Abstract
本发明提供了一种关节镜下胫骨髁间隆突骨折固定装置,包括钛缆、钛缆导入套管、钛缆导出套管和外套管,钛缆导入套管与手柄呈L形连接,钛缆导入套管一端呈弯曲状,与手柄连接的另一端设有外螺纹,钛缆导入套管与外套管螺纹连接;钛缆的一端为尖状端;外套管上设有固定螺栓;钛缆导出套管一端设有U型开口。本发明结构简单,操作简便,能够使强度高柔韧性好的钛缆顺利通过骨隧道及前交叉韧带,对胫骨髁间隆突进行牢固固定,有利于关节进行早期功能锻练。
Description
技术领域
本发明属于医疗器械领域,具体地说涉及一种关节镜下胫骨髁间隆突骨折固定装置。
背景技术
胫骨髁间隆突骨折是前交叉韧带在胫骨髁间附着止点处的斯脱性骨折, 随着运动创伤和交通事故的不断增多, 目前该类骨折的发生率呈不断增高的趋势。由于髁间隆突是交叉韧带的附着点, 不正确的治疗会导致日后膝关节不稳, 并可继发关节内主要结构损伤, 严重影响膝关节功能。随着膝关节镜技术的逐渐成熟、普及,微创治疗胫骨髁间隆突撕脱骨折已成为发展趋复位。目前对胫骨髁间隆突骨折进行固定的器械主要有空心螺钉,可吸收钉,钢丝,可吸收线,Ethibond线及钛缆。空心螺钉及可吸收钉固定仅适用于骨块较大且完整患者,空心螺钉二次手术取出需切开关节,损伤较大,可吸收螺钉强度不够,术后需较长时间制动。钢丝固定符合张力带原则,但细钢丝强度较小,且容易造成组织切割;粗钢丝强度好,但刚度大,柔韧性差,手术操作难度增加。可吸收线及Ethibond线强度不足,难以进行早期锻炼;传统钛缆强度高,柔韧性好,易于关节镜下操作,能够早期进行关节功能锻练,但钛缆的柔韧性使其在穿过前交叉韧带时操作比较困难。
发明内容
本发明为了解决上述问题而提供了一种关节镜下胫骨髁间隆突骨折固定装置。
本发明的技术方案是这样实现的:关节镜下胫骨髁间隆突骨折固定装置,包括钛缆、钛缆导入套管、钛缆导出套管和外套管,钛缆导入套管与手柄呈L形连接,钛缆导入套管一端呈弯曲状,与手柄连接的另一端设有外螺纹,钛缆导入套管与外套管螺纹连接;
钛缆的一端为尖状端;
外套管上设有固定螺栓;
钛缆导出套管一端设有U型开口。
本发明的有益效果是:本发明结构简单,操作简便,适用于所有类型胫骨髁间隆突骨折,能够使强度高柔韧性好的钛缆顺利通过骨隧道及前交叉韧带,对胫骨髁间隆突进行牢固固定,有利于关节进行早期功能锻练。

具体实施方式
为了更好地理解与实施,下面结合附图1-4对本发明作进一步描述:关节镜下胫骨髁间隆突骨折固定装置,包括钛缆1、钛缆导入套管3、钛缆导出套管7和外套管5,钛缆1的一端为尖状端2,有利于钛缆1在套管导引下穿过前交叉韧带;钛缆导入套管3与手柄4呈L形连接,钛缆导入套管3一端呈弯曲状,有利于钛缆1进入关节后改变方向,水平穿过前交叉韧带,手柄4有利于调整钛缆导入套管3的出口方向,与手柄4连接的另一端设有外螺纹,钛缆导入套管3与一端设有内螺纹的外套管5可以刚性连接和分离,增加钛缆1导入时钛缆体外部分的推进力,外套管5上设有固定螺栓6,可以在外套管5与钛缆导入套管3分离时固定外套管5内的钛缆1,钛缆导出套管7头端侧方有U型开口8,用于将钛缆1送入钛缆导出套管7时进行观察。各套管内径均与钛缆1外径匹配,使钛缆1能够顺利通过。
参照附图5,关节镜下证实胫骨髁间隆突骨折并移位后,应用ACL导向器在骨折内缘及胫骨结节内侧建立骨隧道,扩大骨隧道使之允许钛缆导入套管3进入膝关节腔,将钛缆导入套管3弯状出口对准前交叉韧带,连接外套管5与钛缆导入套管3,将钛缆1带锐利尖状端2经外套管5送入钛缆导入套管3,用电钻向前推进钛缆1,直到钛缆1尖端穿过前交叉韧带,必要时将钛缆导入套管3与外套管5分离,挤压固定螺栓6固定钛缆1,电钻固定于外套管5后向前推进钛缆1。应用ACL导向器在骨折外缘及胫骨结节外侧建立骨隧道,插入钛缆导出套管7,应用血管钳在关节镜监视下将钛缆1尖端送入钛缆导出套管7并导出膝关节,撤出套管,将钛缆外侧端经皮下隧道拉入内侧切口并收紧固定。骨折愈合后,在关节外切小口即可取出钛缆,不必切开关节。
本发明结构简单,操作简便,能够使强度高柔韧性好的钛缆顺利通过骨隧道及前交叉韧带,对胫骨髁间隆突进行牢固固定,有利于关节进行早期功能锻练。
Claims (4)
1.关节镜下胫骨髁间隆突骨折固定装置,包括钛缆(1),其特征在于还包括钛缆导入套管(3)、钛缆导出套管(7)和外套管(5),钛缆导入套管(3)与手柄(4)呈L形连接,钛缆导入套管(3)一端呈弯曲状,与手柄(4)连接的一端设有外螺纹,钛缆导入套管(5)与外套管(5)螺纹连接。
2.根据权利要求1所述的关节镜下胫骨髁间隆突骨折固定装置,其特征在于钛缆(1)的另一端为尖状端(2)。
3.根据权利要求1所述的关节镜下胫骨髁间隆突骨折固定装置,其特征在于外套管(5)上设有固定螺栓(6)。
4.根据权利要求1所述的关节镜下胫骨髁间隆突骨折固定装置,其特征在于钛缆导出套管(7)一端设有U型开口(8)。
