Die vorliegende Erfindung betrifft ein Gerät zur chirurgischen Behandlung von Knochenbrüchen und -erkrankungen, das zumindest zwei Paare von quer durch die Knochen zu ziehenden Spei### n umfasst. wobei die Enden der Spei chen jedes Paares an zumindest einem Bügel befestigt sind, welche Bügel durch einstellbare Distanzglieder miteinander verbunden sind.
Solche Geräte können in der Orthopädie und Traumatologie zur Reposition von Knochenbruchstücken bei Knochenbrüchen mit deren nachfolgender Halterung in Kompressionsstellung zum Verlängern eines abgekürzten Knochens sowie zur Kompressionsarthrodese verwendet werden.
Allgemein bekannt sind Geräte zur chirurgischen Behandlung der Knochenbrüche und -erkrankungen (s. beispielsweise Urheberschein der UdSSR Nr. 247454). die mindestens zwei Speichenpaare einschliessen. Die Enden der Speichen jedes Paars sind in einem oder zwei Bügeln befestigt, die miteinander derart verbunden werden, dass jedes Speicherpaar mit den Bügeln ein starres Gefüge bildet. Die Speichen in jedem Gefüge sind durch einen der gegenüberzustellenden Knochen hindurchgezogen. Die genannten Gefüge stehen mit Hilfe von Distanzgliedern miteinander in Verbindung. Die Distanzglieder sind an den beiden starren Gefügen befestigt.
Die Distanzglieder weisen Einschnitte auf und sie sind mittels auseinanderstellbarer Muttern miteinander verbunden, die eine Veränderung des Abstandes zwischen den durch die Distanzglieder verbundenen Bügeln ermöglichen.
Die beiden starren Gefüge sind durch eine zusätzliche Gewindekupplung miteinander verbunden, die unter einem Winkel zu der Längsachse des Geräts angeordnet ist und zur seitlichen Kompression der Knochenbruchstücke dient. Das Anlegen des Geräts beginnt mit dem Ilindurchstechen der Speichen durch die zusammenzustellenden Knochen. Nach diesem Hindurchziehen von je zwei Speichen durch jedes Knochenbruchstück, werden die Speichen gespannt und in den entsprechenden Bügeln befestigt.
Nachdem das Gerät angelegt ist, kann die Reposition der Knochenbruchstücke beginnen. Zur Beseitigung einer Knochenversetzung in der Längsrichtung werden die auseinanderstellbaren Muttern der Distanzglieder gedreht. Zwecks Vermeidung einer seitlichen Versetzung wird die Mutter der Geviindekupplung gedreht.
Das bekannte Gerät bietet jedoch nicht die Möglichkeit, eine vollkommene Reposition der zusammenzustellenden Knochenbruchstücke zu erreichen, da es nicht ermöglicht, die seitlichen Versetzungen der Knochen in der Frontal- und Sagittalebene des menschlichen Körpers zu beheben.
Der Erfindung liegt die Aufgabe zugrunde, ein Gerät zur chirurgischen Behandlung der Knochenbrüche und -erkrankungen zu entwickeln, das eine genaue und dosierte Reposition der Knochenbruchstücke in allen Ebenen unter deren fester Halterung während des Zusammenwachsens ermöglicht.
Diese gestellte Aufgabe wird erfindungsgemäss dadurch gelöst, dass im Bereich der Bügelenden mindestens zweier benachbarter Bügel mindestens zwei je Bügel zueinander parallele. gegenüber jenen des anderen Bügels senkrechte und in den Ebenen der Bügel verlaufende Führungsnuten ausgebildet sind, in welchen mit einer Einrichtung zu ihrer Verstellung und Festlegung in der Führungsnut versehene Gleitstücke angeordnet sind, welche mit den Enden von Distanzgliedern verbunden sind.
Die mit den Gleitstücken verbundenen Distanzglieder können in ihrer Mitte Kardangelenke aufweisen.
Die Mittelteile der den gegenüberliegenden starren Gefügen gehörenden Bügel können durch ein Distanzglied verbunden sein, das in seiner Mitte ein Kardangelenk enthält.
Das erfindungsgemässe Gerät besitzt mehrere Gebrauchsmöglichkeiten im Vergleich zum genannten bekannten Gerät. Es ermöglicht die Durchführung einer genauen Reposition der Knochenbruchstücke bei frischen und auch bei veralteten Brüchen, und zwar auch mit deren starren Festlegung in der Kompressions- oder Distraktionsstellung. Bei Strecken der Extremität in die Länge, bietet das Gerät die Möglichkeit, eine genaue Zentrierung der Knochenbruchstücke während der ganzen Zeit dieses Streckens aufrechtzuerhalten.
Das Gerät ermÏglicht die Durchführung der Kompression der Knochenbruchstücke nicht nur in Längsrichtung bei Querbrüchen. sondern noch über die Breite bei Schräg- und Schraubenbrüchen.
Nachstehend wird ein Ausführungsbeispiel der Erfindung anhand der beiliegenden Zeichnungen näher erläutert. Es zeigen:

Das genannte Gerät zur chirurgischen Behandlung der Gelenkbrüche und -erkrankungen enthält drei Bügel 1, 2, 3.
(Fig. 1). Die Bügel 1 und 2 sind durch drei Distanzschrauben 4 4starr miteinander verbunden. In diesen Bügeln 1 und 2 sind Speichen 5 bzw. 6 und im Bügel 3 zwei Speichen 7 und 8 festgeklemmt. Die Speichen 5, 6 mit den Bügeln 1, 2 bilden ein starres Gefüge, das an einem Knochenbruchstück befestigt wird. Die Speichen 7, 8 bilden mit dem Bügel 3 ein anderes starres Gefüge. das an einem anderen Knochenbruchstück befestigt wird. Die beiden starren Gefüge sind miteinander über zwei Distanzglieder 9 verbunden. Die Rükkenteile der Bügel 2 und 1 können auch durch ein Distanzglied 10 in Verbindung stehen. Die Distanzglieder 9 und 10 weisen in ihrem Mittelteil je ein Kardangelenk 11 auf. Jedes Kardangelenk 11 schliesst zwei einachsige Gelenke 12 und 13 mit zueinander senkrecht liegenden Achsen ein.
Zur Ver änderung des Abstandes zwischen den Bügeln 2 und 3 und demzufolge auch zwischen den im Gerät festgehaltenen Knochenbruchstücken sind die Distanzglieder 9, 10 mit Schraubengewinde versehen und werden an den Bügeln 2, 3 mittels auseinanderstellbarer Muttern 14 befestigt.
Der Bügel 3 enthält im Bereich seiner Enden liegende Führungsnuten 15 (Fig. 2). Jede Führungsnut 15 liegt in der Ebene des Bügels 3. In der jeweiligen Führungsnut gleitet ein Gleitstück 16, durch dessen Bohrung das mit ihm ver hunden Distanzglied 9 hindurchläuft. Als Antriebsmittel zur Verstellung des Gleitstücks 16 in der Führungsnut 15 und zur Halterung des Gleitstücks 16 an einer erforderlichen Stelle dient eine Leitapindel 17.
Der Bügel 2 (Fig. 1), der dem Bügel 3 gegenüberliegt und dem anderen starren Gefüge gehört, weist im Bereich seiner Enden zwei Führungsnuten 18 (Fig. 3) auf, die in der Ebene des Bügels 2 liegen und zu den Führungsnuten 15 (Fig. 2) senkrecht verlaufen.
In jeder Führungsnut 18 (Fig. 3) gleitet ein Gleitstück 19.
durch dessen Bohrung jeweils ein mit ihm verbundenes Distanzglied 9 hindurchläuft. Als Antriebsmittel zur Verstellung des Gleitstücks 19 in der Führungsnut 18 und zur Halterung des Gleitstücks 19 an einer erforderlichen Stelle dient eine Leitspindel 20.
Die Verstellung der Gleitstücke 16 (Fig. 2) und 19 (Fig. 3) ermöglicht die Verschiebung beider starren Gefüge, bestehend aus den Bügeln und den Speichen, in zwei zueinander senkrechten Richtungen.
Das Gerät zur chirurgischen Behandlung der Knochen brüche und -erkrankungen arbeitet wie folgt.
Durch ein Knochenbrnchstüd: werden die Speichen 5 und 6 (Fig. 1) hindurchgezogen, sie werden gespannt und in den Bügeln 1 bzw. 2 festgeklemmt. Durch das andere Knochen bruchstück werden noch zwei Speichen 7 und 8 hindurchge zogen, sie werden gespannt und im Bügel 3 fixiert.
Nach dem erfolgten Anlegen des Geräts und der Erfassung der Knochenbruchstücke kann nun die Reposition dieser I(nochenbruchstücke vorgenommen werden. Zur Beseitigung der Verschiebung der Knochenbruchstücke in der Längsrichtung werden die auseinanderstellbaren Muttern 14 aller Distanzglieder 9 und 10 gedreht. Falls auch die Winkelverschiebung der Knochenbruchstüde in der Frontalebene des Körpers vorliegt, so wird die Mutter 14 des Distanzgliedes 9 auf der Seite des spitzen Winkels mehr gedreht. Während der Behebung der Winkelverschiebung der Knochen bruchstücke in der Frontalebene werden die Gelenkverbin- dungen 12 der Distanzglieder 9 und 10 etwas gelöst, wodurch diese Behebung leichter vor sich geht.
Nach der Beseitigung der genannten Verschiebungsart werden die genannten Ge lenkverbindungen 12 wiederum unbeweglich gemacht.
Zur Behebung der Winkelverschiebung in der Sagittalebene des Körpers dreht man an den auseinanderstellbaren Muttern 14 des Distanzgliedes 10. Während der Behebung dieser Verschiebung werden nun die Gelenkverbindungen 13 der Distanzglieder 9 und 10 etwas gelöst und nach der Behebung der Verschiebung wiederum angezogen.
Die Verschiebung der Knochenbruchstücke über die Breite wird durch Verstellung der Gleitstücke 16 (Fig. 2) und 19 (Fig. 3) beseitigt. Zur Beseitigung der Breitenverschiebung der Knochenbruchstücke in der Frontalebene des Körpers wird die Leitspindel 20 (Fig. 3) gedreht. Die Breitenverschiebung der Knochenbruckstücke in der Sagittalebene des Körpers wird durch die Drehung der Leitspindel 17 (Fig. 2) beseitigt.
Auf diese Weise ermöglicht das vorliegende Gerät eine schrittweise Reposition der Knochenbruchstücke mit deren nachfolgender Festlegung in der Kompressionsstellung durchzuführen, was das Zusammenwachsen der Knochenbruchstücke während einer kürzeren Zeit begünstigt.

Such devices can be used in orthopedics and traumatology for the repositioning of broken bones with their subsequent holding in the compression position to lengthen a shortened bone and for compression arthrodesis.
Devices for the surgical treatment of bone fractures and diseases are generally known (see, for example, copyright of the USSR No. 247454). which include at least two pairs of spokes. The ends of the spokes of each pair are fixed in one or two brackets, which are connected to one another in such a way that each storage pair forms a rigid structure with the brackets. The spokes in each structure are drawn through one of the opposing bones. The structures mentioned are connected to one another with the aid of spacers. The spacers are attached to the two rigid structures.
The spacer members have incisions and they are connected to one another by means of separable nuts which allow the distance between the brackets connected by the spacer members to be changed.
The two rigid structures are connected to one another by an additional threaded coupling which is arranged at an angle to the longitudinal axis of the device and which serves to compress the bone fragments laterally. The application of the device begins by piercing the spokes through the bone to be assembled. After pulling two spokes through each bone fragment, the spokes are tensioned and fixed in the corresponding brackets.
After the device is on, the reduction of the bone fragments can begin. To eliminate a bone dislocation in the longitudinal direction, the disengageable nuts of the spacer members are turned. The nut of the Geviindekupplung is rotated to avoid a lateral displacement.
However, the known device does not offer the possibility of achieving a complete reduction of the bone fragments to be assembled, since it does not make it possible to correct the lateral displacements of the bones in the frontal and sagittal planes of the human body.
The invention is based on the object of developing a device for the surgical treatment of bone fractures and diseases, which enables precise and metered repositioning of the bone fragments in all planes while maintaining them firmly as they grow together.
This object is achieved according to the invention in that in the region of the temple ends at least two adjacent brackets are at least two per bracket parallel to one another. compared to those of the other bracket vertical guide grooves extending in the planes of the bracket are formed, in which sliding pieces provided with a device for their adjustment and fixing in the guide groove are arranged, which are connected to the ends of spacer members.
The spacers connected to the sliders can have cardan joints in their center.
The middle parts of the stirrups belonging to the opposite rigid structures can be connected by a spacer element which contains a universal joint in its center.
The device according to the invention has several possible uses compared to the known device mentioned. It enables the implementation of a precise repositioning of the bone fragments with fresh and also with old fractures, even with their rigid fixation in the compression or distraction position. When extending the limb in length, the device offers the possibility of maintaining precise centering of the bone fragments throughout this extension.
The device enables compression of the bone fragments not only in the longitudinal direction in the case of transverse fractures. but also across the width for diagonal and screw breaks.
An exemplary embodiment of the invention is explained in more detail below with reference to the accompanying drawings. Show it:

The mentioned device for the surgical treatment of joint fractures and diseases contains three brackets 1, 2, 3.
(Fig. 1). The brackets 1 and 2 are rigidly connected to one another by three spacer screws 4 4. In these brackets 1 and 2, spokes 5 and 6 and in bracket 3 two spokes 7 and 8 are clamped. The spokes 5, 6 with the brackets 1, 2 form a rigid structure that is attached to a bone fragment. The spokes 7, 8 form a different rigid structure with the bracket 3. that is attached to another piece of bone. The two rigid structures are connected to one another via two spacers 9. The back parts of the bracket 2 and 1 can also be connected by a spacer member 10. The spacers 9 and 10 each have a cardan joint 11 in their middle part. Each universal joint 11 includes two uniaxial joints 12 and 13 with axes perpendicular to one another.
To change the distance between the brackets 2 and 3 and consequently also between the bone fragments held in the device, the spacers 9, 10 are provided with screw threads and are attached to the brackets 2, 3 by means of nuts 14 that can be separated.
The bracket 3 contains guide grooves 15 located in the region of its ends (FIG. 2). Each guide groove 15 lies in the plane of the bracket 3. In the respective guide groove, a slider 16 slides, through the bore of which the spacer member 9 ver dogged with him runs. A guide pin 17 serves as the drive means for adjusting the slider 16 in the guide groove 15 and for holding the slider 16 at a required location.
The bracket 2 (Fig. 1), which is opposite the bracket 3 and belongs to the other rigid structure, has two guide grooves 18 (Fig. 3) in the area of its ends, which lie in the plane of the bracket 2 and to the guide grooves 15 ( Fig. 2) run vertically.
A slider 19 slides in each guide groove 18 (FIG. 3).
a spacer member 9 connected to it runs through its bore. A lead screw 20 serves as the drive means for adjusting the slide 19 in the guide groove 18 and for holding the slide 19 at a required location.
The adjustment of the sliders 16 (Fig. 2) and 19 (Fig. 3) enables the displacement of both rigid structures, consisting of the brackets and the spokes, in two mutually perpendicular directions.
The device for the surgical treatment of fractures and diseases works as follows.
Through a broken bone: the spokes 5 and 6 (Fig. 1) are pulled through, they are tensioned and clamped in the brackets 1 and 2 respectively. Two spokes 7 and 8 are pulled through the other bone fragment, they are stretched and fixed in the bracket 3.
After the device has been put on and the bone fragments have been captured, these fragments can now be repositioned. To eliminate the displacement of the bone fragments in the longitudinal direction, the mutually adjustable nuts 14 of all spacers 9 and 10 are turned. If the angular displacement of the bone fragments is also in If the frontal plane of the body is present, the nut 14 of the spacer member 9 is turned more on the side of the acute angle Fixing is easier.
After eliminating the aforementioned type of displacement, the aforementioned Ge articulated connections 12 are again made immobile.
To correct the angular displacement in the sagittal plane of the body, turn the separable nuts 14 of the spacer member 10. While this displacement is corrected, the articulated connections 13 of the spacer members 9 and 10 are loosened somewhat and tightened again after the displacement has been corrected.
The displacement of the bone fragments across the width is eliminated by adjusting the sliding pieces 16 (Fig. 2) and 19 (Fig. 3). The lead screw 20 (FIG. 3) is rotated to eliminate the shift in width of the bone fragments in the frontal plane of the body. The shift in width of the bone fragments in the sagittal plane of the body is eliminated by rotating the lead screw 17 (FIG. 2).
In this way, the present device enables a step-by-step reduction of the bone fragments to be carried out with their subsequent fixing in the compression position, which promotes the growing together of the bone fragments over a shorter period of time.
