/或喷雾。在有些实施例中,每种模式基于预先编程的功率曲线操作,该曲线指示在变化的负载(例如,组织)阻抗范围由发生器20输出多少功率。每条功率曲线包括由用户选定的功率设置和测出的最小负载阻抗定义的恒定功率、恒定电压和恒定电流范围。
屏幕46控制由可以插入到连接器60、62中的钳子10执行的双极愈合手术。发生器20通过连接器60、62输出适于愈合被钳子10抓住的组织的能量。屏幕46还控制系统托盘47,以允许用户访问并调整系统设置。系统托盘47可以包括亮度图标43。系统托盘47可以包括菜单图标48。系统托盘47可以包括错误禁用图标41。亮度图标43允许用户调整屏幕42、44、46的亮度。错误禁用图标41利用服务菜单指示错误报警已经被禁用。菜单图标48允许对主菜单的访问,其中用户可以改变对语言、外观和其它操作的选项。
屏幕42控制连接到连接器50和52的单极输出及设备。在有些实施例中,连接器50配置为耦合到器械2,而连接器52可以配置为耦合到提供附加输入(例如,复制发生器20和/或器械2的输入)的脚踏开关(未示出)。例如,在标准的单极模式中,功率输出模式72、74在接口70上指示。用户分别利用上下箭头76、78对每种模式调整功率控制。
屏幕44控制连接到连接器56和58的单极和双极输出及设备。连接器56配置为耦合到器械2,从而允许发生器20给多个器械2供电。连接器58配置为耦合到双极器械。当在单极模式下使用发生器20时(例如,对于器械2),返回电极6耦合到与屏幕42、44相关联的连接器54。
图3示出了包括耦合到外科器械21的发生器20的电外科系统19的示意性框图。发生器包括控制器24、高压DC电源27(“HVPS”)和RF输出级28、电容-数字转换器36以及多路复用器38。器械21包括识别电路14。器械21可以是图1的单极外科器 械2或双极电外科钳10。
HVPS 27连接到AC源(例如,墙上的电源插座)并且向RF输出级28提供高压DC功率,然后,RF输出级28把高压DC功率转换成RF能量并且把RF能量交付给端口或有源端子30。电外科能量经端口或返回端子32返回到其,例如,电外科能量可以通过返回垫(未示出)返回。特别地,RF输出级28生成高RF能量的正弦波形。RF输出级28配置为以多种模式操作,在此期间,发生器20输出具有具体占空比、峰值电压、波峰因数等的对应波形。在另一种实施例中,发生器20可以基于其它类型的合适的电源拓扑结构。
控制器24包括与存储器26可操作通信的处理单元25,其中存储器可以是易失性存储器,例如RAM,和/或非易失性存储器,例如闪存介质、盘介质等。处理单元25包括可操作连接到HVPS 27和RF输出级28的输出端口,从而允许处理单元25根据开环控制方案或闭环控制方案控制发生器20的输出。处理单元25可以包括任何类型的计算设备、计算电路,或者能够执行存储在存储器中的一系列指令的任何类型的处理器或处理电路。处理单元25可以包括多个处理器和/或多核CPU并且可以包括适于执行这里讨论的计算的任何类型的处理器,诸如微处理器、数字信号处理器、微控制器等。
控制器24耦合到电容-数字转换器36,电容-数字转换器36又耦合到多路复用器38。多路复用器38耦合到外科器械21,使得电容-数字转换器36测量识别电路14中的一个或多个电容性元件29,以识别外科器械21。
每个电容性元件29都具有电容。多路复用器38个别地选择其中一个电容性元件29进行询问,因此电容-数字转换器36可以测量各个电容。电容-数字转换器36以数字形式把测出的电容传送到控制器24。控制器24把电容性元件29的每个电容映射到对应的数字。例如,16个不同的电容范围可以对应于16个不同的十六进制数字,例如由位0…9和A…F组成。当由8位的十六进制数字表示时,八个电容性元件29可以对应于4,294,967,296个值。映射值可以对应于用作外 科器械21的识别值的数字,例如,外科器械识别值可以是映射数字的顺序聚合(或者映射数字的串联)。在有些实施例中,对外科器械识别值执行哈希算法,以安全地识别外科器械21。哈希算法可以是消息摘要5(MD5),或者是安全哈希算法(SHA),例如,SHA-0、SHA-1或SHA-2等。哈希后的外科器械识别值可以与数据库中的(例如存储器26中的)值进行比较,以确定外科器械21是否是经过授权的。在有些实施例中,如果确定未授权的外科器械21耦合到发生器20,那么,例如,通过阻止RF输出级28向外科器械21供应能量,控制器24禁用发生器20。附加地或者作为选择,可以执行其它加密方法,例如,高级加密标准(AES)。
外科器械识别值可以附加地,或者作为选择,用来对外科器械21已经使用过的次数保持跟踪。在有些实施例中,例如,通过比较所存储的外科器械21已经使用的次数与预定的阈值,发生器20适于确定是否已经到达外科器械21的寿命终结。在其中外科器械21可以是单次使用的一次性器械的有些情况下,发生器20可以使用外科器械识别值来确定外科器械21之前是否使用过。如果使用过,发生器20就可以适于防止外科器械21被再次使用。
外科器械识别值可以对应于外科器械21的序列号、外科器械21的批号、外科器械21的器械类型,等等。识别电路14可以包括用于确定外科器械21的一个或多个参数的多个值。
外科发生器20从一个或多个传感器23接收反馈,以便在控制器21中形成闭环控制系统(未示出)。控制器24中的控制系统可以基于外科器械21的识别来调整。一个或多个传感器23测量各种组织和能量属性(例如,组织阻抗、组织温度、输出电流和/或电压等)并且向控制器24提供反馈。这种传感器可以包括耦合到发生器20的输出端子30和32的电压和电流传感器。响应于传感器信号,控制器24控制HVPS 27和RF输出级28,然后,HVPS 27和RF输出级28分别调整DC和RF功率供给。控制器24还从发生器20、器械2或者钳子10的输入控制接收输入信号。控制器24利用输入信号来调整 发生器20输出的功率并且对其执行其它控制功能。
图4是根据本公开内容实施例的识别电路400的示意图。识别电路400包括由电容器402-414示意性表示的八个电容性元件。电容器402-414当中的每一个分别具有对应的电容C1、C2、…、C8。电容-数字转换器36可以利用多路复用器38(见图3)通过询问线418个别地询问电容器402-414。图4B和4C示出了通过在基板上淀积银墨水形成的识别电路400的实施例。
目前公开的电容性元件可以通过移转印刷、丝网印刷、光刻、激光打印和/和合适的其它过程来形成。在下面描述的有些实施例中,电容性元件(例如,图4B、4C和5中所示的电容器402-416)可以利用直接写过程形成,例如,Technologies的,以便把材料淀积到表面上。一般而言,术语“直接写”描述了采用计算机化的、运动受控工作台的打印或构图方法,利用不运动的图案生成设备在设计好的图案中把可流动的材料分配到表面上。
是一种使用把可流动材料泵送到表面上的正位移法的微-毛细管技术,可流动的材料通常具有大约5和大约500000厘泊之间的黏度。在有些实施例中,在一个或多个层中,利用直接写精确地控制应用到探针外表面的可流动材料(例如,导电墨水)的体积导致满足电容性元件阵列的具体容限需求的图案的形成,例如,允许探针识别。
图5A是通过在基板418上淀积金属性墨水402A和402B形成的识别电路400的俯视图,而图5B是其侧视图。金属性墨水402A和402B可以由任何合适的导电材料形成,例如,铜、金、银或具有合适电导率值的其它导电金属。电容器402-416当中每一个都可以以这种方式形成。在其它实施例中,其它金属性墨水或导电材料可以淀积在基板418上,以形成一个或多个电容器402-416。基板418可以由各种无机和/或有机材料制成,包括但不限于基于硅的化合物、玻璃、石英、蓝宝石、陶瓷、聚酰亚胺(例如,)、聚酯 (例如,)、聚对苯二甲酸乙二醇酯(PET)、塑料。在有些实施例中,基板418可以由以商标由Polygon公司提供的商业可用的复合材料制成。
在有些实施例中,识别电路400可以在能量施加器或探针的外表面上形成、构图或以别的方式淀积。在有些实施例中,识别电路400可以利用直接写过程形成,例如,Technologies的 或者其它合适的材料淀积技术。
在平行板几何形状的情况下,电容器402-416中每一个的电容可以由公式(1)得出:
其中C是电容器的电容,A是基板418一侧上所淀积的墨水的面积,ε0是电容率常数,εr是基板418的介电常数,而d是两个墨水淀积之间的距离,例如,墨水淀积402a和402B之间的距离。根据本公开内容的各种实施例,电容器的设计可以不是平行板几何形状,而且电容器402-416中每一个的电容可以一般性地由公式(2)定义:
其中C是电容器的电容,Q是电荷,而V是电压。
所淀积的墨水402A-416A和402B-416B的面积可以选择成对应于数字。在有些实施例中,电容器402-416所淀积的墨水的尺寸(例如,面积A)可以选择成使得电容器402-416中每一个都具有对应于一个数字的电容,使得发生器20(图3)可以经识别电路400识别器械。附加地或者作为选择,两个墨水淀积之间的距离可以改变,以改变电容性元件的电容。
图5示出了根据本公开内容实施例的、具有识别电路400的微波外科器械500。识别电路400耦合到、淀积到或者以别的方式附连到微波外科器械500的外部套管,使得微波发生器可以询问识别电路400。
图6A和6B是说明根据本公开内容实施例的、识别外科器械的 方法600的流程图。在步骤602,外科器械连接到发生器,例如,发生器20。在步骤604,多个电容性元件被询问,包括步骤602的外科器械中的电容性元件。步骤604包括步骤606和608以及决定步骤630。
在步骤606,电容性元件被个别询问,例如,多路复用器39个别地询问电容性元件29的一个电容性元件。在步骤608,该电容性元件的电容利用电容-数字转换器来确定。在决定步骤630,确定是否所有电容性元件29都已经被询问过。如果在步骤630确定不是所有电容性元件29都被询问过,则方法600重复步骤606和608,直到所有电容性元件29都被询问过。
在步骤610,每个电容性元件29都映射到一个对应的数字。在步骤612,外科器械识别值由对应的数字形成,例如,通过串联所有电容性元件的所有对应的数字。在步骤614,基于来自电容性元件29的对应数字识别外科器械,例如,利用外科器械识别值。在步骤616,确定对应于电容性元件29的外科器械的生产批号,例如,利用外科器械识别值。在步骤618,对外科器械识别值执行哈希算法。在步骤620,哈希后的外科器械识别值与数据库中的值进行比较。在步骤622,基于哈希后的外科器械值与数据库中的值之间的比较,确定外科器械是否是授权的外科器械。
在步骤624,如果识别出的外科器械是未授权的外科器械或者批号不对应于预定的标准,则发生器被禁用。在步骤646,基于阈值,例如一次性器械=1,确定对应于电容性元件的外科器械是否已达到寿命终结。在步骤628,如果已经到达寿命终结,例如,外科器械已经使用了预定的次数,则发生器被禁用。
图7示出了根据本公开内容实施例的微波外科器械(总体上示为700)。微波外科器械700一般包括能量施加器或探针701,包括定义纵轴“A–A”并包括末端715的拉长构件710。在有些实施例中,拉长的构件710可以包括冷却套714并且可以由任何合适的材料形成,例如,POLYMEDTM复合材料管材。在有些实施例中,至少冷却套 714的一部分具有大于天线组件(未示出)外直径的内直径,由此定义管状流体腔712。
微波外科器械700可以具有端帽或锥形部分750,例如,位于拉长构件710的末端715,该部分可以终止于锋利的尖端,以允许以最小化的阻力插入组织。可适于用作探针701的、具有锋利尖端的直探针的一个例子以由位于美国科罗拉多州博尔德的CovidienSurgical Solution提供的商标EVIDENTTM商业可用。端帽或锥形部分750可以包括其它形状,诸如像圆形、扁平、方形、六边形或圆柱形的尖端。
导电轨迹的第一配置(在图7和9中总体上示为740)在探针701的外表面711的第一部分720上形成、构图或以别的方式淀积。导电轨迹的第一配置740包括多条个别的轨迹744,并且可以由导电墨水742形成,例如,包括导电(例如,银)粒子的墨水。一条或多条导电轨迹744可以包括一个或多个元件连接745。在有些实施例中,如图9中所示,每个元件连接745可以是圆形的,并且可以定义顶针(pogo-pin)或其它隔离的电气连接。元件连接745的形状和尺寸可以与图9中所绘出的配置有所不同。
在图8中,图7的微波外科器械700示为关于其纵轴“A-A”旋转180度。导电轨迹的第二配置(在图8和10中总体上示为840)在探针701的外表面711的第二部分820上形成、构图或以别的方式淀积。导电轨迹的第二配置840包括多条轨迹744的末端部分846,并且可以由导电墨水742形成。第二配置840可以附加地包括接地轨迹830。在有些实施例中,如图8和10中所示,接地轨迹830包括位于末端部分846之间并与其隔开的第一部分831(图10)以及沿纵轴“A-A”附近延伸的第二部分832(图8)。末端部分846和接地轨迹830的形状和尺寸可以与图8和10中所绘出的配置有所不同。
如图9和10中最好地示出的,多条轨迹744及其末端部分846的尺寸设计成关于接地轨迹830具有不同的电容值。在有些实施例中,导电轨迹的第一配置740和导电轨迹的第二配置840一起形成探针701的识别电路。
在有些实施例中,第一配置740和/或第二配置840可以利用直接写过程形成,例如,Technologies的 以便把例如导电墨水742的材料淀积到探针701的外表面711的一个或多个部分上。
上述包括一个或多个识别电路的外科器械可以适用于内窥镜外科手术和/或手辅助的、内窥镜和腹腔镜外科手术。上述包括一个或多个识别电路的外科器械可以适用于开放式外科应用。上述识别外科器械的方法可以用来识别各种外科器械,例如,双极和单极电外科器械。
虽然为了说明和描述的目的已经参考附图具体描述了实施例,但是应当理解,该发明性过程和装置不应当认为受此限制。对本领域普通技术人员来说很显然的是,在不背离本公开内容范围的情况下,可以对以上实施例进行各种修改。
Claims (14)
1.一种用于识别外科器械的方法,所述方法包括:
把所述外科器械连接到发生器;
测量形成在所述外科器械的外表面上的多个电容性元件的多个电容值;
将所述多个电容值中的每个电容值映射到对应的值;
顺序串联所述对应的值中的每个值以生成外科器械识别值;及
基于所生成的外科器械识别值来识别所述外科器械。
2.如权利要求1所述的方法,其中测量多个电容值包括询问所述多个电容性元件。
3.如权利要求1所述的方法,还包括对所述外科器械识别值执行哈希算法。
4.如权利要求3所述的方法,还包括比较哈希后的外科器械识别值与数据库中的值。
5.如权利要求4所述的方法,还包括基于哈希后的外科器械识别值与数据库中的值之间的比较,确定所述外科器械是否是授权的外科器械。
6.如权利要求1所述的方法,其中测量多个电容值包括个别地询问所述多个电容性元件中的每个电容性元件。
7.如权利要求1所述的方法,还包括响应于识别出的外科器械而禁用所述发生器。
8.如权利要求1所述的方法,还包括确定所述外科器械是否已到达寿命终结。
9.如权利要求1所述的方法,还包括确定所述外科器械的生产批号。
10.如权利要求1所述的方法,其中测量多个电容值包括利用电容-数字转换器确定所述多个电容性元件中的每个电容性元件的电容。
11.一种用于识别外科器械的系统,包括:
发生器,所述发生器包括:
多路复用器;
电容-数字转换器;以及
控制器;
所述系统还包括:
外科器械,所述外科器械包括:
外罩,具有附连到所述外罩的、用于治疗组织的末端执行器;
电缆,从所述外罩延伸并配置为把所述外科器械耦合到所述发生器;及
识别电路,包括位于所述外科器械的外表面上并且能由所述发生器读取的多个电容性元件;
其中:
所述多路复用器配置为单独地耦合所述多个电容性元件中的每个电容性元件用于询问,使得所述电容-数字转换器能够测量每个电容性元件的相应的电容;
所述电容-数字转换器配置为将测量到的电容数字地传送给所述控制器;以及
所述控制器配置为:
将测量到的电容中的每个电容映射到对应的值;
顺序串联所述对应的值中的每个值以生成外科器械识别值;以及
基于所生成的外科器械识别值识别所述外科器械。
12.如权利要求11所述的系统,其中多个电容范围对应于多个数字,其中测量到的电容中的每个电容在所述多个电容范围当中的一个电容范围中并且由此对应于与该范围对应的数字。
13.如权利要求11所述的系统,其中所述外科器械是电外科器械、微波外科器械和超声外科器械之一。
14.如权利要求11所述的系统,其中位于基板上的导电墨水定义所述多个电容性元件。
