Description
The invention has for its object to provide a transition piece for receiving bags with a flange and a skin protection that allows the attachment and removal of receiving bags, in particular disposable bags with a fastening disc made of adhesive. The transition piece according to the invention consists of a skin protection consisting of an adhesive layer and a flexible polymer film and of a flat flange made of a polymer, the arrangement of which is defined in claim 1. The skin protection is adjusted around the stoma; a bag is glued to the flat polymer flange.
In order to alleviate or prevent the problems caused by abrasions and irritation of the skin, skin protectors according to US Pat. No. 3,339,546 are used. Such skin protectors are preferred by ileostomy patients who leave the protection and the entire device on the body for several days. In contrast, for economic reasons, colostomy patients using disposable bags are reluctant to use such skin protectors. The colostomy patient needs a skin protector that can remain around the stoma for several days and still allow the filled pouch to be easily removed and the new pouch to be tightly attached.
Other colostomy patients can control the excretion of body exudates with a suitable irrigation. Between these rinses, however, they use a pouch to increase safety. Before rinsing, the receiving bag is removed and, even if it is empty, put down, since the adhesive on the receiving bag does not allow it to be attached to the skin again. However, if the skin protection according to the invention and the flange are used, the colostomy patient can reattach an empty receiving bag to the flat polymer flange.
As an alternative, the colostomy patient currently has an attachment flange which is attached to a skin protection and which allows the attachment and removal of the receiving bag with the aid of an elastic band or using a receiving bag with elastic properties. However, such pouches are extremely expensive and the flange creates an unsightly bulge under the clothing.
Therefore, the transition piece according to the invention with the combined flange and the skin protection is particularly suitable for colostomy patients who use pouches for single use.
Es zeigen: The invention is explained in more detail below with reference to the accompanying drawing. Show it:

Fig. 2 einen vergrösserten Schnitt des Übergangsstücks entlang der Linie 2-2 in Fig. 1, 2 is an enlarged section of the transition piece along the line 2-2 in Fig. 1,
Fig. 3 einen vergrösserten Schnitt einer gegenüber den Fig. 1 und 2 geänderten Ausführungsform, 3 shows an enlarged section of an embodiment modified compared to FIGS. 1 and 2,
Fig. 4 eine Aufsicht einer weiteren Ausführungsform und Fig. 5 einen vergrösserten Schnitt des Übergangsstücks gemäss Fig. 4 entlang der Linie 5-5 in Fig. 4. 4 shows a plan view of a further embodiment and FIG. 5 shows an enlarged section of the transition piece according to FIG. 4 along the line 5-5 in FIG. 4.
1 and 2, the transition piece 10 consists of a skin protection with an adhesive layer 11, which comes into contact with the patient's body, and with a thin, flexible polymer film 12 and an outer, flat flange 13 made of a semi-solid polymer. The receiving bag is glued to the flange 13. In order to ensure a secure attachment between the receiving bag and the transition piece 10, the width 14 of the flange 13 in the attachment area should be at least approximately 19 mm.
The glued receiving bag should never cover the entire flange 13. As a result, the patient can press the uncovered part of the flange 13 down, one hand holding the transition piece 10 securely on the body, while the filled receiving bag is pulled off the flange 13 with the other hand. In the embodiment shown in FIGS. 1 and 2, this is achieved in that the flange 13 is on both sides

However, this can also be achieved in that the flange 13 only protrudes on one side or by suitable dimensioning of the flange 13 so that a part is not glued to the receiving bag, although the flange 13 is not over the adhesive layer 11 and the film 12 extends.
Advantageously, eyelets 15 can be provided in the sections of the flange 13 which extend beyond the adhesive layer 11 and the film 12 to hold a band in order to secure the transition piece 10 to the patient's body.
According to FIGS. 1 and 2, the flange 13 is preferably made of a semi-solid polymer and does not cover the entire surface of the film 12. Preferably, film 12 and adhesive layer 11 are rectangular, each side being from about 76 to about 127 mm long; the flange 13 is preferably elliptical and annular. The actual geometric shape of the transition piece 10 is not critical in the sense of the invention and can vary accordingly. For example, the adhesive layer 11 and the film 12 can be circular or elliptical instead of the rectangular shape, and the flange 13 can be circular instead of the illustrated elliptical configuration.
The diameter of the inner circular region of the film 12, which is not covered by the flange 13, can vary from approximately 12.7 to approximately 63 mm. Depending on the size of the stoma, the patient cuts a hole through this part of the adhesive layer 11 and the film 12. The maximum size of this hole is shown by the dashed line 16 in Fig. 1. However, an edge of at least about 3.16 mm of the film 12 and the adhesive layer 11 should remain between the inner circumference of the flange 13 and the stoma. If the semi-solid polymer of the flange 13 touches or rubs against the stoma, this leads to discomfort and possibly injury. An initial hole is preferably provided in the adhesive layer 11 and the film 12.
The polymer film 12 lying against the stoma in this area serves to reduce the dissolution of the adhesive layer 11 by the body exudates which flow through the stoma into the attached receiving bag.
The transition piece shown in FIG. 3 differs from that of FIGS. 1 and 2 in that the flange 13 bears directly on the adhesive layer 11. The polymer film 12 lies as a cover only over the free areas of the upper side of the adhesive layer 11. An initial hole can also preferably be provided.
The adhesive layer 11 consists of materials which are known to be compatible with the skin and which form a bond which is sufficiently strong to bear the weight of the bag device. Suitable adhesives can contain a pressure-sensitive adhesive component and a hydrocolloid or several hydrololloids. Furthermore, adhesives based on Karaya rubber according to US Pat. No. 3,302,647 can be used. Other suitable adhesives contain guar flour according to US Patent Application No. 804,673 dated June 8, 1977. A preferred adhesive according to US Pat. No. 3,339,546 consists of a homogeneous mixture of 40 percent by weight polyisobutylene, 20 percent by weight pectin, 20 percent by weight gelatin and 20 weight percent sodium carboxymethyl cellulose. The adhesive layer 11 can be from about 0.5 to about 2.29 mm, preferably from about 1.4 to about 1.65 mm, thick.
Die dünne, flexible Polymerfolie 12 besteht aus irgendeinem geeigneten, im wesentlichen wasserunlöslichen Material, vorzugsweise Polyäthylen, Polypropylen und Copoly- The thin, flexible polymer film 12 is made of any suitable, substantially water-insoluble material, preferably polyethylene, polypropylene and copoly-
The film 12 can be from about 0.013 to about 0.13 mm, preferably from about 0.025 to about 0.076 mm, thick.
The flat flange 13 consists of a semi-solid polymer. The surface to which the receiving bag is attached must have sufficient strength so that it does not wrinkle or bend over several days after the adapter has been attached to the patient's body. If wrinkles form, it will be difficult to leak-tightly attach the subsequent pouches. However, if the flange 13 made of a polymer is too rigid, it becomes uncomfortable because the parts that protrude beyond the adhesive layer 11 can press 15 into the skin. The polymer used is preferably low-density polyethylene with a thickness of about 0.38 to about 1.52 mm or high-density polyethylene or polypropylene with a thickness of about 0.25 to about 1.0 mm; low density polyethylene with a thickness of about 0.5 to about 0.76 mm is particularly preferred.
The adhesive connection between the flange 13 and the receiving bag is critical. It must be sufficiently strong to bear the weight of a filled bag and, on the other hand, must allow the bag to be easily detached from the flange 13 in order to throw away the receiving bag without detaching the transition piece from the patient's body. The tear strength of this adhesive connection can be influenced by changing the adhesive on the receiving bag or by changing the surface properties of the flange 30 13. If, for example, a receiving bag with a very good adhesive is used, the top of the flange 13 made of polymeric material can be coated with silicone in order to increase the peelability. When using a receptacle bag with an adhesive of lower adhesive capacity, it is preferred that the polymeric material for the flange 13 is not subjected to a corona treatment. If the receiving bag has a relatively weak adhesive, the top of the flange 13 made of polymeric material is preferably subjected to a treatment by corona discharge.
The embodiment of a transition piece 40 shown in FIGS. 4 and 5 differs from the transition piece shown in FIGS. 1 to 3 in that the 45 outer flange 43 made of semisolid polymer is not annular but extends over the surface of the film 12 . The central portion of the flange 43 has a plurality of circular grooves 44 which are cut to a depth of approximately 2/3 of the thickness of the flange 43. The grooves serve as a cut, so that the patient can cut or tear a hole in the transition piece 40 that is slightly larger than the circumference of the stoma. The area between the stoma and the inner edge of the hole can be filled with various ointments or powders which are known to be suitable for this purpose. The fastening area on the flange 43 made of polymeric material for the receiving bag is essentially flat, the flange also being dimensioned such that a part is not glued to the receiving bag. As explained above, this part of the flange can be used by the patient to hold the transition piece in place when the receiving bag is pulled off. In the embodiment shown in FIGS. 4 and 5, the flange extends beyond the skin protection and can preferably have eyelets 15 for fastening a band.

In this case, the section of the polymer film 12 below the grooved section can be omitted. The grooves also serve as a cut, so that the patient can tear or cut a hole in the transition piece that is slightly larger than the circumference of the stoma.
The transition pieces 10 and 40 according to the invention are assembled in the following manner. The skin protection with the adhesive layer 11 is produced in a known manner and then covered by the thin, flexible polymer film 12. The semi-solid flange 13 or 43 is punched out, cut out by a die or cast in the desired shape. The flange is then glued onto the flexible polymer film 12. Numerous adhesives are suitable for this purpose, the only condition being that the adhesive connection between the flange 13 or 43 and the film 12 must be strong enough that the flange is not displaced when the receiving bag is pulled off. In a particularly suitable adhesive process, commercially available, pressure-sensitive transfer adhesive tape is used. This material is available in roll form, with silicone-coated release paper on both sides of the tape.
The transition piece shown in Fig. 3 is made slightly different. The adhesive layer 11 and the flange 13 are produced in the above manner and connected to one another in a suitable manner. The bond between the adhesive layer 11 and the flange 13 must be stronger than the bond that is established between the flange 13 and the receiving bag. The top of the adhesive layer 11, which is not covered by the flange 13, is then coated with a thin polymer film 12 by evaporation of the solvent or by other known coating techniques.
The packaged transition pieces s according to the invention have a paper sheet coated with a release agent on the free underside of the adhesive layer 11, which is intended to come into contact with the patient's body. Silicone coated paper is preferred. If necessary, this paper can be provided with a cut line in order to make it easier to adapt the transition piece to the stoma.
The transition pieces according to the invention have been described above in connection with single-use colostomy bags. Such pouches may have a pressure sensitive adhesive coating directly on a portion of the pouch or a sleeve welded to the pouch around the stoma opening, with the pressure sensitive adhesive layer on the outside of the sleeve. The transition pieces according to the invention can also be used by patients who use reusable bags with a bottom outlet or with another opening. In addition, receiving bags without an adhesive surface can be attached to the transition piece with the aid of a transfer tape or an additional disc which is coated on both sides with a pressure-sensitive adhesive. The surface of the containment bag or cuff should be subjected to corona discharge treatment or pretreated so that the adhesive remains on the containment bag when it is peeled from the semi-rigid flange.
