Description
The present invention is within the scope of medical technologies and electronic instrumentation, aimed at the medical clinic area, that is, a videolaryngoscope, which consists of a new apparatus, for carrying out intubations, with the aid of imaging, through of a design, which allows it to be smaller, lighter and integrated with other image presentation devices, which makes it a simpler and more practical system than its peers currently available on the market.
The systems allow visualization of the epiglottic and glottic region, facilitated by a camera and light emitter, developed in the 1990s, where we can cite patent documents US 5,973,728 of October 1999 which demonstrates a system for visualizing the region of epiglottis, however, it still uses a Traditional manual Laryngoscope to perform the intubation procedure. This is a complex device with several parts that interferes with the field of vision of the professional who is performing the intubation procedure. Patent document US 8,029,440 B2 of October 2011, which demonstrates a similar device, with the same function of viewing the patient's internal cavities, and can also be used with a Traditional Laryngoscope, however the device consists of two parts, one of which one can undergo a sterilization process and the other cannot, which makes the sterilization process complicated, necessary for procedures with equipment that is inserted into human body cavities. The product developed has only one main part, which avoids accumulation of secretions in areas such as gaps between the two parts of the device. The product presented here is also different in terms of design, without sharp corners that could, in a situation of emergency, lead to injuries to the inside of the patient's mouth. An advantage of the Tracel video laryngoscope is that it does not have such sharp edges, which avoids injuries due to sudden movements of the patient during emergency care, a condition inherent to emergency intubation procedures, with the need for mechanical ventilation to keep the patient's clinical condition stable. patient. In document US 9,351,633 B2 of May 2016, the Video Laryngoscope has image-assisted visualization, however the device can only be used using a cable connected to its own camera, the device presented now has different dimensions to provide better visualization and guiding the tube when performing the procedure. This condition has had its safety and effectiveness proven through studies with patients at the Gaffrée Guinle university hospital at the Federal University of the State of Rio de Janeiro (UNIRIO) and in hospitals in the city of Maricá, meeting the requirements for intubation. By integrating the system with the doctor's smartphone, it is possible to visualize the epiglottis with image transmission via wireless connection, this means allows greater availability of the equipment to perform the procedure by reducing points of failure. In US patent 0137376 of May 2021, the video endoscopy imaging technique and the use of instruments are common and arranged in several devices as in US Patent 0338581 of 2016 or US patent 0258311 of 2017, with imaging assistance for intubation of patients, already known and widely used in the medical field, however ours differs due to the unique format in which it is not necessary to have several blades to serve a varied group of patients, the wireless connection, the arrangement or not of the channel to direct the tube endotracheal system (CUFF), in the selection of the material, a resistant and low-cost plastic polymer was chosen, and in the simplification of components, dispensing with screens and integrated visualization devices, allowing the separation of the sterilized part and the majority of the electronics, allowing the camera was subjected to liquid disinfection procedures, with less risk of cross-contamination, and a significant reduction in the final production cost, making this device accessible to a greater number of hospitals and doctors.
Being aware of the state of the art, the use of laryngoscopes with optical arrangements, to allow the professional to obtain a clear view during endotracheal intubation. In particular, a video camera is usually incorporated into the blade of a laryngoscope to transmit images on a screen or monitor during the intubation procedure, whether the monitor is distant or connected to the laryngoscope.
Sendo usualmente um dispositivo grande e de difícil manuseio pelo profissional de saúde.4- The endotracheal intubation procedure consists of a procedure that allows invasive ventilation, or the introduction of tubes with inspection or actuation instruments into the patient's digestive system, allowing a series of medical procedures to be carried out after the patient's intubation. This is a procedure that is part of the daily routine of anesthetists, emergency doctors and other intensive care professionals. It consists of using an instrument, called a laryngoscope, with a shape that allows the tube to be inserted to be rotated to the correct cavity that accesses the patient's lung or stomach, depending on the procedure to be performed. Devices that have image assistance usually have, in addition to the mechanical part that supports the tube to be introduced, an electronic device, powered by batteries, coupled to a display or monitor that allows viewing the procedure. It is usually a large device that is difficult for healthcare professionals to handle.
Existe um risco na intubação endotraqueal de provocar ferimentos à região glótica, tornando necessário realizar intubações esofagianas.5- A laryngoscope is used in a series of emergency medical procedures, necessary for the treatment of various diseases, especially serious and highly contagious respiratory diseases, in intensive care units, and even serves as an emergency support instrument in rescue units. It is also an instrument used during epidemics, especially those involving respiratory diseases, thus representing a relevant use by society. There is a risk of endotracheal intubation causing injuries to the glottic region, making it necessary to perform esophageal intubations.
In the case of highly contagious epidemics such as that caused by covid 19, or corona virus, distancing the medical professional from the patient is essential to avoid contamination. The equipment developed and as described in this patent application, allows distancing from the patient, and by separating the mechanical, lighting and video system from the viewing system (which could be a smartphone, monitor or other means of viewing the generated image) guarantees a lower cost per device, which allows the use of one device per patient in an intensive care unit, as the simplified equipment as presented here has a lower cost and additional protection for healthcare professionals by avoiding contamination by proximity, necessary for visualize the glottic region, when the patient has a diagnosis or presents symptoms of infectious diseases.
The area of Occupational Medicine and Safety is concerned with the possibility of potential contamination and the rapid assistance of these professionals, so that they are able to return to their posts and continue to respond to any occurrences in the system.
