Description

1 Description The PractiCare System Delivering medication management with reduced risk of cross infection and environmentally friendly practices Six Steps - Blister Packing Medications in Pharmacy Before commencing Wash Hands with antibacterial solution and clean workspace /PractiFiles/Transport bins with antibacterial solution. Check Prescriptions are current and any changes/additions to patients medication regime. Step 1. Using PractiCare Software print details on front label and foil. Step 2. Place the top or bottom labeled Colour Coded PractiPak blister into Platen pack the correct medications according to the patient foil and front label details. Use the following colour coding for medication delivery time slots as listed in 24 hour time. Rose 0800 Lemon 1200 Mandarin 1800 Aqua 2100 White PRN Lilac Drugs of addiction Lime Antibiotics Step 3. Once packed and checked, seal foil on back of PractiPak using either heat seal or cold seal foil sheet. Step 4. Place front label onto PractiPak and have pharmacist check and sign PractiPak where needed. Step 5. Place Patient PractiPaks on PractiFiles or in Transport bins for delivery to facilities.
2 For community use the individual PractiPak is given to the individual or agent. Step 6. Check medications remaining and request any prescriptions which will be required for future packs. Eight Simple Steps to ???Best Practice??? Medication Delivery Step 1. Wash Hands thoroughly with antibacterial solution Step 2. Pack Medication Trolley Step 3. Check Patient Identification (Visually and Verbally) Step 4. Check the pre-packed PractiPak is the patients Step 5. Remove medication from the appropriate blister (Use PillCup to minimise risk of damage to medications and to make removal easier) Step 6. Count tablets and ensure that they match the patients chart. Check the tablets visually against the PractiPak Label. Step 7. Administer the medication Check if there is a reminder card for other medications (eg creams, ointments, etc) administer as required. Step 8. Complete/sign the Medication Reporting Sheet noting any discrepancies. Return to Step 1 At the end of the medication period discard (or recycle) the PractiPak Important Notes: PractiPak has four drugs one in each line for Seven Days. Where more than four drugs are required use a second PractiPak. PractiCare???s 7 Day Unit Dose System Medication and Dosage Changes - replace or eliminate a time slot or replacing the PractiPak.
3 Suspended or Ceased Medications a sticker is added denoting the change. PractiCare???s Software prints the tablet name and strength in each blister row by row on the back of the PractiPak, including directions and the number of tablets in each blister. It also prints a description of each tablet or a graphic of the tablet denoting shape, colour, and markings. PractiCare???s PillCup To use insert the PillCup into the foil with the cup under the pills and tear the backing sheet away in one movement. The PillCup can then be handed directly to the patient. PillCup is single use and replaces a traditional medicine cup. PractiPak: PractiPak is made from Recycled cardboard and PET Plastic. PractiPak is recycled when all medications have been taken. Reduces risk of cross infect from reusable plastic folders and cards. To use insert the PillCup into the backing paper with the cup under the pills and tear the backing sheet away with in one simple movement, The PillCup can then be handed directly to the patient to take the pills from. PractiCare's PillCup is the only aid available which meets best practice. The PillCup is single use and replaces a traditional medicine cup for the easy dispensing of medication
