Medical control device.
The present invention relates to a medical monitoring device for studying the propagation of blood pulses in the organism, intended to enable the practitioner, in diagnosis and therapy, to detect, monitor and verify the physiological and biological reactions present. a patient to disease and treatment.
When the application of a medical treatment, in whatever form it is presented, and for example a medical treatment by hertzian waves better known under the name of d'Arsonvalisation, modifies the physicochemical characteristics of the blood, the lymph , interstitial tissues, etc., especially when it causes changes in glandular secretions and in nerve excitations, the operating characteristics, more particularly the mechanical characteristics of the arterial network can be modified as is well known for many cases that those skilled in the art will appreciate immediately.
These changes may be due to contractions or dilations of blood vessels, changes in stiffness of the arterial walls, changes in permeability of interstitial tissue and capillary vessels.
Such modifications can be interpreted by considering their association with relative modifications of parameters characterizing the blood pulsations which propagate in a treated region of the organism compared to those which characterize the pulsations which propagate in another region of the organism. , unaffected or affected with a different dewlap by this treatment. It will be the same in the case of a disease affecting two different regions of the body with different baleen. The parameters to be considered are the speed, the shape and the duration of the pulsations.
Thus, if a therapeutic application modifies, for example, characteristics of the blood network supplying a right portion of the organism, the observation of the parameters of the blood pulsations of this portion, and the simultaneous observation of the corresponding parameters in another portion, then left for example, of the organism, will make it possible to highlight such modifications; if, moreover, these same parameters are observed in a region situated in the immediate vicinity of the heart, it will become possible to make an advantageous synthesis of these three series of observations.
The object of the invention is to provide a device making it possible to carry out these three series of observations simultaneously while making the results appear in a direct and recordable visual form. It also aims to provide this device such that it delivers the visual indications in a form involving an automatic comparison of the parameters of the blood pulsations in two distinct regions of the organism and, moreover, in a form such that it facilitates the interpretation of these indications, in particular by making it possible to vary the presentation.
The medical control device according to the invention, for the study of the propagation of blood pulses in the organism, is characterized in that it comprises a first oscilloscopic indicator and a second oseilloseopic indicator, two sampling organs of two symmetrical pulse of the body providing, after energy conversion. two series of pulses respectively applied to the vertical and horizontal deflection circuits of the first oscilloscopic indicator, at least one of these series of pulses being also applied to a deflection circuit of the second oscilloscopic indicator,
and a third member for sampling a hair close to the heart supplying, by energy conversion, a series of pulses applied to a circuit for triggering a time base of the second oseilloscopic indicator.
By oseilloscopic indicator is meant any oscilloscope forming on an observation screen im spot lllminous which moves there by at least double deflection controls. Since the phenomena to be observed are of an essentially transient nature, although reproducing with each pulse or excitation, such an oscilloscopic indicator will preferably incorporate a cathode ray tube with long screen persistence, associated with the usual deflector circuits of a cathode spot in at least two directions.
The observation screens of the two oscilloscopic indicators of the device could be placed side by side or one above the other, or even be arranged orthogo nana, the vision being effected by means of a semi-mirror. transparent placed between them at 450.
By pulse sampling device is meant any organ, mechanical (Marey's capsule, sphygmometer, plethysmometer), electrical (electrodes), or electromechanical (microphone, piezoelectric element), which delivers pulses of a physical nature. in response to the biological or physiological excitatory impulses collected.
More particularly, as regards sampling the two symmetrical pulses, two sphygmometers will preferably be used, while the sampling of the third pulse, in the vicinity of the encoder, will be carried out using a microphone.
The pulses thus collected and already transformed by the very nature of the sampling devices are converted into oscillations of the same physical nature, here in oscillations of electric current, if necessary by addition of mano-electric converters, to modulate alternating electric currents controlling the deflector circuits of the oscilloscopic indicators.
It should be noted that, on the first oscilloscopic indicator, comparing the two symmetrical pulses of the organism, there appears a plot giving the phase relation in time of the two pulsations, while the plot which appears on the second oscilloscopic indicator translates the amplitude relation over time of either of these pulsations during the interval of the corresponding heartbeat.
The appended drawing represents, by way of example, an embodiment of the device forming the subject of the invention:
Fig. 1 is a partial diagram of the device,
fig. 2 a diagram of the delay elements entering into the constitution of the device of FIG. 1.
In fig. 1, the first osilloscopic indicator of the device has been indicated at O.
This oscilloscope O has its deflection control circuits driven by. pulses coming from amplifiers H and V, themselves excited by the sampling members D and G, respectively, through organs or delay lines RR and RF. The sampling member D is intended to be applied to a right pulse of the organism and the organ G, to a left pulse symmetrical to the right pulse (both wrists or both instep, for example).
So that the control device can be used even in the case of treatments by Arsonvalisation, these two organs D and 61 consist of Zlare capsules or sphygmometers and are connected by rubber tubes to mano-electric converters, piezoelectric for example, which avoids the presence of conducting wires in the vicinity of the high frequency field created by the Arsonvalisation current generator.
The output current of each set D and G feeds an amplifier of very low frequencies, H and V, respectively. The amplifier g supplies the deflection or horizontal deflection circuit of the oscilloscope-copic indicator O, the amplifier V, the vertical deflection circuit of this indicator. The horizontal time base therefore reproduces the oscillations of the patient's right pulse, the evolution of the right blood pulse serving as a reference along the abscissa on a scale going to the right of the screen. The left pulse oscillations will be reproduced in ordinates increasing from bottom to top of the screen. A new figure will be drawn on the screen during the time of each heartbeat.
The output of the very low frequency amplifier of one of the channels, H or V, is, on the other hand, bypassed to supply the vertical deflection circuit of the second oscilloscopic indicator (not shown). A third pulse, close to the level of the tip of the heart, is preferably taken by a third sampling member, such as a microphone and the output of this microphone is also applied to a very low frequency amplifier, which controls synchronization of the generator of the borizontal time base of this second oscilloscope indicator.
This is ensured of a dewlap known per se in the art, by the provision for this time base of a relaxation generator to which the pulses picked up by the third organ serve as trigger pulses. In this way, a plot is obtained reproducing the variations in the amplitude of the right or left pulsation applied to the vertical deflection circuit as a function of time in the interval of the corresponding heartbeat. This second plot facilitates the identification of the various portions of the first and makes it possible to monitor the proper shape of the pulsation applied to the vertical deflection circuit.
It can also be noted that it is possible, if desired, to display the two plots of the right and left pulsations as a function of time on the screen of this second oscilloscope indicator by deriving the two outputs of the amplifiers H and V on the two inputs of an electronic switch whose output then feeds the vertical deflection circuit of the second indicator.
In the first indicator, the ideal theoretical case, where the two fractions, right and left, of a pulse would be identical and strictly synchronous, would be indicated by the displacement of the spot of the oscilloscope along a line whose inclination is a function of the amplitude ratio.
If a disturbing cause, for example the appearance of a field of high frequency of treatment and its reaction on the organism in a localized location in the left region, slows the propagation of the blood pulses to the left pulse taken, the pulse right will cause the appearance of a line starting with a line close to the horizontal in the direction from the left to the right of the screen from the origin of the scan; then the appearance of the delayed pulsation in the left pulse will manifest itself by the rise of the cathode spot; From this moment on, a complex figure will be drawn on the screen as a function of the relative positions of the undulations which characterize the two compared pulsations.
On the other hand, if the therapeutic agent accelerates the speed of propagation of the blood pulsation between the heart and the left pulse to the point of preceding the right pulsation by the left pulsation, the drawing of the figure is carried out in the opposite direction of travel. , that is to say starting with a rise of the spot on the screen, substantially vertical, until the appearance of the right-hand pulse which makes the plot oblique to the right of the screen. It is understood that, in all cases, the magnitude of the shift can considerably modify the appearance of the plotted figure.
To allow, moreover, a measurement of the offset in time either of the origins or of particular portions of the two pulses compared on the first indicator, provision has been made for the insertion, between the amplifiers H and V and the deflection control circuits respective RR and RP delay units. The delay introduced by the element RR is adjustable, that introduced by RF is fixed, or may only be adjustable, i.e. kept fixed once determined.
By introducing an adjustable delay, the operator will be able to obtain a figure comprising a line segment, more or less extended, either from the origin of the scan, or during a privileged portion of the plot, which then continues. by a complex curve resulting from the composition of the different parts of the pulsations.
Reading this delay, adjusted by button B, associated with a reading dial directly graduated in time (fractions of a second), provides a measure of the order of magnitude of the shift between these partial pulses; the measurement of the inclination of the straight segment obtained provides the order of magnitude of the ratio of their amplitudes and thus reveals the variations of this factor under the effect of a disturbing cause; examining the remainder of the complexly shaped image provides an indication of the relative positions of the various remaining pulsation ripples. In the pulse measurements, it is the first expansion of the reference pulsation which can be used to obtain the inclined segment shown in fig. 1, the rest of the complex curve not being shown.
In principle, it would suffice to introduce a single, adjustable delay in only one of the deflection control channels. It is however preferable to introduce a delay in the two channels to reduce the causes of error or of difficulty of construction vis-à-vis weak shifts: when the two pulsations are close to synchronism, there may also be a tendency in advance than in relative delay. I1 is advantageous to provide the organ
Adjustable RF to allow an initial reset of the reading to correct various errors
Such delay elements or members can be established in various ways, all well known in the art (electrical or hydraulic circuits constituting delay lines, for example).
Fig. 2 shows how the delay lines RR and RF of FIG. 1.
A motor 11f at a constant speed of, for example, one revolution per second, drives by its transmission shaft T two plates or plates of mild steel P1 or P2.
Electromagnetic inscribers I1 and 12 are respectively associated with these plates and cause the recording of the pulses exiting the amplifiers H and V. Readers L1 and L2 retranslate these inscriptions into control pulses of the corresponding deviation circuits of the oscilloseopic indicator. .
After reading, magnetic erasers
E1 and E2 ensure that the location previously assigned to registration on each board is shown blank in front of the registrant. The delay time of each set is obviously determined by the angular difference between the inserter I and its associated reader L, the motor running at constant speed.
The adjustable delay of the line PR is controlled by the button B1 which moves the reader L1 around the circumference of the plate P1. The adjustable delay RF is controlled by button B2, the latter displacement being of much less ampli tilde since it is only a resetting.
These recording, reading and erasing elements need not be described in detail, being well known in the art of magnetic recording.
Such an adjustable delay arrangement can also be introduced into the control of the second oscilloscopic indicator, more particularly when the vertical deflection circuit of this indicator is, as mentioned, controlled from an electronic switch. It then makes it possible to bring back to the same time coordinate the amplitudes which relate to corresponding parts of the two amplitude-time curves of the partial pulsations, which makes it possible to better demonstrate the nature of the changes in the shape of the pulsations between two locations of the blood network.
A change in the setting of the RR delay circuit of this second control causes the lateral displacement of the trace of the pulse thus delayed in its action vis-à-vis the other, which allows the operator to create a superposition throughout. less partial of the two plots, assuming that it also has the usual means for adjusting the amplitude of the deviations. This arrangement is of particular importance for the demonstration of modifications occurring in the state of the capillary network, which result in a variation in the curvature of the descending part, of exponential appearance, from the return to rest after passage of the pulsatile wave. It is also useful for comparing the ascending parts of pulsations, the shape of which can reflect the state of the arteries upstream of the sampling points.
Of course, in this second oscilloscopic indicator, one could be satisfied, instead of an arrangement with delay lines, of shifting the horizontal time bases, if in an alternative embodiment, this second indicator was taken of a type with double cathode beam, each beam being controlled in its horizontal deflection by a distinct time base, and in its vertical deflection, by each of the voltages of the symmetrical pulses, respectively.
It is also possible, if desired, to make a trace appear on the first oscilloscopic indicator which corresponds to the oscillations collected at the two symmetrical pulses following the oscillations of the neighboring pulse of the heart. The modification of the circuits consists in providing three circuits for deviating the spot of this first indicator, therefore in defining three scanning axes instead of two, these three axes being able to
gladly be established at 1200 from each other.
The three outputs of the three amplifiers
low frequencies of the device control
respectively the three deflection circuits
by direct supply or electronically
switched in time sequence. An advantageous arrangement then consists in
bling the indicator so that a ba
vertical layage is controlled by the pulsa
of the pulse near the encoder and that
the two sweeps inclined on either side
to 1200 are controlled respectively by
the other two pulses: the figure obtained is
to appear an origin common to the pulses
symmetrical, which facilitates the point of view
medical interpretation of the tracing because
the direction of the apparent displacements is linked to
the succession of pulses over time.
When also desired, we can establish
the diversion circuits of one and the other
oscilloscopic indicators to allow
be im spotting in direct reading of inter
absolute time values. Just
for this to establish these circuits, as well
known in the art, so that the length
and the duration of the linear sweeps are fixed,
for example so that the duration is of the order
half a second in the case of observations
pulsations in the blood, the spot returning to
its point of origin and remaining there until
the moment when the next pulse appears,
moment of origin of a new ba
layage by electrical release of the circuits
sweep.
Such a provision removes
obviously the end of the lines, but ensures the
direct reading marking of intervals of
time; it can also be optional
ment used by the introduction of a commu
manual switch on and off of the
devia circuit blocking generator
tion.
To further facilitate the identification of
two corresponding portions of the plots ob
held on the oscilloscopic indicators, a
auxiliary relaxation oscillator or a cir
baked one pulse retarder from the base
time of the second oscilloscopic indicator may be incorporated into the device.
The pulse thus selected is applied to luminosity control electrodes (Wehnelt for example) and this pulse simultaneously applied to the two cathode-ray tubes will then control in time correspondence a variation in luminosity during its duration of application, for example a reinforcement. or a decrease in luminosity or even a dotted tragedy of a portion of the two curves if this impulse is applied by means of a vibrator.
The choice of the portion of the traces to be differentiated in this way is preferably made relative to the setting of the second oscilloscopic indicator since the control pulse will preferably be derived from its synchronized time base. The adjustment is ensured by that of the delay circuit, of this pulse or that of the activation delay of the auxiliary relaxation oscillator from the triggering of the main relaxation oscillator.
By this means, corresponding portions of the traces can be simultaneously identified, for example to correspond to the ripple generated by the dicrotism of the pulse.
Finally, it should be noted that, since the device shows traces on oscilloscope screens, the recording of these traces is direct by simple photography of these persistence screens. However, the very formation of the traces provides the observer with a visual impression of which certain elements such as direction and speed no longer appear on the recordings.
To preserve these elements of information, if desired, it is then possible to resort to means, known in the oscillographic technique, consisting in applying modulation voltages to the modulation and brightness control electrodes of the spot (for example cathodes or Wehnelts). such that the corresponding line appears in dotted lines with variable shape and brightness in a determined direction as it develops. This can be achieved in a simple way, for example, by applying to the Wehnelts of the oscilloscope tubes a fraction of the AC component which appears at the terminals of the impedance constituting the input of the high voltage filter through which the device is supplied.
