PATENTANSPRÜCHE
1. Vorrichtung zum Spülen der Blase (1) bei Operationen unter Niederdruckirrigation, mit einer Fördereinrichtung (3) für aufbereitetes Wasser, die einen Warruwasseraufbereiter (10) und einen in die Blase (1) einzuführenden Resektionsschaft (11) enthält, gekennzeichnet - durch eine in der Leitung (12) zwischen dem Warmwas seraufbereiter (10) und dem Resektionsschaft (11) ange ordnete Dosiervorrichtung (13), welche Dosiervorrichtung (13) ausgebildet ist zur Einhaltung eines Druckbereichs in nerhalb der Blase (1) mittels selbsttätiger Regelung der
Wasserzufuhr aus dem Wasseraufbereiter (10), - sowie durch eine mit Saugpumpe (24) und Druckmesser (27, 28) ausgestattete Absaugvorrichtung (2),
die ein mit tels eines Trokars (20) durch die Bauchdecke in die Blase (1) einzuführendes Absaugrohr (22) aufweist.
2. Vorrichtung nach Anspruch 1, gekennzeichnet durch eine Dosiervorrichtung (13) mit einem in der Förderleitung (12) angeordneten Dosiergefäss (30) und einer Steuereinrichtung, die ein zwischen dem Wasseraufbereiter (10) und dem Gefäss (30) in der Förderleitung (12) angeordnetes Regelventil (43) sowie dem Dosiergefäss (30) zugeordnete Füllstandmessorgane (36-36',37-37') aufweist, welche das Ventil (43) elektrisch ansteuern, welche Steuervorrichtung ausgelegt ist zur Einhaltung eines bestimmten Füllstandbereiches innerhalb des Dosiergefässes (30) bei in Betrieb befindlicher Absaugvorrichtung (2).
3. Vorrichtung nach Anspruch 2, gekennzeichnet durch einen aufrechtstehenden, stirnseitig geschlossenen und mit Skala (35) versehenen Glaszylinder als Dosiergefäss (30) mit einem oberseitigen und bodenseitigen Anschluss (33 bzw.34) für die Förderleitung (12), wobei der Resektionsschaft (11) mit dem bodenseitigen Anschluss (34) in Verbindung steht, durch ein Magnetventil als Regelventil (43) sowie durch zwei übereinander angeordnete Lichtschranken (3636', 37-37') als Füllstandmessorgane und einen lichtundurchlässigen Schwimmer (42) im Dosiergefäss (30), welche Lichtschranken (3636', 37-37') an zwei entlang dem Gefäss (30) verlaufenden Führungsstangen (38, 39) befestigt und in Abstand und Höhe verstellbar sind.
4. Vorrichtung nach Anspruch 2 oder 3, gekennzeichnet durch ein vor bzw. nach dem Ventil (43) in der Förderleitung (12) angeordnetes Thermometer (44) bzw. Sterilfilter (45).
5. Vorrichtung nach einem der Ansprüche 2 bis 4, gekennzeichnet durch eine vor dem Dosiergefäss (30) in der Förderleitung (12) angeordnete Saugpumpe (46) und ein an der Saugpumpe (46) angeschlossener, eine Traubenzuckerlösung aufzunehmen bestimmter Behälter (47).
6. Vorrichtung nach einem der Ansprüche 2 bis 5, dadurch gekennzeichnet, dass das Dosiergeläss (30) auf einem verstellbaren Tragarm (5) gehaltert ist.
7. Vorrichtung nach einem der Ansprüche 1 bis 6, gekennzeichnet durch ein im Trokar (20) verschiebbares und unten geschlossenes Absaugrohr (22), dessen Rohrwandung im unteren, aus dem Trokar (20) vorstehenden Rohrbereich perforiert ist.
8. Vorrichtung nach Anspruch 7, dadurch gekennzeichnet, dass die Saugpumpe (24) über eine Absaugleitung (23) und einem darin angeordneten Manometer (28) an das Absaugrohr (22) angeschlossen ist.
9. Vorrichtung nach Anspruch 8, gekennzeichnet durch ein am Trokar (20) angeordnetes weiteres Manometer (27) zur Überwachung des Flüssigkeitsdrucks im Absaugrohr (22).
10. Vorrichtung nach Anspruch 9, gekennzeichnet durch eine aus durchsichtigem Material bestehende Ábsaugleitung (23).
Die Erfindung betrifft eine Vorrichtung zum Spülen der
Blase bei Operationen unter Niederdruckirrigation, mit einer
Fördereinrichtung für aufbereitetes Wasser, die einen Warmwasseraufbereiter und einen in die Blase einzuführenden Resektionsschaft enthält.
Den bekannten Vorrichtungen dieser Art liegen Operationsmethoden zugrunde, bei denen das Absaugen des durch den Resektionsschaft in die Blase eingeleiteten Wassers je weils wieder durch den Schaft erfolgt. Daher erfordert in die sen Vorrichtungen jeder Entleerungsvorgang der Blase die Abtrennung der Fördereinrichtung (Resektionsinstrument).
Der Ablauf der Operation unter Verwendung derartiger Vorrichtungen muss daher nachteiligerweise während den Entleerungsvorgängen häufig unterbrochen werden. Ferner besteht bei diesen Vorrichtungen nur eine beschränkte Möglichkeit, die Menge des in die Blase einfliessenden Wassers zu kontrollieren, so dass die Blase nur unzureichend gegen Überdehnung geschützt ist. Als weiterer Nachteil kann sich bei diesen Vorrichtungen eine unerwünschte Resorption von Flüssigkeit durch das Gewebe, sog. Wassersyndrom, ergeben.
Die Erfindung hat sich dementsprechend zur Aufgabe gestellt, eine Vorrichtung zu schaffen, bei welcher die Entnahme von Flüssigkeit aus der Blase während der Operation ohne Entfernung des Resektionsinstrumentes vonstatten geht, so dass die vorstehenden Nachteile beim Wassertausch vermieden werden. Ferner soll die Vorrichtung jeweils neues Spülwasser ohne Gefahr einer Überdehnung der Blase zuführen und zweckmässig eine etwaige Resorption ausschliessen.
Diese Aufgaben werden erfindungsgemäss gelöst für eine Vorrichtung der obengenannten Art die gekennzeichnet ist durch eine in der Leitung zwischen dem Warmwasseraufbereiter und dem Resektionsschaft angeordnete Dosiervorrichtung, welche Dosiervorrichtung ausgebildet ist zur Einhaltung eines Druckbereichs innerhalb der Blase mittel selbsttätiger Regelung der Wasserzufuhr aus dem Aufbereiter, sowie durch eine mit Saugpumpe und Druckmesser ausgestattete Absaugvorrichtung, die ein mittels eines Trokars durch die Bauchdecke in die Blase einzuführendes Absaugrohr aufweist.
Die Erfindung wird im folgenden für ein Ausführungsbeispiel anhand der beiliegenden Zeichnungen näherer- läutert. Es zeigen

In der schematischen Darstellung von Fig. 1 erkennt man rechtsseitig der Blase 1 die Absaugvorrichtung 2 und linksseitig die Fördereinrichtung 3 (Resektionsinstrument3.
Unterhalb der Blase list der Operationstisch 4 angedeutet, der einen verstellbaren Tragarm 5 haltert.
Die Fördereinrichtung 3 enthält als wesentliche Bestandteile den Warmwasseraufbereiter 10, den Resektionsschaft 11 und eine in der Förderleitung 12 zwischen Aufbereiter 10 und Resektionsschaft 11 angeordnete Dosiervorrichtung 13.
Aufbereitetes Warmwasser von z. B. 300C gelangt nach Durchlauf durch die Dosiervorrichtung 13 und den im Blaseneingang eingesetzten Resektionsschaft 11 in die Blase 1.
Während der Operation ist es wichtig, dass dieses Spülwasser einen genügenden Entfaltungsdruck in der Blase 1 ausübt und ferner austauschbar ist. Erfindungsgemäss wird dies erreicht durch das Zusammenwirken von Dosiervorrichtung 13 und Absaugvorrichtung 2.
Die Absaugvorrichtung 2 gemäss Fig. 3 weist einen
durch die Bauchdecke in die Blase 1 eingeführten Trokar 20 auf. Im Trokar 20 befindet sich ein in Pfeilrichtung 21 verschiebbares Rohr 22 mit geschlossenem Boden und perforierter unterer Rohrwandung. Das Rohr 22 ist über die Absaugleitung 23 an die Wasserstrahlpumpe 24 angeschlossen, die durch den Wasserhahn 25 betätigt wird und das Abwasser aus der Blase 1 durch die perforierte Rohrwandung in die Ablaufleitung 26 absaugt. Hierbei lassen sich etwaige an der perforierten Rohrwandung angesetzte Gewebeteilchen dadurch abstreifen, indem man das Rohr 22 in den Trokar 20 einzieht, also die Verschiebbarkeit des Rohrs 22 dessen Verstopfungsfreiheit gewährleistet.
Die Absaugvorrichtung 2 enthält ferner zwei Manometer 27, 28, wobei das Manometer 27 gemäss Fig. 3 zwecks Überwachung des Rohrdrucks am Trokar 20 angeordnet ist und das der Pumpe 24 vorgelagerte Manometer 28 den Druck in der Absaugleitung 23 misst. Die Absaugleitung 23 besteht vorzugsweise aus durchsichtigem Material, damit die Farbe der abgesaugten Flüssigkeit ständig überprüft und eine eventuell eintretende stärkere Blutung festgestellt werden kann.
Bezugnehmend auf Fig. 1,2 besitzt die Dosiervorrichtung 13 ein Dosiergefäss 30 in Gestalt eines stirnseitig beidseits geschlossenen, aufrechtstehenden Glaszylinders. Das Gefäss 30 ist ober- und unterseitig in je einer Fassung .31,32 gehaltert. Die Bodenfassung 32 ist hierbei am erwähnten verstellbaren Tragarm 5 befestigt, der das Gefäss 30 trägt und eine zur Sichtkontrolle geeignete und fixierbare Einstellung des Dosiergefässes 30 ermöglicht. Zum Einsetzen des Dosiergefässes 30 in die Förderleitung 12 weist das Gefäss oben- und untenseitig den Einlaufstutzen 33 bzw. Auslaufstutzen 34 auf, so dass in die Blase 1 nachgeliefertes Wasser das Gefäss 30 von oben nach unten passiert.
Ersichtlicherweise stellt hierbei die Höhe der Wassersäule im Dosierge läss 30 ein direktes Mass dar für den jeweils in der Blase herrschenden Flüssigkeitsdruck. Die eingangs genannte Bedingung eines einheitlichen Flüssigkeitsdruckbereichs in der Blase 1 kann dementsprechend durch eine entsprechende Dosierung der Höhe der Wassersäule erreicht werden. Hierzu verfügt die Dosiervorrichtung 13 über eine Steureinrichtung, die dafür sorgt, dass die Wassersäule ein passendes oberes und unteres Niveau nicht über- bzw. unterschreitet.
Zwei derartige beispielsweise einzuhaltende Niveaus a, b sind in Fig. 1, 2 gestrichelt dargestellt und ablesbar an der z. B. in Druckeinheiten geeichten Skala 35 des Gefässes 30.
Die Steureinrichtung der Dosiervorrichtung 13 umfasst zwei übereinander angeordnete Lichtschranken 3636' und 37- 37', deren Strahlengänge auf die gewünschten Niveaus a, b einstellbar sind. Zu diesem Zweck erstrecken sich beidseits des Dosiergefässes 30 je eine Führungsstange 38, 39, welche eine höhenverstellbare Halterung der Sender 36, 37 bzw. Empfänger 36', 37' ermöglichen. Die Lichtschranken 36-36',37-37' liegen in bekannter Weise in je einem Stromkreis mit den elektrischen Leitungen 40, 41. Im Gefäss 30 befindet sich ein Schwimmer 42, der die Lichtschranken 3636' bzw. 37-37' unterbricht, wenn der Wasserspiegel das obere bzw. untere Niveau erreicht.
Zur Steureinrichtung der Dosiervorrichtung 13 gehört ferner das Magnetventil 43, welches in der Förderleitung 12 zwischen Wasseraufbereiter 10 und Dosiergefäss 30 angeordnet ist. Das Ventil 43 liegt in den Stromkreisen 40, 41 der Lichtschranken und wird durch letztere angesteuert, regelt also die in das Dosiergefäss 30 geförderte Wassermenge in Abhängigkeit von den Signalen der Lichtschranken 3636', 3W37'. Eine beispielsweise Ansteuerung besteht darin, dass bei Erreichen des oberen Ni- veaus a mit dem Unterbruch der Lichtschranke 3636' das Ventil 43 schliesst, worauf bei laufender Absaugvorrichtung 2 der Wasserspiegel so lange absinkt, bis er das untere Niveau b erreicht hat und der eintretende Unterbruch der Lichtschranke 37-37' wieder die Offenstellung des Ventil 43 auslöst.
Allgemein wird also in das Dosiergefäss 30 bei annähernd konstantem Flüssigkeitsdruck innerhalb der Blase 1 nur soviel Wasser nachgefördert, wie Flüssigkeit mittels der Absaugvorrichtung 2 aus der Blase herausgepumpt wird.
Die Fördervorrichtung 3 enthält gemäss Fig. 1 ausser den vorbeschriebenen Bestandteilen zweckmässig noch ein vor dem Magnetventil 43 angeordnetes Thermometer 44 zum Messen der Wassertemperatur und dem Ventil 43 nachfolgendes Sterilfilter 45 in der Förderleitung 12. Hieran schliesst sich eine Saugpumpe 46, die in Saugverbindung steht mit einem oberhalb der Pumpe 46 angeordneten Behälter 47, der eine Traubenzuckerlösung enthält. Die in das Spülwasser der Förderleitung 12 eingesaugte Traubenzukkerlösung verhindert vorteilhafterweise, dass innerhalb der Blase 1 das Wasser vom Gewebe aufgesogen wird.

PATENT CLAIMS
1. 9. The device according to claim 8, characterized by a arranged on the trocar (20) further manometer (27) for monitoring the liquid pressure in the suction tube (22).

The invention relates to a device for rinsing the
Bladder during surgery under low pressure irrigation, with one
Conveying device for treated water, which contains a water heater and a resection shaft to be inserted into the bladder.
The known devices of this type are based on surgical methods in which the suction of the water introduced through the resection shaft into the bladder is done again through the shaft. Therefore, each emptying process of the bladder in these devices requires the separation of the conveyor (resection instrument).
The course of the operation using such devices must therefore disadvantageously often be interrupted during the emptying processes. Furthermore, there is only a limited possibility with these devices to control the amount of water flowing into the bladder, so that the bladder is insufficiently protected against overexpansion. A further disadvantage of these devices is the undesirable absorption of liquid through the tissue, the so-called water syndrome.
Accordingly, the object of the invention is to provide a device in which the removal of liquid from the bladder during the operation takes place without removing the resection instrument, so that the above disadvantages in water exchange are avoided. Furthermore, the device should in each case supply new rinsing water without the risk of the bladder being overstretched and expediently preclude any resorption.
These objects are achieved according to the invention for a device of the type mentioned above, which is characterized by a metering device arranged in the line between the water heater and the resection shaft, which metering device is designed to maintain a pressure range within the bladder by means of automatic regulation of the water supply from the conditioner, and by a suction device equipped with a suction pump and pressure meter, which has a suction tube to be inserted into the bladder through the abdominal wall by means of a trocar.

The conveying device 3 contains, as essential components, the hot water conditioner 10, the resection shaft 11 and a metering device 13 arranged in the conveying line 12 between the conditioner 10 and the resection shaft 11.
Treated hot water from z. B. 300C after passing through the metering device 13 and the resection shaft 11 inserted in the bladder inlet into the bladder 1.
During the operation, it is important that this rinsing water exerts sufficient pressure in the bladder 1 and is also interchangeable. According to the invention, this is achieved by the interaction of metering device 13 and suction device 2.
The suction device 2 according to FIG. 3 has one
inserted through the abdominal wall into the bladder 1 trocar 20. In the trocar 20 there is a tube 22 which can be displaced in the direction of the arrow 21 and which has a closed bottom and a perforated lower tube wall. The pipe 22 is connected via the suction line 23 to the water jet pump 24, which is actuated by the water tap 25 and sucks the waste water out of the bladder 1 through the perforated pipe wall into the drain line 26. Any tissue particles attached to the perforated tube wall can be wiped off by pulling the tube 22 into the trocar 20, that is, the displaceability of the tube 22 ensures its freedom from clogging.
The suction device 2 also contains two manometers 27, 28, the manometer 27 according to FIG. 3 being arranged on the trocar 20 for monitoring the tube pressure and the manometer 28 upstream of the pump 24 measuring the pressure in the suction line 23. The suction line 23 is preferably made of transparent material, so that the color of the liquid sucked off can be checked continuously and any bleeding that may occur can be determined.
With reference to FIG. 1, 2, the metering device 13 has a metering vessel 30 in the form of an upright glass cylinder which is closed on both ends. The vessel 30 is held on the top and on the bottom in a version .31, 32. The base mount 32 is in this case fastened to the adjustable support arm 5 mentioned, which carries the vessel 30 and enables a suitable adjustment of the metering vessel 30 which is suitable for visual inspection. To insert the metering vessel 30 into the delivery line 12, the vessel has the inlet connection 33 and outlet connection 34 on the top and bottom, so that water supplied into the bladder 1 passes the vessel 30 from top to bottom.
Obviously, the height of the water column in Dosierge 30 is a direct measure of the liquid pressure prevailing in the bladder. The condition mentioned at the outset of a uniform liquid pressure range in the bladder 1 can accordingly be achieved by appropriate dosing of the height of the water column. For this purpose, the metering device 13 has a control device which ensures that the water column does not exceed or fall below a suitable upper and lower level.
Two such levels to be observed, for example, a, b are shown in dashed lines in Fig. 1, 2 and can be read on the z. B. in pressure units calibrated scale 35 of the vessel 30.
The control device of the metering device 13 comprises two light barriers 3636 'and 37-37' arranged one above the other, the beam paths of which can be adjusted to the desired levels a, b. For this purpose, a guide rod 38, 39 extends on both sides of the metering vessel 30, which enable height-adjustable mounting of the transmitters 36, 37 or receivers 36 ', 37'. The light barriers 36-36 ', 37-37' are each in a known manner in a circuit with the electrical lines 40, 41. In the vessel 30 there is a float 42 which interrupts the light barriers 3636 'and 37-37' when the water level reaches the upper or lower level.
The control device of the metering device 13 also includes the solenoid valve 43, which is arranged in the delivery line 12 between the water conditioner 10 and the metering vessel 30. The valve 43 is located in the circuits 40, 41 of the light barriers and is controlled by the latter, that is to say regulates the amount of water conveyed into the metering vessel 30 as a function of the signals from the light barriers 3636 ', 3W37'. An example of control is that when the upper level a is reached, the valve 43 closes with the interruption of the light barrier 3636 ', whereupon the water level drops with the suction device 2 running until it has reached the lower level b and the interruption that occurs the light barrier 37-37 'triggers the open position of the valve 43 again.
In general, only as much water is pumped into the metering vessel 30 at an approximately constant liquid pressure within the bladder 1 as the liquid is pumped out of the bladder by means of the suction device 2.
According to FIG. 1, the conveying device 3 expediently contains, in addition to the components described above, a thermometer 44 arranged in front of the solenoid valve 43 for measuring the water temperature and the sterile filter 45 downstream of the valve 43 in the conveying line 12. This is followed by a suction pump 46 which is in suction connection with a container 47 arranged above the pump 46 and containing a glucose solution. The grape sugar solution sucked into the rinsing water of the delivery line 12 advantageously prevents the water from being sucked up by the tissue within the bladder 1.
