De uitvinding heeft betrekking op niet-steroide ontstekingsvertragende substanties voor de behandeling van ontstekingen bij oogziekten, welke in gebruikelijke oftalmologische waterige dragervloeistoffen oplosbaar zijn, in het bijzonder op lysine-0-methyl-4- (2'-thienyl- earbony 1) feny laeetaat.
Tot hiertoe hebben de werkwijzen voor het vervaardigen van niet steroide waterige ontstekingsvertragende middelen voor het lokaal gebruik in de ooggeneeskunde zieh in hoofdzaak gericht op de optimalisering van uit dergelijke substanties bereide suspensies, omdat het tot nu toe niet mogelijk was stabiele waterige oplossingen voor het lokaal gebruik in de ooggeneeskunde met een actieve substantie te vervaardigen, zoals bijvoorbeeld beschreven in het Amerikaanse octrooischrift 4 087 538, dat betrekking heeft op het vervaardigen van

ooggeneeskunde moet bepaalde wezenlijke eigenschappen bezitten de niet oplosbare actieve substantie mag geen tendens tot snelle afscheiding vertonen,
ten einde gedurende de behandeling een gelijkmatige dosering mogelijk te maken. Bij het vormen van een neerslag tussen de behandelingen moet deze neerslag middels schudden of roeren eenvoudig opnieuw dispergeerbaar zijn. De suspensie mag ook geen te hoge viscositeit hebben, zodanig dat ze goed vloeibaar blijft.

Het genoemde Amerikaanse octrooischrift beschrijft de toepassing van bepaalde vlokkingsmiddelen en vlokkingsvertragende middelen of suspensiemiddelen alsook bepaalde kritische mengverhoudingen van de verschillende bestanddelen van het mengsel met het doel bruikbare suspensies te bekomen die toepasbaar zijn in de ooggeneeskunde.
Anderzijds beschrijft het Europese octrooischrift 460 926 van de aanvraagster het vervaardigen van lysine-tX-methyl-4- (2'-thienyl- carbonyl) fenylacetaat, welk als actieve substantie in oplossingen volgens de uitvinding toepasbaar is in de ooggeneeskunde.
In waterige dragervloeistoffen hebben niet oplosbare actieve vaste stoffen voor lokaal gebruik in de ooggeneeskunde het nadeel dat het vervaardigen van stabiele of ten minste eenvoudig opnieuw dispergeerbare suspensies lastig is, wat het gevaar van een onregelmatige dosering met zieh meebrengt, afgezien van de problemen van verdraagzaamheid, welke daardoor kunnen ontstaan, dat dergelijke niet oplosbare substanties afhankelijk van hun deeltjesgrootte min of meer ernstig als een vreemd lichaam in het oog voelbaar zijn. Een middel voor het wegruimen van deze nadelen zou het vervaardigen zijn van een waterige oplossing, welke de gewenste eigenschappen van een recept voor lokale toepassing in de ooggeneeskunde in zieh verenigt.
Oplossingen van niet in water oplosbare vaste stoffen laten zieh door toepassing van organishce oplosmiddelen vervaardigen, welke echter dikwijls onverdraagbaar zijn, doordat zij gebruik maken van oliën, welke echter na toepassing daarvan tot irritaties leiden, omdat ze niet met tranen vermengbaar zijn, of door het toevoegen van oppervlakteactieve middelen, welke echter tot verandering van de oogvloeistof en het hoornvlies leiden en de opname van de actieve substantie beinvloeden kunnen. Deze nadelen zijn door toepassing van oplosbare actieve substanties vermijdbaar.

Lysine-C < .-methyl-4- is een niet steroid ontstekingsvertragend middel dat geschikt is voor de behandeling van oogontstekingen en bovendien oplosbaar is in gebruikelijke oftalmologische waterige dragervloeistoffen, zodanig dat het voor lokale toepassing in ooggeneeskunde buitengewoon geschikt is.

Hieromtrent is een eerste doel van de uitvinding het realiseren van een samenstelling van genoemde actieve substanties in de vorm van een gebruiksklare of desgewenst bereidbare oplossing, welke 0, 05 tot 10 % (gewicht per volume) van deze actieve substantie alsook ten minste een van de volgende conserveringsmiddelen bevat : methyl-p-hydroxybenzoaat, propyl-p-hydroxybenzoaat of thiomersal, en elke bovendien desgewenst een buffersubstantie bevat, zoals bijvoorbeeld een substantie op basis van fosfaten of boorzuur/natriumboraat ;
wiens viscositeit desgewenst middels hydrofiele polymeren zoals bijvoorbeeld hydroxypropylmethylcellulose regelbaar is, en welke bovendien verdere substanties kunnen bevatten, zoals bijvoorbeeld manniet of polyvinylpyrrolidon, welke desgewenst uit een dragervloeistof van de actieve substantie kunnen stammen, welke dragervloeistof mogelijkerwijze van tevoren gelyofiliseerd werd, alsook verder een isotonerend middel zoals bijvoorbeeld natriumchloride. De pH-waarde van de oplossing kan tussen 6 en 8 liggen.
Een tweede doel van de uitvinding is de lokale toepassing van lysine-0 (-methyl-4- (2'. : hienylcarbonyl) fenylacetaat voor de behandeling van oogontstekingen.
Een derde doel van de uitvinding is uiteindelijk het realiseren van een werkwijze voor het vervaardigen van een waterige oplossing van lysine-O(X-methyl-4- (2'. thienylcarbonyl) fenylacetaat voor lokale toepassing in de ooggeneeskunde. Deze werkwijze is daardoor gekenmerkt dat ze volgende stappen omvat :
A) oplossen in zuiver water van lyslne- O'--methyl-4- (2'-thlenylcarbonyl) fenyl- acetaat, indien noodzakelijk toevoegen van een middel voor het isotoniseren van de bereide oplossing, instellen van de pH-waarde tussen 6 en 8, bijstellen van het volume met zuiver water, toepassen van een steriele filtratie, onder omstandigheden en ter regeling van de pH-waarde middels roeren en op eender welk tijdstip gedurende de bereiding van de oplossing, van een buffersubstantie op basis van fosfaten of boorzuur/natriumboraat ;

B) oplossen van een of meer conserveringsmiddelen in zuiver water, in koude toestand, zoals bijvoorbeeld in het geval van thiomersal, of na verwarmen van het zuiver water tot een temperatuur van 90 tot 95OC, zoals bijvoorbeeld in het geval van methyl-en/of propyl-p-hydroxybenzoaat, indien noodzakelijk, middels roeren afkoelen van de oplossing tot kamertemperatuur, bijstellen van het volume met zuiver water, toepassen van een steriele filtratie, onder omstandigheden, middels schudden, op eender welk tijdstip gedurende de bereiding van de oplossing toevoegen van een hydrofiel polymeer zoals bijvoorbeeld hydroxypropylmethylcellulose en/of een isotonisch middel voor het verhogen van de viscositeit respectievelijk het isotoniseren van de oplossing, C)
mengen en aftappen onder steriele omstandigheden van de oplossingen volgens A) en B) of D) aftappen van de oplossing volgens B) en lyofiliseren onder steriele omstandigheden van de oplossing volgens A voor het bereiden indien nodig van de gebruiksklare oplossing.
De therapeutische werking van lysine--methyl-4-- (2'-thienylcarbonyl) fenylacetaat en de daarmee vervaardigde middelen werden in verschillende farmaceutische testen en onderzoekingen bewezen. Hieronder zijn dergelijke onderzoekingen en hun resultaat kort samengevat : Onderzoekingen met betrekking tot de werkzaamheid van lysine-o-methyl-4- (2Lthienylcarbonyl) fenylacetaat in waterige oplossing : 1) Onderzoekingen van de penetratiediepte"in vivo"bij konijnen : de resultaten zijn grafisch in figuur l weergegeven.
2) Onderzoekingen van de penetratiediepte"in vitro"in een voor dat doeleinde ontwikkelde kamer onder toepassing van het ooghoornvlies van een konijn : de resultaten zijn grafisch in figuur 2 weergegeven.
3) Onderzoekingen van de ontstekingsvertragende en heilende werkingen bij verschillende doseringen, gebruik makend van een door punctie ontstoken konijnenoog. De ontstekingsvertragende en heilende werking is in de hieronder gegeven tabel I samengevat :

2) Onderzoek naar de invloed van een 5 %-ige oplossing van lysine--methyl-4- (2'-thienylcarbony1) fenylacetaat op de oogdruk bij acute en chronische toepassing :
de invloed bewees zich als niet significant. 3) Onderzoekingen van de invloed van een 5%-ige oplossing van lysine-o (. -methyl-4- (2'-thienylcarbony1) fenylacetaat op het genezingsproces aan het ooghoornvlies van konijnen na een gebruikelijke verwonding : er werd vastgesteld dat de actieve substantie het normale genezingsproces niet beinvloedt.
4) Onderzoeking van subacute (22 dagen) toxische werkingen van l %-ige en 5%-ige oplossingen van lysine--methyl-4- (2'-thienylcarbony1) fenylacetaat op het oog van een albino konijn : er werden geen tekens van lokale toxische werkingen (volgens Draize) waargenomen, waarvan de oorzaak bij de behandeling te zoeken zou zijn. Het histopathologische onderzoek van de oogappel leverde geen tekenen van signifikante veranderingen van het epitheel ter hoogte van het trabicularium.
Bovendien werd de volgens de uitvinding verkregen verbinding door verschillende galenische onderzoekingen gewaardeerd zoals het testen van de lyofilisering, de verdraagzaamheid met conserveringsmiddelen, de invloed van viskositeitstoevoegsels, de pH-waarde en de buffersubstanties, alsook onderzoekingen met betrekking tot stabiliteit bij verscheidene temperaturen en van verscheidende pentratiedieptes respectievelijk de opname door het oog. Bij deze verschillende onderzoekingen werd in het bijzonder naar voren gebracht dat :
1) Lyofilisering van lysine-o-methyl-4- (2'-thienylcar- bonyl) fenylacetaat alleen of met toevoeging van een stof zoals maniet of polyvinylpyrrolidon.
Er werd vastgesteld dat de actieve substantie als gevolg van de lyofilisering op geenerlei wijze gedegradeerd werd. Het onderzoek van de fysische en fysisch-chemische eigenschappen van de gelyofiliseerde verbinding en van de opnieuw bereide oplossing bewees de vatbaarheid van de werkwijze.
2) Onderzoek naar de houdbaarheid door het testen van de invloed van conserveringsmiddelen, viscositeitstoevoegingen, van de pH-waarde en van de buffersubstanties : de resultaten van deze onderzoeken wezen uit dat de actieve substantie in geen van de onderzochte gevallen, na

het in voorraad houden gedurende een maand bij 55OC, gedegradeerd was.
3) Onderzoek naar de stabiliteit van de gebruiksklare oplossingen van de in de voorbeelden l tot 6 aangeduide samenstelling : bij deze onderzoeken wees zich een hoge stabiliteit van de verschillende oplossingen uit. De fysisch-chemische eigenschappen bleven onveranderd, en het chromatografische onderzoek (HPLC) gaf geen enkele aanduiding op het aanwezig zijn van afvalprodukten van 0 (-methyl-4- (2Lthienyicarbonyl) fenyiachtige zuren.
4) Farmaceutische onderzoeken van kinetische oftalmologische aard met 13 monsters van oplossingen, bij dewelke de invloed van de pH-waarde (3 waarde), van de viscositeit (drie concentraties van viscositeit inkrementerende toevoegsels) en de aanwezigheid of afwezigheid van buffersubstanties op de penetratiediepte van het geneesmiddel bij konijnen werden onderzocht : als resultaat van deze onderzoeken werd de conclusie getrokken dat een eenvoudige waterige oplossing een goed penetratievermogen had en dat de verzuringsmiddelen van een buffersubstantie alsook de toevoeging van een viscositeitverhogend middel het penetratievermogen van het geneesmiddel positief beinvloeden.
I1Írte beschrijving van de tekeningen Figuur 1 toont de resultaten van de onderzoeken met betrekking tot het penetratievermogen "in vivo". De waarden zijn in jjg van de zuren van de actieve substantie per ml van de waterige oplossing aangegeven. De gebruikte oplossing bevatte 10 % (gewicht/volume) van de actieve substantie.

Figuur 2 laat het resultaat zien van het onderzoek met betrekking tot het penetratievermogen De waarden zijn in ug van de zuren van de actieve substantie per ml van een kunstmatig waterige oplossing aangegeven. De onderzochte oplossing bevatte 10 % (gewicht/volume) actieve substantie.
Hieronder zijn verschillende voorbeelden beschreven voor de voorbereiding van het preparaat volgens de uitvinding. Deze voorbeelden houden geen beperking in van de beschermingsomvang van de uitvinding, welke alleen door de conclusies gegeven wordt.

Tijdens het roeren wordt hydroxypropylmethylcellulose toegevoegd, totdat een transparante en homogene oplossing wordt verkregen welke vervolgens gedecanteerd wordt om eventuele klonten af te scheiden.
C) De oplossingen volgens A) en B) worden gemengd en afgetapt, of D) de oplossing volgens B) wordt met zuiver water tot 100 ml verhoogd, gehomogeniseerd en afgetapt. de oplossing volgens A) wordt in flessen gedoseerd en gelyofiliseerd. In deze vorm is het preparaat, indien nodig, voor de bereiding klaar.

C) De oplossingen volgens A) en B) worden onder sterile omstandigheden gemengd en afgetapt.
