Technisches Gebiet
Die vorliegende Erfindung betrifft durch Umwelteinflüsse wie die Luftverschmutzung (Pollution), das Rauchen oder andere Noxen verursachte krankhafte Veränderungen der Nasenschleimhaut im Sinne von Zell-Hyper-, -Meta- oder -Dysplasien.
Stand der Technik
Krankhafte Zellveränderungen der Bronchialschleimhaut, die auf die genannten Einflüsse zurückzuführen sind, sind seit längerem bekannt. Erst jetzt wurde dagegen eine Studie veröffentlicht (Glück U. & Gebbers J.-O.: Cytopathology of the Nasal Mucosa in Smokers: A Possible Biomarker of Air Pollution? American Journal of Rhinology, Vol. 10, No. 1, 1996), die entsprechende Veränderungen auch an der Nasenschleimhaut nachweisen konnte. Zytopathologisch untersucht wurde hierbei die Nasenschleimhaut von 60 Zigarettenrauchern und ebenso vielen Nichtrauchern. Während bei den Nichtrauchern der Befund in 46 Fällen normal war und in 14 Fällen höchstens eine Plattenepitehlmetaplasie aufzeigen konnte, präsentierten die Raucher in 52 Fällen Epitheldysplasien unter schiedlichen Grades. Nur bei 8 Rauchern war lediglich eine Metaplasie nachweisbar.
In der Studie wird ausgeführt, dass der zytopathologischen Untersuchung der Nasenschleimhaut mittels Bürstenzytologie in der HNO-Heilkunde bisher wenig Beachtung geschenkt wurde, bedingt teils durch mangelnde Erfahrung, teils durch das Vorherrschen anderer Fragestellungen. Insbesondere zur Erforschung der Entzündungsvorgänge sowie der immunologischen Reaktionen an der Nasenschleimhaut stünden dem Arzt andere Methoden zur Verfügung, welche dafür zuverlässigere Informationen lieferten.
Die Studie nennt keinerlei Mittel zur Behandlung der erwähnten krankhaften Zellveränderungen der Nasenschleimhaut. Derartige Mittel scheinen bis anhin auch noch nicht bekannt zu sein. Die Studie setzt sich lediglich zum weiteren Ziel herauszufinden, ob die Nasenschleimhautzytologie als einfache und reproduzierbare Untersuchungsmodalität als Screening-Vorsorge-Untersuchung eingesetzt werden kann.
Capsaicin, der scharfe Bestandteil der roten Pfefferschote und anderer Nachtschattengewächse, weist günstige therapeutische Wirkungen für die unterschiedlichsten Krankheitsbilder auf, darunter die vasomotorische bzw. hyperreflektorische Rhinopathie. Die chemische Bezeichnung von natürlichem Capsaicin lautet 8-Methyl-N-Vanillyl-6-Nonenamid, die von synthetischem Capsaicin N-Vanillyl-nonanamid (CAS-Nr.: 2444-46-6).
Zur Rhinopathiebehandlung wurde das Capsaicin bisher zumeist in hochkonzentrierter Form auf die Nasenschleimhaut appliziert. Wegen des durch den Wirkstoff verursachten starken Brennens war dies nur nach vorgängiger Lokalanästhesie möglich und musste durch den Arzt durchgeführt werden.
Aus der EP-A1 646 372 ist vor kurzem jedoch auch ein Capsaicin-Nasenspray bekannt geworden, der den Wirkstoff (synthetisches Capsaicin) in neutraler wässriger Lösung in einer vergleichsweise sehr geringen Konzentration enthält. Dieser Nasenspray kann durch den Patienten selbst mehrfach täglich problemlos angewendet werden.
Darstellung der Erfindung
Im Zuge der Markteinführung des vorgenannten Capsaicin-Nasensprays wurde dieser im Rahmen einer klinischen Studie auch daraufhin überprüft, ob eine längerfristige Anwendung über einen Zeitraum von mehreren Wochen eventuell toxische Auswirkungen auf die Nasenschleimhaut haben könnte. Derartige negative Auswirkungen konnten nicht festgestellt werden.
Die im Rahmen der Studie gewonnenen Ergebnisse zeigten jedoch bislang noch unbekannte und überraschende positive therapeutische Effekte des Wirkstoffes Capsaicin: Vor der Capsaicin-Behandlung festgestellte Becherzellen-Hyperplasien sowie schuppige Zellmeta- und Dysplasien hatten sich zurückgebildet.
Die vorliegende Erfindung schlägt deshalb vor, Capsaicin als Wirkstoff zur Herstellung eines Arzneimittels zur lokalen Behandlung von pathologischen Veränderungen der Nasenschleimhautzellen im Sinne von Hyper-, Meta- oder Dysplasien zu verwenden.
Als weiteres überraschendes Ergebnis zeigten die erwähnten Untersuchungen eine Verbesserung des nasalen mukoziliären Transportsystems. Die Erfindung schlägt deshalb weiter vor, Capsaicin als Wirkstoff zur Herstellung eines Arzneimittels zur lokalen Applikation auf die Nasenschleimhaut zwecks Verbesserung des mukoziliären Transportsystems zu verwenden.
Im Sinne der bewährten Formulierung ist Gegenstand der Erfindung auch ein flüssiges Arzneimittel enthaltend Capsaicin in Lösung als Wirkstoff zur lokalen Behandlung von krankhaften Veränderungen der Nasenschleimhaut im Sinne von Zell-Hyper-, -Meta- oder -Dysplasien oder als Wirkstoff zur lokalen Applikation auf die Nasenschleimhaut zwecks Verbesserung des mukoziliären Transportsystems, wobei die Wirkstofflösung jeweils eine neutrale, wässrige Lösung mit einer Wirkstoffkonzentration zwischen 0,5 und 14 mu g/ml ist.
Vorzugsweise ist das flüssige Arzneimittel als Nasenspray zubereitet, insbesondere als Nasen-Dosierspray, wobei in diesem Fall pro Spray-Stoss 0,035 bis 1,0 mu g Wirkstoff freigesetzt bzw. zerstäubt wird.
Das flüssige Arzneimittel kann auch in Form von Nasentropfen zubereitet sein. Insbesondere im Hinblick auf Nasenpflegeanwendungen kommen weiters in Betracht festere Darreichungsformen wie Salben, Crèmes, Gele, Emulgele in Tuben oder spezielle Dosierapplikatoren. Schliesslich ist auch ein Pulver oder ein Puder als Schnupfmittel denkbar.
Die Präparate können ausser dem Wirkstoff Capsaicin, Lösungsmittel, Träger und/oder Füllstoffe noch andere Substanzen enthalten, die für die Pflege der Nasenschleimhaut nützlich sind, wie beispielsweise Meersalz oder bei Schnupfen schleimhautabschwellende Mittel und/oder Antibiotika oder Desinfizienten.
Als Lösungsmittel kommen in Betracht Wasser, physiologische Kochsalz-Lösung, Propylenglykol oder ölige Lösungen, die Glycerin, flüssige Polyethylenglykole oder partiell verestertes Glycerin oder Polyethylenglykol enthalten. Als Träger für Pulver und Puder können verwendet werden Stärke, Silikate, Stearate und dergleichen.
Als Grundlage für Salben und Crèmes kommen in Betracht Vaseline, Paraffine, pflanzliche oder tierische Fette und \le, Wachse, synthetische Glyceride, Carboxyvinylpolymere (Carbopol) und Polyalkylsiloxane. Besonders geeignet sind hydrophile Salben aus Gemischen von flüssigen und festen Macrogolen, das sind beispielsweise Polyethylenglycole, Polyethylenglycolester wie Poly(oxyethylenoxyadipoyl), Poly-(oxyethylenoxysuccinyl), Macrogolstearat, Macrogol-glycerol-hydrostearat.
Crèmes sind im vorliegenden Fall Zubereitungen aus einer lipophilen und einer wässrigen Phase, enthaltend Emulgatoren vom Wasser-in-\l-Typ wie Wollfett, Sorbitanester und Monoglyceride oder Natrium- oder Triethanolaminseifen, sulfatierte Fettalkohole, Polysorbate oder Carboxyvinylpolymere. Gele bestehen aus Zubereitungen von Wasser, Gylcerin oder Propylenglycol mit geeigneten Quellstoffen wie Traganth, Stärke, Cellulosederivaten, Carboxyvinylpolymeren oder Magnesium-Aluminium-Silikaten.
Den Nasenmitteln können noch Geruchs- und Geschmacksstoffe, wie beispielsweise Kampfer, Eucalyptus- oder Nelkenöl sowie Antibiotika, wie beispielsweise Tyrothricin oder Bacitracin und/oder Desinfizientia wie Benzalkoniumchlorid oder Dequaliniumchlorid zugesetzt werden.
Nachstehend wird die klinische Studie, die zu den überraschenden Erkenntnissen führte, eingehender beschrieben:
Untersuchte Gruppe:
Untersucht wurden acht männliche und acht weibliche Personen, im Durchschnitt 28-jährig, im Büro tätig und gesund. Die Testpersonen wiesen insbesondere keine krankhaften Anomalien der Nase sowie der paranasalen Sinuse auf. 7 Personen waren Zigaretten- Raucher/innen, vier litten anamnestisch an einer saisonalen, allergischen Rhinopathie.
Formulierung:
Für alle Untersuchungen wurde ein Capsaicin als Wirkstoff enthaltender Dosierspray verwendet, welcher bei jedem Spraystoss 0,125 mu g Wirkstoff liefert. Zur Herstellung von insgesamt 1000 ml dieses Dosiersprays wurden 28,56 mg synthetisches Capsaicin in 20 g Polyethylenglykol 200 bei erhöhter Temperatur (ca. 50-60 DEG C) gelöst. 1,25 g der so gewonnenen Lösung wurden zu 1000 ml Wasser verdünnt.
Zytopathologische Untersuchung:
Die Probanden applizierten während 27 (+/-4) Tagen 3 x täglich je einen Spraystoss in jedes Nasenloch. Die zur Untersuchung benötigten Zellen wurden bei allen Probanden mit einem kleinen Bürstchen unter der Maxilloturbinale entlang des inferioren Muschelansatzes jeweils vor sowie nach der Capsaicin-Behandlung (2 Stunden nach der letzten Applikation) entnommen, nach Papanicolaou gefärbt und schliesslich zytopathologisch untersucht.

Der Vergleich der vorstehenden Untersuchungsergebnisse ergibt, dass Capsaicin in vorteilhafter Weise eine Eutrophisierung des Zellzyklus der Nasenschleimhautzellen bewirkt.
Untersuchung des mukoziliären Transports mittels Saccharin-Test
Den Probanden wurde vor sowie 5 Minuten nach der letzten von vier Capsaicin-Applikationen, die innerhalb von 15 Minuten stattfanden, 1 mg Saccharin auf der unteren Nasenmuschel deponiert und nachfolgend die Zeit bis zum Auftreten eines intensiven Süssgeschmacks im Mund gemessen. Die nachfolgende Tabelle gibt die Messergebnisse im Einzelnen wieder:

Kampfer und Eucalyptusöl werden mit etwa der Hälfte der Stärke angerieben. Die andere Hälfte der Stärke wird zum Anreiben des Capsaicin benutzt. Beide Teile werden vereinigt, homogenisiert, gemischt und in geeignete Behälter abgefüllt.

Die in den vorstehenden Beispielen beschriebenen Nasenmittel werden in Dosen von etwa 0,1 bis etwa 1 g Salbe, Crème oder Gel durch die Naris in die Nase appliziert. Pulverförmige Darreichungsformen können durch die Naris eingeschnupft werden.

Only 8 smokers could only detect one metaplasia.
The study states that the cytopathological examination of the nasal mucosa by means of brush cytology has so far received little attention in otolaryngology, partly due to a lack of experience and partly due to the predominance of other questions. In particular for researching the inflammatory processes and the immunological reactions on the nasal mucosa, the doctor would have other methods available which provided more reliable information.
The study does not name any means of treating the aforementioned pathological cell changes in the nasal mucosa. Such means do not seem to be known yet. The study only aims to find out whether nasal mucosal cytology can be used as a simple and reproducible examination modality as a screening screening examination.
Capsaicin, the hot ingredient of the red chilli pepper and other nightshade family, has beneficial therapeutic effects for a wide variety of diseases, including vasomotor and hyperreflective rhinopathy. The chemical name of natural capsaicin is 8-methyl-N-vanillyl-6-nonenamide, that of synthetic capsaicin N-vanillyl-nonanamide (CAS No .: 2444-46-6).
To treat rhinopathy, capsaicin has so far mostly been applied to the nasal mucosa in a highly concentrated form. Because of the strong burning caused by the active ingredient, this was only possible after prior local anesthesia and had to be carried out by the doctor.
However, a capsaicin nasal spray has recently become known from EP-A1 646 372, which contains the active ingredient (synthetic capsaicin) in neutral, aqueous solution in a comparatively very low concentration. This nasal spray can easily be used by the patient several times a day.
Presentation of the invention
In the course of the market launch of the aforementioned capsaicin nasal spray, it was also examined in a clinical study to determine whether long-term use over a period of several weeks could have toxic effects on the nasal mucosa. No such negative effects were found.
However, the results obtained during the study showed previously unknown and surprising positive therapeutic effects of the active ingredient capsaicin: goblet cell hyperplasia as well as flaky cell meta- and dysplasia found before the capsaicin treatment had regressed.
The present invention therefore proposes to use capsaicin as an active ingredient in the manufacture of a medicament for the local treatment of pathological changes in the nasal mucosal cells in the sense of hyper-, meta- or dysplasia.
As a further surprising result, the studies mentioned showed an improvement in the nasal mucociliary transport system. The invention therefore further proposes to use capsaicin as an active ingredient in the manufacture of a medicament for topical application to the nasal mucosa in order to improve the mucociliary transport system.
In the sense of the proven formulation, the invention also relates to a liquid medicament containing capsaicin in solution as an active ingredient for the local treatment of pathological changes in the nasal mucosa in the sense of cell hyper-, meta or dysplasia or as an active ingredient for local application to the nasal mucosa for the purpose of improving the mucociliary transport system, the active substance solution in each case being a neutral, aqueous solution with an active substance concentration between 0.5 and 14 μg / ml.
The liquid medicament is preferably prepared as a nasal spray, in particular as a nasal dosing spray, in which case 0.035 to 1.0 μg of active ingredient is released or atomized per spray burst.
The liquid drug can also be prepared in the form of nose drops. In particular with regard to nasal care applications, solid dosage forms such as ointments, creams, gels, emulgels in tubes or special dosing applicators are also considered. Finally, a powder or powder is also conceivable as a snuff.
In addition to the active ingredient capsaicin, solvents, carriers and / or fillers, the preparations can also contain other substances which are useful for the care of the nasal mucosa, such as, for example, sea salt or, in the case of colds, decongestants and / or antibiotics or disinfectants.
Suitable solvents are water, physiological saline, propylene glycol or oily solutions which contain glycerol, liquid polyethylene glycols or partially esterified glycerol or polyethylene glycol. Starch, silicates, stearates and the like can be used as carriers for powder and powder.
Vaseline, paraffins, vegetable or animal fats and oils, waxes, synthetic glycerides, carboxyvinyl polymers (Carbopol) and polyalkylsiloxanes can be used as the basis for ointments and creams. Hydrophilic ointments from mixtures of liquid and solid macrogols are particularly suitable, for example polyethylene glycols, polyethylene glycol esters such as poly (oxyethyleneoxyadipoyl), poly (oxyethyleneoxysuccinyl), macrogol stearate, macrogol glycerol hydrostearate.
In the present case, creams are preparations from a lipophilic and an aqueous phase, containing water-in-l type emulsifiers such as wool fat, sorbitan esters and monoglycerides or sodium or triethanolamine soaps, sulfated fatty alcohols, polysorbates or carboxyvinyl polymers. Gels consist of preparations of water, glycerol or propylene glycol with suitable swelling agents such as tragacanth, starch, cellulose derivatives, carboxyvinyl polymers or magnesium aluminum silicates.
Fragrance and flavoring agents, such as camphor, eucalyptus or clove oil, and antibiotics, such as tyrothricin or bacitracin and / or disinfectants such as benzalkonium chloride or dequalinium chloride, can also be added to the nasal agents.
The clinical trial that led to the surprising findings is described in more detail below:
Investigated group:
Eight male and eight female persons, average age 28, were examined and worked in the office and healthy. In particular, the test subjects showed no abnormalities in the nose or paranasal sinuses. 7 people were cigarette smokers, four had a history of seasonal allergic rhinopathy.
Formulation:
A dosing spray containing capsaicin as the active ingredient was used for all investigations, which delivers 0.125 μg of active ingredient with each spray burst. To produce a total of 1000 ml of this dosing spray, 28.56 mg of synthetic capsaicin were dissolved in 20 g of polyethylene glycol 200 at elevated temperature (approx. 50-60 ° C.). 1.25 g of the solution thus obtained were diluted to 1000 ml of water.
Cytopathological examination:
The test subjects applied one burst of spray to each nostril 3 times a day for 27 (+/- 4) days. The cells required for the examination were removed from all subjects with a small brush under the maxilloturbinal along the inferior mussel attachment before and after the capsaicin treatment (2 hours after the last application), stained for Papanicolaou and finally examined cytopathologically.

Examination of mucociliary transport using the saccharin test
Before and 5 minutes after the last of four capsaicin applications that took place within 15 minutes, the test subjects were placed 1 mg saccharin on the lower turbinate and then the time until an intense sweet taste appeared in the mouth. Dequalinium chloride is dissolved in water, the solution is mixed with the sodium carboxymethyl cellulose and stirred. Camphor and eucalyptus oil are rubbed with about half the starch. The other half of the starch is used to rub the capsaicin. Both parts are combined, homogenized, mixed and filled into suitable containers.

An ointment obtained as in Example 1 is added to benzalkonium chloride instead of dequalinium chloride.
The nasal agents described in the above examples are applied in doses of about 0.1 to about 1 g of ointment, cream or gel through the Naris into the nose. Powdery dosage forms can be sniffed through the Naris.
