La presente invenzione concerne l'uso della L-metioninamide dell'acido ursodesossicolico avente la seguente struttura chimica.

Tale principio attivo è descritto nel brevetto europeo EP 272 462 e si ottiene attraverso due possibili vie di sintesi direttamente dall'acido ursodessossicolico. Le due sintesi adottate sono:
1) Dal cloruro dell'acido ursodesossicolico ottenuto direttamente con cloruro di ossalile a basse temperature in solventi di diossano e acetone anidro facendolo poi reagire con L metionina.
2) Dall'anidride mista dell'acido ursodesossicolico con fenilcloroformiato in acetone anidro facendolo poi reagire con L metionina in mezzo acquoso.
Il prodotto finito puo essere utilizzato direttamente come acido oppure come un suo sale inorganico od organico formato con basi farmacologicamente accettabili, in particolare come sale di lisina od arginina o di ioni come Ca ed Na.
La preparazione del prodotto avviene preferibilmente secondo il metodo di sintesi sopra riportato al punto (2).
Si pesano accuratamente gr. 200 di acido ursodesossicolico e si pongono in litri 1 di acetone anidro. Si abbassa la temperatura di reazione a +5 DEG C e si aggiungono ml 60 di fenilcloroformiato. Lentamente, mantenendo la temperatura costantemente intorno a +5 DEG C, si cala l'equivalente stechiometrico di trietilammina, sciolta in acetone anidro.
Terminata la calata, sempre sotto agitazione, si porta la temperatura a +35 DEG C e si mantiene per due ore al fine di completare la reazione.
A parte si prepara una soluzione acquosa costituita da gr. 58 di L metionina, trattata con la quantita stechiometrica di trietilammina, formandone il sale solubile.
Tale soluzionc di L metionina viene calata nella soluzione acetonica dell'anidride dell'acido ursodesossicolico portata ad una temperatura interna +5 DEG C. Mano a mano che avviene l'aggiunta si ha una soluzione finale completa che viene riscaldata per due ore a temperatura di 35 DEG C e quindi calata in una soluzione acida di acido cloridrico al 1096. Precipita il prodotto finito che viene filtrato e lavato abbondantemente con acqua.
Se la presenza di acido ursodesossicolico libero supera limiti analitici del 2%-3% bisogna ricorrere ad una purificazione selettiva attraverso il suo sale sodico.
Il prodotto si decompone a circa 90 DEG C e fonde in forma perfettamente limpida a 107 DEG -109 DEG .
Il prodotto così ottenuto è stato utilizzato per produrre compresse contenenti ciascuna mg. 400 di L-metioninamide dell'acido ursodesossicolico ed aventi la seguente composizione qualitativa quantitativa:
L-metioninamide dell'acido ursodesossicolico g. 0,4 - Lattosio Famracopea Ufficiale g. 0,1 - Amido di mais g. 0,057 - Talco g. 0,03 - Magnesio stearato g. 0,01 - Idrossipropilcellulosa g. 0,03.
Non si esclude comunque l'utilizzo del prodotto come sale in forme farmaceutiche solubili realizzate con la L-metioninamide dell'acido ursodesossicolico trattata con basi organiche o inorganiche.
Il prodotto nel seguito indicato con la sigla F.R. 530 è stato sottoposto ad una valutazione preliminare di screening comparativo con acido ursodesossicolico e sono state individuate proprieta sorprendenti sotto il profilo terapeutico.
Preventivamente all'utilizzo sull'uomo si era proceduto ad una attenta valutazione tossicologica del prodotto dalla quale è possibile trarre le seguenti conclusioni:
gli studi di tossicita acuta (DL 50 > 5000 mg/kg e 1210 mg/kg rispettivamente p.o. e per i.p. sia nel ratto che nel topo) non hanno mostrato un'attivita lesiva di F.R. 530 su specifici organi o apparati;
gli studi di tossicità per somministrazioni prolungate, nel ratto e nel cane, non hanno evidenziato particolari alterazioni imputabili a F.R. 530;
nel ratto, fino alla dose di 300 mg/kg, F.R. 530, non e stato osservato alcuno effetto sulla fertilità e sulle capacità riproduttive;
gli studi di teratogenesi, condotti nel coniglio e nel ratto, mostrano che, fino alla dose di 300 mg/kg, F.R. 530 non causa malformazioni e/o anomalie;
;
negli studi di tossicità peri e post natale, condotti nel ratto, non sono mai stati evidenziati particolari effetti negativi;
lo studio sull'attività mutagena di F.R. 530, effettuato mediante 5 differenti tests (Ames, Aberrazioni Cromosomiche, test dei Micronuclei, DNA synthesis) non ha evidenziato condizioni che possano far presupporre effetti mutageni imputabili al preparato in esame.
Prima di procedere alla valutazione della attività comparativa con l'acido ursodesossicolico (UDCA) si e accertato il possibile arricchimento biliare in UDCA nell'uomo.
Per tale studio sono stati arruolati 5 pazienti volontari con diagnosi di colelitiasi trattati per 6 settimane con 1 cpr da 400 mg somministrata per 2 volte al giorno. Le determinazioni analitiche svolte mediante cromatografia liquida ad alta pressione (HPLC) sono state effettuate su succo duodenale, prelevato ai pazienti con la tecnica dell'entero test.
I risultati ottenuti sono stati decisamente positivi; le concentrazioni di acido ursodesossicolico sotto forma di tauro-glicoconiugato, raggiunti nella bile durante la somministrazione di F.R. 530 (vedere tabella 1) risultano allineati con quelli descritti in letteratura a seguito di somministrazioni di 600 mg di UDCA con trattamenti più prolungati nel tempo.

Queste prove preliminari hanno confermato le attività ipotizzate nel succitato EP 272 462.
I risultati sperimentali che seguono hanno invece indicato, sorprendentemente, una specifica attivita di F.R. 530 in patologie quali epatopatie e cirrosi epatiche.
Per tale ragione si è iniziato un trattamento su particolari pazienti affetti da cirrosi biliare primitiva (CBP) sottoposti ad un trattamento nel tempo al fine di una valutazione obiettiva delle varianti dei parametri ematochimici significativi in queste patologie.
Sono stati trattati 20 pazienti di cui 9 per un periodo di 90 giorni, mentre un paziente ha protratto le somministrazioni per 9 mesi.
Bisogna considerare che l'arruolamento di tali pazienti ha creato non poche difficoltà poichè la CBP è una patologia che rimane per diverso tempo asintomatica e successivamente progredisce con la comparsa di tutte le caratteristiche complicanze di una vera e propria cirrosi. È estremamente importante sottolineare che, nella CBP, nessun trattamento farmacologico, ad eccezione dell'UDCA, è in grado di modificare i parametri ematochimici di necrosi e di colestasi: questi risultano, nel corso della malattia, notevolmente alterati, soprattutto nei valori espressi dalla Gamma Glutamyl Transpeptidasi e dalla Fosfatasi Alcalina.
Le tabelle seguenti illustrano la buona attività di F.R. 530, nel ridurre i valori dei parametri di funzionalità epatica, che si esplica sia a seguito di trattamenti a breve termine sia a seguito di trattamenti prolungati, evidenziando, rispetto alle condizioni basali, differenze statisticamente significative (p.<0.01)(p<0.05)(tabelle 2, 3 e 4).
Inoltre, va ricordato che tali risultati sono almeno paragonabili e per alcuni parametri superiorì a quelli descritti in letteratura per UDCA.
Di notevole rilevanza è stata anche l'ottima tollerabilità generale e la buona compliance dei pazienti ai trattamenti.

Un ulteriore aspetto significativo di tali risultati e quello di poter raggiungere valori ematochimici soddisfacenti in tempi abbastanza ristretti rispetto a medesimi risultati ottenuti nella stessa patologia con trattamenti con ursodesossicolico; infatti dalla letteratura emerge che riduzioni analoghe con uso di solo acido ursodesossicolico avvengono con trattamenti da 4 a 6 mesi.
I risultati brillanti del farmaco hanno consigliato un suo utilizzo in casistiche molto allargate in pazienti utilizzando il prodotto nello studio delle epatopatie croniche.
The present invention relates to the use of ursodeoxycholic acid L-methioninamide having the following chemical structure.

This active principle is described in the European patent EP 272 462 and is obtained through two possible ways of synthesis directly from ursodeoxycholic acid. The two summaries adopted are:
1) From ursodeoxycholic acid chloride obtained directly with oxalyl chloride at low temperatures in solvents of dioxane and anhydrous acetone making it then react with L methionine.
2) From the mixed anhydride of ursodeoxycholic acid with phenylchloroformate in anhydrous acetone making it then react with L methionine in an aqueous medium.
The finished product can be used directly as acid or as an inorganic or organic salt thereof formed with pharmacologically acceptable bases, in particular as lysine or arginine salt or ions such as Ca and Na.
The preparation of the product preferably takes place according to the synthesis method reported above in point (2).
Weigh accurately gr. 200 of ursodeoxycholic acid and are placed in 1 liters of anhydrous acetone. The reaction temperature is lowered to +5 DEG C and 60 ml of phenylchloroformate are added. Slowly, keeping the temperature constantly around +5 DEG C, the stoichiometric equivalent of triethylamine, dissolved in anhydrous acetone, decreases.
Once the drop is finished, always under stirring, the temperature is brought to +35 DEG C and is kept for two hours in order to complete the reaction.
Separately, an aqueous solution consisting of gr. 58 of L methionine, treated with the stoichiometric amount of triethylamine, forming its soluble salt.
This solution of L methionine is lowered into the acetone solution of the ursodeoxycholic acid anhydride brought to an internal temperature of +5 DEG C. As the addition occurs, a complete final solution is obtained which is heated for two hours at 35 DEG C and then lowered into an acid solution of hydrochloric acid to 1096. The finished product precipitates which is filtered and washed abundantly with water.
If the presence of free ursodeoxycholic acid exceeds analytical limits of 2% -3%, it is necessary to resort to a selective purification through its sodium salt.
The product decomposes at about 90 DEG C and melts in a perfectly clear form at 107 DEG -109 DEG.
The product thus obtained was used to produce tablets containing each mg. 400 of L-methioninamide of ursodeoxycholic acid and having the following qualitative and quantitative composition:
Ursodeoxycholic acid L-methioninamide g. 0.4 - Lactose Famracopea Officer g. 0.1 - Corn starch g. 0.057 - Talc g. 0.03 - Magnesium stearate g. 0.01 - Hydroxypropylcellulose g. 0.03.
However, the use of the product as a salt in soluble pharmaceutical forms made with L-methioninamide of ursodeoxycholic acid treated with organic or inorganic bases is not excluded.
The product indicated below with the abbreviation F.R. 530 has undergone a preliminary evaluation of comparative screening with ursodeoxycholic acid and surprising therapeutic properties have been identified.
Prior to its use on humans, a careful toxicological evaluation of the product was carried out from which it is possible to draw the following conclusions:
the acute toxicity studies (LD 50> 5000 mg / kg and 1210 mg / kg respectively p.o. and for i.p. both in the rat and the mouse) did not show a harmful activity of F.R. 530 on specific organs or systems;
toxicity studies for prolonged administration, in rats and dogs, did not reveal any particular alterations attributable to F.R. 530;
in the rat, up to a dose of 300 mg / kg, F.R. 530, no effect was observed on fertility and reproductive capacity;
teratogenicity studies, conducted in rabbits and rats, show that, up to the dose of 300 mg / kg, F.R. 530 does not cause malformations and / or anomalies;
;
in the peri and post natal toxicity studies conducted in the rat, no particular negative effects were ever highlighted;
the study on the mutagenic activity of F.R. 530, carried out through 5 different tests (Ames, Chromosomal Aberrations, Micronuclei test, DNA synthesis) did not highlight conditions that could suggest mutagenic effects attributable to the preparation under examination.
Before proceeding with the evaluation of the comparative activity with ursodeoxycholic acid (UDCA), the possible biliary enrichment in UDCA in humans was ascertained.
5 volunteer patients diagnosed with cholelithiasis treated for 6 weeks with 1 400 mg tablet administered 2 times a day were enrolled for this study. Analytical determinations carried out by high pressure liquid chromatography (HPLC) were carried out on duodenal juice, taken from the entero test technique to patients.
The results obtained have been decidedly positive; the concentrations of ursodeoxycholic acid in the form of tauro-glycoconjugate, reached in the bile during the administration of F.R. 530 (see table 1) are aligned with those described in the literature following doses of 600 mg of UDCA with more prolonged treatments over time.

These preliminary tests confirmed the activities hypothesized in the aforementioned EP 272 462.
The experimental results that follow have instead indicated, surprisingly, a specific activity of F.R. 530 in diseases such as liver disease and cirrhosis of the liver.
For this reason, treatment has been started on particular patients with primary biliary cirrhosis (CBP) who have undergone treatment over time in order to objectively evaluate the variants of the significant blood chemistry parameters in these pathologies.
20 patients were treated including 9 for a period of 90 days, while one patient continued the administrations for 9 months.
It must be considered that the enrollment of these patients has created many difficulties as CBP is a pathology that remains for some time asymptomatic and subsequently progresses with the appearance of all the characteristic complications of a real cirrhosis. It is extremely important to underline that, in CBP, no pharmacological treatment, with the exception of UDCA, is able to modify the blood chemistry parameters of necrosis and cholestasis: these are significantly altered during the disease, especially in the values expressed by the Gamma Glutamyl Transpeptidase and Alkaline Phosphatase.
The following tables illustrate the good activity of F.R. 530, in reducing the values of the liver function parameters, which is carried out both following short-term treatments and following prolonged treatments, highlighting statistically significant differences compared to basal conditions (p. <0.01) (p <0.05 ) (tables 2, 3 and 4).
Furthermore, it should be remembered that these results are at least comparable and for some parameters superior to those described in the literature for UDCA.
The excellent general tolerability and good compliance of patients with treatments was also of significant importance.

A further significant aspect of these results is that of being able to reach satisfactory blood chemistry values in fairly short times compared to the same results obtained in the same pathology with treatments with ursodeoxycholic; in fact, from the literature it appears that similar reductions with the use of ursodeoxycholic acid only occur with treatments from 4 to 6 months.
The brilliant results of the drug have recommended its use in very large cases in patients using the product in the study of chronic liver diseases.
