Abstract
A metal or plastic profile pre-fabricated for connecting and stabilising natural teeth, and drilling and grinding instruments matched thereto, as well as drilling templates, are described. The recesses to be produced with these auxiliary instruments correspond to the pre-fabricated profiles, and as a result any number of teeth can be connected to one another in a single treatment session. In contrast to other profiles, however, need the recess be aligned only for two teeth. The special shape of the profile ensures high stability coupled with little invasion of tooth substance and little expenditure on work and material. This method can be employed for connection both of natural teeth to one another and of artificial crowns to natural teeth. In addition, it is also possible to replace individual teeth by this system. <IMAGE>

Beschreibung
Die Zeichnungen zeigen unter Fig. la das im Patentanspruch 1 beschriebene Profil mit zwei zylindrischen Wülsten im Querschnitt und in der Ansicht.
Unter Fig. Ib zeigen sie das im Patentanspruch 2 beschriebene Profil mit zwei halbkreiszylindrischen Wülsten.
Fig. 2 zeigt ein doppelt Y-förmiges Profil.
Fig. 3 zeigt einen Abschnitt des Profils der Fig. ib, der in eine Ausnehmung, die mit der Schleifscheibe Fig. 5 oder Fig. 6 und dem zylindrischen Schleifer der Fig. 7 hergestellt wurde, passt.
Fig. 4 zeigt einen Abschnitt des Profils der Fig. 1 a, bei dem die Wülste hohl gehalten sind zur Aufnahme von Stiften mit Gewinden.
Unter Fig. 5 ist eine Schleifscheibe zu sehen, die nur am Rande mit Diamantkörner belegt ist und daher einen ausreichend masshaltigen Schnitt erzielen kann. Sie ist am Schaft so geformt, dass sie in die Mitte des Zwischenraumes zwischen zwei Zähnen hinein gleitet. Dem gleichen Zweck dient eine dem Zentrum der Schleifscheibe aufgesetzte Spitze.
Fig. 6 zeigt eine solche Schleifscheibe ohne Spitze.
Fig. 7 zeigt zylindrischen Bohrkörper aus Stahl oder Hartmetall.
Fig. 8 zeigt einen ebensolchen diamantierten Schleifkörper wie Fig. 7, Beide sind so ausgerüstet, dass sie nur auf einem Teil ihrer Länge schneiden und durch den nicht schneidenden Teil eine ausreichend genaue Führung erzielt wird.
Dazu sind sie mit einem Tiefenanschlag versehen, der die Eindringtiefe genau begrenzt.
Fig. 9 zeigt im Querschnitt eine kreuzförmige Bohrschablone mit zwei im rechten Winkel zueinander stehenden Lamellen a + b und einem Zylinder c, die in einen zuvor angelegten Schlitz zwischen zwei Zähne gesteckt wird und den Abstand, die Tiefe des Eindringens und die Richtung der zylindrischen Bohrungen genau vorschreibt und begrenzt.
Fig. 10 zeigt den Längsschnitt der Fig. 9 mit einem eingeführten zylindrischen Schleifer.
Fig. 11 + 12 zeigen die eingegliederten Profile zur Verbindung dreier natürlicher Zähne untereinander.
Fig. 13 + 14 zeigen ein Profil, das einerseits in einen natürlichen Zahn eingegliedert, andererseits an einer künstlichen Krone angelötet ist.
Fig. 15 zeigt die Anwendung des Profils zwischen zwei natürlichen Zähnen zum Ersatz eines natürlichen Zahnes.
Fig. 16 zeigt die hierfür erforderliche verstellbare Bohrschablone. Figur 17 zeigt eine Bohr- und Schleifeinrichtung mit zwei parallel laufenden Werkzeugen und Figur 18 zeigt eine Bohr- und Schleifeinrichtung mit einem Führungsstift.
Ausführungsbeispiele
An einem Beispiel soll eine Anwendung des Profils gezeigt werden: Mit einer Schleifscheibe wird in etwa senkrecht zur Kaufläche zweier benachbarter Zähne und möglichst nahe an ihrem Berührungspunkt ein Schlitz so angebracht, dass er in beide Zähne gleich tief eindringt. Der Schlitz ist auf das Zentrum beider Zähne gerichtet und läuft gleich wie die Zahnachsen. Nun wird eine kreuzförmige Bohrschablone (Fig. 10) in den Schlitz und zwischen die beiden Zähne gesteckt, wodurch sie fixiert wird. Zusätzlich kann sie mit einer erhärtbaren Masse beschickt werden. Durch den Zylinder am Rande der Querlamelle wird eine zylindrische Bohrung an dem einen Rande des Schlitzes gebohrt, danach nach Drehung der Schablone um 1800 eine dazu parallel laufende am anderen Rande.
Die Bohr- und Schleifinstrumente, die Bohrschablone und die Verbindungsprofile sind in allen Abmessungen so aufeinander abgestimmt, dass in eine solcherart hergestellte Ausnehmung ein Abschnitt des Verbindungsprofils satt anliegend eingebracht werden kann. Lediglich die nach oben und unten offenen Kanten müssen noch durch Beschneiden, Schleifen und Polieren angepasst werden. Diese Anpassung kann ausserhalb des Mundes geschehen, wenn zuvor die Uberstände mit einem spitzen Instrument angezeichnet oder eingekratzt wurden. Danach kann die endgültige Verbindung hergestellt werden, indem die Ausnehmung mit einem der üblichen Befestigungszemente ausgefüllt und das Verbindungselement unter Druck und leichten Hammerschlägen in die Endlage gebracht wurde.
Auf diese Weise können beliebig viele nebeneinander stehende Zähne miteinander verbunden und damit stabilisiert werden.
Nach einer Variante des Verfahrens kann ein mit einer künstlichen Krone zu versehender Zahn mit einem benachbarten natürlichen, nicht zu überkronenden Zahn verbunden werden. Eine solche Krone kann notwendig sein wegen kari öser Zerstörung der Zahnkrone oder, wenn dieser zur Ankupplung eines abnehmbaren Zahnersatzes versehen werden soll. Hierbei wird die Ausnehmung wie bei dem zuvor dargestellten Verfahren an beiden Zähnen vor dem Beschleifen des zu überkronenden Zahnes angebracht. Danach wird der zu überkronende Zahn auf übliche Weise beschliffen und mit einem Abschnitt des Verbindungsprofils in üblicher Weise eine Abformung über beiden Zähnen gewonnen. Dieser Profilabschnitt kann entweder aus einer anlötbaren oder eingiessbaren Edelmetall-Legierung bestehen - dann wird er angelötet oder eingegossen (Fig. 13 + 14). Er kann aber auch aus Stahl oder Nickel bestehen.
Dann wird nach dem Guss der Edelmetallkrone das unedle Metallprofil durch Aussäueren, z.B. in Salz- oder Schwefelsäure, entfernt. Der so gewonnene Hohlraum kann dann nach dem Eingliedern der Krone im Munde mit einem Profilabschnitt aus Edelmetall ausgefüllt werden, der die Krone mit dem vorbereiteten natürlichen, nicht überkronten Nachbarzahn verbindet.
Das Verfahren kann aber auch analog in einer weiteren Variante zur Herstellung einer kleinen Brücke angewandt werden, die ohne Überkronung der Nachbarzähne einen verlorenen Zahn ersetzen soll (Fig. 15). Hierfür wird man vorzugsweise einen zweckmässig dimensionierten und aus einer Legierung von hoher mechanischer Festigkeit bestehenden Profilabschnitt anwenden. Dabei benötigt man eine verstellbare Bohrschablone (Fig. 16), um den Zwischenraum zu überbrücken. Nachdem die Ausnehmung an beiden Zähnen angebracht ist, werden zwei Profilabschnitte eingebracht und darüber die übliche Abformung genommen. Auf dem danach gewonnenen Modell kann dann im Labor ein Kunstzahn als Ersatz in die offenen Profilflächen einpolymerisiert werden. Eine derartige Brücke wird danach in gewohnter Weise einzementiert.
Vorteile
Das erfindungsgemässe Profil hat den Vorteil, nicht nur einfach und wenig aufwendig anwendbar zu sein, sondern dazu praktisch unsichtbare, hygienische, stabile und grazile Verbindungen herzustellen. Zudem können derartige Schienungen jederzeit nachträglich auf weitere Zähne leicht ausgedehnt werden. Sie erfordern gegenüber anderen ähnlichen, individuell herzustellenden nur einen geringen materiellen und zeitlichen Aufwand.
Das erfindungsgemässe Verbindungssystem kann auch leicht repariert oder erneuert werden, wenn sich im Laufe des Gebrauchs eines der Profile lösen oder am Rande eine Karies entstehen sollte, da es von allen Seiten zugänglich ist und eine direkte Verbindung immer nur zwischen zwei Zähnen besteht.
Beim Ausfall eines Zahnes in einer Zahnreihe können zudem kleine Brücken zum Ersatz eines solchen Zahnes hergestellt werden, indem Profile in den beiden, der Zahnlücke benachbarten Zähnen eingelassen werden, an denen ein künstlicher Ersatzzahn verankert wird. Derartige Brücken können als zeitweiliger, aber auch als dauernder Ersatz in einem parodontal vorgeschädigten Gebiss angewandt werden. Ihr bedeutsamer Vorteil ist darin zu sehen, dass sie keine Volloder Teilkronen als Anker benötigen, sondern lediglich die vorgefertigten Profilabschnitte, für deren Eingliederung nur wenig Zahnsubstanz der noch gesunden Nachbarzähne geopfert werden muss. Dazu kommt, dass Vollkronen im parodontal geschädigten Gebiss nur schwer herzustellen sind, da die Verjüngungen der Zähne wurzelwärts verhältnismässig zuviel Abtragung der natürlichen Kronen verlangen.

It is shaped on the shaft so that it slides into the middle of the space between two teeth. A tip placed on the center of the grinding wheel serves the same purpose.

Fig. 7 shows cylindrical drill bodies made of steel or hard metal.
Fig. 8 shows the same diamond-coated abrasive body as Fig. 7, both are equipped so that they cut only over part of their length and a sufficiently precise guide is achieved by the non-cutting part.
For this purpose, they are provided with a depth stop that precisely limits the depth of penetration.
Fig. 9 shows in cross section a cruciform drilling template with two mutually perpendicular blades a + b and a cylinder c, which is inserted into a previously created slot between two teeth and the distance, the depth of penetration and the direction of the cylindrical bores prescribes exactly and limits.

Embodiments
An example of an application of the profile is to be shown: With a grinding wheel, a slot is made approximately perpendicular to the occlusal surface of two adjacent teeth and as close as possible to their point of contact so that it penetrates the same depth in both teeth. The slot is aimed at the center of both teeth and runs in the same way as the tooth axes. Now a cross-shaped drilling template (Fig. 10) is inserted into the slot and between the two teeth, whereby it is fixed. In addition, it can be loaded with a hardenable mass. A cylindrical bore is drilled on one edge of the slot through the cylinder on the edge of the transverse lamella, and then, after the template has been rotated by 1800, a hole running parallel to it on the other edge.
The drilling and grinding instruments, the drilling template and the connecting profiles are matched to one another in all dimensions so that a section of the connecting profile can be inserted snugly into a recess produced in this way. Only the edges open at the top and bottom still have to be adjusted by trimming, grinding and polishing. This adjustment can take place outside the mouth if the protrusions have previously been marked or scratched with a pointed instrument. The final connection can then be made by filling the recess with one of the usual fastening cements and bringing the connecting element into the end position under pressure and light hammer blows.
In this way, any number of teeth standing side by side can be connected to one another and thus stabilized.
According to a variant of the method, a tooth to be provided with an artificial crown can be connected to an adjacent natural tooth that is not to be crowned. Such a crown may be necessary due to carious destruction of the tooth crown or if this is to be provided for coupling a removable denture. Here, the recess is made on both teeth, as in the previously described method, before the tooth to be crowned is ground. Then the tooth to be crowned is ground in the usual way and an impression is obtained over both teeth in the usual way with a section of the connecting profile. This section of the profile can either consist of a solderable or pourable precious metal alloy - then it is soldered or cast (Fig. 13 + 14). But it can also consist of steel or nickel.
Then, after casting the precious metal crown, the base metal profile is acidified, e.g. in hydrochloric or sulfuric acid. The cavity thus obtained can then be filled in with a profile section made of precious metal after fitting the crown in the mouth, which connects the crown to the prepared natural, uncrowned neighboring tooth.
However, the method can also be used analogously in a further variant for producing a small bridge, which is intended to replace a lost tooth without crowning the neighboring teeth (FIG. 15). For this purpose, a suitably dimensioned profile section consisting of an alloy with high mechanical strength will preferably be used. You need an adjustable drilling template (Fig. 16) to bridge the gap. After the recess has been made on both teeth, two profile sections are inserted and the usual impression is taken. On the model obtained afterwards, an artificial tooth can be polymerized into the open profile surfaces as a replacement in the laboratory. Such a bridge is then cemented in the usual way.
advantages
The profile according to the invention has the advantage not only of being simple and inexpensive to use, but also of producing practically invisible, hygienic, stable and delicate connections. In addition, such splinting can be easily extended to other teeth at any time. Compared to other similar, individually manufactured ones, they require only a small amount of material and time.
The connection system according to the invention can also be easily repaired or renewed if one of the profiles becomes loose during use or if caries develops on the edge, since it is accessible from all sides and there is only a direct connection between two teeth.
If a tooth fails in a row of teeth, small bridges can also be made to replace such a tooth by inserting profiles in the two teeth adjacent to the tooth gap, to which an artificial replacement tooth is anchored. Such bridges can be used as a temporary but also as a permanent replacement in a periodontally damaged dentition. Their significant advantage can be seen in the fact that they do not need full or partial crowns as anchors, but only the prefabricated profile sections, for the integration of which only little tooth substance has to be sacrificed to the still healthy neighboring teeth. In addition, full crowns in periodontally damaged teeth are difficult to manufacture, since the tapering of the teeth towards the root requires too much removal of the natural crowns.
