【発明の属する技術分野】
この発明は、被検体を挟んでX線照射手段と透過X線像検出手段とが対向配置されたアームを、互いに直交する3軸の各軸周り方向に自在に回動させるアーム回動機構を有するX線診断装置に関し、特に、アームを回動制御する技術に関する。

【従来の技術】
従来のX線診断装置としては、例えば、被検体の血管造影撮影を行うものがある。このX線診断装置は、互いに対向配置されたX線管とイメージインテンシファイア(適宜「I・I管」と略記)との間に、被検体が載置された天板を挿入させ、この被検体にX線管からX線を照射し、被検体を透過したX線をI・I管で検出して透過X線像を得るものである。このようなX線診断装置の一例として、図5に示すように、C字状をなすC形アーム101を有するものがある。C形アーム101の一端にはX線管102が取り付けられ、C形アーム101の他端にはX線管102と対向してI・I管103が取り付けられている。また、C形アーム101は保持部104にて保持され、この保持部104は支柱部105を介してスライド部材106に固定されている。スライド部材106は、天端107に対してスライドできるようになっている。また、このX線診断装置は、C形アーム101が保持部104に対してスライドする方向への回動(α方向)、保持部104の支柱部105に対しての回動(β方向)、支柱部105の軸周り方向への回動(γ方向)がそれぞれ可能であり、これらの各方向に回動させることによってX線管102の照射方向を任意に設定できる。なお、各方向(α,β,γ)の回動制御はそれぞれ単独に行われる。例えば、α,β方向への回動は、天板の側面側に取り付けられた操作レバー(図示省略)を操作者が操作することで行われ、γ方向への回動は、回動スイッチ(図示省略)を操作者が操作することで行われる。

ところで、このようなC形アーム101を有するX線診断装置では、通常、図5に示す矢印「A」の方向(この矢印「A」の方向をγ角度0度とする)から被検体Mの頭部を進入させ、γ方向の角度をこの0度の状態で固定し、C形アーム101をα,β方向にそれぞれ回動させることにより、図6に矢印で示すような被検体Mに対する軌道を得ていた。すなわち、C形アーム101をα方向に回動させることにより、被検体Mの頭部方向(CRANIAL方向)あるいは下肢方向(CAUDAL方向)に移動可能であり、C形アーム101をβ方向に回動させることにより、被検体Mの左方向(LAO方向)あるいは右方向(RAO方向)に移動可能である。

なお、上述した被検体Mの頭部方向(CRANIAL方向),下肢方向(CAUDAL方向),左方向(LAO方向),右方向(RAO方向)は、診断学上において、被検体Mを診断する方向(透視方向)として非常に重要な意味を持っており、図7に示すように、臨床角θ,ηとして定義されている。すなわち、臨床角θは、天板の被検体載置面の鉛直方向(この鉛直方向を0度とする)を基準に、頭部方向(CRANIAL方向)あるいは下肢方向(CAUDAL方向)にどの程度傾けた角度からこの被検体Mを診断しているのかを示す角度である。また、臨床角ηは、前述と同様の鉛直方向(この鉛直方向を0度とする)を基準に、左方向(LAO方向)あるいは右方向(RAO方向)にどの程度傾けた角度からこの被検体を診断しているのかを示す角度である。

【発明が解決しようとする課題】
しかしながら上記の従来装置では、次に示すような問題がある。すなわち、実際には、臨床時の周辺機器との干渉上の問題や、臨床上の必要性の問題などの理由から、図5に示した矢印「A」の方向(γ角度が0度である方向)から被検体Mを挿入させた状態で常に撮影するとは限らず、斜めの方向(γ角度が0度でない方向)から被検体Mを挿入させた状態で撮影する場合がある。このような場合においては、被検体Mの撮影方向が図6の場合と異なるので、例えば図8に示すように、矢印で示す軌道とは別の軌道にC形アーム101が回動することになってしまう。つまり、γ方向の角度(C形アーム101の挿入角度)を変更すると、α方向,β方向への回動をそれぞれ単独に制御するだけでは、図6に矢印で示した被検体Mの頭部方向(CRANIAL方向),下肢方向(CAUDAL方向),左方向(LAO方向),右方向(RAO方向)の軌道を得ることができないという問題がある。

例えば、図5に示した矢印「A」の方向(γ角度が0度である方向)から被検体Mを挿入させるとともに、C形アーム101をα,β方向にそれぞれ任意の角度に回動させた状態で、被検体Mの透過X線像を得ていたとする。その後、臨床上の必要性の理由などにより、C形アーム101のγ方向の角度(C形アーム101の挿入角度)を変更したとすると、被検体Mを別方向から透視することになるので、当然にγ角度変更後の透過X線像は元の透過X線像と一致しないものになる。そこで、操作者が上述の操作レバーを操作して、元の透視方向と一致させるようにC形アーム101を各回動方向(α,β方向)に回動させるが、この各回動方向(α,β方向)は上述した臨床角θ,ηの方向、すなわち、被検体Mの頭部方向(CRANIAL方向),下肢方向(CAUDAL方向),左方向(LAO方向),右方向(RAO方向)に不一致となっているので、C形アーム101の各回動方向(α,β方向)を交互に小刻みに回動させたりして目的の位置に移動させるように回動操作しなければならず、回動操作が煩雑で非常に困難となり、操作者の操作負担になるという問題がある。

この発明は、上記の事情に鑑み、アームを所望の軌道に迅速に回動させることができるとともに、アームの操作性を向上させたX線診断装置を提供することを目的とする。

【課題を解決するための手段】
前記課題を解決するために、請求項1の発明に係るX線診断装置は、
(A)X線照射手段と透過X線像検出手段とが対向配置されたアームと、
(B)当該アームをα方向にスライド移動可能に支持するスライド機構と、
(C)当該スライド機構をα方向の回転軸と直交する回転軸周りのβ方向に回転可能に支持する回動機構と、
(D)当該回動機構を前記β方向の回転軸と直交する鉛直軸周りのγ方向に回転可能に支持する旋回機構と、
(E)第3軸を鉛直方向とする直交座標系において、その幅方向が前記直交座標系の第1軸方向に、その長手方向が前記直交座標系の第2軸方向に、その厚み方向が前記直交座標系の第3軸方向に合うようにして配設され、被検体を載置する天板とを有し、
(F)前記X線照射手段からのX線照射に伴って、前記透過X線像検出手段で検出される前記被検体の透過X線像を撮影するX線診断装置において、
(G)前記X線照射手段と前記透過X線像検出手段とを結ぶアイソセンターの前記直交座標系に対する回動方向及び速度が入力される入力手段と、
(H)前記β方向の回転軸が前記第1軸又は第2軸と平行である状態において、前記アイソセンターの回動方向及び速度が、前記入力された回動方向及び速度と一致するように、前記スライド機構及び前記回動機構を制御するとともに、前記γ方向への回転により前記β方向の回転軸が前記第1軸又は第2軸と平行でない状態となった場合において、前記アイソセンターの回動方向及び速度が、前記β方向の回転軸が前記第1軸又は第2軸と平行である状態のときと一致するように、前記スライド機構及び前記回動機構を制御するアーム回動機構制御手段と
(I)を備えることを特徴とするものである。

〔作用〕
次に、この発明に係るX線診断装置における作用を説明する。請求項1の発明に係るX線診断装置では、スライド機構は、X線照射手段と透過X線像検出手段とが対向配置されたアームをα方向にスライド移動可能に支持する。回動機構は、当該スライド機構をα方向の回転軸と直交する回転軸周りのβ方向に回転可能に支持する。旋回機構は、当該回動機構を前記β方向の回転軸と直交する鉛直軸周りのγ方向に回転可能に支持する。天板は、第3軸を鉛直方向とする直交座標系において、その幅方向が前記直交座標系の第1軸方向に、その長手方向が前記直交座標系の第2軸方向に、その厚み方向が前記直交座標系の第3軸方向に合うようにして配設され、被検体を載置する。入力手段は、前記X線照射手段と前記透過X線像検出手段とを結ぶアイソセンターの前記直交座標系に対する回動方向及び速度が入力される。アーム回動機構制御手段は、前記β方向の回転軸が前記第1軸又は第2軸と平行である状態において、前記アイソセンターの回動方向及び速度が、前記入力された回動方向及び速度と一致するように、前記スライド機構及び前記回動機構を制御するとともに、前記γ方向への回転により前記β方向の回転軸が前記第1軸又は第2軸と平行でない状態となった場合において、前記アイソセンターの回動方向及び速度が、前記β方向の回転軸が前記第1軸又は第2軸と平行である状態のときと一致するように、前記スライド機構及び前記回動機構を制御する。したがって、γ方向への回転によりβ方向の回転軸が第1軸又は第2軸と平行でない状態となった場合において、入力手段によってアーム(アイソセンター)の回動方向を入力したときに、β方向の回転軸が第1軸又は第2軸と平行である状態と同じようにアームがスライド移動及び回転するので、アームが迅速に回動され、アームの操作性が向上され、操作者の作業負担が軽減される。

また、請求項2の発明に係るX線診断装置では、アーム回動機構制御手段は、アイソセンターを天板の被検体載置面の鉛直方向を基準として第1軸方向および第2軸方向に傾けた角度で表される現在の臨床角の値と、入力手段によって入力された回動方向及び速度とに基づいて算出されるアームの角速度指令に応じて、アームを入力された回動方向に回動させる。したがって、目的の臨床角のみを与えてアームを回動させる場合に比べて不要な演算処理をかけることがない。

また、請求項3の発明に係るX線診断装置では、前述の数1の式に基づいて、アームの角速度指令を算出する。したがって、請求項3に係る発明を好適に実現することができる。また、前述の数1のA〜Dの係数をCPU部のメモリに記憶すれば、演算速度が十分でないCPUを採用することができる。

【発明の実施の形態】
以下、この発明のX線診断装置の一実施例を図面を参照しながら説明する。図1は、実施例に係るX線診断装置の構成を示すブロック図である。図2(a)は、実施例装置のC形アームを側面から見た側面図であり、図2(b)は、実施例装置のC形アームを正面から見た正面図である。

本実施例に係るX線診断装置は、図1に示すように、天板1上に載置された被検体Mを挟んでX線管2とI・I管3とが対向配置されたC形アーム4と、このC形アーム4を互いに直交する3軸の各軸周り方向に自在に回動させるC形アーム回動機構部5と、天板1を駆動する天板駆動部6と、X線管2を制御する照射制御部7と、I・I管3を制御するI・I管制御部8と、I・I管3により得られた光学像を撮影するTV(テレビ)カメラ9と、このTVカメラ9で撮影されたデータ画像を処理するデータ画像処理部10と、このデータ画像処理部10からのデータ画像を画像表示するモニタ11と、C形アーム回動機構部5と天板駆動部6と照射制御部7とI・I管制御部8とデータ画像処理部10を制御する撮像制御部12となどを備えている。上述したX線管2は本発明におけるX線照射手段に相当し、I・I管3は本発明における透過X線像検出手段に相当し、C形アーム4は本発明におけるアームに相当する。以下に各部の構成について詳細に説明する。

C形アーム回動機構部5は、図1,図2に示すように、C形アーム4を保持した状態でα方向にスライド移動できる保持部21と、この保持部21を支持する支柱部22と、この支柱部22の上端側に固定されたスライド部材23と、このスライド部材23が取り付けられた天端24とを備えている。この支柱部22の下側端部には、C形アーム4を保持部21ごとβ方向に回動する回動機構部25が備えられ、この支柱部22の上側端部には、この支柱部22をγ方向に回動する旋回機構部26が備えられている。この天端24には、例えば、天井面27にx方向,y方向に敷かれた天井レール(図示省略)が備えられている。なお、図1には、X方向を便宜上斜め方向に図示しているが、このX方向は図1紙面に垂直方向であることを示すものである。スライド部材23は、このx方向,y方向に敷かれた天井レール(図示省略)を走行することで、x方向,y方向にスライド移動できるようになっている。こうすることで、図2(a)に示した、X線管2とI・I管3との対向方向(アイソセンター)Isoが、被検体Mの関心部位からずれないように位置合わせされる。上述したC形アーム回動機構部5は、本発明におけるアーム回動機構に相当する。

天板1は、被検体Mを載置したままで水平移動自在に構成されていて、天板1の上に載置された被検体Mと、X線管2及びI・I管3との位置関係が、被検体Mの体軸J方向に相対的に変位可能になっている。この天板1の移動は天板駆動部6により行われる。なお、この天板1は、その幅方向が第1軸方向としてのx方向に、その長手方向が第2軸方向としてのy方向に、その厚み方向が第3軸方向としてのz方向に一致するように配設されている。

また、図3に示すように、天板1の側面側1aには、C形アーム4の回動方向とその回動速度とに関する操作指令が入力される操作レバー13と、支柱部22をγ方向に回動させるための回動スイッチ14とが備えられている。この操作レバー13は、上下方向に傾斜させることができ、左右に回転させることができる。具体的には、操作者が操作レバー13を上方向に傾斜させると、I・I管3が遠ざかる方向、すなわち、被検体Mの左方向(LAO方向)に移動する。操作者が操作レバー13を下方向に傾斜させると、I・I管3が近づいてくる方向、すなわち、被検体Mの右方向(RAO方向)に移動する。また、操作者が操作レバー13を左回りに回転させると、I・I管3が被検体Mの頭部方向(CRANIAL方向)に移動する。操作者が操作レバー13を右回りに回転させると、I・I管3が被検体Mの下肢方向(CAUDAL方向)に移動する。また、X線管2とI・I管3とは対向するようにC形アーム4に配設されているので、I・I管3がある方向に移動するということは、その移動分だけX線管2がI・I管3の移動方向とは反対方向に移動することは言うまでもない。なお、図1では操作レバー13および回動スイッチ14を天板1の側面側1aに図示すると、却って見にくくなるので、便宜上、天板1の外部に図示している。

このように操作者から操作レバー13に対して操作方向が与えられると、C形アーム4はその操作方向に回動する、すなわち、支柱部22がγ方向に任意の角度に回動された状態であっても、図7に示した臨床角θ,ηの方向にC形アーム4が回動するように、後述するC形アーム回動機構制御部15によって制御されている。

さらに、操作レバー13の上下方向の傾きの大きさ、および、操作レバー13の左右への回転角の大きさに応じた回動速度Vθ,Vηで、C形アーム4は回動することになる。なお、上述した操作レバー13は本発明における入力手段に相当する。

また、回動スイッチ14には、例えば、右回転ボタン14aと左回転ボタン14bとが備えられている。操作者が右回転ボタン14aを押している間だけ、γ方向に右回りに支柱部22が回動する。操作者が左回転ボタン14bを押している間だけ、γ方向に左回りに支柱部22が回動する。ここでは、この回動スイッチ14は、支柱部22をγ方向の任意の角度に回動できるものを採用しているが、予め設定されたγ方向における特定の角度(例えば、0 度,22.5度,45度,…のように22.5度刻みの角度)に支柱部22を回動するものであっても良い。

X線管2は、天板1上の被検体Mに対して接離自在に構成されている。また、X線管2には、高電圧発生装置(図示省略)が接続されていて、所定の管電圧,管電流が供給されて、X線束を被検体Mに向けて照射する。また、X線管2には、X線コリメータ(図示省略)が取り付けられていて、X線管2から照射されるX線束がI・I管3の入射面に入射されるように絞られる。X線管2に関するこれらの動作は照射制御部7により行われる。

被検体Mを透過したX線は、I・I管3により可視光に変換される。I・I管3は、電極電圧を調節することで、入射面の視野サイズが大小に切り換えられるようになっている。

X線管2によるX線照射に伴ってI・I管3の出力面に被検体Mの撮影部位(関心部位)の透過X線像が光学像として結像する。I・I管3の出力面の光学像は、I・I管3の後部に取り付けられたTVカメラ9によって撮影されてアナログ映像信号がX線透視像としてデータ画像処理部10に出力される。データ画像処理部10では、TVカメラ9からのX線透視像に所定の画像処理を施して画像メモリ10aに記憶する。この画像メモリ10aに記憶されたX線透視像が読み出されてモニタ11に出力される。モニタ11は、被検体Mの血管造影撮影されたX線透視画像を表示する。

撮像制御部12は、X線照射や、I・I管3の視野サイズの切り換えや、天板1の水平方向(体軸J方向)への駆動や、操作指示に基づくC形アーム4の回動などを制御して、被検体Mの血管造影撮影する。撮像制御部12には、操作者からの各種の操作指示が入力される操作部16が接続されている。この操作部16には、上述した操作レバー13や回動スイッチ14や、撮影開始を撮像制御部12に指示するためのフットスイッチ17などが備えられている。

撮像制御部12は、図4に示すように、操作レバー13からの操作指令に応じてC形アーム4を回動させるようにC形アーム回動機構部5を制御するC形アーム回動機構制御部15を備えている。このC形アーム回動機構制御部15は、操作レバー13からの操作指令としての回動速度Vθ,Vηと、回動スイッチ14からの支柱部22の回動速度Vγとをアナログデジタル変換して出力するA/D変換器31と、このA/D変換器31でデジタル化された回動速度Vθ,VηをC形アーム4の角速度指令Vα,Vβに変換してC形アーム回動機構部5を制御するCPU(中央演算処理装置)32とを備えている。

CPU32は、入力された回動速度Vθ,Vηを時間微分した値を、前回の臨床角θ,ηに加算して、現在の臨床角θ,ηの値を算出する。このCPU32には、回動スイッチ14の操作により変更された支柱部22の回転角度γも入力される。さらに、このCPU32は、前記の算出した現在の臨床角θ,ηの値と、回動スイッチ14からの支柱部22の回転角度γと、入力された回動速度Vθ,Vηとを、次に示す演算式(1)〜(7)に基づいて、C形アーム4の角速度指令Vα,Vβとを算出する。なお、この演算式(1)〜(7)は、CPU32内のメモリ32aに記憶されている。

C形アーム回動機構部5は、CPU32からのC形アーム4の角速度指令Vα,Vβ,Vγを電力増幅してそれぞれのモータMα,Mβ,Mγに出力する各速度サーボ回路Sα,Sβ,Sγと、C形アーム4をα,β,γ方向に駆動させる各モータMα,Mβ,Mγと、各モータMα,Mβ,Mγの回転角度を検出する各ポテンショメータPα,Pβ,Pγと、各ポテンショメータPα,Pβ,Pγで検出された回転角度をアナログデジタル変換して出力するA/D変換器33とを備えている。CPU32は、上述したように、操作レバー13からの回動速度Vθ,Vηと、回動スイッチ14からの支柱部22の回動速度Vγとに基づいて、現在の臨床角θ,ηを算出可能であるが、算出した現在の臨床角θ,ηにC形アーム4が実際に位置しているかどうかを確認するために、A/D変換器33を介して入力される、各ポテンショメータPα,Pβ,Pγで検出された各モータMα,Mβ,Mγの回転角度を用いている。

また、CPU32には、X線管2とI・I管3との対向方向(アイソセンター)Isoにおける第1軸方向への回転角度成分(x成分)としての臨床角ηと第2軸方向への回転角度成分(y成分)としての臨床角θとを表示する角度表示器18が接続されている。この角度表示器18は、CPU32で算出されたC形アーム4の現在の臨床角θ,ηと、支柱部22のγ方向への回転角度とを表示する。上述した角度表示器18は本発明における角度表示手段に相当する。

続いて、以上の構成を有する実施例のX線診断装置の動作を説明する。

なおここでは、図5に示した矢印「A」の方向(γ角度が0度である方向)から被検体Mを挿入させて、C形アーム4をα,β方向にそれぞれ任意の角度に回動させた状態で、被検体Mの透過X線像を得ていたとする。その後、臨床上の必要性の理由などにより、C形アーム4のγ方向の角度(C形アーム4の挿入角度)を変更したとすると、被検体Mを別方向から透視することになるので、当然にγ角度変更後の透過X線像は元の透過X線像と一致しないものになる。そこで、元の透視方向と一致させるように、操作者が操作レバー13を操作してC形アーム4を回動させる場合について説明する。

操作者は、元の透視方向と一致させるように、操作レバー13を上下あるいは左右回りに操作して、C形アーム4を回動させる。このとき、C形アーム回動機構制御部15により、C形アーム4は、被検体Mの頭部方向(CRANIAL方向),下肢方向(CAUDAL方向),左方向(LAO方向),右方向(RAO方向)のうちで操作レバー13の操作方向に応じた方向に回動することになる。さらに、このC形アーム4は、操作レバー13の上下方向の傾きの大きさ、あるいは、操作レバー13の左右への回転角の大きさに比例した回動速度で回動することになる。
