L'invention a pour objet une composition pharmaceutique destinée au traitement préventif ou curatif d'affections à Chlamydiae humaines ou animales telles par exemple le trachome, les conjonctivites à inclusions, les infections uro-génitales y relatives.
L'invention a plus particulièrement pour objet une composition telle que définie ci-dessus, contenant à titre de facteur immunogène une souche ou un mélange de souches de sérotypes différents de bactéries de l'espèce Chlamydia trachomatis ou Chlamydia psittaci.
Les muqueuses, qui présentent une vaste surface en contact avec le milieu extérieur, sont l'objet d'agressions constantes et possèdent un système de défense très puissant pour faire face à ces attaques, avec des facteurs antimicrobiens non spécifiques et spécifiques. Les systèmes spécifiques de protection des muqueuses conjonctivales font partie du large système MALT (mucosa associated lymphoïd tissue) comprenant pour l'essentiel le système GALT associé au tube digestif et des systèmes annexes, dont le système BALT associé aux bronches ou le système CALT (conjonctiva associated lymphoïd tissue).
L'immunité spécifique des muqueuses est due essentiellement aux immunoglobulines secrétoires (IgAs) qui sont synthétisées sur place par des plasmocytes provenant de lymphocytes B activés qui ont reçu une stimulation antigénique dans n'importe quelle partie des muqueuses, in situ ou à distance. Il a été démontré, par des méthodes chimiques et par immunofluorescence, qu'une partie des IgA sécrétoires se fixe sur le mucus conjonctival et à la surface des cellules épithéliales, et forme ainsi une barrière continue d'anticorps qui s'oppose aux agressions extérieures.
Dans le cas des affections mentionnées plus haut, il a été trouvé de façon surprenante que la meilleure stimulation pour la formation d'anticorps de type IgAs au niveau des épithélias conjonctivaux, ou uro-génitaux par exemple, se situait au niveau de l'intestin.
Grâce à la composition objet de la présente invention, il est désormais possible d'obtenir une stimulation locale efficace des conjonctives, par stimulation antigénique au niveau de l'intestin, après absorption des antigènes y relatifs par voie orale. L'invention est conforme à la revendication 1.
Dans le traitement préventif ou curatif d'affections oculaires humaines ou animales, on obtient de bons résultats à l'aide de compositions contenant, à titre de facteur immunogène, au moins un des sérotypes A à C, ou selon les cas tous ces sérotypes, de l'espèce Chlamydia trachomatis.
Dans le cas d'affections oculo- ou uro-génitales humaines ou animales, on utilise de préférence comme facteur immunogène au moins un des sérotypes D à K, ou selon les cas l'ensemble de ces sérotypes, de l'espèce Chlamydia trachomatis. Dans le cas d'affections pulmonaires humaines, on utilise de préférence au moins un des sérotypes, ou tous selon les cas, L1 à L3 de l'espèce Chlamydia trachomatis ou le sérotype TWAR.
Dans le cas d'affections essentiellement animales, on utilise avantageusement un ou plusieurs sérotypes de l'espèce Chlamydia psittaci.
Selon l'invention, la composition pharmaceutique contient à titre de facteur immunogène des bactéries vivantes, atténuées ou tuées, ou encore un extrait ou un lyophilisat de ces bactéries. De bons résultats ont été obtenus à l'aide de Chlamydia trachomatis vivantes ou de leur lyophilisat.
Selon l'invention également, la composition pharmaceutique peut contenirt un excipient ou diluant inerte, phar maceutiquement acceptable. Elle peut en outre contenir, selon les cas, un ou plusieurs adjuvants ou stimulants immunologiques appropriés.
La composition selon l'invention s'administrant aisément par voie orale, elle peut se présenter sous toute forme appropriée, par exemple sous la forme de suspension de bactéries vivantes ou atténuées, de leur lyophilisat ou de leur extrait, sous une forme galénique gastro-résistante telle une gélule ou un comprimé gastro-résistant, sous forme de comprimé à faire fondre dans la bouche ou encore sous forme de liposomes. Il est du ressort de l'homme du métier de déterminer la forme d'administration la plus adéquate.
Pour obtenir l'immunisation souhaitée, il convient d'administrer la composition de manière répétée, sur une période de 15 à 30 jours et d'effectuer un rappel après 3 mois, le cas échéant après 1 an. Par exemple, on peut administrer la dose prescrite à J1, J2, J3 puis à J15, J16 et J17 et effectuer ensuite un rappel à 90 jours puis au bout d'un an, ou encore à J1, J15 et J30, les rappels subséquents étant également effectués à 90 jours, puis au bout d'un an. Bien entendu, tout autre rythme d'administration peut être envisagé.
L'efficacité de compositions conformes à l'invention peut être illustrée comme suit. Cette illustration n'est pas limitative.
a. Chlamydia trachomatis
a.1. Expérimentation sur le lapin
On prépare une suspension antigénique à partir de culture de Chlamydia trachomatis sérotype D en procédant comme suit: les antigènes sont obtenus en culture de Chlamydia sur cellules McCOY ou HELA 229, et comportent un mélange de corps réticulés et de corps élémentaires. Ces cel lules sont remises en suspension et ultrasoniquées ou séparées par tout autre système mécanique.
Les anticorps sont recherchés et dosés à partir du 8ème jour dans le sang et les larmes, l'absence d'anticorps ayant été vérifiée avant toute injection. Les anticorps se positivent dès le 8ème jour à un titre faible dans le sérum (1/4 à 1/8) et se précisent franchement dès le 15ème jour (3ème détermination: J-1, J8, J15).
Le tableau suivant donne les titres observés pour cinq lapins répartis dans les cages 114, 115, 116, 117 et 119, en fonction du temps écoulé depuis l'immunisation:

On a ainsi pu observer que l'ingestion, trois jours consécutifs à deux reprises, de particules chlamydiennes vivantes entraîne chez le lapin l'apparition d'anticorps spécifiques à un titre élevé, aussi bien dans le sérum que dans les larmes.
a.2. Autres applications
Des expérimentations identiques ont été menées sur le lapin avec les sérotypes A et B ainsi que A, B et C pour ce qui concerne le trachome et avec des sérotypes D à K pour ce qui concerne les conjonctivites à inclusion.
Chlamydiae trachomatis ne présentant aucune pathogénécité par voie buccale et digestive, la forme active vivante donne d'excellents résultats.
Des résultats comparables ont été obtenus avec les lyophilisats correspondants ou avec des bactéries atténuées par du formol à 1%, durant une heure à 4 DEG C.

b. Chlamydia psittaci
b.1. Recherche de l'effet protecteur chez le cobaye
Afin de confirmer l'apparition d'anticorps chez l'animal immunisé et non immunisé, secondairement infecté par la souche GPIC (Guinea Pig Inclusion Conjunctivitis) de Chlamydia psittaci, on a procédé comme suit.
Deux groupes de 5 cobayes ont été choisis:
- Un groupe A est immunisé selon le protocole décrit. Dès l'apparition des anticorps dans le sérum et les larmes, 2 gouttes d'une suspension d'une culture de 48 heures en cellules McCOY de la souche sont instillées dans l'oeil droit et gauche Cette souche avait été préalablement atténuée par l'adjonction de formaldéhyde à 1 ml pour 100 ml en contact 24 heures à 37 DEG C (6 flacons de 75 cm<3>). Les cellules injectées formolées sont receuillies, lavées trois fois en eau physiologique. Le culot a été repris par 45 ml d'eau physiologique et conservée en aliquotes de 3 ml à + 4 DEG C. Ces aliquotes servent pour l'immunisation per os.
. Les anticorps apparaissent dans les larmes dès le 15ème jour et dans le sérum dès la 3ème semaine dans le groupe des cobayes immunisés. Les titres dans les larmes atteignent 1/1024 pour certains animaux et 1/64 dans le sérum.
. Après instillation de la souche vivante dans les conjonctives, des frottis sont effectués deux fois par semaine et le germe est recherché par culture. L'expérience a duré un mois et demi.
- Un groupe B de témoins non immunisés. Les cultures ont été positive jusqu'à la fin pour le groupe non immunisé où il y avait encore persistance de mucus mais sans autre signe.
Dans le groupe immunisé, les cultures se sont partiellement négativées 15 jours avant la fin de l'expérience. Une recherche de l'effet dose a cependant été entreprise devant la difficulté à conclure à une efficacité réelle.
b.2. Recherche de l'effet dose
En vue de déterminer la plus petite quantité de germes provocant une conjonctivite, on a procédé comme suit:
Dix groupes de deux cobayes ont été inoculés (instillation oculaire bilatérale), avec deux gouttes de la suspension de Chlamydia psittaci vivantes (culture de 48 heures en cellules McCOY) et des dilutions logarithmiques croissantes de cette même suspension jusqu'à 10<-><9>.
Des frottis ont été effectués tous les 8 jours ainsi que des prélèvements pour l'isolement en culture pendant 4 semaines.
. Les cultures s'avèrent positives pour la dilution 10<-5> jusqu'au 15ème jour
. Les cultures s'avèrent positives pour la dilution 10<-><4> jusqu'au 21ème jour
. Les cultures s'avèrent positives pour la dilution 10<-><2> jusqu'à la 4ème semaine.
Le titre infectant à utiliser a été évalué à 10<-><4>.
b.3. Effet de l'immunisation sur l'infection
Deux groupes de 4 cobayes ont été utilisés:
- Les cobayes témoins A - B - C - D ont été instillés avec deux gouttes de la souche, diluée de 10<-><1> (un cobaye avec chaque dilution) jusqu'à 10<-><4>. Les prélèvements sont effectués chaque semaine en vue d'isoler le germe des sécrétions oculaires.
- Le groupe immunisé (4 cobayes): 1 - 2 - 3 - 4. Une instillation oculaire bilatérale a été effectuée dès l'apparition des anticorps (3ème semaine après la première ingestion, soit une semaine après la 2ème ingestion)(taux 1/64 - 1/128).
Chez les témoins (non immunisés), on note la présence d'une conjonctivite. Des cultures positives sont obtenues jusqu'à cinq semaines après l'instillation pour les dilutions 10<-><2>.
Chez les animaux immunisés, on note l'absence de conjonctivite. Dès la deuxième semaine, il n'est plus possible d'obtenir des cultures positives aux mêmes dilutions. Pour les dilutions 10<-><3> et 10<-><4>, il n'y a plus de culture positive une semaine après.
Il y a donc manifestement une protection contre l'infection.

The mucous membranes, which have a large surface in contact with the external environment, are the object of constant aggressions and have a very powerful defense system to face these attacks, with non-specific and specific antimicrobial factors. The specific conjunctival mucosa protection systems are part of the broad MALT system (mucosa associated lymphoid tissue) essentially comprising the GALT system associated with the digestive tract and related systems, including the BALT system associated with the bronchi or the CALT system (conjunctiva associated lymphoid tissue).
Specific mucosal immunity is mainly due to secretory immunoglobulins (IgAs) which are synthesized on the spot by plasma cells from activated B cells which have received antigenic stimulation in any part of the mucous membranes, in situ or at a distance. It has been shown, by chemical methods and by immunofluorescence, that part of the secretory IgA binds on the conjunctival mucus and on the surface of the epithelial cells, and thus forms a continuous barrier of antibodies which opposes external aggressions. .
In the case of the aforementioned conditions, it has been surprisingly found that the best stimulation for the formation of IgAs-type antibodies in the conjunctival epithelia, or urogenital, for example, is in the intestine. .
Thanks to the composition which is the subject of the present invention, it is now possible to obtain effective local stimulation of the conjunctiva, by antigenic stimulation in the intestine, after absorption of the related antigens by the oral route. The invention is in accordance with claim 1.
In the preventive or curative treatment of human or animal ocular conditions, good results are obtained using compositions containing, as immunogenic factor, at least one of serotypes A to C, or as the case may be, all these serotypes, of the species Chlamydia trachomatis.
In the case of human or animal oculo- or uro-genital disorders, at least one of the serotypes D to K, or as the case may be, all of these serotypes, of the species Chlamydia trachomatis, is preferably used as immunogenic factor. In the case of human pulmonary affections, at least one of the serotypes is used, or all as the case may be, L1 to L3 of the species Chlamydia trachomatis or the serotype TWAR.
In the case of essentially animal diseases, one or more serotypes of the species Chlamydia psittaci are advantageously used.
According to the invention, the pharmaceutical composition contains, as an immunogenic factor, living bacteria, attenuated or killed, or an extract or a lyophilisate of these bacteria. Good results have been obtained using live Chlamydia trachomatis or their lyophilisate.
Also according to the invention, the pharmaceutical composition may contain an inert, pharmaceutically acceptable excipient or diluent. It may also contain, as appropriate, one or more adjuvants or appropriate immunological stimulants.
Since the composition according to the invention can be administered easily by the oral route, it can be in any suitable form, for example in the form of a suspension of living or attenuated bacteria, of their lyophilisate or of their extract, in a galenic form resistant such as a capsule or gastro-resistant tablet, in the form of a tablet to be melted in the mouth or also in the form of liposomes. It is up to the person skilled in the art to determine the most suitable form of administration.
To obtain the desired immunization, the composition should be administered repeatedly, over a period of 15 to 30 days and a booster should be given after 3 months, if necessary after 1 year. For example, you can administer the prescribed dose on D1, D2, D3 then on D15, D16 and D17 and then perform a booster at 90 days then after a year, or again on D1, D15 and D30, the subsequent booster also being performed at 90 days, then after a year. Of course, any other rhythm of administration can be envisaged.

at. Chlamydia trachomatis
a.1. Rabbit experiment
An antigenic suspension is prepared from a culture of Chlamydia trachomatis serotype D by proceeding as follows: the antigens are obtained in culture of Chlamydia on McCOY or HELA 229 cells, and comprise a mixture of crosslinked bodies and elementary bodies. These cells are resuspended and ultrasonic or separated by any other mechanical system.
Antibodies are sought and assayed from the 8th day in blood and tears, the absence of antibodies having been checked before any injection. Antibodies are positive from the 8th day on a low titer in the serum (1/4 to 1/8) and are clearly defined from the 15th day (3rd determination: D-1, D8, D15).
The following table gives the titers observed for five rabbits distributed in cages 114, 115, 116, 117 and 119, according to the time elapsed since the immunization:

It has thus been observed that the ingestion, on three consecutive days twice, of living chlamydial particles causes in rabbits the appearance of specific antibodies with a high titer, both in serum and in tears.
a.2. Other applications
Identical experiments were carried out on rabbits with serotypes A and B as well as A, B and C with regard to trachoma and with serotypes D to K with regard to inclusion conjunctivitis.
Chlamydiae trachomatis does not present any pathogenicity by mouth and digestive, the living active form gives excellent results.
Comparable results were obtained with the corresponding lyophilisates or with bacteria attenuated by 1% formalin, for one hour at 4 DEG C.

b. Chlamydia psittaci
b.1. Research of the protective effect in guinea pigs
In order to confirm the appearance of antibodies in the immunized and non-immunized animal, secondarily infected with the GPIC (Guinea Pig Inclusion Conjunctivitis) strain of Chlamydia psittaci, the procedure was carried out as follows.
Two groups of 5 guinea pigs were chosen:
- A group A is immunized according to the protocol described. As soon as the antibodies appear in the serum and the tears, 2 drops of a suspension of a 48 hour culture in McCOY cells of the strain are instilled in the right and left eye. This strain had been previously attenuated by the addition of formaldehyde to 1 ml per 100 ml in 24 hour contact at 37 DEG C (6 bottles of 75 cm <3>). The injected formalin cells are harvested and washed three times in physiological water. The pellet was taken up in 45 ml of physiological water and stored in aliquots of 3 ml at + 4 DEG C. These aliquots are used for oral immunization.
. Antibodies appear in tears from the 15th day and in the serum from the 3rd week in the group of immunized guinea pigs. The tears are 1/1024 for some animals and 1/64 for serum.
. After instillation of the living strain into the conjunctiva, smears are performed twice a week and the germ is sought by culture. The experiment lasted a month and a half.
- A group B of non-immune controls. The cultures were positive until the end for the non-immunized group where there was still persistence of mucus but without other signs.
In the immunized group, the cultures were partially negative 15 days before the end of the experiment. A search for the dose effect was however undertaken in view of the difficulty in concluding that it was really effective.
b.2. Dose effect research
In order to determine the smallest quantity of germs causing conjunctivitis, we proceeded as follows:
Ten groups of two guinea pigs were inoculated (bilateral ocular instillation), with two drops of the suspension of live Chlamydia psittaci (48 hour culture in McCOY cells) and increasing log dilutions of this same suspension up to 10 <-> < 9>.
Smears were taken every 8 days as well as samples for isolation in culture for 4 weeks.
. The cultures are positive for dilution 10 <-5> until the 15th day
. The cultures are positive for dilution 10 <-> <4> until the 21st day
. The cultures are positive for dilution 10 <-> <2> until the 4th week.
The infectious titer to be used was evaluated at 10 <-> <4>.
b.3. Effect of immunization on infection
Two groups of 4 guinea pigs were used:
- Control guinea pigs A - B - C - D were instilled with two drops of the strain, diluted from 10 <-> <1> (one guinea pig with each dilution) up to 10 <-> <4>. Samples are taken weekly to isolate the germ from eye secretions.
- The immune group (4 guinea pigs): 1 - 2 - 3 - 4. A bilateral ocular instillation was carried out as soon as the antibodies appeared (3rd week after the first ingestion, or one week after the 2nd ingestion) (rate 1 / 64 - 1/128).
In the controls (not immunized), the presence of conjunctivitis is noted. Positive cultures are obtained up to five weeks after instillation for 10 <-> <2> dilutions.
In immunized animals, the absence of conjunctivitis is noted. From the second week, it is no longer possible to obtain positive cultures at the same dilutions. For dilutions 10 <-> <3> and 10 <-> <4>, there is no longer a positive culture one week later.
There is therefore clearly protection against infection.
