技术领域
本发明涉及用于颈椎和腰椎的椎间盘的全置换的椎间盘假体。
背景技术
用于椎间盘的保持功能的人造置换物的构想比四肢的人造关节置换更年轻,但也超过了50年[Buttner-Janz,Hochschuler,McAfee(Eds):TheArtificialDisc,SpringerVerlag,Berlin,Heidelberg,NewYork2003]。其应对生物力学的考虑、颈椎和腰椎融合手术的不令人满意的结果、邻近融合的运动脊椎节段的疾病,以及具有更好的滑动性质和更长寿命的新材料的开发。
借助于保持功能的椎间盘植入物,有可能避免融合手术,即,用以保持、用以修复或用以改善在椎间盘空间内的运动性。在体外实验中,在很大程度上有可能通过人造椎间盘的植入实现脊椎运动节段的生物力学性质的正常化。
目前,临床上使用了许多保持功能的椎间盘植入物。腰椎上的全椎间盘置换始于CHARITéArtificialDisc,之后接着是PRODISC,MAVERICK,FLEXICORE,MOBIDISC,KINEFLEX;ACTIVL,XL-TDR,DYNARDI,PHYSIO-L,INMOTION,M6L,FREEDOM和其它椎间盘假体。颈椎上的许多保持功能的椎间盘植入物也是公知的,举例来说,BRYAN,PRESTIGE,PRODISC-C,KINEFLEXC,MOBIC,ACTIVC,DISCOCERV,DISCOVER,PCM,CERVICORE,M6C,GALILEO,GRANVIA,NUNEC和BAGUERAC。
根据接合部分的数目,且根据生物力学的考虑和颈椎以及腰椎椎间空间内的直接的功能相关情况,存在颈椎和腰椎全椎间盘假体的不同分类。目前,所使用的是具有两个接合表面、一个接合表面或没有接合表面的假体。取决于功能构件的数目和材料,假体具有生物力学上的固定的或移动的旋转中心。尽管更以生理学方式设计了具有两个滑动表面的假体(滑动表面具有移动的旋转中心),但仅具有两个功能性相关的部分和一个滑动表面的假体能够在多节段植入中更好地稳定脊椎。
人造椎间盘可经由腹部、侧腹部、侧部或背部途径来植入。依据该途径,人造椎间盘构造成具有不同的形状、尺寸和器械器件。椎间盘假体的不同尺寸基于假体板的尺寸、关于假体构件高度的不同高度,以及关于假体构件角度的不同前凸角度。梯形的天然椎间盘形状主要负责腰椎和颈椎的前凸,此外,椎体在很小的程度上有助于前凸。在椎间盘的假体置换期间,应保持或重新构造前凸。由于从更长远的来看,可能会发生痛苦的面关节退化,因此要避免操作的脊椎节段的过度前凸角度。过度前凸的椎间盘空间也是对于减少的节段运动范围的前提条件。在该情况下,可能不会实现相邻运动节段免于椎间盘退化的目标。
用于滑动的椎间盘植入物的常用材料为与聚乙烯组合的金属或金属对金属。同时,使用用于全椎间盘的部分新材料,包括用于椎间盘植入物的涂布以获得用于MRI诊断以及植入物的更好滑动性质的机会,以避免任何修正手术。
健康的椎间盘在其与功能性脊椎单元的其它元件相互作用时,允许伸展运动和屈曲运动以及向右和向左的侧向弯曲运动和轴向旋转运动的不同范围内的有限运动。向前运动和向后运动与旋转运动组合,而侧部运动与其它运动方向组合;这是所谓的"联接运动"问题。就伸展(向后弯曲)和屈曲(向前弯曲)以及向右和向左的侧向弯曲和轴向旋转运动来说,健康的椎间盘的运动幅度是不同的。尽管具有一般的基本特点,但在颈椎和腰椎的运动幅度之间也存在差异。
目前在临床上使用的用于全椎间盘置换的所有假体没有完全覆盖颈椎或腰椎的运动节段的自然功能,包括,自然的运动范围。在长期的经历中,由于假体椎间盘的过度活动性和面关节的相关功能障碍,故可能在椎间盘植入的相同水平和/或附近出现面关节退化和面关节疾病。在形成骨刺的情况下,可能出现面关节的磨蚀(关节炎、脊椎关节炎)。结果,可能刺激神经结构,以及直接引起面关节中的疼痛。
迄今仍无证据的是,脊椎内需要冲击吸收来挽救组织或特定的解剖结构;面关节通常不是水平地加载。由于自然的椎间盘不具有像球窝那样的典型接合部分,故冲击吸收的主要功能似乎允许了椎间盘空间内的运动。通过具有椎间盘的冲击吸收,可改变椎间角度,而同时不会显著地降低椎间盘高度。
一些椎间盘假体其功能包括冲击吸收,但又没有模拟节段的生理运动范围。手术后,可观察到后凸椎间盘空间具有的对于面关节过度活动性和面关节退化及疾病的危险的潜在可能。因此,从长远来看,仍未实现通过椎间盘假体的植入的保持功能的椎间盘置换以达到脊椎运动节段的不痛或无痛的稳定的原始目的。

所需的是一种用于颈椎和腰椎的椎间盘假体,其允许较高质量和生理量的生理运动。椎间空间内的三维运动范围应模仿天然椎间盘的联动运动,包括在前后向视图和前部视图中的生理平移。在特殊的解剖学和/或生物力学局部状况的情况下,或在修正手术的情况下,有时需要不允许在手术后在所有方向上的运动的椎间盘假体。也需要用于经由侧向途径植入的椎间盘假体。
在全椎间盘置换后发生任何问题的情况下,最后通常执行融合手术,有时包括椎间盘假体的除去。尤其是在具有用于固定在椎体上的大锚定器件的椎间盘假体中,所植入和向内生长的椎间盘假体的除去导致同时除去骨头的需要,以使得有可能移出椎间盘假体。取决于该问题,在手术进行前或手术进行中,需要组合的可用的全椎间盘假体用于保持运动,且如果需要,还根据外科医生的决定而用于融合手术,用以替换假体构件,由于在手术后没有椎间运动而不除去固定的假体板。
仅存在很少的具有公知的防脱臼的构件的假体,例如,FLEXICORE。具有还包括生理平移且通过其构成或构造避免构件的任何脱臼(例如,在事故之后的颈椎过度屈伸下)的假体将是全椎间盘置换的较大的新优点,允许了更加运动性的活动或甚至给飞行员做手术。
发明内容
本公开内容提供了一种用于颈椎或腰椎内的椎间盘的全置换的椎间盘假体,包括适于以其外表面牢固地与上椎体组装在一起的至少一个滑动部分和适于以其外表面牢固地与下椎体组装在一起的另一个相邻滑动部分,且相邻的滑动部分经由在其面对的内侧上的接合表面接合,包括在一个滑动部分上具有凸面且在另一个滑动部分上具有凹面的接合区域,其中相邻滑动部分中的第一个具有凹口,凹口具有底部表面和四个侧表面,其中凹口定位在第一滑动部分的凸面或凹面内,且与第一滑动部分接合的相邻滑动部分中的第二个具有突起,该突起具有四个侧表面和尖端表面,其中该突起布置在第二滑动部分的凹面或凸面内;以及嵌体,嵌体包括用于收纳突起的切口,其中嵌体定位在相邻滑动部分中的第一个的凹口内,且嵌体具有包绕切口的上凸形的接合表面或上凹形的接合表面,上凸形接合表面或上凹形接合表面具有与相邻滑动部分中的第一个的凸形接合表面或凹形接合表面相同的曲率半径;以及,包绕切口的下凹形接合表面或下凸形接合表面,下凹形接合表面或下凸形接合表面具有与相邻滑动部分中的第一个的凹口底部的凹形接合表面或凸形接合表面相同的曲率半径;以及,四个外侧表面,外侧表面包括面对相邻滑动部分中的第一个的凹口的四个侧表面的嵌体的外周,以及四个内侧表面,内侧表面包括面对相邻滑动部分中的第二个的突起的四个侧表面的嵌体的内周,其中相邻滑动部分中的第一个与相邻滑动部分中的第二个之间的相对于彼此围绕前后向轴线、前部轴线和纵轴线的运动范围由凸面和凹面的曲率半径、凹口的尺寸和形状、嵌体的尺寸和形状、切口的尺寸和形状和突起的尺寸和形状,以及凹口的侧表面与嵌体的外周之间的空间的尺寸和/或嵌体的内周与突起的侧表面之间的空间的尺寸来限定。
至少一个接合表面可部分地或完全地由边缘包绕,以使在前后向轴线和前部轴线上的各个方向上的最终运动范围内的运动限制接触区域最大化。
包绕至少一个接合表面的边缘应由柔性材料制成,使得能够柔软限制至前后向轴线和前部轴线中的各个方向的最终运动范围。
凹口的形状、嵌体的外周和内周的形状,以及突起的形状应为矩形、圆形、圆柱形、角锥形、圆锥形(包括截头角锥和截头圆锥),或前面提到的形状的组合。凹口侧表面的形状、包括嵌体的外周和内周的侧表面的形状,以及突起的侧表面的形状应为平面、弯曲的、成角的或圆形的或它们的组合。
凸面和对应的凹面可具有球形、圆柱形、环面状、螺旋形和/或圆锥形形状或它们的组合,而凸面和对应的凹面可具有相同的或不同的曲率半径。
还打算的是,凹口的尺寸和形状以及嵌体的外周的尺寸和形状可以构造成使得相邻滑动部分有可能围绕前后向轴线或前部轴线旋转。凹口的尺寸和形状以及嵌体的外周和内周的尺寸和形状还可以构造成使得有可能围绕纵轴线附加地有限旋转。
在本公开内容的另一个实施例中,嵌体的内周的尺寸和形状以及突起的尺寸和形状可以构造成使得相邻滑动部分有可能围绕前后向轴线或前部轴线旋。嵌体的内周的尺寸和形状以及突起的尺寸和形状还可以构造成使得有可能围绕纵轴线的附加地有限旋转。
还打算的是,凹口和嵌体的外周的形状和/或嵌体的内周和突起的形状可允许围绕纵轴线无限制的旋转。
突起的两个相对的侧表面可以以形状配合方式与嵌体的内周的两个相对的侧表面接触,和/或嵌体的外周的两个相对的侧表面可以以形状配合方式与凹口的两个相对的侧表面接触。
突起的尖端表面可为凹形或凸形,其具有对应于相邻滑动部分中的第一个的凹口的底部表面的凹面曲率或凸面曲率的曲率半径,使得尖端表面和底部表面能够接合。

在本公开内容的又一个实施例中,嵌体和/或至少一个滑动部分或滑动部分的一部分可由柔性材料制成,以阻尼椎间的冲击或施加的负载。
上滑动部分和/或下滑动部分可具有在其外表面上的用于与椎体组装在一起的至少一个十字形锚,十字形锚可选为与锚定齿组合。
还打算的是,用于与椎体组装在一起的上滑动部分和下滑动部分的十字形锚和/或外表面可为使器械在植入和移出期间保持假体的器件或可具有用于使器械在植入和移出期间保持假体的器件。
在本公开内容的又一个实施例中,具有上接合表面和下接合表面的中间滑动部分布置在上滑动部分的内侧与下滑动部分的内侧之间,其中中间滑动部分的上接合表面与上滑动部分的面对的内侧的接合表面接合,而中间滑动部分的下接合表面与下滑动部分的面对的内侧的接合表面接合,导致上接合区域和下接合区域,其中嵌体布置在上接合区域和/或下接合区域内,其中嵌体如上文指出那样构造。中间滑动部分上方和下方的接合区域可相同地构造或不同地构造。
发明的详细描述
由本发明提供的假体旨在用于经由腹部、侧腹部或侧部途径的主要的全椎间盘置换,其允许相邻脊柱之间的椎间盘空间内的相关的生理运动。假体还旨在在之前的保持功能的椎间盘植入之后的修正手术,通过插入全新的椎间盘假体或通过嵌体的替代或替换,如果需要,例如,在手术后,不向右侧向弯曲和/或向左侧向弯曲,或排除任何手术后的运动。
由于其位置在一个相邻滑动部分的凹口内,故保护了根据本公开内容的假体的嵌体免于脱臼。穿过嵌体的附加的突起保护相邻滑动部分免于脱臼。
根据本发明的假体基本上包括至少三个部分或构件,即,两个滑动部分和一个嵌体。在上滑动部分与下滑动部分之间的中间滑动部分的布置导致具有四个部分或五个部分的假体,即,三个滑动部分和一个或两个嵌体,这取决于在上接合区域和/或下接合区域内嵌体的布置。
就本发明的描述和描绘来说,"接合区域"包括相邻滑动部分的面对的接合表面,其与彼此接触或与彼此接合。因此,接合区域包括相邻滑动部分的至少两个接合表面。在本公开内容的意义内,假体的部分经由其凸形表面和凹形表面接合,包括滑动部分的凸形部分和凹形部分、嵌体的上侧和下侧,以及突起的尖端表面和凹口的底部表面或在底部表面之下的孔的表面。用语接合表面与用语滑动表面同义。
尽管它们可移动且与彼此接触或可滑动到彼此上,但凹口的侧表面、嵌体的外周和内周的侧表面,以及突起的侧表面在本公开内容的意义内不会接合。这些表面和它们之间的空间用于确定假体的运动范围,以便允许或防止围绕前后向轴线、前部轴线或纵轴线的旋转。例如,因此可排除围绕前部轴线的旋转(即,向右和向左的侧向弯曲),同时,在生理运动范围中,还允许作为伸展移动和屈曲移动的围绕前后向轴线的旋转和作为轴向旋转移动的围绕纵轴线的旋转。允许无限制的围绕纵轴线旋转的假体也在本公开内容的范围内。为了实现围绕纵轴线的没有限制的运动,至少凹口的侧表面的形状和嵌体的外周的形状或嵌体的内周的形状,以及突起的形状必须为圆形,其中所有相邻侧表面也可具有圆形形状。
二维表面接触指的是由于它们具有对应的形状,至少两个表面接触,使得不只是发生点接触或线接触。这意味着,只要围绕相应的轴线的旋转(包括以最大的可能旋转)将实现表面的二维接触,表面就不是必须为平面,而是也将具有弯曲的形式或平面和弯曲的组合。
三个空间轴线将限定为椎间盘空间内的伸展和屈曲功能的"前后向旋转轴线",其从前向后穿过身体,或反之亦然。围绕前后向轴线的旋转沿背部和腹部(背部-腹部)方向。围绕"前部旋转轴线"执行向椎间盘空间的右侧和左侧的弯曲功能,前部旋转轴线从身体的右侧向左侧,或反之亦然。"纵轴线"用于向右/向左轴向旋转,意思是围绕沿身体的头-尾方向延伸的竖直轴线旋转。该旋转也被称为"轴向旋转"。
就本发明的来说,由于切割平面从前向后竖直延伸,或反之亦然,故三个切割平面应由以下用语限定:"前后向截面"或"前后向视图"描述了来自侧向的视图。
用语"前部的"与"腹部的"和"前面的"同义,而用语"后部的"与"背部的"和"后面的"同义。"前部截面"或"前部视图"是从身体的右侧向侧到左侧向侧的垂直截面,或反之亦然。
用语"侧向的"代表向一侧的,而侧-侧向意思是从一侧到另一侧,从右到左,或反之亦然。由于前后向截面和前部截面它们两者都在从身体和椎间盘空间的头到尾(或反之亦然)的垂直平面中延伸,故为垂直截面,但彼此旋转90°。"横向平面"或"横向截面"中的视图示出了假体上的顶部视图,而椎体的端板的切割平面为水平截面。
可中心地、向右或向左的侧向地、背部地、腹部地、尾部地或头部地定位或移位切割截面或旋转轴线。附加的切割平面和旋转轴线可与彼此成角,且不必与彼此垂直。
用语"对应的",就接合的滑动表面或侧表面来说,不但指全等的凸面和凹面或其它面对的滑动表面,而且也指在彼此之间具有公差的凸形表面和凹形表面或其它形状的表面,因此指并不全等的接合和其它滑动表面。在一方面,所选择的材料和形状可引起关于接合或对应的接合且滑动的构件的其它面对的表面和滑动表面的此类"偏差"或公差。另一方面,还可打算的是,接合的表面和其它面对的表面以及滑动表面不是完全全等的,例如,以便直接地限定接合的滑动部分和假体的其它部分的最大可能的或有限的旋转或运动,或允许从接合的表面和侧表面中输送出磨蚀物。此类公差还可用于穿过该表面输送体液或其它材料。
由于三个旋转轴线的数目对应于三个接合方向的最小数目,故根据本发明的假体通过凹口的侧表面与嵌体的外周表面的相互作用和/或嵌体的内周表面与突起的侧表面的相互作用来提供限定(以允许或防止意义)围绕各个轴线的旋转可能性。两个相互作用都可用于限定围绕前后向轴线、前部轴线和纵轴线的旋转,但是为了限制围绕纵轴线的旋转,使用凹口的侧表面与嵌体的外周侧表面之间的相互作用更合适。因此,在考虑水平平面或水平区段中的组合平移或同时平移的情况下,根据本公开内容的假体的所提到的部分的形状或设计必须仅适于相对于背部-腹部旋转方向、侧-侧向旋转方向或向右/向左旋转方向以及头尾向旋转方向中的一个的生理状况。
按照本发明,边缘表示位于相应滑动部分的外边沿与接合区域之间的区域。边缘完全地或部分地包绕接合区域,即,凸面和凹面。相应滑动部分的边缘水平地延展和/或倾斜地延展,且具有平面表面或弯曲表面。对边缘的表面的形状重要的是,在滑动部分朝彼此的末端倾斜期间,实现了穿过滑动部分的边缘之间的最大可能的区域的间隙闭合。假若该边缘不具有平面表面,则在任何情况下,它们都将设计成使得在间隙闭合期间,在它们之间出现的最大可能的二维接触。
否则,边缘不必直接紧挨着滑动部分的接合表面开始。也在本公开内容的范围内的是,在接合表面与周围的边缘之间存在过渡区域或甚至间隙。
包绕至少两个面对的接合表面的边缘的接触也不应理解为在本公开内容的意义内的接合。此类接触,尽管其可能包括滑动且导致沿三个轴线中的至少一个的特定滑动部分的运动范围的限制,但不会出现或发生在根据本公开内容的接合区域内。
球形的球窝关节没有关于相对于关节的两个部分的竖直旋转或轴向旋转的限制,但此类关节为在两个相邻滑动部分的二维面对的表面的间隙闭合期间的压力的最佳吸收提供了较好的前提。本公开内容提供了用于没有围绕竖直轴线(即,纵轴线)的无限制的旋转的球窝关节的设计。嵌体布置到在两个滑动部分之间的凸面或凹面的凹口中不但容许轴向旋转的限制,而且容许前后向旋转和前部旋转的限制。
所公开的假体的设计利用了球形球窝形状的接合区域的优点,但不限于此类凸面和凹面的形状。根据本公开内容的假体的另一个优点在于,由于其位置分别在凸面或凹面的凹口内,故保护了嵌体免于脱臼。必须注意的是,突起不必为半球体,而是可通过在两个相对侧竖直地将其切割或通过沿一个轴线拉深半球体而源自半球体。
根据本公开内容的假体允许围绕以上限定的轴线中的至少两者的相关运动,其中运动程度将适于颈椎或腰椎的对应节段内的平均生理运动范围。假体的对应的限制元件的形状将特别考虑面关节及其最大运动范围的程度,以便防止面关节的非自然的磨蚀、退化和疾病。
为了实现已经提到的和打算的二维表面接触,包绕接合区域的边缘起着重要的作用。伸展、屈曲和向右侧向弯曲和向左侧向弯曲的限制还可通过相邻滑动部分的边缘的表面接触限制,甚至与接合区域的对应构件的形状(即,凸面、凹面、嵌体和突起的形状和尺寸)组合来限制。还打算的是,边缘可包括柔软的或柔性的材料,以实现此类功能。
按照本公开内容,椎间盘假体的另一个非限制性的优点在于,在某些实施例中,除其大概的运动角度(其接近于自然的运动程度)外,旋转由凹口的侧表面与嵌体的外周的接触区域和/或嵌体的内周的侧表面与突起的侧表面的接触区域来限定。
本发明提供了一种椎间盘假体,其通过从一组现有的部分或构件中选择具有适合的凹口的滑动部分、适合的嵌体和对应的滑动部分的匹配的突起,在分段的前提条件下改变。
还打算的是,滑动部分的接合表面和/或侧表面可牢固地但可逆地固定到滑动部分上。在修正手术的情况下,至少上滑动部分和下滑动部分可留在位置中,但可改变嵌体,以用于进一步的运动保持或限制功能,或如果需要,用于脊椎节段的融合。如果经由侧向途径或侧腹部途径执行修正手术,则可植入新的部分,以用于允许到背部和腹部方向的运动以及头尾向轴向旋转,但是不允许侧向弯曲,以避免在前部视图中成角的椎间盘空间。
根据本发明的假体还允许通过仅选择形状配合的构件来防止三个部分朝彼此的任何运动或旋转,以便在凹口与嵌体之间不存在空间,且嵌体的切口与突起的所有四个侧表面形状配合接触。通过简单地替换嵌体,可容易地实现任何运动的此类阻挡。
在侧向视图中上滑动部分和/或下滑动部分的外侧成角的情况下,根据本公开内容的假体适于颈椎或腰椎的前凸的均衡。在前部视图中上滑动部分和/或下滑动部分的外侧存在角度的情况下,根据本公开内容的假体适于侧凸的腰椎或颈椎节段的均衡。
还打算的是,旋转中心可沿背部位移高达3mm,以便使旋转中心适应生理状况,或以便补偿相应的椎间空间内的功能障碍。

具体实施方式
图1示出了用于颈椎的三构件椎间盘假体的不同视图。图1a示出了假体的分解透视图,图1b为分解侧视图,图1c示出了组装状态中的假体。图1d和图1e示出了如由图1c中的对应截面线指出的前后向截面视图和前部截面视图。假体包括三个不同的部分或构件:上滑动部分1、下滑动部分2,以及位于下滑动部分2的凹口7中的嵌体3。两个截面视图示出了怎样组装假体的三个构件。上滑动部分1具有突起4,其中其基点或起点在上滑动部分1的内侧的凹面5中(还见图2h和图2j)。突起4配合到嵌体的切口6中。嵌体3和突起4两者都位于凹口7中,凹口7定位在下滑动部分2的内侧的凸面8内。

图1e中的前部截面视图示出了类似的组件。然而,在该实施例中,间隙15,16位于嵌体3的外周的侧表面与凹口7的相应面对的侧表面17之间。因此,还存在围绕上滑动部分1与下滑动部分2之间的前后向轴线的有限旋转移动的可能性,其对应于植入假体的脊椎单元处的侧向弯曲。
上述旋转移动由假体的实施例来限制。显然,一旦a)突起4和嵌体3与彼此接触,b)嵌体3与凹口7的侧表面17接触,或c)上滑动部分1的边缘18与下滑动部分2的边缘19接触彼此,则不同的旋转移动就停止。

如可从图2b、图2c、图2e、图2f、图2g至图2j中以及图1d和图1e中看出的那样,嵌体3的上侧11、下滑动部分2的内侧的凸面8,以及上滑动部分1的内侧的凹面5具有相同的曲率半径。实际上,前面提到的所有三个表面都位于如由图1d和图1e中的虚线圈所指出的相同球体S2上。同样,嵌体3的下侧12、凹口7的底部14,以及突起的尖端13具有相同的曲率半径。所有的三个表面都位于如由图1d和图1e中的虚线圈所指出的相同球体S1上。显然,S1具有比S2小的半径,但两个球体都具有相同的中心点。

图3a至图3d示出了具有最大伸展的假体。清楚地可见的是,由于若干表面与彼此接触,故防止了进一步伸展:突起4的后侧接触嵌体的切口6的对应表面(因此闭合了后面的间隙10且加宽了前面的间隙9),且上滑动部分1的边缘18与下滑动部分2的边缘19彼此接触。
图4a至图4d示出了具有最大屈曲的假体。又由突起4的侧表面与嵌体3的内周的对应表面接触(因此闭合了前面的间隙9且加宽了后面的间隙10)和上滑动部分1的边缘18与下滑动部分2的边缘19接触彼此的事实防止了进一步屈曲。
图5a至图5d示出了处于最大的向右侧弯曲状态的假体。由于嵌体的周向的右侧表面接触凹口的右侧(因此闭合了右侧间隙15且加宽了左侧间隙16),且由于上滑动部分1的边缘18与下滑动部分2的边缘19接触彼此,故抑制了进一步弯曲。
总之,在伸展和屈曲期间,上滑动部分1围绕嵌体3和下滑动部分2两者旋转(比较图3d和图4d)。相反,在向右侧向弯曲和向左侧向弯曲期间,上滑动部分1和嵌体3两者围绕下滑动部分2旋转(比较图5d)。
图6a至图6e示出了处于最大的向右轴向或纵向旋转状态的假体。由嵌体3的周向的后侧接触凹口7的后侧和嵌体3的前侧接触凹口7的前侧的事实抑制了进一步轴向旋转(比较图6b、图6c和图6e)。在轴向旋转期间,上滑动部分1和嵌体3两者围绕纵轴线旋转。像用于伸展和屈曲的旋转中心那样,轴向旋转中心也沿背部放置,离生理学位置较近(比较图6e)。
图7示意性地示出(以二维)了假体的基本构造。存在对应于下滑动部分2的外部部分24。外部部分24包括(在对应于凹口7的开口内)对应于嵌体3的中间部分25。最后,中间部分25包括对应于突起4的内部部分26。前面的间隙9和后面的间隙10,以及侧向间隙15和侧向间隙16对应于前述附图中所示的间隙。如果外部部分24在空间中是固定的,则中间部分25和内部部分26两者仅可在某个有限范围内平移。内部部分26的向上的平移对应于假体的伸展,而向下的平移对应于屈曲。中间部分25和内部部分26两者的侧向平移对应于假体的向右侧向弯曲和向左侧向弯曲。如可在图7a至图7i中看出的那样,不同平移的所有组合都是可能的。然而,与之前描述的假体相反,轴向旋转不可能。
图8示出了与图7相同的构造原理。然而,所有部分都已经旋转了90°。现在,内部部分26的侧向平移将对应于侧向弯曲,而内部部分26和中间部分25的向上平移和向下平移一起将对应于伸展和屈曲。
最后,图9示出了之前已经描述的假体的准确的二维方案。现在,中间部分25具有成角的侧表面,允许中间部分25在外部部分24内的有限轴向旋转。四个角20-23对应于图21中示出的嵌体3的角。显然,由于不存在用于内部部分26单独地旋转的空间,故中间部分25和内部部分26仅可一起旋转。此外,中间部分25仅可在外部部分24内水平地平移,而内部部分26仅可在中间部分25内竖直地平移。因此,内部部分26和中间部分25的水平平移和竖直平移以及轴向旋转的量全部可单独地被限定和被限制。因此,还可单独地限定和限制由假体所允许的伸展、屈曲、侧向弯曲和轴向旋转的量。图9的八个最底部图片示出了内部部分26和中间部分25的组合的平移和旋转的各种可能性。如可看出的那样,向上平移和向下平移独立于侧向平移,且两种类型的平移都独立于轴向旋转。
图10示出了允许独立地限定平移和旋转的量的另一个方案。然而,这里内部部分26在中间部分25内既可平移也可旋转。
图11示出了又一个类似的可能性。这次,允许内部部分26和中间部分25两者旋转。
图12示出了不再限制围绕纵轴线的轴向旋转的假体。在该假体中,以前粗略地定形成像角锥的截头锥体的突起现在定形成像圆锥的截头锥体27。此外,在嵌体中的切口28已经适用于接受突起27的新形状。
在图13中,绘出了类似的构造。这里,圆锥形嵌体29的外周定形成像圆锥的截头锥体,且因此改变了下滑动部分2中的凹口30的形状。如前文所述,该构造不会限制轴向旋转。
图14示出了一个假体,其很类似于图1至图6中示出的假体。主要差异在于在图14中突起的长度已经最大化。伸展的突起31的尖端现在位于在最初的凹口7中的单独的凹口(孔)32内,增加了上滑动部分1和下滑动部分2的总的竖直重叠,使假体甚至更加安全以防脱臼。
图15示出了其中已经交换了突起4和凹口7的位置的假体。这里,凹口33置于上滑动部分1的凹面5中,且突起34是下滑动部分2的凸面8的一部分。然而,凹口33、突起34和嵌体3的功能保持相同,且该假体还基于在图9及以下附图中示出的构造方案。
图16示出了包括五部分的用于腰椎的假体:上滑动部分35和下滑动部分36(分别具有突起4)、两个相同的嵌体3,以及具有两个相同的凹口7的单个中间滑动部分37,一个凹口在其上侧,而一个凹口在其下侧。如果该假体将由位于中间滑动部分37的中心的水平面来分成两半,则基本上将得到两个相同的假体,两个假体将看上去很类似于在图1至图6中示出的假体。因此,上滑动部分35和下滑动部分36对应于前述假体的上滑动部分1和下滑动部分2,而中间滑动部分37的上表面和下表面对应于前述假体的上滑动件1和下滑动部分2的内侧。在该假体中的显著差异为中间滑动部分37的边缘38的形状,其具有燕尾形状(如可在两个截面视图中看到的那样),即,其竖直厚度沿周向连续地增加。因此形成上滑动部分35和下滑动部分36的边缘,即,其厚度沿周向减小。
图17至图20示出了在滑动部分与相邻脊柱之间提供良好固定的不同器件。在图17中示出了十字形工艺。根据椎体的通常成凹形的端板,侧-侧向横臂39和前-后横臂40两者定形成类似于弧,即,它们在其中间比在其端部更高。侧-侧向横臂39略微比前后臂40更高,以具有穿过脊柱端板的骨头的逐步切割,以用于进行更容易和更安全的固定。相比于假体的侧-侧向脱位,更重要的是避免假体的前后脱臼,所以从右到左的横臂略微更高。
图18示出了类似的构造。侧-侧向臂39仍略微高于前后臂40。此外,现在侧-侧向臂39中存在两个孔41,以用于在假体植入期间收纳器械。所述用于接受器械的器件还便于滑动部分的移出和准确定位。
图19示出了固定器件,该器件包括十字形工艺和四个钉42。最后,图20示出了具有六个钉42的滑动部分的外面,其中在中心的两个钉42略微地比在外周中的钉42长,以补偿椎体的端板的凹形。
图21示出了两个假体端板的八角形状。较小的形状43旨在用于在颈椎植入物中使用,而较大的形状44用于在腰椎植入物中使用。在附图中指出了四个相关的解剖方向。假体端板的形状设计成既用以在假体端板与相邻脊柱端板之间产生较大的接触区域,也在几何上很简单。部分平面侧部便于经由前面途径、前面-侧向途径(仅腰椎)和侧向途径(仅腰椎)植入的器械的固定。

Claims (21)

1.一种用于颈椎或脊椎内的椎间盘全置换的椎间盘假体,所述假体包括上滑动部分和相邻的下滑动部分,所述上滑动部分适于以其外表面与上椎体牢固地组装在一起,所述下滑动部分适于以其外表面与下椎体牢固地组装在一起,且所述相邻的上和下滑动部分经由接合表面接合在其面对的内侧上,包括在一个滑动部分上具有凸面和在另一个滑动部分上具有凹面的接合区域,其中,
a.所述相邻的上和下滑动部分中的第一个具有凹口,所述凹口具有底部表面和四个侧表面,其中所述凹口定位在所述相邻的上和下滑动部分中的第一个的凸面或凹面内,以及
b.与所述相邻的上和下滑动部分中的第一个接合的所述相邻的上和下滑动部分中的第二个具有突起,所述突起具有四个侧表面和尖端表面,其中所述突起布置在所述相邻的上和下滑动部分中的第二个的凹面或凸面内,以及
c.嵌体包括用于收纳所述突起的切口,其中所述嵌体定位在所述相邻的上和下滑动部分中的第一个的凹口内,以及
d.所述嵌体具有
i.包绕所述切口的上凸形接合表面或上凹形接合表面,所述上凸形接合表面或所述上凹形接合表面具有与所述相邻的上和下滑动部分中的第一个的凸形接合表面或凹形接合表面相同的曲率半径,以及
ii.包绕所述切口的下凹形接合表面或下凸形接合表面,所述下凹形接合表面或所述下凸形接合表面具有与所述相邻的上和下滑动部分中的第一个的凹口底部的凹形接合表面或凸形接合表面相同的曲率半径,以及
iii.四个外侧表面,其包括面对所述相邻的上和下滑动部分中的第一个的凹口的四个侧表面的所述嵌体的外周,以及
iv.四个内侧表面,其包括面对所述相邻的上和下滑动部分中的第二个的突起的四个侧表面的所述嵌体的内周,其中
e.相对于彼此的围绕前后向轴线、前部轴线和纵轴线在所述相邻的上和下滑动部分中的第一个与第二个之间的运动范围由以下限定:
i.所述凸面和所述凹面的曲率半径,
ii.所述凹口的尺寸和形状、包括其切口的所述嵌体的尺寸和形状,以及所述突起的尺寸和形状,以及
iii.在所述凹口的侧表面与所述嵌体的外周之间的空间的尺寸和/或在所述嵌体的内周与所述突起的侧表面之间的空间的尺寸。
2.根据权利要求1所述的椎间盘假体,其特征在于,至少一个接合表面部分地或完全地由边缘包绕,以使在所述前后向轴线和所述前部轴线中的各个方向上的最终运动范围处的运动限制接触区域最大化。
3.根据权利要求2所述的椎间盘假体,其特征在于,包绕至少一个接合表面的边缘由柔性材料制成,允许在所述前后向轴线和前部轴线中的各个方向上的最终运动范围处的柔软限制。
4.根据权利要求1-3中任一项所述的椎间盘假体,其特征在于,所述凹口的形状、所述嵌体的外周和内周的形状,以及所述突起的形状为矩形、圆形、圆柱形、角锥、包括截头角锥和截头圆锥的圆锥,或前面所提到的所述形状的组合。
5.根据权利要求1-3中任一项所述的椎间盘假体,其特征在于,所述凹口的侧表面的形状、包括所述嵌体的外周和内周的侧表面的形状,以及所述突起的侧表面的形状为平的、弯曲的、成角的,或圆形的,或它们的组合。
6.根据权利要求1-3中任一项所述的椎间盘假体,其特征在于,凸面和对应的凹面具有球形、圆柱形、环面状、螺旋形和/或圆锥形的形状或它们的组合,且凸面和对应的凹面具有相同的曲率半径或不同的曲率半径。
7.根据权利要求1-3中任一项所述的椎间盘假体,其特征在于,所述凹口的尺寸和形状和所述嵌体的外周的尺寸和形状被构造成使得相邻的上和下滑动部分有可能围绕所述前后向轴线或所述前部轴线旋转。
8.根据权利要求7所述的椎间盘假体,其特征在于,所述凹口的尺寸和状态和所述嵌体的外周的尺寸和形状被构造成使得有可能附加地限制围绕所述纵轴线的旋转。
9.根据权利要求1-3、8中任一项所述的椎间盘假体,其特征在于,所述嵌体的内周的尺寸和形状和所述突起的尺寸和形状被构造成使得相邻的上和下滑动部分有可能围绕所述前后向轴线或所述前部轴线旋转。
10.根据权利要求9所述的椎间盘假体,其特征在于,所述嵌体的内周的尺寸和形状和所述突起的尺寸和形状被构造成使得有可能附加地限制围绕所述纵轴线的旋转。
11.根据权利要求1-3、8、10中任一项所述的椎间盘假体,其特征在于,所述凹口和所述嵌体的外周的形状和/或所述嵌体的内周和所述突起的形状允许围绕所述纵轴线的无限制的旋转。
12.根据权利要求1-3、8、10中任一项所述的椎间盘假体,其特征在于,所述突起的两个相对的侧表面以形状配合的方式与所述嵌体的内周的两个相对的侧表面接触,和/或所述嵌体的外周的两个相对的侧表面以形状配合的方式与所述凹口的两个相对的侧表面接触。
13.根据权利要求1-3、8、10中任一项所述的椎间盘假体,其特征在于,所述突起的尖端表面为凸形或凹形,其曲率半径对应于所述相邻的上和下滑动部分中的第一个的凹口的底部表面的凸面曲率或凹面曲率,使得尖端表面和底部表面能够接合。
14.根据权利要求1-3、8、10中任一项所述的椎间盘假体,其特征在于,所述突起的尖端表面与布置在所述凹口的底部表面下方的孔的表面接合,且所述突起的尖端表面的曲率半径对应于所述孔的表面的面对的曲率半径。
15.根据权利要求1-3、8、10中任一项所述的椎间盘假体,其特征在于,各个滑动部分和/或所述嵌体均包括相同的材料或不同的材料,或涂布有相同的材料或不同的材料。
16.根据权利要求1-3、8、10中任一项所述的椎间盘假体,其特征在于,各个滑动部分和/或所述嵌体构造成一件或由至少两件牢固地但可逆地组装。
17.根据权利要求1-3、8、10中任一项所述的椎间盘假体,其特征在于,所述嵌体和/或所述上和下滑动部分中的至少一个或其一部分由柔性材料制成,以阻尼椎间的冲击或施加的负载。
18.根据权利要求1-3、8、10中任一项所述的椎间盘假体,其特征在于,所述上滑动部分和所述下滑动部分在其用于与椎体组装在一起的外表面上具有至少一个十字形的锚,所述锚可选为与锚定齿组合。
19.根据权利要求1-3、8、10中任一项所述的椎间盘假体,其特征在于,用于与椎体组装在一起的所述上滑动部分和/或所述下滑动部分的外表面为用于在植入和移出期间保持所述假体的器械的器件,或具有用于在植入和移出期间保持所述假体的器械的器件。
20.根据权利要求1-3、8、10中任一项所述的椎间盘假体,其特征在于,具有上接合表面和下接合表面的中间滑动部分布置在所述上滑动部分的内侧与所述下滑动部分的内侧之间,其中所述中间滑动部分的上接合表面与所述上滑动部分的面对的内侧的接合表面接合,而所述中间滑动部分的下接合表面与所述下滑动部分的面对的内侧的接合表面接合,产生上接合区域和下接合区域,其中嵌体布置在上接合区域和/或下接合区域内,且与嵌体的接合区域如根据前述权利要求的任一项指定那样构造。
21.根据权利要求20所述的椎间盘假体,其特征在于,所述中间滑动部分上方和下方的所述接合区域相同地构造或不同地构造。
