Auf das obere Ende des zylindrischen Teils wird ein Kopfelement mit Innengewinde aufgeschraubt, das lateral vorspringende Rippen zum Aufbau der Zahnkrone aufweist. Je nach Art der Zahnkrone werden Kopfelemente unterschiedlicher äusserer Konfiguration verwendet, die entsprechend vorrätig sein müssen. Beim Eindrehen solcher Stifte mit selbstschneidenden Gewinden in den präparierten Zahnwurzelkanal kann es wegen der auftretenden hohen Kräfte leicht zur Fraktur der Zahnwurzel kommen. Auch lässt sich ein Lokkern des aufgeschraubten Kopfteils bei mechanischer Belastung der Zahnkrone, wie beispielsweise beim Kauen, nicht sicher vermeiden, was zu einem Herausbrechen des Wurzelstiftes aus dem Wurzelkanal führen kann.
