Abstract
A new endovascular prosthesis (stent) was developed by bending 0.13 mmstainless steel wire in a zigzag fashion, the straight edges of the saidzigzag are 2 to 3 mm long and the point of inflection forms an angle between40 and 50 degrees. The length of the stent varies from 8 -20 mm. The stentcan be used in blood vessels which are at least 2.5 mm and less than 5 mmwide. This stent is to be put in position using the conventional ballooncatheter.

EEN NIEUWE ENDOVASCULAIRE PROTHESE (IDS-STS-2) TER BEHANDELING VAN GEDISSECTEERDE EN VERNAUWDE BLOEDVATEN.
BESCHRIJVING Behandeling van bloedvatvernauwingen door middel van een balloncatheter is een populaire methode. Vorig jaar werden op deze manier in ons land meer dan 6000 patienten met coronaire bloedvatvernauwingen behandeld. Probleem van deze behandelingsmethode is enerzijds het gevaar dat tijdens het opblazen van de ballon een scheur (dissectie) ontstaat waardoor het bloedvat kan dichtvallen en alzo een akuut myocardinfarkt kan veroorzaken, anderzijds is het goed gedocumenteerd dat deze behandelingsmethode gepaard gaat met een frequente teruggroei (restenose) van de bloedvatvernauwing binnen de 6 maand na de behandeling. Om deze problemen optelossen werden enerzijds medicijnen uitgetest om de teruggroei van de vernauwing te voorkomen en anderzijds werden nieuwe behandelingsmethoden ontwikkeld.
Een van die nieuwe behandelingsmethoden bestaat erin een metalen intravasculaire prothese (stent) ter hoogte van de vernauwing te plaatsen. Deze methode is zeer efficient voor het behandelen van vaatscheuren die kunnen ontstaan tijdens de ballondilatatie en recente studies hebben aangetoond dat het plaatsen van zo een metalen ondersteuning de kans op restenose significant vermindert. Probleem echter met de tot op heden gecommercialiseerde prothesen is dat ze op zichzelf ook trombogeen zijn en aanleiding kunnen geven tot een trombotische occlusie van het bloedvat. Anderzijds bleek dat door het implanteren van een metalen prothese in een bloedvat het lichaam met een afweerreactie kan reageren waardoor terug bloedvatvernauwingen (restenose) kunnen ontstaan binnen de 6 maand na implantatie van de prothese.

gebogen wordt in een zig-zag vorm (cfr. figuur waarvan de rechte stukken 2 à 3 mm lang zijn en de buighoekken 40 à 500 Hierdoor ontstaat een soort veertje.
Deze endovasculaire prothese kan gemonteerd worden op de balloncatheter die gebruikt wordt voor het openrekken van de bloedvatvernauwing en wordt aangebracht ter hoogte van het bloedvat door de ballon opteblazen tot 8 atm en dan weer leegtezuigen. De endoprothese blijft alzo ter plaatse ter hoogte van de dissectie in het bloedvat of ter hoogte van de te behandelen bloedvatvernauwing.
De lengte van de stent kan varieren van 8 mm tot 20 mm.
Hierdoor is het mogelijk scheuren en vernauwingen van verschillende lengte adequaat te behandelen. Hierbij wordt ernaar gestreefd een stent te kiezen die zowel proximaal als distaal de stenose met ongeveer 2 mm overbrugt.
De finale diameter van de stent wordt bepaald door de dikte van de balloncatheter die gebruikt wordt voor het openblazen van de stent. Het is aangewezen hierbij een ballon te gebruiken die ongeveer 0. 5 mm groter is dan het distale referentie punt.

Proefondervindelijk onderzoek bij het varken heeft aangetoond dat dit systeem zeer gemakkelijk te gebruiken is.
Het was mogelijk de stent zeer distaal in de bloedvaten te plaatsen en het was ook mogelijk om scherpe bochten zonder enig probleem te passeren.
Verder bleek dit systeem ook bijzonder veilig te zijn. We hebben nooit problemen gehad met het exact plaatsen van de stent op de gewenste plaats, tijdens het opblazen van de ballon hebben we nooit ballonrupturen gezien en het plaatsen van de stent gaf nooit aanleiding tot problemen met het terugtrekken van de ballon, problemen die klassiek zijn met de huidige gecommercialiseerde stents wanneer ze gebruikt worden in dezelfde experimentele omstandigheden.
Controle angiografie na het plaatsen van de stents toonde een mooi open vaatlumen zonder intravasculaire opklaringen en tekenen van distale of proximale dissecties. Verder bleken ook de zijtakken ter hoogte van het gestentte vaatsegment intact.
Vervolgonderzoek na 7 dagen toonde aan dat al de stent open bleven zonder dat er thrombotische verwikkelingen ontstonden, niettegenstaande geen anticoagulantia therapie gegeven werd in dit studiemodel.
Vervolgonderzoek na 6 weken toonde aan dat er geen belangrijke reactieve neointimale groei uitgelokt werd door deze stent.

3. Behandeling van complicaties ontstaan tijdens diagnostische procedures voor het opsporen van cardiale en vaataandoeningen.

DESCRIPTION Treatment of blood vessel strictures by means of a balloon catheter is a popular method. Last year, more than 6,000 patients with coronary artery strictures were treated in this way in our country. The problem of this treatment method is on the one hand the danger that a rupture (dissection) occurs during the inflation of the balloon, which can cause the blood vessel to close and thus cause an acute myocardial market, on the other hand, it is well documented that this treatment method is accompanied by frequent regrowth (restenosis). ) of the blood vessel constriction within 6 months after treatment. To solve these problems, drugs were tested on the one hand to prevent the regrowth of the narrowing and on the other hand new treatment methods were developed.
One of these new treatment methods is to place a metal intravascular prosthesis (stent) at the level of the narrowing. This method is very efficient for treating vascular tears that can develop during balloon dilatation and recent studies have shown that placing such a metal support significantly reduces the risk of restenosis. However, the problem with the prostheses commercialized to date is that they themselves are also thrombogenic and can lead to a thrombotic occlusion of the blood vessel. On the other hand, it has been found that by implanting a metal prosthesis in a blood vessel, the body can react with an immune response, which in turn can lead to narrowing of the blood vessels (restenosis) within 6 months after implantation of the prosthesis.

is bent in a zig-zag shape (cf. figure whose straight pieces are 2 to 3 mm long and the bending angles 40 to 500. This creates a kind of feather.
This endovascular prosthesis can be mounted on the balloon catheter used to stretch the blood vessel constriction and is placed at the blood vessel by inflating the balloon to 8 atm and then evacuating it again. The endoprosthesis thus remains in place at the level of the dissection in the blood vessel or at the level of the blood vessel constriction to be treated.
The length of the stent can range from 8 mm to 20 mm.
This makes it possible to adequately treat cracks and constrictions of different lengths. This seeks to select a stent that bridges the stenosis both proximally and distally by about 2 mm.
The final diameter of the stent is determined by the thickness of the balloon catheter used to inflate the stent. It is recommended to use a balloon approximately 0.5 mm larger than the distal reference point.

Experimental research in the pig has shown that this system is very easy to use.
It was possible to place the stent very distally into the blood vessels and it was also possible to pass sharp bends without any problem.
Furthermore, this system also proved to be very safe. We have never had any problems with the exact placement of the stent in the desired location, we have never seen balloon ruptures while inflating the balloon, and the placement of the stent never gave rise to balloon retraction problems, which are classic with current commercialized stents when used in the same experimental conditions.
Control angiography after stent placement showed a nice open vascular lumen with no intravascular clearing and signs of distal or proximal dissections. Furthermore, the side branches at the height of the stented vascular segment were also found to be intact.
Follow-up study after 7 days showed that all of the stent remained open without developing thrombotic complications, despite no anticoagulant therapy being given in this study model.
Follow-up study at 6 weeks showed that no significant reactive neointimal growth was elicited by this stent.
POSSIBILITIES OF APPLICATION OF THE SYSTEM 1. Treatment of blood vessel strictures in humans and animals.
2. Treatment of complications arising from other treatment methods of blood vessel strictures.
3. Treatment of complications arising during diagnostic procedures for the detection of cardiac and vascular disorders.
