Abstract
The hip joint prosthesis which is assisted by electro stimulation by means of a positive connection (7) on the ball portion and a negative connection (6) near the femur (4) has a ball joint consisting of a spherical headed portion (1) and a stem portion (2) the latter being inserted down the femur, with a gap between the inside of the spherical part and the spigot (8) on the top of the stem part, which is filled with a cement (3), the two parts being made of a cobalt/nickel/chromium/molybedenum alloy. The negative connection is made on a plate (5) through a pair of screws (5a) passing through it, in contact with the stem (2).
Description
Die Erfindung betrifft eine neue zweiteilige Hüftgelenk Endoprothese, die dadurch gekennzeichnet ist, dass sie durch Elektrostimulation eine permanente Verbindung mit dem Femurknochen eingeht.
Die bis jetzt bekannten Hüftgelenkprothesen, die verschiedene Formen aufweisen, wirkten nur biomechanisch.
Zweck der Erfindung ist, neben einer rein biomechanischen Wirkung durch Elektrostimulation die Festigkeit und Zähigkeit der Verbindung der Prothese mit dem Knochen dauernd zu erhalten.
In der Zeichnung ist eine Ausführungsform der erfindungsgemässen Prothese dargestellt.
Wenn man den Zwischenraum zwischen dem Zapfen 8 und der Gelenkkugel 1 mit elektrisch isolierendem Knochenzement ausfüllt, kann man durch Anschluss der Gelenkkugel 1 an den positiven Pol (die Anode) einer Gleichstromquelle und des Schaftteiles 2 an den negativen Pol (die Kathode) der Gleichstromquelle einen elektrischen Strom mit einer Stromstärke von 50 mA in den ersten 24 Stunden und von 15 bis 20 mA in den nächsten 20 Tagen, d. h. mit Stromstärken im rein physiologischen Bereich, fliessen lassen. Dadurch wird auf der Oberfläche der Prothese durch Elektrophorese langsam ein Knochenbelag gebildet. Da die Oberfläche der Prothese uneben oder rauh ausgebildet ist, ergibt sich durch die Ober flächenvergrösserung eine bessere Verankerung im Knochen.
Im Falle einer Osteoporose des elektrophoretisch gebildeten Knochens (z. B. bei Diabetik-Acidosis) kann die Elektrostimulation des Schaftteiles der Prothese wiederholt werden.
Dadurch kann eine Restabilisierung der Prothese ohne operativen Eingriff erreicht werden. Im Falle eines Prothesenwechsels auf Grund von Brechen oder Amortisation der Prothese ist ein erneutes Verbinden des Schaftteils der Prothese, diesmal aber mit dem positiven Pol der Gleichstromquelle, not wendig. Dadurch wird eine Enveichung des elektrophoretisch gebildeten Knochens bewirkt und der Prothesenwechsel erleichtert.
Beide Teile der Prothese sind aus einem Stahl gefertigt, der der schweizerischen Norm für chirurgische Implantate (SNV 129) entspricht.

If you fill the space between the pin 8 and the joint ball 1 with electrically insulating bone cement, you can connect the joint ball 1 to the positive pole (the anode) of a direct current source and the shaft part 2 to the negative pole (the cathode) of the direct current source electrical current with a current strength of 50 mA in the first 24 hours and from 15 to 20 mA in the next 20 days, i.e. H. let flow with currents in the purely physiological range. As a result, a bone coating is slowly formed on the surface of the prosthesis by electrophoresis. Since the surface of the prosthesis is uneven or rough, the surface enlargement results in better anchoring in the bone.
In the case of osteoporosis of the electrophoretically formed bone (e.g. in the case of diabetic acidosis), the electrostimulation of the shaft part of the prosthesis can be repeated.
In this way, the prosthesis can be restabilized without surgery. In the event of a replacement of the prosthesis due to breakage or amortization of the prosthesis, it is necessary to reconnect the shaft part of the prosthesis, this time to the positive pole of the direct current source. This causes the electrophoretically formed bone to soften and the replacement of the prosthesis is made easier.
Both parts of the prosthesis are made of a steel that complies with the Swiss standard for surgical implants (SNV 129).
