Abstract

目前的乙状结肠和直肠部分切除手术中,经肛门插入圆形吻合器完成结直肠吻合是常规方法,吻合前需要充分扩张润滑吻合口远端的乙状结肠和直肠肠腔,圆形吻合器才能顺利插入,当吻合部位超出肛门以上15cm至乙状结肠段时,由于肠腔内涩滞,放置规格较长的圆形吻合器比较困难,且放置过程中容易导致增加手术并发症风险的因素,故需要设计专用器械以实现乙状结肠和直肠的充分扩张和润滑,本发明提供一种乙状结肠扩张棒,可将充足的润滑剂带入乙状结肠和直肠肠腔,并充分扩张肠腔,以解决以上临床问题。

乙状结肠扩张棒
技术领域
本发明涉及外科手术器械,是为直肠上段至乙状结肠的部分切除手术设计的扩张润滑器械,以利于吻合器吻合顺利进行。
背景技术
目前的乙状结肠和直肠部分切除手术中,经肛门插入圆形吻合器完成结直肠吻合是常规方法,吻合前需要充分扩张润滑吻合口远端的乙状结肠和直肠肠腔,圆形吻合器才能顺利插入,对于距肛门15cm以内的直肠进行扩张润滑较易作到,而当吻合部位超出肛门以上15cm至乙状结肠段时,由于肠道粘膜的涩滞,放置规格较长的圆形吻合器比较困难,且放置过程中反复调整容易导致肠粘膜或肠壁损伤,放置部位不理想等问题,增加手术并发症风险,故需要设计专用器械以实现乙状结肠和直肠的充分扩张和润滑。
发明内容
本发明提供一种乙状结肠扩张棒,用于对乙状结肠和直肠肠腔进行充分扩张润滑,以使圆形吻合器能顺利插入,易化手术操作,降低手术并发症风险。
本发明的技术方案为:乙状结肠扩张棒为一根呈钝角弯曲的金属棒,其横截面为圆形,直径15mm,总长度35cm,中部弯曲角度约150°,钝角弯曲有利于扩张棒适应直肠的解剖弯曲而进入结直肠腔,其一端为长径55mm、最大直径35mm的椭球形膨大体,膨大体表面中部均匀分布4条“S”形弯曲浅槽,沿长径走行,用于携带润滑剂进入肠管,并作为气体通道导出肠腔内积存气体,弯曲形状可增加浅槽容积,浅槽横截面底边为圆弧形,浅槽长度40mm,宽度5mm,最大深度2mm,浅圆弧底槽腔易于清洗消毒,扩张润滑器另一端为长度7cm的圆柱体手柄,直径20mm,金属棒及手柄部位标有长度刻度,以便掌握进入肠管深度,乙状结肠扩张棒的具体实施方式如下:将润滑剂注入扩张棒头端的浅槽内,再将扩张棒经肛门渐次插入直肠至乙状结肠,过程可反复进行,即可将足够的润滑剂带入肠腔,并充分扩张肛门和结直肠腔,同时通过浅槽排出肠腔内蓄积的气体,根据扩张棒表面刻度可掌握进入肠管的深度,经充分润滑扩张后的乙状结肠和直肠肠腔易于置入圆形吻合器,完成吻合。

具体实施方式
将润滑剂注入乙状结肠扩张棒头端的浅槽内(图1,图2),再将扩张棒经肛门渐次插入直肠至乙状结肠(图4),过程可反复进行,即可将足够的润滑剂带入肠腔,并充分扩张肛门和结直肠腔,同时通过浅槽排出肠腔内蓄积气体,根据扩张棒表面刻度(图1)可掌握进入肠管的深度,经充分润滑扩张后的结直肠腔易于置入圆形吻合器完成吻合。
图4所示的实施例中,乙状结肠扩张棒置入结直肠,带入润滑剂润滑肠腔,充分扩张肠腔,并经浅槽排出肠腔内积存气体,以利于顺利置入吻合器,根据扩张棒表面刻度可掌握置入深度。

1.一种乙状结肠扩张棒,其特征是:一根呈钝角弯曲的金属棒,其横截面为圆形,一端为椭球形膨大体,膨大体表面分布数条弯曲浅槽,浅槽横截面底边为圆弧形,另一端为圆柱体手柄,扩张棒表面标有长度刻度。
2.根据权利要求1所述的乙状结肠扩张棒,其特征是:一根呈钝角弯曲的金属棒,横截面为圆形。
3.根据权利要求1所述的乙状结肠扩张棒,其特征是:一端为椭球形膨大体。
4.根据权利要求1所述的乙状结肠扩张棒,其特征是:膨大体表面分布数条弯曲浅槽。
5.根据权利要求1所述的乙状结肠扩张棒,其特征是:浅槽横截面底边为圆弧形。
6.根据权利要求1所述的乙状结肠扩张棒,其特征是:一端为圆柱体手柄。
7.根据权利要求1所述的乙状结肠扩张棒,其特征是:扩张棒表面标有长度刻度。
