Die Erfindung betrifft ein chirurgisches Instrumentarium, bestehend aus einem Markhöhlen- stift mit einem Kleeblatthohlprofil und einem axialen Längsschlitz und Einrichtungen zum Ein- schlagen bzw. Herausziehen des Markhöhlenstiftes.
Es sind bereits Markhöhlenstifte bekannt, die ein kleeblattförmiges Profil aufweisen und mit einem axialen Längsschlitz versehen sind. Diese Art von Markhöhlenstiften ist unter dem Na- men Küntscher-Stift bekannt und in Markhöhlenoperationen mit Erfolg verwendet, weil sie wegen des Längsschlitzes elastisch sind. Dabei können sie sehr wirtschaftlich hergestellt werden. Zum
Einschlagen dieser Stifte stehen aber keine Werkzeuge zur Verfügung, die eine eindeutige Füh- rung des Einschlagwerkzeuges gewährleisten könnten. Die beim Herausziehen der Küntscher-Stifte verwendeten Spiesse brechen wegen ihrer geringen Auflageflächen oft ab oder verschmieren sich, was bei Operationen eine Verzögerung bzw. Komplikationen hervorrufen kann.
Der Zweck der Erfindung besteht in der Behebung der oben bezeichneten Schwierigkeiten.
Die Aufgabe kann in der Schaffung eines chirurgischen Instrumentariums zum Einschlagen bzw. Herausziehen von Markhöhlenstiften mit Kleeblattprofil und einem axialen Längsschlitz be- zeichnet werden, mit welchem derartige Markhöhlenstifte in wohlbestimmter Lage bzw. bei ge- nauer Führung des Einschlagwerkzeuges eingeschlagen bzw. bei genauer gegenseitiger Lage des
Markhöhlenstiftes und der Werkzeuge herausgezogen werden können.
Die Erfindung geht davon aus, dass zur Lösung der Aufgabe die Konzeption im Verhältnis zu zylindrischen Markhöhlenstiften grundsätzlich geändert werden muss, indem vor allem auf die elastische Verformbarkeit und die Aufschlitzung der Stifte Rücksicht genommen werden muss. Von. dieser Konzeption ausgehend konnte dann ein chirurgisches Instrumentarium zum Einschlagen bzw.
Herausziehen von Markhöhlenstiften mit Kleeblattprofil und axialem Längsschlitz entwickelt wer- den, bei welchem gemäss der Erfindung neben dem Markhöhlenstift erstens eine Hülse, welche senkrecht zur Längsachse des Markhöhlenstiftes auf das dem Einschlagwerkzeug zugewandte Ende des Markhöhlenstiftes aufschiebbar ist, zweitens ein Gewindestift, welcher zur Anlage an das En- de des Markhöhlenstiftes in eine Gewindebohrung der Hülse einschraubbar ist, drittens ein auf die Hülse aufsetzbarer Hohlschlüssel und viertens ein in den Gewindestift einsetzbarer Stangen- schlüssel zum Einschrauben des Gewindestiftes in die Hülse vorgesehen sind.
Zweckmässig wird der Markhöhlenstift des Instrumentariums derart ausgebildet, dass sein äusseres Ende mit Quernuten zur Aufnahme eines inneren Bundes einer auf den Markhöhlenstift aufschiebbaren Hülse, und mit einem konischen Sitz zur Aufnahme des konischen Endes eines Gewindestiftes versehen ist, wobei die Nuten im Querschnitt des Markhöhlenstiftes eine V-Form bilden.
Die Hülse des Instrumentariums kann an einem axial aufgeschlitzten Abschnitt miteinander eine V-Form bildende innere Bunde zum Eingreifen mit den Nuten des Markhöhlenstiftes aufweisen, wobei sein unaufgeschlitzter Abschnitt als ein Mehrkant mit Gewindebohrung zur Auflage in einem Hohlkant des Hohlschlüssels ausgebildet ist.
Der Gewindestift dieses Instrumentariums wird erfindungsgemäss derart ausgebildet, dass sein mit einem Aussengewinde versehener Abschnitt länger ist als die Gewindebohrung der Hülse, wobei am einen seiner Enden ein Hohlkant zur Aufnahme des Schlüsselkopfes des Stangenschlüssels und am andern Ende ein Konus zur Auflage im konischen Sitz des Markhöhlenstiftes vorgesehen sind.
Zweckmässig wird beim Hohlschlüssel des Instrumentariums der am freien Ende des Hohlschlüssels zur Aufnahme des mehrkantförmigen unaufgeschlitzten Abschnittes der Hülse ausgebildete Hohlkant mit einer umlaufenden Nut mit eingelegter Ringfeder versehen.
Es ist ferner erfindungsgemäss, den Arbeitskopf des Stangenschlüssels als Mehrkant zur Auflage im Hohlkant des Gewindestiftes auszubilden.
Die oben beschriebene Ausbildung des erfindungsgemässen chirurgischen Instrumentariums hat den Vorteil der einfachen und billigen Herstellungsweise, sowie der zuverlässigen und doch einfachen und dabei schonungsvollen Handhabe.
Weitere Einzelheiten der Erfindung werden an Hand der Zeichnungen erläutert, die verschiedene Ausführungsformen der Teile des erfindungsgemässen chirurgischen Instrumentariums beispielsweise darstellen. Fig. 1 ist dabei eine Ansicht eines Ausführungsbeispiels des Erfindungsgegen-

Wie aus Fig. 1 der Zeichnungen hervorgeht, besteht das chirurgische Instrumentarium gemäss der Erfindung aus einem Markhöhlenstift --20--, einer Hülse --22--, einem Gewindestift --24--, einem Hohlschlüssel --26-- und einem Stangenschlüssel --28--.
Einzelheiten des Markhöhlenstiftes --20-- gehen aus den Fig. 2 bis 5 hervor. Der Markhöhlenstift --20-- weist ein kleeblattförmiges Profil auf, dessen eine Seite durch einen axialen Längsschlitz --30-- unterbrochen ist. Das in den Zeichnungen obere äussere Ende des Markhöhlenstiftes --20-- ist in einem Abstand von etwa 3 bis 5 mm vorn Rand mit etwa 2 mm breiten Nu- ten --32 bzw. 34-- versehen, welche im Querschnitt des Markhöhlenstiftes eine V-Form bilden, wie dies aus Fig. 5 der Zeichnungen hervorgeht, wo der Winkel der V-Form mit --36-- bezeichnet ist.
Der Winkel --36-- und die Tiefe der Nuten sind derart zu wählen, dass Zug- und Druckbeanspru- chungen möglichst gleich gross werden. Ferner ist der Markhöhlenstift --20-- mit einem konischen
Sitz --38-- versehen, der zur Aufnahme eines entsprechenden Konus des Gewindestiftes --24-- ge- eignet ist, wie dies nachstehend noch näher erläutert wird.
Die Hülse --22-- besteht in einem axial aufgeschlitzten Abschnitt --22a-- und einem unaufgeschlitzten Abschnitt --22b--. Am axial aufgeschlitzten Abschnitt --22a-- der Hülse --22-- sind innere Bunde --40 bzw. 42-- vorgesehen, die miteinander ebenfalls eine V-Form bilden, die der V-Form-36-- der Nuten-32 und 34-- des Markhöhlenstiftes --20-- entspricht und deshalb mit demselben Bezugszeichen --36-- bezeichnet ist. Der unaufgeschlitzte Abschnitt --22b-- ist dabei als ein Mehrkant --44-- ausgebildet, der beim dargestellten Ausführungsbeispiel aus praktischen Gründen, die Form eines Sechskantes aufweist, wie dies aus Fig. 8 hervorgeht. Der Mehrkant --44-- weist eine Gewindebohrung --46-- auf.
Beim dargestellten Ausführungsbeispiel ist das in den Zeichnungen untere Ende der Hülse --22-- unter einem Winkel von 40 abgeschrägt. Diese Abschrägung ist zum Einschlagen von sogenannten"TIBIA"-Stiften erforderlich.
Die Ausführungsform der Hülse --22-- gemäss Fig.10 bis 13 unterscheidet sich von der vorherigen insofern, dass das in den Zeichnungen untere Ende der Hülse --22-- keine Abschrägung aufweist, wie dies bei Schenkelstiften vorteilhaft ist.
Im Abschnitt --22b-- ist übrigens bei beiden Ausführungen eine umlaufende Nut --48-- zur Aufnahme einer nachstehend noch zu beschreibenden Haltefeder vorgesehen.
Der Gewindestift --24-- ist an einem seiner Enden, u. zw. am in den Zeichnungen oberen Ende mit einem Hohlkant --50-- versehen, während sein anderes Ende konisch ausgebildet ist, wobei der konische Teil mit dem Bezugszeichen --52-- bezeichnet ist (Fig. 1 b). Der konische Teil --52-ist bestimmt, im konischen Sitz --38-- des Markhöhlenstiftes --20-- aufzuliegen. Der Gewindestift --24-- ist aussen mit einem Gewinde --54-- versehen, das mit der Gewindebohrung --46-- der Hülse --22-- einzugreifen bestimmt ist. Der Abschnitt mit dem Aussengewinde --54-- ist so lang, dass er in Arbeitslage um etwa 10 bis 15 mm aus der Hülse --22-- herausragt, damit verschiedene gewünschte Werkzeuge bzw. Geräte angeschlossen werden können.
Einzelheiten der beiden Schlüssel gehen ebenfalls aus Fig. 1 hervor.
Wie ersichtlich, weist der Hohlschlüssel --26-- an seinem freien Ende einen Hohlkant --56-auf, in dem eine umlaufende Nut --58-- vorgesehen ist (Fig. 1a). In die Nut --58-- ist eine Ring-

grösser als der Aussendurchmesser des nicht dargestellten Leitspiesses.
Das dargestellte Ausführungsbeispiel des Stangenschlüssels --28-- zeichnet sich dadurch aus, dass sein Arbeitskopf als ein Mehrkant --64-- ausgebildet ist, der genau in den Hohl-

kant --50-- des Gewindestiftes --24-- passt.
Sowohl der Hohlschlüssel -26--, wie auch der Stangenschlüssel --28-- ist mit einem Griff-66 bzw. 68-- versehen.
Das dargestellte Ausführungsbeispiel des erfindungsgemässen chirurgischen Instrumentariums kann in folgender Weise gehandhabt werden :
Wie aus Fig. 1 ersichtlich, ist die Hülse --22-- auf den Markhöhlenstift --20-- derart aufge- schoben, dass ihre Bunde-40 bzw. 42-- in die Quernuten --32 bzw. 34-- zu liegen kommen. Der unaufgeschlitzte Abschnitt der Hülse --22--, der als ein Sechskant ausgebildet ist, passt in den Hohlkant --56-- des Hohlschlüssels --26--, wobei die Ringfeder --60-- in der Nut --58-- des Hohl- schlüssels --26- in die Nut --48-- des unaufgeschlitzten Abschnittes --22b-- der Hülse --22-- einschnappt.
Auf diese Weise wird die gegenseitige axiale Lage von Markhöhlenstift --20--, Hül- se --22-- und Hohlschlüssel --26-- eindeutig festgelegt. Der Gewindestift --24-- ist mit der Hül- se -22-- derart verschraubt, dass sein konischer Teil --52-- im konischen Sitz des Mark- höhlenstiftes --20-- aufliegt.
Wird nun der Stangenschlüssel --28-- durch die Bohrung --62-- des Hohlschlüssels --26-- hindurch mit dem Gewindestift -24-- derart in Verbindung gebracht, dass der Mehrkant --64-- des Stangenschlüssels --28-- mit dem Hohlkant --50-- des Gewindestiftes --24-- eingreift, kann der Gewindestift --24-- durch Drehen des Stangenschlüssels --28-- mittels des Griffes --68-- derart gedreht werden, dass der konische Teil -52-- immer mehr in den konischen Sitz --38-- eindringt und die beiden Schenkel des kleeblattförmigen Profils des Markhöhlenstiftes --20-- auseinander spreizt.
Durch diese Spreizung wird eine gleichsam feste Verbindung zwischen Markhöhlen- stift --20--, Hülse --22-- und Hohlschlüssel --26-- erreicht, so dass nach Entfernung des Stan- genschlüssels --28-- der Markhöhlenstift --20-- mit dem Hohlschlüssel --26-- als eine Einheit ge- handhabt werden kann.
Werden die beiden Schlüssel entfernt, dann ist das Aussengewinde --54-- des Gewindestif- tes-24-- zum Verschrauben mit einem Einschlageinstrument freigelegt.
Nach erfolgtem Einschlagen wird das Einschlagewerkzeug vom Gewindestift --24-- abge- schraubt und der Gewindestift um etwa 5 mm nach oben geschraubt, wobei dann die Hülse --22-- durch eine seitliche Verschiebung um etwa 4 mm vom Markhöhlenstift --20-- herabgenommen wer- den kann.
Beim Herausziehen der Markhöhlenstifte --20-- wird in der umgekehrten Weise vorgegangen.
PATENTANSPRÜCHE :
1. Chirurgisches Instrumentarium, bestehend aus einem Markhöhlenstift mit einem Kleeblatthohlprofil und einem axialen Längsschlitz und Einrichtungen zum Einschlagen bzw. Herausziehen des Markhöhlenstiftes, dadurch gekennzeichnet, dass neben dem Markhöhlenstift (20) erstens eine Hülse (22), welche senkrecht zur Längsachse des Markhöhlenstiftes (20) auf das dem Einschlagwerkzeug zugewandte Ende des Markhöhlenstiftes (20) aufschiebbar ist, zweitens ein Gewindestift (24), welcher zur Anlage an das Ende des Markhöhlenstiftes (20) in eine Gewindebohrung (46) der Hülse (22) einschraubbar ist, drittens ein auf die Hülse (22) aufsetzbarer Hohlschlüssel (26) und viertens ein in den Gewindestift (24) einsetzbarer Stangenschlüssel (28) zum Einschrauben des Gewindestiftes (24) in die Hülse (22) vorgesehen sind.
