Abstract
Изобретение относитс к медицинской технике, а именно к устройствам дл наложени анастомоза. Целью изобретени вл етс снижение несосто тельности шва и уменьшение заброса инфицированного содержимого пищевода в дыхательные пути. Устройство дл соединени трубчатых органов содержит втулку 13 с канавками 14 и 15, гибкую т гу 8, цилиндрическое утолщение 9 с упором 11, наружный стакан 1, внутренний стакан 2, втулку 12, эластичное кольцо 3. В резецированный конец пищевода ввод т втулку 13 с предварительно заведенной в нее гибкой т гой 8 так, чтобы цилиндрическое утолщение 9 заходило во втулку 13, после чего конец т ги 8 проводитс по пищеводу в полость рта, а край пищевода фиксируют лигатурой в канавке 14 втулки 13. Затем в конец кишки, предназначенный дл анастомозировани ввод т втулку 12 с буртиками и закрепл ют кишку на втулке лигатурой. После чего через дополнительный разрез на кишке ввод т в устройство. Затем второй конец гибкой т ги провод т через втулку 12. Смещением гибкой т ги 8 вт гивают внутренний стакан 2 в стакан 1. Эластичное кольцо 3 соскальзывает с внутреннего стакана 2, попадает в канавку 15 и выравнивает со стороны просвета кишки стенки кишки и инвагинированного пищевода в кишку. Устройство извлекают из кишки, гибкую т гу извлекают через рот.

Изобретение относитс к медицинской технике, а именно к устройствам дл наложени анастомоза в трубчатые органы.
Цель изобретени - снижение не- состо тельности шва и уменьшение заброса инфицированного содержимого пищевода в дыхательные пути.
На фиг,1 показано предлагаемое устройство , общий вид на фиг.2 - про- ведение гибкой т ги с цилиндрическим утолщением, заходным и упором через втулку и все устройство и фиксаци резецированных концовч пищевода и кишки на втулках} на фиг.З - погружение во внутренний стакан устройства втулок с фиксированными кишкой и пищеводом} на фиг.4 - анастомоз , сформированный с помощью устройства
Устройство дл соединени трубчатых органов содержит телескопически соединенные наружный цилиндрический стакан 1 и внутренний цилиндрический стакан 2 с осевыми отверсти ми в днищах - внутренний стакан короче наружного на 5-3,5 мм, эластичное кольцо 3, размещенное по наружному периметру внутреннего стакана в раст - нутом состо нии. Наружный стакан снабжен соосной трубчатой руко ткой 4, соединенной с отверстием в днище наружного стакана, предохранительный шток 5, расположенный снаружи

вдоль трубчатой руко тки, рабочий конец 6 которого через отверстие в днище заходит в полость наружного стакана, а противоположный конец соединен с фиксатором положени внутреннего стакана, например байонетом 7.
Внутри устройства проходит гибка т га 8 с цилиндрическим утолщением 9 с заходным- конусом 10 на дистальном конце и упором 11 на противоположном конце. Цилиндрическое утолщение расположено примерно посередине гибкой т ги.
В конструкцию устройства вход т втулка с буртиками 12 и втулка 13 с двум кольцевыми канавками 14 и 15.
Устройство работает следующим образом.
В резецированный конец пищевода 16 ввод т втулку 13 с предварительно заведенной в нее гибкой т гой 8 так, чтобы цилиндрическое утолщение 9 заходило во втулку 13, после чего конец т ги, что выше упора 11, проводитс по пищеводу в полость рта, а край пищевода фиксируют лигатурой в канавке 14 втулки 13, затем в конец кишки 17, предназначенной дл анастомозировани , ввод т втулку с буртиками 12 и заркепл ют кишку на втулке лигатурой. После этого через дополнительный разрез на кишке ввод т в кишку устройство в собранном виде до встречи с втулкой с буртиком 12, закрепленной в кишке. Затем второй конец гибкой т ги провод т через втулку с буртиками 12 и все устройство до выхода наружу (фиг,2). Далее начинают смещать гибкую т гу, при этом происходит плотное соприкосновение втулок, погружение фик-1 сированной кишки во внутренний стакан устройства и пищевода в кишку. Т гу перемещают до упора втулки с буртиками 12 в днище внутреннего стакана (фиг.З). При этом исключаютс попадани тканей между втулкой с буртиками 12 и втулкой с кольцевыми канавками и днищем внутреннего стакана 2, т.е. гарантируетс местоположение канавки 15 по отношению к эластичному кольцу 3. После чего с помощью байонета 7 вывод т рабочий конец 6 предохранительного штока 5 из полости наружного стакана и продолжают нат гивать т гу, смещают внутренний стакан с втулками до упора в днище наружного стакана, при
этом эластичное кольцо 3 сталкивают с внутреннего стакана и оно попадает, точно в канавку 15, сдавливает со стороны просвета кишки стенки кишки и инвагинированного пищевода в кишку.
В св зи с тем, что внутренний стакан выполнен, короче, пищевод дополнительно инвагинируетс в воронкообразную складку кишечной стенки, что позвол ет увеличить площадь сраста- ни стенок пищевода с кишкой, а также закрепить их несколькими узлевыми швами 18, проход щими через поверхностные слои стенок пищевода и кишки.
Устройство извлекают из кишки че- рез вспомогательное отверстие в кишке , которое ушивают, гибкую т гу извлекают через рот. При этом внутренние отверсти втулок остаютс открытыми , что обеспечивает питание больного и свободный отток пищевод Ю 15 0

ного содержимого естественным путем Через 7-10 дней сдавленные эластичным кольцом 3 и омертвевшие ткани кишки и пищевода вместе с втулками и эластичным кольцом отторгаютс и удал ютс естественным путем.
Вследствие того, что срастание стенок раст нутой кишки со стенками пищевода происходит в асептических услови х, анастомоз надежен, легко раст жим, практически лишен рубцовой ткани.
Claims (2)
1. Устройство дл соединени трубчатых органов, содержащее телескопически соединенные наружный и внутренний стаканы с осевыми отверсти ми в днищах и эластичное кольцо, размещенное по наружному периметру внутреннего стакана, трубчатую руко тку, соединенную с отверстием в днище наружного стакана, зонд, размещенный в трубчатой руко тке, на котором установлена втулка с кольцевой канавкой , отличающеес тем, что, с целью снижени несосто тельности шва и уменьшени заброса инфицированного содержимого пищевода в дыхательные пути, оно снабжено до- полнительнбй втулкой с несколькими кольцевыми канавками, размещенной на зонде, зонд выполнен в виде гибкой т ги с цилиндрическим утолщением, с заходным конусом на одном конце и с упором на другом.
2. Устройство по п.1, отличающеес тем, что оно снабжено предохранительным штоком, рабочий конец которого расположен в полости наружного стакана, а противоположный конец соединен с фиксатором положени , внутреннего стакана, например байо- нетом.
