Einleitung
[0001] Für medizinische Untersuchungen, Eingriffe und Analysen werden oft Endoskope eingesetzt. Um mit möglichst kleinen Eingriffen für den Patienten das Endoskop zur Untersuchungs- oder Operationsstelle zu bringen, wird versucht, ein Endoskop mit möglichst kleinem Durchmesser einzusetzen. Andererseits besteht die Bestrebung, die Ortsauflösung der endoskopischen Kamera zu erhöhen. Ausserdem ist bei den meisten endoskopischen Anwendungen nicht nur ein Endoskop zur Visualisierung zur Operations-/Untersuchungsstelle zu bringen, sondern auch medizinisches Gerät, z.B. mikromechanische Schneidwerkzeuge. Bei herkömmlichen Endoskopen wird diesem Anspruch so Rechnung getragen, dass der im Endoskop zur Verfügung stehende Durchmesser zwischen dem optischen Kanal und einem Arbeitskanal der zum Einführen von medizinischem Gerät aufgeteilt wird.
Dadurch wird aber die mögliche Ortsauflösung der endoskopischen Kamera reduziert.
[0002] Ziel der offengelegten Erfindung ist es, eine Lösung vorzustellen, die es ermöglicht, den gesamten Endoskopdurchmesser für die endoskopische Kamera zu verwenden und trotzdem mechanisches Werkzeug über einen Arbeitskanal zur Operationsstelle einzuführen.
Stand der Technik
[0003] Heutzutage werden Endoskope typischerweise so aufgebaut, dass der zur Verfügung stehende Durchmesser des Endoskopes in einen Arbeitskanal (3) und einen optischen Kanal (1) aufgeteilt wird. Siehe auch Abbildung 1. Ein weiterer Teil des zur Verfügung stehenden Durchmessers wird für die Beleuchtung verwendet (2). In früheren Endoskopen wurde der optische Kanal sowie die Beleuchtung meist mittels Glasfaserbündeln realisiert. In neueren Endoskopen wird teilweise die Beleuchtung mittels LEDs und/oder der optische Kanal mittels einer elektronischen Kamera realisiert, wobei das Bildsignal mittels elektrischer Signalübertragung zum Endoskopende und Visualisierungsgerät übertragen wird.
Beschreibung der Erfindung
[0004] Die in dieser Beschreibung aufgeführten oder in den Abbildungen dargelegten Realisierungsmöglichkeiten der offengelegten Erfindung sind einzig als erläuternde Beispiele zu verstehen. Die Beispiele schränken nicht die Generalität der Erfindung ein. Die Ansprüche zum Schutz der offengelegten Erfindung sind einzig in den entsprechenden Patentansprüchen dargelegt.
[0005] Die offengelegte Erfindung bezieht sich auf Endoskope, bei welchen der optische Kanal mittels einer elektronischen Kamera realisiert wird (Chip in Tip Endoskope). Siehe auch Abbildung 2. Der maximale Durchmesser des Endoskopes richtet sich nach der engsten Stelle, durch welche das Endoskop zum Untersuchungs- oder Operationsort eingeführt werden muss. Oft steht am Operationsort allerdings mehr Platz zur Verfügung, oder kann durch elastisches Gewebe mehr Platz geschaffen werden, als der maximal mögliche Durchmesser des Endoskopes auf Grund der Zuführung erlauben würde.
[0006] Im erfindungsgemässen Endoskop wird der optische Kanal mittels einer elektronischen Kamera realisiert, und die Bilddaten werden mittels bekannter Signalübertragungstechnik zum Visualisierungsgerät ausserhalb der Operationsstelle übertragen. Das erfindungsgemässe Endoskop ist so ausgelegt, dass zum Einführen des Endoskopes bis an die Operations-/Untersuchungsstelle die Kamera vollständig im Arbeitskanal liegt. Dadurch steht für die Kamera annähernd der gesamte Endoskopdurchmesser zur Verfügung, was die Verwendung eine Kamera mit höherem Durchmesser und dadurch höherer Ortsauflösung oder Lichtstärke ermöglicht Gleichzeitig kann der Arbeitskanal vergrössert werden, was die Verwendung von grösseren Werkzeugen oder das Einsammeln von grösseren Proben ermöglicht.
Alternativ kann auch der Gesamtdurchmesser des Endoskopes bei gleichbleibender Kamera- und Arbeitskanalgrösse verkleinert werden, wodurch das Endoskop leichter eingeführt und durch kleinere Zugangsöffnungen verwendet werden kann.
[0007] Das erfindungsgemässe Endoskop bietet die Möglichkeit, am Operations-/Untersuchungsort die Kamera aus dem Arbeitskanal zu entfernen und lateral zum Arbeitskanal verschoben anzubringen, so dass über den Arbeitskanal andere Werkzeuge oder Proben zur Operations-/Untersuchungsstelle zu- oder weggeführt werden können. Dabei kann die Kamera sowohl längs aus dem Arbeitskanal ausgeführt und seitlich angebracht werden oder lateral neben den Arbeitskanal verschoben werden. Siehe Abbildung 3 und 4 resp.
[0008] Abbildung 3a zeigt eine Variation des erfindungsgemässen Endoskops mit der Kamera (4) in der Einführungsposition, komplett im Arbeitskanal (3) platziert. In Abbildung 3bist die Position der Kamera am Untersuchungs-/Operationsort gezeigt. Die Kamera wird längs aus dem Arbeitskanal nach vorne ausgefahren und auf eine leicht seitliche Position gebracht. Dadurch ist der Arbeitskanal (3) frei für die Einführung von Werkzeugen. Im Falle der Verwendung von LED-Beleuchtung am Tip kann diese unter Umständen zusammen mit der Kamera aus dem Arbeitskanal verschoben werden. Für den Fall der Verwendung von fiberoptischer Beleuchtung kann diese, wie in Abbildung 3c ersichtlich, seitlich des Arbeitskanals oder in Form eines Rings, den Arbeitskanal komplett umfassend, angebracht werden.
[0009] Abbildung 4a zeigt die Position der Kamera im Arbeitskanal zum Einführen des Endoskopes. Abbildung 4b zeigt die seitlich ausgefahrene Kamera (4) in Arbeitsposition. Der Arbeitskanal (3) ist frei für das Einführen von Werkzeugen und Proben. Abbildung 3czeigt die mögliche Platzierung von fiberoptischer Beleuchtung für diese Realisierungsmöglichkeit.
[0010] Eine beispielhafte Realisierungsmöglichkeit der seitlichen Verschiebung der Kamera ist in Abbildung 5gezeigt. Mittels einer Feder (6) wird die Kamera (4) in ihrer Position im Arbeitskanal gehalten. Sowohl die Kamera wie deren Halterung im Arbeitskanal weisen eine abgeschrägte Führungseinrichtung auf. Um den Arbeitskanal für das Vortreiben von Werkzeugen oder das Entnehmen von Proben freizumachen, wird die Kamera mittels eines Zugkabels (5) über die abgeschrägte Führungseinrichtung seitlich aus dem Arbeitskanal verschoben. Um die Anzahl von Kabeln im Endoskop zu reduzieren, kann optional das Zugkabel (5) durch eine elektrische Signalleitung oder eine Versorgungsleitung der Kamera (4) oder durch einen Lichtwellenleiter zur Beleuchtung ausgebildet werden.
[0011] Optional kann der Datenstrom der Kamera von einer automatischen Bildverarbeitungssoftware analysiert werden, die anhand der Bilddaten feststellt, wenn das Endoskop vom Operations-/Untersuchungsort zurückgezogen wird, so dass auf einem geeigneten Ausgabegerät eine Warnung generiert werden kann, falls sich beim Zurückziehen des Endoskops die Kamera noch in der ausgelenkten Position befindet.

For medical examinations, procedures and analyzes often endoscopes are used. In order to bring the endoscope to the examination or surgical site with as little intervention as possible for the patient, an attempt is made to use an endoscope with the smallest possible diameter. On the other hand, there is the desire to increase the spatial resolution of the endoscopic camera. In conventional endoscopes, this claim is taken into account so that the available diameter in the endoscope between the optical channel and a working channel for the introduction of medical device is divided.
However, this reduces the possible spatial resolution of the endoscopic camera.
The aim of the disclosed invention is to present a solution that makes it possible to use the entire endoscope diameter for the endoscopic camera while still introducing mechanical tool via a working channel to the surgical site.
State of the art
Nowadays, endoscopes are typically constructed so that the available diameter of the endoscope is divided into a working channel (3) and an optical channel (1). See also Figure 1. Another part of the available diameter is used for lighting (2). In earlier endoscopes, the optical channel and the illumination were mostly realized by means of glass fiber bundles. In newer endoscopes, the illumination is partially realized by means of LEDs and / or the optical channel by means of an electronic camera, wherein the image signal is transmitted by means of electrical signal transmission to the endoscope end and visualization device.
Description of the invention
The implementation possibilities of the disclosed invention set forth in this description or illustrated in the figures are to be understood as illustrative examples only. The examples do not limit the generality of the invention. The claims for the protection of the disclosed invention are set forth solely in the corresponding claims.
The disclosed invention relates to endoscopes in which the optical channel is realized by means of an electronic camera (chip in tip endoscopes). See also Figure 2. The maximum diameter of the endoscope depends on the narrowest point through which the endoscope must be inserted to the examination or surgical site. Often, however, more space is available at the surgical site, or more space can be created by elastic tissue than would allow the maximum possible diameter of the endoscope due to the feed.
In the inventive endoscope, the optical channel is realized by means of an electronic camera, and the image data are transmitted by known signal transmission technology to the visualization device outside the surgical site. The endoscope according to the invention is designed so that the camera is completely located in the working channel for insertion of the endoscope up to the surgical / examination site. As a result, almost the entire endoscope diameter is available for the camera, which allows the use of a camera with a larger diameter and thus higher spatial resolution or light intensity. At the same time, the working channel can be enlarged, which allows the use of larger tools or the collection of larger samples.
Alternatively, the total diameter of the endoscope can also be reduced while the camera and working channel size remains the same, which makes it easier for the endoscope to be inserted and used through smaller access openings.
The inventive endoscope offers the ability to remove the operation / examination site, the camera from the working channel and laterally mounted to move to the working channel, so that on the working channel other tools or samples to the operation / examination center or can be led away. In this case, the camera can be performed both longitudinally from the working channel and attached laterally or laterally displaced next to the working channel. See Figures 3 and 4 resp.
Figure 3a shows a variation of the inventive endoscope with the camera (4) in the insertion position, completely placed in the working channel (3). Figure 3b shows the position of the camera at the examination / surgical site. The camera is extended lengthwise out of the working channel and brought to a slightly lateral position. As a result, the working channel (3) is free for the introduction of tools. In the case of using LED lighting on the tip, it may be possible to move it out of the working channel together with the camera. In the case of the use of fiber optic illumination, this can, as shown in Figure 3c, the side of the working channel or in the form of a ring, the working channel completely comprehensive, are attached.
Figure 4a shows the position of the camera in the working channel for insertion of the endoscope. Figure 4b shows the laterally extended camera (4) in working position. The working channel (3) is free for the insertion of tools and samples. Figure 3c shows the possible placement of fiber optic illumination for this realization option.
An exemplary realization of the lateral displacement of the camera is shown in Figure 5. By means of a spring (6), the camera (4) is held in position in the working channel. Both the camera and its holder in the working channel have a beveled guide device. To free the working channel for propelling tools or removing samples, the camera is laterally displaced out of the working channel by means of a pulling cable (5) via the slanted guide means. In order to reduce the number of cables in the endoscope, optionally the pull cable (5) can be formed by an electrical signal line or a supply line of the camera (4) or by an optical waveguide for illumination.
Optionally, the data stream of the camera can be analyzed by an automatic image processing software, which determines from the image data when the endoscope is withdrawn from the operation / examination site, so that on a suitable output device, a warning can be generated if the withdrawal of the Endoscope the camera is still in the deflected position.
